ABSTRACT

We report a 45-year-old woman presenting with a painful subungual epidermal cyst on the 4th finger nail bed. Based on clinical features only, the lesion was initially suspected to be a glomus tumor. However, histopathological examination revealed an epidermal cyst on the nail bed. Subungual epidermal cysts are rare disorders that may cause pain and tenderness that increase according to tumor growth. They can mimic glomus tumors owing to their similar clinical manifestations. To differentiate subungual epidermal cysts from glomus tumors, imaging tools such as ultrasonography and magnetic resonance imaging can be used. Dermatologists should be aware that several kinds of tumors including epidermal cysts with clinical similarities may occur in the subungual area.

ABSTRACT

OBJETIVO: Analisar os principais aspectos de imagem dos tumores glômicos subungueais. MATERIAIS E MÉTODOS: Realizado estudo retrospectivo de oito casos de tumores glômicos subungueais, pertencentes a arquivos de duas clínicas particulares de Goiânia, GO, Brasil. Foram obtidas radiografias em cinco casos, ultrassonografia com Doppler em seis casos e ressonância magnética em cinco casos. RESULTADOS: A idade média de acometimento no presente estudo foi de 39 anos, com predomínio do sexo feminino, na proporção de 7:1. Os tumores não apresentaram predileção por nenhum dedo e a maioria localizava-se na região subungueal mediana. A radiografia foi positiva em três casos, demonstrando erosões de pressão. A ultrassonografia com Doppler foi positiva em cinco casos, evidenciando nódulo sólido, hipoecoico e hipervascularizado. A ressonância magnética, em todos os casos, demonstrou nódulo sólido com hipossinal em T1, hipersinal em T2 e captação homogênea do meio de contraste. Em todos os pacientes foi realizada excisão cirúrgica com confirmação anatomopatológica. CONCLUSÃO: A maioria dos tumores glômicos tem localização subungueal. O diagnóstico é clínico, porém geralmente tardio. Os métodos de imagem auxiliam no diagnóstico precoce, além auxiliar no planejamento terapêutico, cujo tratamento de escolha é a excisão cirúrgica.


OBJECTIVE: To evaluate main imaging findings of subungual glomus tumors. MATERIALS AND METHODS: Retrospective study of eight cases of subungual glomus tumors from the archives of two private clinics in Goiânia, GO, Brazil. Plain radiographs were obtained in five cases, Doppler ultrasonography in five, and magnetic resonance imaging in five cases. RESULTS: Mean age of the patients included in the present study was 39 years, with female predominance (7:1). Tumors didn't present prevalence in any specific finger and in most of cases the tumor was located in the medial subungual region. Plain radiography was positive in five cases, demonstrating pressure erosion. Doppler ultrasonography was positive in five cases, demonstrating a hypoechoic and hypervascularized solid nodule. Magnetic resonance imaging was positive in all of the cases, showing a solid nodule, hypointense on T1-, hyperintense on T2-weighted image, with homogeneous contrast uptake. All the patients underwent surgical excision of the lesion with histopathological confirmation. CONCLUSION: In most of cases, glomus tumors are subungual. The diagnosis is clinical and generally is lately achieved. Imaging methods are useful tools for the early diagnosis besides aiding in the surgical planning, considering that the treatment of choice is surgical excision.

ABSTRACT

PURPOSE: Glomus tumors are neoplasms that are composed of modified smooth muscle cells of the glomus body and multiple glomus tumor comprises 10% of all glomus tumors. We report a case of disseminated multiple glomus tumors. METHODS: A 14-year-old boy presented with multiple subcutaneous purple nodules on the right cheek, back, right arm, right hand dorsum, right fourth finger, and left ankle. Nodules on the back and right fourth finger were completely excised under local anesthesia and histopathologic examination was followed. RESULTS: Histopathologic findings showed numerous dilated, cavernous-like, thin-walled vascular spaces surrounded by one or a few layers of glomus cells. On immunohistochemical examination, glomus cells stain for smooth muscle actin, and endothelial cells stain for CD31. Those revealed multiple glomangiomas. CONCLUSION: A review of Korean literature revealed only one reported case of disseminated multiple glomus tumors, so this is the second case to be reported in the Korean literature. In case of multiple soft tissue tumors, thorough physical examination and preoperative evaluation are needed.

ABSTRACT

We report a case of multiple glomus tumors showing histopathologic feature of glomangiomyoma with a review of the literature. The patient presented with multiple, tender, subcutaneous nodules on the back, thigh, and arm. Skin biopsy from the back revealed glomangioma. Three years later, new lesions appeared on the waist and soles and pain was noted on the thigh and back lesions. Additional skin biopsy at the same site was performed to reveal glomangiomyoma.

ABSTRACT

We report herein a case of multiple glomus tumors on the scrotum in a 14-year-old boy. The lesions were present at birth and had gradually increased in size and number. Pea to thumb tip sized, dark colored nodules without pain sensation were confined to the scrotum. Histopathologically, there were many dilated and irregular vascular spaces lined by a single layer of flattened endothelial cells in the dermis. In the periphery of the endothelial cells, there are a few layers of glomus cells. No capsule was seen. Total excision of the tumors was performed. No recurrence was observed for an year after surgical excision.
