ABSTRACT

ObjectiveTo evaluate the value of tensionfree herniorrhaphy for inguinal hernia. Methods(1)Comparison was made in operating time, early postoperative response, hospitalization time, postoperative complications and recurrence rate between the two herniorrhaphy groups.In tensionfree herniorrhaphy group( n=137),hernias were repaired with polypropylene mesh plug or mesh patch.(2)In conventional herniorrhaphy group (n=98 ) ,hernias were repaired with Bassini operation. Mean Follow up time was 22 month in the 2 groups. Results The operating time, analgesic use, early response and hospitaliz ation time in tensionfree herniorrhaphy group were significantly lower than those in conventional herniorrhaphy group(P

ABSTRACT

Objective To evaluate the clincal therapeutic efficacy of mesh-plug tension-free hernia repair in elder patients with inguinal hernia.Methods The clinical data of 127 elder patients with inguinal hernia treated with mesh-plug tension-free hernia repair in our hospital were analyzed retrospectively.Results All cases were treated by tension-free hernia repair with mesh-plug case-hardened products which manufactured by American Bard Company.All the patients were cured.Average operation time was 40 min,and average(hospital) stay was 4.6d.The patients were followed up for 6~24 months,only 1 case had recurrence of(hernia)(recurrence rate 0.8%).Conclusions Mesh-plug tension-free hernia repair is a perfectly sound physiologic and anatomic operation.It is a simple,minitraumatic procedure,with advantages of quick (recovery),low recurrence rate and wide indications.It is especially suitable to elder patients or patients with the type II or larger hernial.

ABSTRACT

Objective To evaluate the effect of various herniorraphy in treating inguinal hernia. Methods The clinical data of 108 patients who underwent conventional Bassini herniorraphy (BH), tension free herniorraphy (TH), or laparoscopic herniorraphy (LH),respectively were analysed and comparated. Results The operation time in TH was less than that in BH, and in LH was longer than that in TH. The ability recovery, hospita stay and postoperative complication in TH and LH were less than those in BH.All the patients were followed up for at least 1 year.Of them, 1 recurred after BH, but there was no recurrence after TH and LH. The blood neutrophils, serum level of fibrin, C-reactive protein and alpha-1 antitrypsin increased significantly in the patients undergoing herniorraphy, which were significantly higher in the patients underwent TH and LH. Conclusions The curative effect of TH and LH is much better than that of BH in treating ingumal hernia. Application of marlex for herniorraphy might increase inflammation response, but it will not increase the incidence of infection.

ABSTRACT

Objective To study the effect of appling a new tension-free three dimension with bilayer mesh herniorraphy(TFTDBMH) in treating inguinal hernia in order to decrease the compilcation and the recurrence rates after herniorraphy. Methods The clinical data of 47 male patients with ingular hernia treated by TFTDBMH were analyzed. All the 47 patients were males with an average age of sixty-five years old. There were 36 patients with indirect hernias and 11 direct hernias in this series. During operation, their inguinal regions were extensively dissected and the transverse fascias were separated from peritoneum after the hernia sacs were completely dissected to reach the hernia sac necks. The base layer of the belayed patch mesh was put into the space between the transverse fascia and the peritoneum, then the hernia sac was put into abdominal cavities. The upper layer of the bilyer patch mesh was saw up to the inguinal ligaments and the arcade inferior boarder,then a three - dimension structures were formed at the inguinal regions. Results After operation, all the patients had no any bad reaction. They could be out-of-bed activity on the first day after operation. They could be discharged on the third day after operation.All the patients were followed up for one to two years, these patients felt well,no scar formation on the operative area,and no recurrence postoperatively. Conclusions This operation could be applied for the patients with inguinal hernia and the inguinal posterior walls need to be repaired. This operation fits to the human body anatomy. The postoperative complication and postoperative recurrence rate are low.This operative procedure is more complicated. It request that the operators must have higher operative technique.

ABSTRACT

Objective To evaluate the effect of laparoscopic herniorrhaphy(LHR)for inguinal hernia. Methods From December,1998 to December,2001, 176 patients with inguinal hernia were treated by LHR using laparoscopic purse string to ligate the neck of hernia sac and to place the intraperitoneal mesh.There were 160 patients with indirect inguinal hernia, 15 patients with direct inguinal hernia and 1 with indirect and direct hernia in this series. Results In this series,the mean operation time was 30 minutes, and was no intraoperative or postoperative complications; the hospital stay was 2~3 days. All the patients were followed-up for 10~36 months, only 2 recurrences(1.2%) were found. Conclusions LHR is a new herniorrhaphy which is safe and effective.The advantages of LHR are simple procedure, low complication rate, faster postoperative recovery, low recurrence rate.This operation should be widely adopted.

ABSTRACT

Objective To study the results of an improved method of laparoscopic treatment of inguinal hernia in children. Methods Between January 1999and October 2003,94 inguinal hernias in 86 children between 2~14 years of age were treated by laparoscopic surgery using a single trocar hole. 54 children showed a right inguinal hernia, 32 a left hernia,and 8 cases presented bilateral hernias. Of the unitaleral hernias, 10 were occult hernias. All the patients were treated by purse-string suture around the internal ring under direct vision with laparoscopy. Results Operative time was from 10-30(average 16) minutes. There were no intra-or post-operative complications. All were followed up for 4-60 months, with no recurrences . Conclusions This method of laparoscopic hernia repair is less traumatic, postoperative recovery is fast and stitch removal is not necessary. It can also be used for the treatment of occult and bilateral inguinal hernias.
