ABSTRACT

This study aimed to investigate the diagnostic value of heparin-binding protein (HBP) in the cerebrospinal fluid of children with purulent meningitis (PM). This study included 118 children with PM diagnosed at our hospital from January 2018 to January 2020, 110 children with viral meningitis (VM) and 80 children with suspected meningitis who were ruled out by cerebrospinal fluid (CSF) analysis during the same period. HBP and white blood cell (WBC) count in the CSF, and inflammatory factors, including C-reactive protein (CRP), tumor necrosis factor (TNF)-α, and procalcitonin (PCT), were measured. Receiver-operator characteristic curves were used to analyze the predictive value of HBP, CRP, PCT, and TNF-α levels in the diagnosis of PM by CSF analysis. HBP levels in the CSF of children with PM were higher, while the CRP and serum PCT and TNF-α levels were elevated in all groups (P<0.05). In addition, HBP levels in the CSF were more accurate for the diagnosis of PM than traditional diagnostic indexes. HBP levels in the CSF can be used as an important reference for early diagnosis of PM.

ABSTRACT

Resumen: La monitorización ambulatoria de la presión arterial (MAPA) es una herramienta clínica útil para el diagnóstico y confirmación de hipertensión arterial en pediatría y permite igualmente el diagnóstico de condiciones especiales como la hipertensión de delantal blanco e hipertensión enmascarada. Exis ten recomendaciones internacionales para su realización e interpretación, sin embargo, aún quedan interrogantes por resolver. En esta guía se resume la bibliografía disponible y se intenta estandarizar, a través de consenso de especialistas nacionales, la aplicación de esta técnica. Se necesitan más estudios de investigación en niños que aporten nuevos valores de referencia y que determinen la relación de alteraciones en MAPA con resultados clínicos a largo plazo.


Abstract: Ambulatory blood pressure monitoring (ABPM) is a useful clinical tool for the diagnosis and confir mation of arterial hypertension in pediatrics, and also allows the diagnosis of special conditions such as white coat hypertension and masked hypertension. There are international recommendations for its implementation and interpretation, however, there are still unresolved questions. This guide summarizes the available literature and attempts to standardize, through consensus of national specia lists, the application of this technique. More research studies are needed that provide new reference values and determine the relationship of alterations in ABPM with long-term clinical results.

ABSTRACT

Objective To identify specific peptides binding to hepatitis B virus ( HBV) DNA pol-ymerase TP region ( HBP-TP) and to study their effects on HBV replication. -ethods The phage polypep-tide display library was screened using the HBP-TP recombinant protein expressed in Escherichia coli as a substrate to obtain the specific phage, the polypeptide region of which was then sequenced. Polypeptide se-quences with a repetition rate greater than 10% were counted and commercially synthesized. Changes in HBV replication were detected after treatment of HBV-expressing cells with synthetic peptides as drugs. Re-sults Eight peptides targeting HBP-TP recombinant protein were screened out from the phage display librar-y. One of the peptides was found to have a significant inhibitory effect on HBV replication at cellular level. Conclusions A HBP-TP protein-targeting polypeptide with an inhibitory effect on HBV replication was ob-tained.

ABSTRACT

The aim of this study was to investigate the effect and mechanisms of miR-29a in migration and inva-sion of human breast cancer MCF-7 cells in vitro. MCF-7 cells were treated with miR-29a mimic or miR-29a inhibitor to up-regulate/down-regulate the expression level of miR-29a. Wound-healing assay and transwell chamber were employed to determine cell migration and invasion in vitro. The target gene of miR-29a was predic-ted with the Targetscan7. 1 database and verified through luciferase reporter method. The effects of miR-29a on the expression of the potential target were detected by Western blot and real-time PCR. Results showed that in vitro migration and invasion ability of MCF-7 cells was increased significantly by miR-29a,which could target HBP1 in the 3′-UTR region. The protein expression of HBP1 was decreased by miR-29a overexpression. However, the alteration of miR-29a had no significant effect on the expression of HBP1 mRNA. The results validated that miR-29a,highly expressed in breast cancer,could down-regulate HBP1 ,which in turn promotes migration and invasion ability of breast cancer cells,thus promoting breast cancer metastasis.

@#<p style="text-align: justify;"><strong>BACKGROUND:</strong> The burden of sepsis is global despite measures to improve its prompt recognition. However, there is no single reliable parameter for its early detection. Heparin-binding protein (HBP) is a new and promising biomarker for sepsis. Presently, there are no published reports in children apart from a limited study on UTI.</p><p style="text-align: justify;"><strong>OBJECTIVE:</strong> To evaluate the role of HBP as a diagnostic tool and prognostic marker of sepsis syndrome among pediatric patients.</p><p style="text-align: justify;"><strong>METHODS:</strong> This prospective cohort study enrolled pediatric patients who were categorized as SIRS or sepsis syndrome. HBP assay was determined on Day1. Likewise, blood culture was taken. A 7-day observation period using PELOD scoring was done. Final category as SIRS or sepsis syndrome was done on Day7. Statistical analysis was done to know relationship of HBP level to SIRS and sepsis.</p><p style="text-align: justify;"><strong>RESULTS:</strong> 106 patients were included in this study. There was statistical significance in the correlation of HBP assay with presence of growth in blood culture and toxic granulations, length of ventilator support, and development of complications including mortality. The cutoff point was >125ng/mL. Sensitivity and specificity for HBP in sepsis syndrome were 98.31% and 97.87% respectively. Positive predictive value was 98.3%. Negative predictive value was 97.9%. Positive likelihood ratio was 46.2. Negative likelihood ratio was 0.017. Risk ratio was 47.6. Subjects with HBP level of >125 ng/mL had 47.6 times the risk of having sepsis syndrome as compared to those with level</p><p style="text-align: justify;"><strong>CONCLUSION & RECOMMENDATIONS</strong>: Elevated HBP level is a useful diagnostic and prognostic marker for childhood sepsis syndrome. Determination of HBP levels at different time intervals within a longer observation period may give a more accurate description of subject's clinical improvement or progression to MODS or mortality.</p>

ABSTRACT

Aging is an intricate process modulated by different molecular and cellular events, such as genome instability, epigenetic and transcriptional changes, molecular damage, cell death and senescence, inflammation, and metabolic dysfunction. Particularly, protein quality control (chaperone systems) tends to be negatively affected by aging, thus leading to cellular senescence in metabolic tissues and, as a consequence, to the increasing dissemination of inflammation throughout the body. The heat shock (HS) response and its associated expression of the 70 kDa family of heat shock proteins (HSP70),which are anti-inflammatory molecular chaperones, are found to be markedly decreased during muscle inactivity and aging, while evidence supports the loss of HSP70 as a key mechanism which may drive muscle atrophy, contractile dysfunction, and reduced regenerative capacity. In addition, abnormal stress response is linked with higher incidence of neurodegenerative diseases as well as low-grade inflammatory diseases that are associated with physical inactivity and obesity. Therefore, strategies to increase or, at least, to maintain the levels of HSP70, and its accompanying HS response to stress, are key to reduce biological cell dysfunctions that occur in aging. In this sense, physical exercise is of note as it is the most powerful inducer of the HS response, comparable only to heat stress and fever-like conditions. On the other hand, the amino acidL-glutamine, whose production within the skeletal muscle and liberation into the bloodstream is dependent on muscle activity, is a potentializer of HSP70 expression and HS response, particularly via its entering in hexosamine biosynthetic pathway (HBP). Herein, we discuss the collaborative role of glutamine (and its donors/precursors) and physical exercise (mostly responsible for glutamine release into the circulation) as potential tools to increase HSP70 expression and the HS response in the elderly.

Objective To investigate the expression and clinicalsignificance of neutrophils heparin binding protein (HBP) and CD64 in the chronic obstructive pulmonary disease to explore the clinical value. Methods From March 2015 to December 2015 , 197 patients with chronic obstructive pulmonary disease (COPD), 223 patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and 265 healthy persons were recruited in this study. 223 patients with AECOPD were divided into two groups based on bacteria concentration in lower respiratory. Infection group was defined as patients with bacteria concentration exceeded 107 cfu/mL group. The rest of patients were in the non-bacterial infection group. Flow cytometry was used to analyzeexpression of CD64 and convert to index of CD64. Enzyme-linked immunosorbent assay (ELISA) was used to analyze expression of HBP. PCT, hs-CRP and WBC were measured at the same time. Results CD64 , HBP , CRP , WBC and PCT were highly expressed inthe acute exacerbation of chronic obstructive pulmonary patients than those in the chronic obstructive pulmonary patients and healthy group (P<0.01). CD64, HBP , CRP , WBC and PCT were highly expressed inthe acute exacerbation of chronic obstructive pulmonary bacterial infection group patients before treatment than thenon-bacterial infection group (P<0.01). HBP, CD64, CRP , WBC and PCT decline dramatically inthe AECOPDbacterial infection group patients after treatment , which were close to the levels in non-bacterial infection group.When the intercept value of CD64 index was defined as 4.0, the sensitivity to diagnose AECOPD bacterial infection was 92.2% with specificity of 86.2%. When the intercept value of HBP index was defined as 49 ng/mL , the sensitivity to diagnose AECOPD bacterial infection was 87.3% with specificity of 92.3%. Therefore, 4.0 of CD64 index and 49ng/ml of HBP index could be used as sensitive indicator of AECOPD detectedby bacterial infections. Conclusion HBP,CD64 were highly expressed in the acute exacerbation of chronic obstructive pulmonary patients than the those of chronic obstructive pulmonary patients and healthygroup, and decline dramatically after treatment. HBP and CD64 has high sensitivity and specificity to diagnose AECOPD bacterial infection. HBP and CD64 could be used as the indicators of auxiliary diagnosis of acute exacerbation of chronic obstructive pulmonary disease and to guide the clinical medication.

ABSTRACT

Objective To explore the clinical value of the different components of glycosylated hemoglobin in pa-tients with coronary artery disease (CAD). Methods A total of 217 patients were divided into 3 groups:CAD group (groupⅠ, n=60), CAD patients without acute coronary syndrome (ACS) and with diabetes mellitus group (groupⅡ, n=60) and ACS patients with diabetes mellitus group (groupⅢ, n=97). Fifty-eight healthy volunteers in the same time period were selected as control group. The values of fructose glycosylated hemoglobin (HbA1a), lactose glycosylated hemoglobin (HbA1b), glucose glycosylated hemoglobin (HbA1c), hemoglobin P3 component (HbP3), hemoglobin A0 component (HbA0), unstable glycosyl-ated hemoglobin (LA1c/CHb1) and alkali-resistant hemoglobin (HbF) were measured. These parameters were compared be-tween 4 groups. Logistic regression was used to analyze factors that influencing CAD and CAD with diabetes mellitus. The re-ceiver operating characteristic (ROC) curve was used to analyze the diagnostic efficiency of these factors. Results There were significant differences in titers of HbA1b, HbA1c, HbP3 and HbA0 between groupⅠ, groupⅡ, groupⅢand control group (P<0.01, or P<0.05). There were no significant differences in levels of glycosylated hemoglobin parameters between groupⅡand groupⅢ(P>0.05). Logistic regression and ROC curve analysis showed that HbA1c and HbP3 were indepen-dent effects of CAD, and there were some diagnostic efficiency of CAD. The diagnostic efficiency of ROC curve was consis-tent in HbA1c and HbP3 between group Ⅱ and group Ⅲ. Conclusion Levels of HbA1b, HbA1c, HbP3 and HbA0 are closely related to CAD and CAD with diabetes mellitus. HbA1c and HbP3 are independent effects of CAD and, there are some diagnostic efficiency in CAD.

É possível que a expressividade de alguns elementos do plasma seminal dos bovinos, como proteínas e hormônios, possa servir como marcadores para sêmen de alta ou baixa fertilidade. Vários estudos têm demonstrado a associação de proteínas do plasma seminal com a fertilidade de touros. Dentre as mais estudadas, destacam-se aquelas com afinidade à heparina, que exercem importantes papéis na capacitação espermática e na reação acrossômica. Alguns fatores endócrinos e/ou locais, podem estar associados à expressividade e/ou função destas proteínas, auxiliando nas condições espermáticas favoráveis à fecundação. Dentre estes, destacam-se a insulina, a leptina e o fator de crescimento semelhante à insulina do tipo I. Assim sendo, evidenciam diferenças entre animais, estando associados à estrutura e as condições metabólicas da célula espermática, auxiliando na determinação da qualidade do plasma seminal. Desta maneira, o estudo das proteínas do plasma seminal asociado à condição metabólica destes hormônios, presentes neste meio, pode servir como importante parâmetro de avaliação da condição reprodutiva do macho


It is possible that the expression of some elements in seminal plasma, such as proteins and hormones, may act as markers of quality. Several studies have demonstrated an association of seminal plasma proteins with fertility. Among the most studied, there are the proteins with affinity to heparin, which play an important role in sperm capacitation and acrosome reaction. Some endocrine factors and/or locals may be associated with the expression and function of these proteins, aiding in sperm condition conducive to fertilization. Among them stand out as insulin, leptin and growth factor insulin-like type I. Thesefactors may highlight the differences between these animals because they are associated with the metabolic condition and structure of sperm cells, aiding in determining the quality of seminal plasma. Thus, the study of seminal plasma proteins associated with the metabolic condition of these hormones are present in this medium can serve as a new parameter for assessment of male reproductive condition...

Se realizó una revisión del tema relacionado con la hipertensión arterial en el paciente diabético tipo 2, con el objetivo de actualizar algunos elementos particulares en este grupo de pacientes. Se plantea que su prevalencia es considerablemente más alta que en la población no diabética, donde la hiperinsulinemia y la disfunción endotelial cobran un papel protagónico en su etiopatogenia. Authors made a review of the topic related to arterial high blood pressure in the patient presenting with Type 2 diabetes mellitus to update some particular elements in this group of patients. We propose that its prevalence is higher than that present in non-diabetic population, where the hyperinsulinism and the endothelial dysfunction have a leading role in its ethiopathogenesis. The arterial high blood pressure in these patients clinically is manifested as a non-dipper pattern increasing the morbidity and mortality. Drug therapy is commonly necessary and as first line-drugs are used the converting-enzyme inhibitors, the agiotensin-II receptors antagonists, and the antagonists-calcium, although the low-dose thiacidic diuretics...

ABSTRACT

O objetivo deste trabalho foi avaliar a utilização de fitoterápicos a base de saw palmetto na terapia sintomática da hiperplasia benigna da próstata (HBP) por médicos urologistas da cidade de Porto Alegre. Consistiu em um estudo transversal, exploratório, por meio de entrevista semi-estruturada aplicada a urologistas de Porto Alegre. A amostra foi obtida utilizando catálogo do plano de saúde UNIMED- Porto Alegre. A randomização foi realizada através de sistemática aleatória, sendo sorteados trinta e cinco médicos, dos quais 21 foram selecionados para realização da pesquisa através de questionário. Todos os urologistas entrevistados avaliam e tratam pacientes com hiperplasia benigna da próstata. O saw palmetto não foi citado como terapia medicamentosa de 1ª ou 2ª escolhas no tratamento da HBP. O grupo farmacológico mais freqüentemente utilizado para o tratamento da HBP foi ∝-bloqueadores. Mais da metade dos médicos entrevistados relata ter conhecimento sobre a utilização do saw palmetto, principalmente através de artigos científicos. Os resultados indicam que o saw palmetto não é prescrito pelos urologistas em Porto Alegre, todavia a maior parte destes profissionais tem conhecimento sobre sua utilização.


The aim of this work was to assess the prescription of saw palmetto phytomedicines for improving symptoms of benign prostatic hyperplasia (BPH) by urologists in Porto Alegre (Brazil). The study was transversal and exploratory consisting of a semi-structured interview with urologists. The sample was obtained from the UNIMED Catalog - Porto Alegre (Brazil). The randomization was made by chance, being selected thirty-five physicians. The first twenty-one who accepted to participate were interviewed. All the interviewed urologists treat patients with benign prostatic hyperplasia. The saw palmetto was not mentioned as the first or second choice for treating BPH. The α-blockers were the most mentioned pharmacological group. However, more than half of urologists have heard about the use of saw palmetto and most of them gathered the information in scientific papers. The results indicate that saw palmetto is not prescribed for urologists in Porto Alegre, although most of them have knowledge about this phytomedicine.

ABSTRACT

Las semillas de calabaza (Cucurbita pepo L.) son utilizadas para la fitoterapia de la hiperplasia benigna de próstata (HBP) en forma de extractos estandarizados con ácidos grasos. Se realizó el estudio fitoquímico preliminar mediante cromatografía gaseosa de la composición de los ácidos grasos presentes en el aceite obtenido de las semillas de Cucurbita pepo L. cultivada en la Estación Experimental de Plantas Medicinales “Dr. Juan Tomás Roig”, ubicada en la provincia de La Habana, Cuba. Se evidenció la presencia y concentración para cada uno de los ácidos grasos determinados, con valores de 32 % de palmítico, 2,2 % de esteárico, 18 % de oleico y 47,7 % de linoleico.


The pumpkin seeds (Cucurbita pepo L.) are used for the phytotherapy of benign prostatic hyperplasia (BPH) as standardized extracts with fatty acids. A preliminary phytochemical study was undertaken by gas chromatography of the composition of the fatty acids present in the oil obtained from Cucurbita pepo L. seeds cultivated in “Dr. Juan Tomas Roig” Experimental Station of Medicinal Plants, located in Havana Province, Cuba. The presence and concentration for each fatty acid determined were evidenced, with values of 32 % of palmitic acid, 2.2 % of stearic acid, 18 % of oleic acid, and 47.7 % of linoleic acid.

La hipertensión arterial complica 6-8% de todos los embarazos, es una de las causas principales de morbi-mortalidad materna y fetal siendo especialmente fatal en países con bajo nivel socioeconómico. La importancia de su estudio radica en que una atención prenatal adecuada previene o atenúa las alteraciones que provoca. Con el presente trabajo se intento buscar una relación PRONOSTICA entre parámetros laboratoriales tales como: perfil lipidico, ácido úrico, recuento plaquetario en pacientes preeclampticas, la correlación con la severidad del cuadro y los resultados perinatales obtenidos. Para tal efecto se realizó un estudio descriptivo, transversal y prospectivo donde 57 pacientes cumplieron los criterios de inclusión. No se observaron alteraciones en las lipoproteínas de alta y baja densidad, así como del colesterol total, pero se observo que un 35 % de las pacientes presentaron niveles de triglicéridos alterados, de ellas la mayor parte con diagnostico de preeclampsia severa.


The high blood pressure complícate 6-8% of all pregnancy. It is a major cause of mother and fetal morbi mortality, being specially fatal in low developed countries. It is important study because a adecuated pre partum care can prevent or at least attenuate its ateration. In the present work we search a pronostic relationship between laboratorial parameters such as : lipidic profile, uric acid, plateleds count in pre eclamptic patients, the correlation between severity and perinatal outcomes. In order to these we give an descriptive, transversal and prospective study, in which were included 57 patients that accomplished the inclusión criteria. It was not observed alterations on high and low density lipoproteins, neither total cholesterol, it was observed in 35% of patients triglycerides levels alterations, the most of them have been diagnosticated with severe preeclampsia.

Extensive hepato-biliary-pancreatic (H-B-P) surgery is still associated with a high postoperative morbidity and mortality than other intraabdominal organ operation. Especially, iatrogenic pseudoaneurysm of splanchnic artery after major H-B-P surgery, although not frequent, can be a devastating and often leads to fatal hemorrhage due to aneurysm rupture. In a series of 300 patients who underwent major hepato-biliary-pancreatic surgery between 1989 and 1995, 6 patients with iatrogenic pseudoaneurysm were reviewed retrospectively. There were 1 hepatic artery pseudoaneurysm after hepatopancreaticoduodenectomy and portal vein resection for Klatskin tumor, 1 proper hepatic artery pseudoaneurysm after extended right hepatectomy and bile duct resection for Klatskin tumor, 1 superior mesenteric artery and common hepatic artery pseudoaneurysm after pancreatoduodenectomy for periampullary cancer, 1 hepatic artery pseudoaneurysm after pancreatoduodenectomy for periampullary cancer,1 pseudoaneurysm of anastomotic site between hepatic artery and splenic artery after type II regional total pancreatectomy for periampullary cancer, and 1 pseudoaneurysm of iliac artery conduit for hepatic artery reconstruction after orthotopic liver transplantation. Five of these 6 pseudoaneurysms were resulted from aggressive radical surgery including skeletonization of hepatoduodenal ligament. Pseudoaneurysm should be highly suspected when continued leukocytosis, fever and gastrointestinal bleeding such as hematemesis or melena are shown. Celiac angiogram should be considered for the early detection of pseudoaneurysm. Angiographic embolization might be considered as a primary treatment, however, if it is not successful, early surgical intervention might be an another life-saving option.
