ABSTRACT

Lumbar disc herniation is a major health problem, affecting the most productive population globally. It has closest resemblance with Gridhrasi mentioned in Ayurvedic classics. Treatments available in conventional sciences have limitations such as relapse of acute episodes. Here an effort was made to treat a case of L5-S1 disc extrusion with radiculopathy using a comprehensive Ayurveda treatment protocol. The protocol includes a set of treatment procedures along with certain internal medicines. Changes were analysed with the help of VAS scale, SLRT and Oswestry disability index and showed significant improvements. This case is an evidence to demonstrate the effectiveness of Ayurveda treatments in case of LDH with radiculopathy.

ABSTRACT

The symptomatology of Gridhrasi given in Ayurvedic classics are pain in chronological order starting from Sphikpurva, Kati, Prushta, Uru, Janu, Jangha, upto Pada, Sakthna kshepo nighruniyat. The clinical features of lumbar spondylosis can be correlated with Gridhrasi and condition can be treated on the lines of Gridhrasi and Vatavyadhi. Objectives: To evaluate the efficacy of Katibasti, Ksheerabasti and Vatagajankusha rasa as Shamanoushadhi in the management of Gridhrasi vis a vis lumbar spondylosis. Result: The therapies Katibasti, Ksheerabasti and Vatagajankusha rasa along with the suitable Anupana as Shamanoushadhi chosen for the treatment showed very good results in terms of reducing acute pain as well as radiculopathy. Conclusion: As the long term or extensive use of NSAID’s, Cox-inhibitors like etoricoxib, muscle relaxants, intervertebral steroid injections and even surgery in the management of lumbar spondylosis will induce serious adverse effect on body, safe and effective therapies like Katibasti, Ksheerabasti and Vatagajankusha rasa along with the suitable Anupana as Shamanoushadhi were selected for treatment. These therapies have not caused any adverse events and improved the condition by arresting the pathogenesis at their respective particular stages. It was observed that the above mentioned Ayurvedic therapies were found to be safe and effective in Gridhrasi vis a vis Lumbar spondylosis.

ABSTRACT

Kati Basti is an Ayurvedic treatment approach that includes several acts such as Bahyasnehana (external oleation), and Swedana Chikitsa (fomentation therapy). Katibasti is not mentioned in any of the classical sources. Even though it is not injected into the body, it is nevertheless referred to as Basti because medicated and lubricated substances are applied to certain parts of the body for a set period. In Kati (back) and Adhah shakagata vikaras, Katibasti is beneficial (disorders of the lower part of the body). This type of external Basti is very beneficial for muscle spasms, lower back tightness, and bone tissue strengthening in the back area. Backache is the common term used for trending diseases of today's lifestyle due to the engagement of people in improper postures, one of them is sciatica. Sciatica (Gridhrasi) is a word for low back pain that travels down the inside of the leg, through the hip, to the back of the thigh, and down the inside of the leg. In general, 5-10% of people with low back pain have sciatica, but the lifetime prevalence of low back pain is believed to be between 49 and 70 percent. Modern science's treatment of sciatica is unsatisfactory, involving the use of analgesics for pain alleviation and surgical techniques that are frequently associated with side effects. Gridhrasi or sciatica is treated with a variety of methods described in Ayurveda. Kati Basti is a one-of-a-kind therapeutic technique due to its simple, non-invasive, and effective approach. The present review focus of Kati basti, its scientific understanding, its mode of action along its application in sciatica.

ABSTRACT

In present era, attentively engaged in professional and social life, eventually forgotten normal sit and lie postures. Hustles like Jerky movements during travelling, poor postures, bending and lifting awkward etc., creates extreme pressure on the spine which affects functional ability of lower limbs. Hence Gridhrasi is taken for present clinical study and is approximately correlated to sciatica. Sciatica is a set of symptoms including pain that may be caused by general compression or irritation of one of five nerve roots that give rise to the sciatic nerve. This can also happen by compression or irritation of the sciatic nerve itself. The pain is felt in the lower back, buttock, thigh, leg and foot. Basti karma is praised by all the Acharyas in the management of Gridhrasi to relive from the pain, to improve functional disability and restore functional abilities & controls the condition. The study was conducted with the aim of comparing Ashtakatwaratailamatrabasti with Sahacharatailamatrabasti in the management of Gridhrasi. The clinical trial included 40 patients of Gridhrasi categorized into two groups. 20 patients recieved 75 ml of Sahachara Taila Matrabasti and 20 patients recieved 75 ml of Ashtakatwara Taila Matrabastiafter sthanika abhyanga and swedana for 7 days. Both the groups are having similar effect on reducing the symptoms statistically. Among the subjective and objective parameters B group (Ashtakatwara taila matrabasti) showed better reduction% in Ruk, Sthambha, Toda, right lateral flexion, left lateral flexion and extension of lumbar movement, walking time, VAS, Oswesry disability index.

ABSTRACT

Background: Large numbers of population suffer from the low back-pain. Prevalence increases linearly from third decade of life on, until the 60 years of age, being more prevalent in woman. Sciatica most commonly occurs when a herniated disk, bone spur on the spine or narrowing of the spine (spinal stenosis) compresses part of the nerve. It is commonly occurs due to the compression of lumbar nerves L4 or L5 or sacral nerves S1, S2 or S3 or by the compression of the sciatic nerve itself. This causes inflammation, pain and often some numbness in the affected leg. Gridhrasi is the one amongst the 80 types of Nanatmaja disorders of Vatavyadhi, It starts from Kati- prishta (pelvic region and Lumbosacral) radiating to Jangha (thigh) and Pada (feet) with impairment of lifting the leg. The gait of the person is very similar to vulture (Gridhra) hence the name is given as Gridharsi. Material and Method: A case controlled clinical study on a known case of Gridharsi has been done in All India Institute of Ayurveda Hospital. The treatment was planned on the basis of principles of Ayurveda mentioned in the context of Gridharsi including Erand-mooladi basti and oral medications. Result: after the whole course of treatment, patient got 70% relief in all sign and symptoms and changes in gait limping gait to normal gait. Conclusion: It can be concluded that Erand-mooladi basti and oral medications is more effective in Gridhrasi management.

ABSTRACT

In India low back ache is one of the most common emerging diseases which are affecting day to day life of people physically as well as psychologically. Low back ache is the second most common disorder causing disability. Among its various causes the most commonly appearing one is Sciatica. Sciatica is a condition in which pain starts from low back and radiates to leg mostly unilateral but in some, it may be bilateral also. In Ayurvedic perspective its features resemble to those of Gridhrasi. In Ayurvedic classics two types of Gridhrasi have been described namely Vataja and Vata-kaphaja. The case study being presented is of a 44yr old male patient suffering from Vataja gridharsi. Basti karma is the choice of treatment in cases of Gridhrasi where as Snehana/ Snigdha svedana is choice of treatment for any Vataja disorder. So, Panchakarma therapy in this case includes Patrapindasveda, Sarvang vashpasveda and Basti therapy along with oral medication assessed on the basis of Oswestry disability index. The results indicate that the patient improved in presenting features up to 68.75% as well as day to day activities were also performed effectively by him.

ABSTRACT

Gridhrasi as mentioned in Ayurveda explains a disorder where in pain starts from Sphik pradesha and radiates downwards to Kati, Prusta, Uru, Janu, Jangha and Pada, Due to this, the patient is unable to walk properly. Gridhrasi can be compared with Sciatica. Sciatica is a relatively common condition with a life time incidence varying from 13% to 40%. The corresponding annual incidence of an episode of Sciatica ranges from 1 to 5%. The incidence of Sciatica is related to age. It is rarely seen before the age of 20. Incidence peaks in fifth decade declines thereafter. Snehana and Swedana are considered as a general line of treatment for Vata Vyadhi which can be taken as a line of treatment for Gridhrasi also. Patra pinda Sweda (Vatashamak) and Nadi sweda (Vata Kapha shamak) is one such treatment modality comes under Snehana and Swedana is known for relieving the symptoms. Murchita Tila Taila due to its Vata Shamaka has been taken for the Sthanik abhyanga. 30 patients of Gridhrasi were randomly selected and divided into 2 groups. Group A: 15 patients received Patra pinda sweda and Group B: 15 patients received Nadi sweda up to Samyak Swinna lakshana. Overall assessment of both groups, out of 30 patients, 2 (6.67%) got complete remission, 13 (43.34%) patients got marked response, 6 (20%) patients got moderate response and 9 patients got mild response.

ABSTRACT

Introduction: Gridhrasi is a condition characterized by Ruk, Toda, Stambha, Spandana in Sphik pradesha and radiates downwards to Kati, Prusta, Uru, Janu, Jangha and Pada. Gridhrasi can be compared with Sciatica. Siravyadhais the major line of treatment mentioned in Ayurveda classical texts. Case study: A female patient aged 35 years; presenting with cardinal clinical signs and symptoms of Gridhrasi visited OPD with history of eight years and worsen in last two days. She was examined thoroughly and detailed history of illness was recorded. She was treated with Siravyadha atantara kandara gulpha sandhi by following proper Purva, Pradhana and Paschyat karma. Observation and Result: patient got relief in subjective parameters i.e. Ruk and Stambha. There was marked improvement in SLR test, mild improvement in Rt. Lateral Flexion and Backward extension of lumbar spine Movement. Forward Flexion and Lt. Lateral flexion of lumbar spine Movement remained unchanged. Conclusion: Siravyadha is administered in Tridoshadushti and Sarvangagatadushti. In Ghridrasi, Rakta and Kandara are Dushya and Vyana vata is major Dosha. Siravyadha corrects these imbalances by letting out the vitiated blood. The procedure was simple, economical and can be done in OPD level. It gives immediate relief of pain and stiffness.
