ABSTRACT

El Granuloma piógeno (GP), es una lesión vascular benigna común en piel y mucosas, que se incluye en la clasificación de anomalías vasculares de la “International Society for the study of vascular anomalies (ISSVA)”, dentro de “otros” tumores vasculares, a pesar de su controversial denominación y su patogenia “reaccional”. Presentamos un caso de granuloma piógeno en una paciente de 18 años de edad, en el segundo trimestre de embarazo con una presentación atípica por su gran tamaño y localización acral, con excelente respuesta al tratamiento quirúrgico.


Pyogenic Granuloma (GP) is a common vascular lesion of the skin and mucous membranes, usually presented as a solitary neoplasm with a bright red, smooth surface; usually sessile or pedunculated that grows rapidly to reach 1 to 2 inches in a few weeks and then remains stationary. It is included in the classification of vascular anomalies of the International Society for the study of vascular anomalies (ISSVA) to under "other" vascular tumors despite their controversial name and their "reactional" pathogenesis. Our goal is to present a case of pyogenic granuloma in an 18-year-old rural worker in the 2nd trimester with an atypical presentation for its size (4 x 4 cm) and acral location with excellent response to surgery and emphasize that such damage can result from repeated trauma, impaired wound healing and neovascularization and overexpression of the growth factor vascular endothelial (VEGF) at this period of life.

ABSTRACT

We describe a case of a giant pyogenic granuloma affecting the left index finger of a 23-year-old pregnant lady requiring surgical excision. Pyogenic granuloma gravidarum is a benign hyperplastic lesion that commonly presents on oral mucosa, typically the gums, in approximately 5% of pregnant women. Skin lesions over the fingers are an unusual site during pregnancy with only a few cases reported in the literature. The lesion was excised and a diagnosis of lobular capillary haemangioma was made based on histopathological evaluation. We delayed surgery until after parturition since recurrence is likely to be lower due to lower levels of circulating oestrogen and progesterone. The prognosis is usually excellent following resection. However, recurrences have been reported regardless of the treatment method.

ABSTRACT

Granuloma gravidarum (GG) is an inflammatory lesion, which develops in the oral mucosa of pregnant women in response to chronic low-grade irritants, under the influence of hormonal factors. Objective To characterize GG clinically by means of a retrospective study of the cases of the Oral Medicine Division, São Lucas Hospital, Brazil. Material and Methods Cases of GG diagnosed between 1980 and 2012 were analyzed. Data were obtained referring to the age of the patients, lesion location, clinical features, as well as the presence of local irritants. The gestation period in which the lesion developed was also investigated. Results Forty-one cases of GG were found. The lesions developed predominantly in the third trimester of pregnancy (51.22%) and the mean age of the patients was 28 years. Most GG was found in the gingiva (73.17%), was reddish color and had a mean diameter of 1.5 cm. Local irritants were involved in 75.6% of the cases. Conclusions The hormonal conditions of pregnancy can have an impact on the oral cavity, predisposing the patient to inflammatory lesions such as GG. .

ABSTRACT

Oral pyogenic granuloma is a hyperplastic inflammatory lesion commonly associated to local irritation or trauma. Females are more affected than men probably due to the vascular effects of hormones that occur during puberty, pregnancy and menopause. In the pregnancy, the lesions are known as "pregnancy tumor" and tend to occur more frequently during the second and third trimester. In the oral cavity, histopathological examination is required for diagnosis, since the lesion is clinically indistinguishable from other reactive lesions and, usually, there is no evidence of bone involvement. The authors report a rare case of pyogenic granuloma with destruction of alveolar bone mimicking a malignant tumor in a 20-year-old woman in the 19th week of pregnancy.


El granuloma piógeno oral es una lesión hiperplásica inflamatoria asociada a la presencia de irritación local o trauma. Las mujeres son más afectadas que los hombres, probablemente debido a los efectos vasculares de las hormonas que se producen durante la pubertad, el embarazo y la menopausia. En el embarazo, las lesiones se conocen como "tumor del embarazo" y tienden a ocurrir con más frecuencia durante el segundo y tercer trimestres. En la cavidad oral, el examen histopatológico es necesario para el diagnóstico, ya que la lesión es clínicamente indistinguible de otras lesiones reactivas y, por lo general, no hay evidencia de afectación ósea. Los autores divulgan un caso raro de granuloma piógeno con la destrucción del hueso alveolar simulando un tumor maligno en una mujer de 20 años de edad, en la semana 19 de embarazo.

ABSTRACT

El hemangioma capilar lobular (granuloma piógeno) ha sido informado con más frecuencia en piel (88%) que en mucosas (12%). El granuloma gravidarum es un granuloma piógeno que se presenta frecuentemente durante el embarazo a nivel de mucosa oral. Ha sido asociado con la acción de los estrógenos y una respuesta inflamatoria exagerada de las mucosas. En este artículo informamos el primer caso de granuloma gravidarum en piel cabelluda.


Lobular capillary hemangioma (pyogenic granuloma) has been reported more frequently in the skin (88%) than in the mucous membranes (12%). Granuloma gravidarum is a pyogenic granuloma that develops during pregnancy, often in the oral mucosa. Estrogens apparently increase the inflammatory response of mucous membranes. In this article, we describe the first case of a granuloma gravidarum of the scalp.
