ABSTRACT

Sobreviventes ao suicídio são pessoas que têm suas vidas profundamente afetadas e apresentam sofrimento psicológico, físico ou social após serem expostas a esse fato. O objetivo deste estudo foi analisar a experiência de sobreviventes ao suicídio de jovens, a partir do luto. Participaram sete sobreviventes entre familiares, amigos e parceiros amorosos de jovens que cometeram suicídio. A análise de conteúdo de entrevistas narrativas apontou que os participantes utilizam explicações racionalizadas ou dissociadas, criando uma distância entre o evento e eles mesmos. Como formas de lidar com o sofrimento podem buscar o isolamento, apoio entre amigos, prática religiosa e/ou a dedicação ao trabalho. Reafirma-se a dimensão do luto diante dessa experiência, além da importância da prevenção ao suicídio e da posvenção aos sobreviventes.(AU)


Suicide survivors are people who have their lives deeply affected; they experience psychological, physical, and social suffering following the occurrence. The aim of this study is to analyze the experience of survivors of youth suicide attempts, based on grief. Seven survivors participated among family, friends, and romantic partners of young people who committed suicide. The content analysis of narrative interviews showed that the participants use rationalized or dissociated explanations, creating a distance between the event and themselves. As ways to deal with suffering, they seek isolation, support among friends, religious practice, and/or dedication to work. The dimension of grief in the face of this experience is reaffirmed, as well as the importance of suicide prevention and postvention for survivors.(AU)


Este artículo tiene como objetivo presentar la construcción metodológica desarrollada en una investigación de maestría, en la que sostenemos la escritura de escenas como método de investigación de la escucha clínica. Las escenas del trabajo en cuestión se recogieron a lo largo del tiempo desde la experiencia en un proyecto de extensión universitario de atención a la niñez y adolescencia en situación de vulnerabilidad social aplicado en una comunidad periférica. En este texto, presentamos los interrogantes que se elaboraron en torno a la elección por el trabajo con escenas y compartimos el rescate histórico de las mismas como un método de escribir la clínica y la reanudación del análisis a partir de la tradición psicoanalítica. Amparadas en el psicoanálisis y en lecturas y contribuciones del filósofo francés Jacques Derrida, nos basaremos en la noción de que la escena se constituye como un lugar de producción, engendrando la configuración particular de elementos significantes en los procesos de subjetivación y de construcción social. La escena no es aquí una representación de lo que pasa en la clínica, sino un modo de producir escucha y sus procesos de investigación.(AU)

ABSTRACT

Resumen La reflexión se plantea desde las formas de tramitación del sufrimiento social producto de la violencia política en Colombia, recogiendo las experiencias locales y comunitarias. Se desprende desde el análisis de contenido realizado a partir de tres tesis de pregrado en el área de la Psicología Social. Se plantean categorias: elaboraciones simbólicas, memoria o liderazgo social, como formas de tramitación y resistencias comunitarias. Las comunidades del dolor se erigen como comunidades de esperanza y apoyo mutuo. Se reafirma el compromiso etico-político de las Ciencias Sociales, frente a la construcción de conocimiento desde las voces y comunidades dolientes, reconociendo saberes otros.


Abstract: The reflection is based on the ways of processing the social suffering resulting from political violence in Colombia, by gathering local and community experiences. It is derived from the content analysis, which is carried out from three undergraduate theses in the area of Social Psychology. The following categories are proposed: symbolic elaborations, memory, or social leadership, as forms of processing and community resistance. The communities of pain are erected as communities of hope and mutual support. The ethical-political commitment of the Social Sciences is reaffirmed, by facing the construction of knowledge from the voices and suffering communities, by recognizing other areas of knowledge.

ABSTRACT

Abstract Communicating bad news is one of the most frequent activities in hospitals, for which some recommendations have been adapted to the needs within the coronavirus-2 disease (COVID-19) context. This document presents nine steps to deliver bad news (face to face or remotely) adapted to the COVID-19 context from two international protocols (SPIKES and GRIEV_ING). The importance of promoting physical and emotional self-care skills in health personnel is also described, as well as psychological first aid strategies to address the emotional response of the family member who receives the news. Finally, the limitations and advantages of the proposal should be considered.


Resumen La comunicación de malas noticias es una de las actividades más frecuentes en los hospitales dentro del contexto de la COVID-19. A pesar de su alta frecuencia, existen pocas recomendaciones adaptadas a las necesidades que el contexto de la COVID-19 demanda. Debido a lo anterior, en el presente escrito se presentan nueve pasos para dar malas noticias (cara a cara o por vía remota) de dos protocolos internacionales (SPIKES y GRIEV_ING) adaptados a las necesidades de transmisión de información de la COVID-19. Se describe también la importancia de promover habilidades de autocuidado físico y emocional en el personal de salud, así como estrategias de primeros auxilios psicológicos para el abordaje de la respuesta emocional del familiar que recibe la noticia. Finalmente, se deben considerar las limitaciones y ventajas de la propuesta.

ABSTRACT

El proceso de duelo en la etapa de fin de vida del paciente en situación oncológica al que se enfrenta la enfermera está dado por varias etapas que requieren exploración con el propósito de comprender mejor la experiencia desde la perspectiva profesional. Objetivo: Describir la vivencia del profesional de enfermería en el proceso de duelo en la muerte de personas en situación oncológica. Metodología: estudio de tipo cualitativo exploratorio empleando la teoría fundamentada y utilizando la técnica de análisis de Glaser y Strauss, con lo cual se genera un planteamiento teórico. Se realizaron 7 entrevistas a profundidad a enfermeras que tuvieron experiencias significativas en la muerte de pacientes en situación oncológica. Resultados: En este estudio surgieron 5 categorías a saber reconociendo mi yo y mi realidad, sintiéndose derrotado, actuando para el buen morir, aprendiendo de las pérdidas y trascendiendo a partir de las múltiples pérdidas. Estas categorías ayudaron al surgimiento de la teoría "Resignificando la muerte y el duelo para empoderar mi "Yo" enfermera". Conclusiones: El planteamiento teórico se entiende como la transformación del significado de fin de vida desde la concepción dolorosa y de que no hay nada más que ofrecer, por un pensamiento proactivo donde se puede ampliar el concepto de cuidado a lo espiritual, social y cultural y no solamente al físico.


The grieving process as the end of life in patients with an oncological situation that a nurse has to face is given in various stages that require exploration in order to better understand the experience from a professional perspective. Objective: Describe the life experience of the nursery staff members in the grieving process in people with an oncological situation. Methodology: is an exploratory qualitative study which applies the grounded theory and uses the Glaser and Strauss Analysis aimed to generate a theoretical approach. Seven in-depth interviews were conducted to nurses who had meaningful experiences in the death of patients with an oncological situation. Results: Five categories emerged from this study: Recognizing myself and my reality, feeling defeated, acting for the dying well, learning from loses and transcending from the multiple losses of life. These categories help to the resurgence of the Re-signifying Theory in the nurse experience in the grieving processes in patients with an oncological situation Conclusions: The theoretical approach it refers to the transformation in the meaning of someone's end of life. It can change from the painful assumption and the idea that there is nothing more to offer to a proactive thinking where it is possible to broaden the concept of care in the spiritual, social and cultural aspects and not only the physical one.

ABSTRACT

O objetivo deste estudo foi compreender como o processo de luto antecipatório se instala em pacientes a serem submetidos ao Transplante de Células Tronco-hematopéticas (TCTH). Foi realizado um estudo transversal, na abordagem clínico-qualitativa. Amostra de 17 pacientes, a maioria mulheres casadas, entre 20 e 42 anos de idade e diagnóstico de Leucemia Mieloide Crônica. Dados coletados por um roteiro de entrevista semiestruturada, aplicado individualmente e submetidos à análise de conteúdo temática. Os resultados indicam que a perda da saúde impõe um luto em uma história de vida já permeada por privações e perdas prematuras. Constatou-se que as reações esperadas frente ao luto normal foram expressas pelos participantes e que a estratégia de enfrentamento privilegiada foi o apego à fé. Os planos futuros envolviam alcançar a cura, retorno à normalidade e reinserção profissional. Os resultados podem auxiliar as equipes multiprofissionais a compreenderem as implicações emocionais do adoecimento/tratamento para implantação de estratégias preventivas e interventivas. Como pontos críticos, a equipe deve atentar para a forma de comunicar o diagnóstico e a possibilidade de traçar um plano terapêutico que mantenha vivo o espírito de luta do paciente, reforçando o elo de confiança com os profissionais de saúde.


The scope of this study was to understand how the process of anticipated grieving is imbued in patients undergoing Hematopoietic Stem Cell Transplantation (HSCT). A cross-sectional clinical-qualitative study was conducted on a sample of 17 patients, mostly women, married, aged between 20 and 42 years and diagnosed with Chronic Myeloid Leukemia. Data was collected by semi-structured interviews applied individually and subjected to thematic content analysis. The results indicate that the loss of health imposes a new challenge in a life history already permeated by great hardships and premature losses. It was found that the expected reactions faced with normal grieving were expressed by the participants and that the most prevalent coping strategy was holding steadfast to their faith. Future plans involved being healed, returning to normality and vocational rehabilitation. The results may help the multidisciplinary teams to understand the emotional implications of the illness/treatment for implementing both preventive and intervention strategies. The critical aspect is that staff must be attentive as to how to communicate the diagnosis and the possibility of outlining a therapeutic plan, in order to augment the fighting spirit of the patient and strengthen the bond of trust with health professionals.

ABSTRACT

This study sought to investigate the significance of death for doctors who work with terminally ill patients in the Bone Marrow Transplant (BMT) context. The research was based on a qualitative, methodological and collective case study approach, and used Dejours' psychodynamics of work as a theoretical framework. Five practitioners linked to a Bone Marrow Transplant Unit in a university hospital in São Paulo, Brazil, participated in the research. A semi-structured script was applied which addressed the personal, academic and professional experiences elicited by a terminal situation. Data were organized into two categories: "Work and professional identity: the doctor's battle against death" and "Vocation and medical training: it all beggins at home." The results show that, in the medical and hospital context, death is almost always reduced to technical rationality and this must be avoided at all costs. The significances attributed to death are rooted in the values of contemporary society and subjectively perceived through the physician's life trajectory and academic training, which influence how these professionals experience the dying process of the patients.


Este estudo teve como objetivo investigar o significado da morte para médicos que atuam com pacientes em condição crítica no contexto do Transplante de Medula Óssea (TMO). A pesquisa foi fundamentada no enfoque metodológico qualitativo e no estudo de caso coletivo, e utilizou o referencial teórico da psicodinâmica do trabalho. Participaram os cinco médicos vinculados a uma Unidade de TMO de um hospital universitário do interior paulista. Aplicou-se um roteiro semiestruturado, que abordava as vivências pessoais, acadêmicas e profissionais suscitadas pela situação de terminalidade. Os dados foram sistematizados nas categorias: "Trabalho e identidade profissional: a peleja do médico contra a morte" e "Vocação e formação médica: tudo começa em casa". Os resultados mostram que, no âmbito médico-hospitalar, a morte é quase sempre reduzida à racionalidade tecnicista e por isso deve ser combatida a qualquer custo. As significações atribuídas à morte são ancoradas nos valores da sociedade contemporânea e subjetivadas nos percursos e percalços enfrentados pelo médico ao longo de sua história de vida e na formação acadêmica, que influenciam o modo como os profissionais vivenciam o processo do morrer dos pacientes.

ABSTRACT

O presente estudo teve por objetivo conhecer as vivências de luto de mulheres que interromperam a gestação sob autorização judicial, devido à malformação fetal incompatível com a vida. Participaram do estudo dez mulheres atendidas no Setor de Medicina Fetal do Hospital das Clínicas de Botucatu. Para coleta dos dados foi realizada entrevista semiestruturada quarenta dias após a interrupção. As entrevistas foram audiogravadas, transcritas na íntegra e tiveram os dados analisados na perspectiva da análise temática de conteúdo. Os resultados revelaram que as mães buscaram explicações e significados para a perda, sendo muito frequentes respostas religiosas e autoculpabilizantes. Os relatos marcaram sentimentos de tristeza, saudade e sensação de vazio pela perda do filho, revelando também a necessidade das mães de manterem-se ligadas a ele. As mães estavam e continuaram vinculadas aos seus filhos e a interrupção da gestação, embora tenha sido uma escolha para minimizar a dor de uma perda inevitável, não as poupou de vivências de grande sofrimento. O estudo traz subsídios para a discussão e planejamento de abordagens e cuidados com a saúde de mulheres que interrompem a gestação por malformação fetal letal, mediante autorização judicial.


The scope of this study was to investigate the grieving experiences of women who terminated pregnancies under judicial authorization, due to life-incompatible fetal malformation. Ten women attended in the Fetal Medicine Department of Botucatu Clinical Hospital participated in the study. Data collection was conducted by means of semi-structured interviews forty days after termination. The interviews were recorded and transcribed in full, with the data analyzed from the thematic content analysis perspective. The results revealed that the mothers sought explanations and meanings for the loss, with religious responses and self-blame being very frequent. The reports were marked by feelings of sadness, longing and sensations of emptiness due to the loss of the child, revealing the need of the mothers to dwell on the issue. The mothers were and continued to be linked to their children; the termination of the pregnancy, although being a choice to minimize the pain of an inevitable loss, did not spare the women from experiences of great suffering.>The study includes input for the discussion and planning of health approaches and care for women who terminate their pregnancy due to lethal fetal malformation, by means of judicial authorization.
