ABSTRACT

Objectives: The purpose of this study was to see the effect of parenteral testosterone injection on penile length, glans diameter, and diameter at the base of the penis in hypospadias patients before surgery. Material and Methods: The study was conducted from September 2019 to March 2021. This study comprised a total of 20 hypospadias patients. At a dose of 2 mg/kg body weight, an injection testosterone propionate was administered deep intramuscularly in three doses with a 3-week gap before reconstructive surgery. Before surgery, the penile length, glans diameter, and penile diameter at the base of the penis were all measured. Results: The mean increase in penile length, glans diameter, and diameter at the base of penis following parenteral testosterone therapy was 1.07 ± 0.23 cm (P < 0.001), 1.01 ± 0.46 cm (P < 0.001), and 0.92 ± 0.12 cm (P < 0.001), respectively. All three measurements were statistically significant. Conclusion: Intramuscular testosterone increased penis size, glans diameter, and penile diameter without causing any notable side effects. Development of fine pubic hair, acne, and aggressiveness is minor adverse effects.

ABSTRACT

SUMMARY: The corporo-glans ligament is the ligament connecting the corpus cavernosum and the glans of the penis. The anatomical description of the corporo-glans ligaments shape is still uncertain, this knowledge affects penile reconstructive procedures. The anatomy of the corporo-glans ligament was analyzed and recorded via observing sagittal sections of 10 different penile P45 plastination sections. According to the P45 plastination sections, the corporo-glans junction displayed a fibrous tissue band connecting the distal ends of the two corpus cavernous (CC) with the glans penis (GP). The fibrous band was a round-obtuse shape and ran deep into the glans of the penis and occupied about 2/3 of the whole GP. The original end was laid in a socket embedded in the GP. The density of the fibers of the ligament at the original end close to the tunica albuginea was less than that of the other parts. The fibers originating from the tunica albuginea, directly extended to the blind end of the two CC, covering the distal end of the two CC.


El ligamento cuerpo cavernoso-glande es el ligamento que conecta el cuerpo cavernoso y el glande del pene. La descripción anatómica de la forma de los ligamentos cuerpo cavernoso -glande aún es incierta; este conocimiento afecta los procedimientos reconstructivos del pene. La anatomía del ligamento cuerpo cavernoso-glande se analizó y registró mediante la observación de 10 secciones sagitales diferentes del pene a través de plastinación P45. Según las secciones de plastinación, la unión cuerpo-glande mostraba una banda de tejido fibroso que conectaba los extremos distales de los dos cuerpos cavernosos con el glande del pene. La banda fibrosa tenía una forma redonda y obtusa y se adentraba profundamente en el glande del pene ocupando alrededor de 2/3 de él. En su origen se coloca en un espacio profundo en el glande del pene. La densidad de las fibras del ligamento cuerpo cavernoso-glande en su origen cercano a la túnica albugínea era menor que el de las otras partes. Las fibras que se originan en la túnica albugínea, se extienden directamente hasta el extremo ciego de los dos cuerpos cavernosos, cubriendo el extremo distal de estos.

ABSTRACT

【Objective】 To investigate the effects of lamellar surgical techniques with urethral plate to strengthen the tissue of glans penis to widen the two flanks of glans penis on the basis of Duckett method in the treatment of congenital hypospadias with small glans penis deformity. 【Methods】 A total of 22 patients admitted to our hospital during Jun.2017 and Oct.2020 were involved. Urethral plate was used to replace the glans penis tissue to widen the two flanks of glans penis based on Duckett method. Lamellar surgical techniques were adopted to fully dissociate the two flanks of glans penis and urethral plate for urethroplasty. 【Results】 Of the 22 operations, 19 were successful,with a success rate of 86.3%. The success rate of penile head urethroplasty reached 96.1%. 【Conclusion】 Widening the glans penis by using the urethral plate based on Duckett method combined with lamellar surgical techniques can improve the success rate of glans penis urethroplasty.

ABSTRACT

Repairing glans dehiscence after failed hypospadias repair is challenging for pediatric surgeons. Here, we introduced and evaluated a newly modified Mathieu technique, Mathieu combined tunnel (MCT), which involves multiple custom-designed flaps for the shortage of flap source material after repeated operations; we also constructed a tunnel to avoid the glans incision that may carry new risks of dehiscence. This retrospective study included 26 patients who were consecutively admitted to the First Affiliated Hospital of Sun Yat-Sen University (Guangzhou, China) for glans dehiscence repair after failed hypospadias repair from October 2014 to October 2020; sixteen patients underwent surgery using the MCT (MCT group) and ten patients underwent surgery using the tubularized incised plate (TIP) technique (TIP group). The operative time, blood loss, postoperative complications, normal urethral meatus rate, success rate, and Hypospadias Objective Penile Evaluation (HOPE) score were compared between the two groups. The MCT group achieved an overall satisfactory penile appearance and voiding function, with a higher rate of normal urethral meatus (15/16, 93.8%) and a lower rate of glans dehiscence (1/16, 6.2%), compared with the TIP group (70.0% and 30.0%, respectively). However, these differences were not statistically significant, possibly because of the limited number of patients (all P > 0.05). Mean postoperative HOPE scores were similar in the MCT group (mean ± standard deviation: 8.83 ± 0. 89) and TIP group (8.94 ± 0.57) (P > 0.05). No significant differences were found between the two groups in terms of blood loss and success rate, nor in the rates of various complications (e.g., fistula, urethral stricture, and glans dehiscence). In conclusion, the MCT technique appears to be feasible and reliable for repairing glans dehiscence after failed hypospadias repair.

Resumen 22. El linfangioma circunscripto (LC) es una afección rara y benigna, caracterizada principalmente por la malformación de los vasos linfáticos de la piel. De etiología congénita o adquirida,esta última debido a causas secundarias como radioterapia, infecciones o procedimientos quirúrgicos. 23. Presentamos el caso de un paciente de 75 años de edad, con antecedente quirúrgico de fimosis a los 29 años, y posterior desarrollo de linfangioma circunscripto de glande diagnosticado 46 años después. El tratamiento definitivo planteado en interconsulta con Urología fue la resección quirúrgica, motivados por la escasa frecuencia de recidivas cuando se usa este método.


Summary 27. Circumscribed lymphangioma (CL) is a rare and benign condition, characterized mainly by malformation of the lymphatic vessels of the skin. It has a congenital or acquired etiology, the latter due to secondary effects such as radiotherapy, infections or surgical procedures. 28. We present the case of a 75-year-old patient, with a surgical history of phimosis at 29 years of age, and subsequent development of circumscribed lymphangioma of the glans penis diagnosed 46 years later. The definitive treatment proposed in consultation with Urology was surgical resection due to the low frequency of recurrences.

ABSTRACT

We aim to design a new glanuloplasty procedure combined with spongiosum to reduce the incidence of glans dehiscence and coronal fistula after proximal hypospadias repair. Patients who underwent urethroplasty by dorsal preputial island flap for proximal hypospadias between January 2014 and December 2016 were reviewed in this retrospective cohort study. Those who underwent spongiosum-combined glanuloplasty comprised the new-maneuver group, whereas those who underwent conventional glanuloplasty comprised the control group. The incidence of complications was then compared. In the new-maneuver group, dysplastic corpus spongiosum alongside lateral Buck's fascia (0.3-0.4 cm wide) on both sides of the urethral plate was separated from the proximal normal spongy tissue, joining into the glans wings to increase tissue volume and covering the neourethra in the glans penis. In the control group, the neourethra was covered with superficial fascia under the coronal sulcus. As a result, the new-maneuver and control groups comprised 47 and 28 patients, respectively. In the new-maneuver group, no glans dehiscence was detected; however, two (4.3%) patients had coronal fistula, two (4.3%) had urethral stricture, and four (8.5%) had diverticulum. In the control group, two (7.1%) patients had glans dehiscence, eight (28.6%) had coronal fistula, four (14.3%) had urethral stricture, one (3.6%) had diverticulum, and one (3.6%) had penile curvature recurrence. The new-maneuver group had less incidences of coronal fistula (P < 0.001), glans dehiscence (P = 0.033), and urethral stricture (P = 0.008) but had a higher incidence of diverticulum than the control group (P = 0.040). It clearly demonstrates that spongiosum-combined glanuloplasty can significantly reduce the incidences of coronal fistula and glans dehisce.

We reviewed our experience in reconstructing forked corpus spongiosum (FCS) in distal/midshaft hypospadias repair and analyzed the efficacy of this surgical technique. From August 2013 to December 2018, 137 consecutive cases of distal/midshaft hypospadias operated by the same surgeon in Urology Department, Children's Hospital of Fudan University (Shanghai, China), were retrospectively analyzed. Sixty-four patients who underwent routine tubularized incised plate (TIP) or onlay island flap (ONLAY) surgery were included in the nonreconstructing group, and 73 patients who underwent reconstructing FCS during TIP or ONLAY surgery were included as the reconstructing group. Thirty-eight cases underwent TIP, and 26 underwent ONLAY in the nonreconstructing group, with a median follow-up of 44 (range: 30-70) months. Twenty-seven cases underwent TIP, and 46 underwent ONLAY in the reconstructing group, with a median follow-up of 15 (range: 6-27) months. In the nonreconstructing/reconstructing groups, the mean age at the time of surgery was 37.55 (standard deviation [s.d.]: 29.65)/35.23 (s.d.: 31.27) months, the mean operation duration was 91.95 (s.d.: 12.17)/93.84 (s.d.: 14.91) min, the mean neourethral length was 1.88 (s.d.: 0.53)/1.94 (s.d.: 0.53) cm, and the mean glans width was 11.83 (s.d.: 1.32)/11.56 (s.d.: 1.83) mm. Twelve (18.8%)/5 (6.8%) postoperative complications occurred in the nonreconstructing/reconstructing groups. These included fistula (5/2), glans dehiscence (3/0), diverticulum (1/2), residual chordee (3/0), and meatus stenosis (0/1) in each group. There was a significant difference in the overall rate of complications (P= 0.035). These results indicate that the technique of reconstructing FCS provides excellent outcomes with fewer complications in distal/midshaft hypospadias repair.

ABSTRACT

A ultrassonografia é um exame complementar não invasivo ainda pouco utilizado na avaliação prepucial e sem descrições detalhadas da parte livre do pênis (PLP) de bovinos. Este estudo objetivou padronizar a ultrassonografia prepucial em bovinos hígidos da raça Nelore com um ano de idade e, para tanto, descreveu o aspecto da lâmina interna prepucial (LIP) e da PLP, determinando as principais janelas acústicas, a viabilidade da utilização de contraste no lúmen da cavidade prepucial (LCP) e o melhor posicionamento do animal para a realização do exame. O escaneamento foi realizado com os animais na posição quadrupedal e em decúbito lateral direito. Foram determinadas cinco janelas acústicas a partir do óstio até a identificação do recesso prepucial. Os escaneamentos foram realizados nos planos longitudinais, transversais e dorsais, antes e após a infiltração de solução fisiológica a 0,9% no LCP. Como conclusões, o exame ultrassonográfico permitiu identificar a LIP e a PLP em todos os planos e em todas as janelas acústicas propostas. O melhor ponto de referência é a identificação da glande. O posicionamento quadrupedal é mais adequado para a realização do exame e o uso de contraste é recomendado para delimitação topográfica das estruturas.(AU)


Ultrasonography is a noninvasive complementary exam that is still rarely used in the preputial evaluation and without detailed descriptions of the bovine free end of the penis. The purpose of this study was to standardize preputial ultrasonography in one-year-old healthy Nelore cattle, describing the main acoustic windows, the viability of using contrast in the preputial cavity and the best positioning of the animal for performing the ultrasound examination. The scanning was performed with the animals in standing and decubitus position. Five acoustic windows were determined from the preputial ostium to the preputial fornix, at the free end of the penis. The scans were performed in longitudinal, transverse and dorsal planes, before and after infiltration of 0.9% saline solution into the preputial cavity. As conclusions, the ultrasound examination is able to identify the internal layer and the free part of the penis in all proposed planes and acoustic windows. The best landmark is the identification of the glans. Furthermore, standing positioning is the most appropriate to perform the exam and the use of contrast is recommended for topographic delimitation of the structures.(AU)

Objective@#To explore the feasibility of glans-preserving surgery (GPS) in the treatment of superficial penile squamous cell carcinoma (PSCC) with the lesion diameter of ≥2 cm.@*METHODS@#We retrospectively analyzed the clinical data on 69 cases of superficial PSCC (≤T1aN0) treated by GPS (n = 36) or radical surgery (total or partial penectomy, n = 33) from July 2007 to July 2017.@*RESULTS@#The mean tumor diameter and depth of invasion were 3.16 (2.0-6.0) cm and 0.89 (0.5-2.0) cm in the GPS group and 3.56 (2.0-6.0) cm and 1.89 (0.6-4.0) cm respectively in the radical surgery group. The patients were followed up for 10-102 (mean 42) months, during which, 5 patients in the GPS group developed local recurrence at 40 days and 2, 4, 7 and 9 months postoperatively, again underwent gansectomy, partial penectomy or GPS, and experienced no more recurrence during the follow-up of 54, 34, 39, 66 and 70 months. No local recurrence was observed in the radical surgery group, and none of the 69 patients experienced lymph node metastasis or died during the follow-up.@*CONCLUSIONS@#GPS is safe and efficient for the treatment of superficial PSCC with the lesion diameter of ≥2 cm.

ABSTRACT

There is no safe and effective standard method for glans penis augmentation. Furthermore, there has been scant research on glans penis augmentation due to a poor understanding of glans anatomy, technical difficulty, and a lack of suitable substances for augmentation. Cross-linked dextran gel is a newly developed filler for soft-tissue augmentation. We evaluated the efficacy and safety of using a novel technique to inject cross-linked dextran gel for glans penis augmentation during a 24-week follow-up study. This prospective, single-arm, multicenter study enrolled twenty healthy adult men who underwent glans penis augmentation between June and August 2013. Cross-linked dextran gel was injected into the glans penis using a simple and easy technique. The sizes of the glans penis and individual satisfaction were assessed. Any adverse event was also reported. A total of 18 individuals were analyzed; two of them were lost to follow-up. The mean procedure time and injected volume were about 30 min and 6.6 ± 0.9 ml, respectively. The mean surface areas of the glans at baseline and 24 weeks were 20.0 ± 3.5 cm2 and 33.6 ± 5.4 cm2, respectively, representing a mean increase of 68.7% ± 14.0% (P < 0.001). Sixteen individuals (88.9%) were satisfied with the outcomes, and none were dissatisfied. There were no serious adverse events during the study. Cross-linked dextran gel injection for glans penis augmentation was easy and showed a significant augmentative effect on the glans penis, good durability, and was well tolerated without serious adverse events. Therefore, cross-linked dextran gel injection may be an effective, new technique for glans penis augmentation.

Objective To explore treatment scheme and surgery for hypospadias with small glans.Methods Between January 2014 and January 2017,40 cases of hypospadias with small glans were enrolled in the study including 13 cases with penile hypospadias and 27 cases with penoscrotal hypospadias.The boys's average age was 1.5 years,aged from 1 to 5 years old.The boys whoes glans width less than 10 mm received the treatment of topical testosterone propionate smeared on penis once a day for 1 or 2 months.The patients received operation when their glans width reached 10 mm to 14 mm.Among of 27 cases of severe penile recurvation patients,17 cases received staging operation,one-stage was correction of penile recurvation and two-stage was Tubularized incised plate with pull-through procedure,10 cases received adopted modified Duplay combined with Duckett operation,then overturn split-skin graft for incision in the urethra located in glans penis.Among of 13 cases with mild and moderate recurvation,after penile correction,3 cases received Tubularized incised plate urethroplasty and 10 cases received Onlay island flap urethroplasty.All of the patients received modified plastic surgery for glans.Results All of patients were followed-up,from 3 months to 3 years.The surgical success rate was 70% (28/40).12 cases got operative complications,including urinary fistula 6 cases and glans dehiscence 1 case,who were cured by reoperation,external orifice stricture 1 case and urethral stricture 4 cases who were healed by Urethra dilatation.All patients acquired good appearance glans and satisfied void stream.Conclusions Topical use testosterone propionate for hypospadias with small glans helps the growth of penis.Individualized diagnosis and treatment plan should be made for the particular case of hypospadias with small glans.The modified plastic surgery of glans is suitable for hypospadias with small glans,which could improve surgery success rate and reduce the postoperative complication.

Objective: To investigate chemical constituents and new antimicrobial agents among essential oils from the rhizomes of Curcuma aeruginosa(C.aeruginosa)Roxb.,Curcuma glans K.Larsen&J.Mood and Curcuma cf.xanthorrhiza Roxb. Methods: The essential oils were obtained by hydro-distillation and analyzed by gas chromatography/mass spectroscopy.Agar-well diffusion assay was used to study the anti-microbial activity and also broth-micro dilution techniques were examined for minimum inhibitory concentration (MIC)against four bacterial strains and yeast. Results: The gas chromatography/mass spectroscopy analysis showed monoterpenes predominantly(88.53%)in the rhizome oil of Curcuma cf.xanthorrhiza.Sesquiterpenes (50.10%) was the most abundant component in the essential oil of C. glans, while monoterpenes(45.55%)and sesquiterpenes(45.81%)were found in C.aeruginosa with a significant amount. The major components of C. aeruginosa were characterized as camphor (29.39%) and germacrone (21.21%). Germacrone (15.76%),β-pinene (9.97%) and camphor (9.96%) were found as major compounds in the rhizome oils of C. glans while α-terpinolene (24.86%) and p-cymen-7-ol(12.17%) were found as major compo-sitions in Curcuma cf.xanthorrhiza.The essential oils were tested against four bacterial strains and yeast.As a result,the rhizome oil of C.aeruginosa exhibited potent activity against Staphylococcus aureus [inhibition zone (21.94 ± 0.24) mm, MIC 125 μg/mL], Bacillus cereus [inhibition zone (20.83 ± 0.36) mm, MIC 125 μg/mL], and Candida albicans[inhibition zone(11.60 ± 0.30)mm,MIC 250 μg/mL]. Conclusions: The essential oils from three Curcuma species possessed greater activity against the gram-positive bacteria (Staphylococcus aureus and Bacillus cereus) than gram-negative bacteria (Escherichia coli and Pseudomonas aeruginosa). The results suggest that the essential oils from the fresh rhizome of Curcuma spp. might be a po-tential source of natural antimicrobial substances.

ABSTRACT

Epidermoid cysts are benign lesions occurring anywhere in the body, but it is a rare entity over glans penis. Even though a few cases are been reported in adult age group, epidermoid cyst occurring at glans penis in young age is very rare. The etiology in penile area is not specific, but it may represent a monolayer teratoma of germ cell origin or an abnormal embryogenic closure of the median raphe. Being benign its harmless, but secondary infections are common which gives an indication for surgical removal of the cyst. Here, we report a case of epidermoid cyst in a 14-year-old boy who came with painless swelling over the glans penis.

ABSTRACT

The present study analyses the glans penis and baculum morphology of three Brazilian tuco-tucos, Ctenomys torquatus Lichtenstein, 1830, Ctenomys minutus Nehring, 1887 and Ctenomys flamarioni Travi, 1981, in order to identify possible variations and understand some more about this taxonomically complex group. We used fixed penis from 15 previously listed adult specimens. For a more detailed baculum analysis, the penis underwent dissection and diaphanisation, whereas to analyse the glans penis surface we used Scanning Electron Microscopy (SEM). Results showed striking differences in baculum morphology among the three species. While C. minutus have a particular V-shaped proximal baculum tip, C. flamarioni baculum is thin throughout the shaft with rounded proximal and distal tips. Ctenomys torquatus have a shorter and larger baculum, similar to what has previously been described for the species. Glans penis surface microstructure analyses also revealed inter-specific differences, with penial spines varying in shape, size and, especially density. Although C. torquatus has a relatively small penis, it has the largest penial spine density, which suggests a more complex penial ornamentation in this species.


O presente estudo analisa a morfologia do glans penis e do baculum de três tuco-tucos do Brazil - Ctenomys torquatus Lichtenstein, 1830, Ctenomys minutus Nehring, 1887, e Ctenomys flamarioni Travi, 1981 - com a finalidade de identificar possíveis variações e elucidar mais sobre grupo taxonômico complexo. Foram usados pênis fixados de 15 indivíduos adultos das espécies listadas anteriormente. Para uma análise mais detalhada do baculum, o pênis foi submetido a dissecções e diafanização, enquanto que, para a análise da superfície do glans penis, foi usada a técnica de microscopia eletrônica de varredura (MEV). Os resultados exibiram significativas diferenças na morfologia do baculum entre as três espécies. Enquanto C. minutus apresenta um baculum com uma ponta em forma de V característica, o baculum de C. flamarioni é fino ao longo do osso, com as pontas proximal e distal arredondadas. Ctenomys torquatus apresenta um baculum mais curto e largo, similar ao descrito anteriormente para a espécie. Análises da microestrutura na superfície do glans penis revelaram diferenças interespecíficas, com os espinhos penianos variando em forma, tamanho e, especialmente, densidade. Apesar de C. torquatus ter um pênis relativamente pequeno, apresenta a maior densidade de espinhos penianos, o que sugere uma complexa ornamentação peniana nesta espécie.

ABSTRACT

Milia are small, superficial keratin cysts believed to be derived from the pilosebaceous follicle, which arise most commonly on the cheeks and eyelids. We report a case of milium occurring on the glans penis in a 18-year-old man, who had a solitary dome-shaped, 2x2 mm sized, yellowish papule. The histopathological examination showed a small keratin-filled, intradermal cyst. The cyst was lined by a stratified epithelium that was a few cell layers thick. This is the first case report in the Korean dermatological literature of milium occurring on the glans penis without coexistence of other dermatoses.

ABSTRACT

The term phimosis is used when the prepuce cannot be reytracted behind the glans. This condition is present in almost all newborns, fulfilling mostly a protective function. In childhood, a gradual separation occurs, which is completed in adolescence. Through this process, complications may happen such as urinary tract infections, balanitis, or paraphimosis. Circumcision has been considered the treatment of choice, although some factors must be weighed: degree of narrowing, complications, opinión and religious beliefs of the parents. In the past 15 years, publications have suggested topical steroids as an alternative treatment, with variable success. Treatment of phimosis will continue to be controversial.


El término fimosis se utiliza cuando el prepucio no puede ser retraído por detrás del glande. Esta condición está presente en casi todos los recién nacidos cumpliendo una función principalmente protectora. Durante la infancia se produce una separación gradual del prepucio, que se completa, en la mayoría de los casos, al llegar la adolescencia. Mientras ocurre el proceso de desprendimiento pueden presentarse complicaciones como; infección del tracto urinario, balanitis o parafimosis. La circuncisión ha sido tradicionalmente el tratamiento de elección, sin embargo, se debe tener en cuenta diversos factores antes de tomar una decisión quirúrgica. Debe ser considerado: el grado de estrechez, las complicaciones, la opinión y las creencias religiosas de los padres. En los últimos 15 años se han publicado numerosos trabajos sobre el uso de esteroides tópicos como alternativa terapéutica, con resultados de éxito variable. El tratamiento de la fimosis seguirá siendo un tema controvertido.
