ABSTRACT

Abstract Objective: Orthodontic force application releases multiple enzymes in gingival crevicular fluid (GCF) for activation, resorption, reversal, deposition of osseous elements and extracellular matrix degradation. The current systematic review critically evaluated all existing evidence on enzymes in orthodontic tooth movement. Methods: Literature was searched with predetermined search strategy on electronic databases (PubMed, Scopus, Embase), along with hand search. Results: Initial search identified 652 studies, shortlisted to 52 studies based on PRISMA. Quality assessment further led to final inclusion of 48 studies (13 moderately and 35 highly sensitive studies). Primary outcomes are significant upregulation in GCF levels of enzymes-aspartate aminotransferase (AST), alkaline phosphatase (ALP), matrix metalloproteinases (MMPs), lactate dehydrogenase (LDH), β-glucuronidase (βG), tartrate resistant acid phosphatase (TRAP), acid phosphatase (ACP) and down regulation in cathepsin B (Cb). Site specificity is shown by ALP, TRAP, AST, LDH, MMP9 with levels at compression site increasing earlier and in higher quantities compared with tension site. ALP levels are higher at tension site only in retention. A positive correlation of LDH, ALP and AST is also observed with increasing orthodontic force magnitude. Conclusions: A strong evidence of variation in enzymes (ALP, AST, ACP TRAP, LDH, MMPs, Cb) in GCF is found in association with different magnitude, stages and sites of orthodontic force application.


Resumo Objetivo: a aplicação da força ortodôntica libera múltiplas enzimas no fluído crevicular gengival (FCG), desencadeando a ativação, reabsorção, reversão, deposição de elementos ósseos e degradação da matriz extracelular. A presente revisão sistemática avaliou criticamente toda a evidência disponível sobre os níveis de enzimas durante a movimentação ortodôntica. Métodos: utilizando-se estratégias predeterminadas, foram realizadas buscas em bases de dados eletrônicas (PubMed, Scopus, Embase), sendo também feitas buscas manuais. Resultados: a busca inicial identificou 652 estudos e, com base nas diretrizes do PRISMA, foram selecionados 52 estudos. A avaliação qualitativa resultou na inclusão final de 48 estudos (13 estudos com moderada sensibilidade e 35 com alto nível de sensibilidade). Os desfechos primários foram o aumento significativo dos níveis no FCG das enzimas aspartato aminotransferase (AST), fosfatase alcalina (FA), metaloproteinases de matriz (MMPs), lactato desidrogenase (LDH), β-glucuronidase (βG), fosfatase ácido-resistente ao tartarato (TRAP), fosfatase ácida (FAC) e baixa regulação de catepsina B (Cb). Especificidade quanto ao local foi mostrada para FA, TRAP, AST, LDH e MMP9 com os níveis no lado de compressão aumentando mais rápido e em maiores quantidades, quando comparado ao lado de tensão. Os níveis de FA foram maiores no lado de tensão somente no período de contenção. Uma correlação positiva de LDH, FA e AST também foi observada à medida que a magnitude de força ortodôntica aumentou. Conclusões: há fortes evidências indicando que as variações nas enzimas (FA, AST, FAC, TRAP, LDH, MMPs, Cb) presentes no FCG estão associadas a diferentes magnitudes, estágios e locais de aplicação da força ortodôntica.

ABSTRACT

Objective:To study the effects of fluor-hydroxyapatite (FHA) coating titanium alloy on the osseointegration and peri-implantitis of orthodontic micro-implant.Methods:Titanium of FHA alloy (FHA group) and titanium alloy(control group) orthodontic micro-implants were respectively planted into buccal alveolar bone in mandibular premolar area of rabbits.Scanning electron microscope was used to observe the osseointegration around the micro-implants.ELISA was employed to detect TNF-α in the gingival crevicular fluid around the implants.Results:The FHA-coating titanium alloy orthodontic micro-implants led to higher bone density,smaller marrow cavity,and lower TNF-α level and shorter lasting period of TNF-α over-expression than the controls (P < 0.05).Conclusion:The FHA-coating titanium alloy orthodontic micro-implant has better histocompatibility and may inhibit peri-implantitis.

ABSTRACT

Objective:To examine the adiponectin level in gingival crevicular fluid(GCF)in patients of chronic periodontitis with dia-betes mellitus type 2.Methods:20 patients of diabetes mellitus type 2 with chronic periodontitis(DM&CP),20 of periodontitis(CP) and 20 health subjects(H)were included.The periodontal indexes (SBI,PLI,PD and AL)were measured,GCF samples were quan-tified by periotron 8000,the adiponectin content in GCF was tested by adiponectin ELISA kit.The relationship between the adiponectin level in GCF and the periodontal indexes of the DM&CP patients was analyzed statistically.Results:The adiponectin level in GCF in group DM&CP was significantly lower than that in the other 2 groups(P 0.05).Conclusion:Decrease of adiponectin in GCF may play a role in the development of DM&CP.

ABSTRACT

Objective:To detect the correlativity between clinical periodental indexes and C-terminal telopeptides of type I collagen (CTX)concentration in gingival crevicular fluid(GCF).Methods:77 teeth with periodontitis in 60 patients from the international Bethune peace hospital were included.The concentration of CTX in GCF was detected by enzyme-linked immunosorbent assay.The plague indexe(PL),gingival indexe(GI),probing depth(PD)and attachment loss (AL)level were recorded at clinic.The correla-tion of CTX concentration and clinic indexes was analysed by Kruskal-Wallis test using SPSS 1 1 .0 software.Results:The Spearman correlation coefficient of PL,GI,PD and AL with CTX was 0.386(P <0.01 ),0.366(P <0.05),0.382(P <0.05)and 0.31 4(P <0.05)respectively.Conclusion:PL,GI,PD and AL are positively correlated with the concentration of CTX in GCF(P <0.05).

ABSTRACT

The purpose of this study was to determine the minimal inhibitory concentration (MIC) of cefuroxime axetil, semisynthetic cefalosporin, for some putative periodotopathogens; F. nucleatum, A. actinomycetemcomitans P. intermedia and P. gingivalis. To investigate the efficacy of cefuroxime axetil, several antibiotics, amoxicillin, metronidazole, and ciprofoxacine, were used as control. The MIC was measured by Murray's method. The MIC of cefuroxime axetil against some putative microbes, as a single use regimen, was relatively high in comparison with that of the other antibiotics used in this study. The MIC of cefuroxime axetil/metronidazole against some putative microbes, as a simultaneous regimen, was similar to that of the other antibiotics used in this study. The manimal level of cefuroxime concentration in gingival fluid was 9 microgram/ml at 36hr after the first dose. In conclusion, within the limited experiment, metronidazole/ cefuroxime axetil therapy of periodontitis may provide a therapeutic benefits in reducing the periodontopathogens.

ABSTRACT

Objective:To study the effects of periodontal scaling on microorganism in periodontal pocket and gingival crevicular fluid in patients with periodontitis.Methods:60 cases of periodontitis were selected and divided into 2 groups randomly with 30 in each group.The patients in scaling group were treated by periodontal scaling, those in control group by gargle with chlohexidine.Before and after treatment the microorganism in pocket bottom of each patient was measured by Congo red negative dyeing.Gingival index(GI), probing depth(PD) and gingival crevicular fluid(GCF) were also measured, the relationship between periodontal scaling and clinical indices was analysed.Results:In scaling group the percentage of coccoid cells in pocket bottom increased more after scaling,that of bacillus,spirochetes decreased, PD,GCF value were all decreased (P0.05).Conclusion:Periodontal scaling can decrease the percentage of pathogenesis bacteria in periodontal pocket bottom and decrease PD,GI and GCF values.

ABSTRACT

Objective:To evaluate the effect of periodontal therapy and local application of Kouling(gargle of composite of phenols in tea)on the subgingival bacteria and levels of endotoxin in gingival crevicular fluid of adult periodontitis.Methods:Seventy-eight adult periodontitis(156teeth)were divided into three groups randomly.The experimental group was treated with periodontal scaling and root planning,and meanwhile administered gargle of composite of phenols in tea.The negative control group was treated with periodontal scaling and root planning,and administered distilled water as placebo.And the positive control group was treated with periodontal scaling and root planning,and administered0.2%chlorhexidine. Levels of endotoxin in gingival crevicular fluid were measured and the subgingival bacteria were cultivated before and after therapy.Results:After therapy,levels of endotoxin in gingival crevicular fluid and numbers of bacteria all decreased remarkably in three groups(P

Objective To examine the changes of matrix metalloproteinase-2,9 (MMP-2,9) in gingival crevicular fluid(GCF) after phase 1 periodontal treatment of adult patients with periodontitis.
