ABSTRACT

Objective To investigate the relationship between infection of helicobaeter pylori(Hp) and iron deficiency anemia(IDA) and to explore effective clinical treatments for patients with Hp associated IDA . Method 1. The Hp infection ratio of 40 chronic gastritis with IDA and 42 patients without IDA were counted up respectively. 2. 36 patients with Hp-positive chronic gastritis were divided into two groups randomly. One group was treated by using Hp eradication therapy in conjunction with oral iron supplement and the other using iron supplement only. The hema-tological parameters before and after treatment are measured and the effectiveness of the treatment was evaluated. Re-sult The Hp infection ratio in chronic gastritis patients with IDA is |figher than that in patients without IDA and the difference is significant. After Hp eradication therapy in conjunction with iron supplement Hb. serum iron and serum ferritin level in Hp associated IDA patients are increased significantly while that for patients treated by using iron sup-plement only have no significant improvement. Conclusion It appears that Hp infection may be related to IDA. When iron supplement treatment has no obvious effect to an IDA patient,lt may suggest a ease of Hp associated IDA. The i-ton supplement treatment has positive effects to IDA patients after Hp eradication.

ABSTRACT

Objective To investigate the effect of Helicobacter pylori (H. pylori) eradication on reversing gastric antrum atrophy and intestinal metaplasia (IM). Methods Biopsy specimens obtained from gastric antrum of patients who underwent gastroscopy were examined for pathology and the status of H. pylori infection. The H. pylori infected patients who had chronic gastritis with antrum atrophy or IM were recruited and were divided into either H. pylori eradicated group (n= 48)or control (non-eradicated) group (n=38). The gastroscopic follow-up was carried out at 1 year and 5years. The specimens examined for changes of histology and reverse of gastric antrum and IM were obtained at the same site. Results The reversing rate of gastric antrum atrophy in H. pylori eradicated group was significantly higher than that in control group (37.1% vs. 12.0%). In H. pylori eradicated group, the reversing rate of gastric antrum atrophy at 5-year was significantly higher than that at 1-year, and it was higher in patients aged below 45 years than those aged above 45 years.Whereas there was no association of reversing gastric antrum atrophy with follow-up time and age in control group. However there was no difference in reversing rate of IM between two groups in 1- or 5-year follow-up and H. pylori eradication. Conclusions H. pylori eradication can not reverse the IM of gastric antrum mucosa, but it can reverse the atrophy of gastric mucosa, which is associated with follow-up time and the age of patients. Therefore, H. pylori must he eradicated as early as possible in H. pylori infected patients with gastric antrum atrophy.

ABSTRACT

Objective To investigate the effect of esomeprazole in the treatment of digestive ulcer with heli- eobacter pylori(Hp)infection.Methods A total of 60 patients with digestive ulcer and Hp infection were randomly divided into group A(30 cases)treated with esomeprazole(20mg),amoxicillin(1000mg)and clarithromycin(500 mg)twice daily and group B(30 cases)treated with omeprazole(20mg),amoxicillin(1000mg)and clari thromycin (500mg)twice daily.The treatment lasted for 7 days,after which abdominal pain refief rate,ulcer healing rate and eradication rate of Hp as well as adverse effects of the medication were evaluated and compared.Results Ulcer heal- ing rate and Hp eradication rate were 90.1% and 94,2 % in group A and 83.9 % and 83.7 % in group B,showing significant statistical diferences between the two groups(P
