ABSTRACT

Background: Recession is a mucogingival condition affecting teeth causing hypersensitivity. Although many techniques are there for recession coverage, semilunar vestibular incision technique (SVIT) is a novel procedure for management of multiple gingival recession in maxillary teeth. Aim: To evaluate the efficacy of root coverage in maxillary teeth with multiple gingival recession using SVIT. Methodology: Twenty systemically healthy patients were recruited with Miller’s class I and II gingival recessions in maxillary teeth. Parameters such as recession height (RH), recession weight (RW), avascular surface area (ASA), width of keratinized gingiva (WKG), width of attached gingiva (WAG), and clinical attachment level (CAL) were measured at baseline three and six months post?surgery. Results: The outcome measures were statistically significant at baseline, three and six months. A reduction of 86% was achieved in terms of RH and RW. Gain in WKG and WAG as achieved at six?month follow?up was 31.5% and 55%, respectively. An 87% decrease in ASA was obtained and reduction in CAL was 82.4%. Between three and six months there was a significant increase in WAG. Conclusion: SVIT results in improved measures of attached gingiva on six?month follow?up.

ABSTRACT

Objective: To observe the clinical efficacy of acellular dermal matrix combined with modified tunnel in the treatment of gingival recessions. Methods: 27 teeth with gingival recessions in 8 patients were treated by the means of acellular dermal matrix and modified tunnel technique and were followed up for 1 year. The clinical parameters of gingival recession depth,recession width,keratinized tissue height,probing depth and clinical attachment level were assessed. Percentage of root coverage(PRC),complete root coverage(CRC) and patient satisfactions were evaluated. Results: 1 years after treatment PRC reached to 76%,19 teeth got complete root coverage. Recession depth and recession width were significantly decreased(P < 0. 05). Keratinized tissue height increased by(1. 25 ± 0. 24) mm. Clinical attachment level increased by(2. 75 ± 0. 59) mm. Conclusion: Acellular dermal matrix allograft combined with modified tunnel technique is effective in the treatment of gingival recessions.

ABSTRACT The low prevalence of gingival recessions observed in orthodontic clinical practice may be assigned to the fact that in studies in which dehiscences and bone fenestrations are described as frequent, they were diagnosed based on: 1) dry skull studies; 2) areas with periosteal reflection together with flap; and 3) imaging techniques with low sensitivity to detect these defects, which have a delicate structure and function. In areas of pseudo-dehiscences and fenestrations, the periosteum and the alveolar cortical bone are very thin; also, they either have been removed during preparation of the dry specimens in the areas for analysis, or, alternatively, have not been investigated using an ideal imaging method.


RESUMO A ausência de prevalência elevada das recessões gengivais relacionadas à prática clínica ortodôntica se explica, provavelmente, porque, nos trabalhos em que as deiscências e fenestrações ósseas são descritas como frequentes, essas foram diagnosticadas a partir de: 1) estudos em crânios secos; 2) áreas com rebatimento periosteal junto com o retalho; e 3) uso de métodos imagiológicos que falham em sensibilidade para captá-las, pela sua delicadeza estrutural e funcional. Nessas áreas de pseudodeiscências e fenestrações, existe periósteo e cortical óssea alveolar muito fina, que foram eliminados nos procedimentos de preparação dos espécimes secos, nas áreas para a análise ou, então, não se aplicou um método imagiológico ideal.

ABSTRACT

Background: Gingival recession is both unpleasant and unaesthetic. Gingival recession associated with dentinal hypersensitivity is one of the commonest issues addressed by the periodontist. The treatment of choice for such cases should address biological as well as patient chief complaint. Meeting the esthetic and functional demands of patients with multiple gingival recessions remains a major therapeutic challenge. Coronally advanced flaps for root coverage are the most commonly used technique for multiple teeth recessions. This case report describes a case of Class I gingival recession due to faulty brushing technique associate with dentinal hyper sensitivity treated with Zucchelli’s modification of conventional coronally advanced flap with PRF membrane.

El tratamiento de recesiones gingivales atrae un gran interés no solo para los odontólogos sino de pacientes, los cuales muestran gran preocupación, principalmente al factor estético asociado a esta condición. Diversas técnicas han sido propuestas para la obtención del recubrimiento radicular, entre ellas, el injerto de tejido conjuntivo sub - epitelial, el cual se convirtió en una de las formas mas difundidas entre los periodoncistas. El grado de recubrimiento es muy relativo, variando principalmente de acuerdo con las características clínicas y el grado de experiencia y destreza manual del cirujano. Entre las ventajas asociadas a esta técnica resaltamos la previsivilidad de la técnica y la estética obtenida


The treatment of gingival recessions has been attracted interest from both the dental practice as well as patients concerned mainly with the aesthetic factor associated with this condition. A variety of techniques have been offered to obtain root recovering. One of them, the subepithelial grafting of connective tissue became one of the tools most prevalent among the periodontists. The quantity of recovery is relative, depending mainly of the clinical aspects and the experience and manual dexterity of the surgeon. Among the advantages related to this technique, the predictably of the technique and the aesthetic reached are emphasized

ABSTRACT

Objective:To study the effect of subepithelial connective tissue grafting in covering an exposed root surface.Methods:Eight cases with gingival recession on upper and lower anterior teeth and premolars receited connective tissue grafting received connective tissue grafting were presented with 12 months of follow-up.Results:Initial gingival recession averaged 4.3 mm,with a mean probing pocket depth of 2.3 mm.At the 12-month follow-up,a mean of 3.0 mm of root coverage was observed(ie,71% root coverage,range 60% to 80%).Probing pocket depth averaged 1.6 mm,indicating a 3.6 mm gain of clinical attachment(range 2 to 5 mm).Conclusion:The application of connective tissue graft results in predictable root coverage and clinical attachment while maintaining shallow pockets.
