ABSTRACT

Presentamos tres casos de dermatitis irritativa de la zona del pañal de aparición poco común, el eritema pápulo-erosivo de Sevestre-Jacquet y el granuloma glúteo infantil, que junto con la erupción pápulo-nodular pseudo-verrucosa perianal, son raras variedades morfológicas de esta frecuente entidad. Se consideran presentaciones patológicas de la dermatitis irritativa del área del pañal, con un amplio espectro de manifestaciones clínicas.


We present three cases of dermatitis of the diaper area of rare occurrence, papulo-erosive erythema of Sevestre-Jacquet and granuloma gluteale infantum, which together with the perianal pseudo-verrucous papules and nodules, are very rare morphological varieties of clinical manifestation of this common entity. These pathological presentations are considered as part of a wide spectrum of clinical manifestations of irritative dermatitis in the diaper area.

ABSTRACT

Granuloma gluteale infantum has been applied to a condition arising as a complication of primary irritant dermatitis. Plastic pants, and the use of topical corticosteroids to treat diaper dermatitis are believed to be important precipitating factors. Candida albicans has been considered as an etiologic agent. The characteristic lesions are violaceous, oval nodules, occuring on the convexities of the diaper area in 2- to 9-month-old infants with a history of diaper dermatitis. We report a case of granuloma gluteale infantum that developed in a 3 month-old infant who had multiple violaceous nodules over the diaper area of the genitocrural region.

ABSTRACT

Granuloma gluteale infantum is a granulomatous lesion in the diaper area of infants. Athough usually seen in infants, it has been described in incontinent elderly patients. The causes of granuloma gluteale infantum are not clear, although child care equipment(ditergents1, diapers2, and starch3), corticosteroids2,4,5,6, candidial infection3 urine and feces7 have been postulated for its etiology. We report a case of granuloma gluteale infantum in a 5-month-old boy. He was healthy except for an asymtomatic 1.5×1.5cm sized, round, Rat-topped brownish and firm nodule on the perianal area, and two, 1×3cm and 1×6cm sized brownish plaques on the intertriginous area of the neck. The lesions were resistant to the treatment of systemic antibiotics. These nodules completely resolved in the three months after we tried to keep the diaper area clean without using any corticosteroids or other ointments and after changing from plastic diapers to cotton o A histological examination of the nodule on the perianal area showed mild hyperkeratosis, spongiosis, and focal infiltration of neutrophils in the epidermis.
