ABSTRACT

Anemia is a common symptom of haematological disorders in people of all ages .The spectrum of haematological disorders differs significantly between developing and industrialized countries. The primary use of bone marrow aspirate is for cytological examination. It enables study into the cellularity of the bone marrow as a whole, the detection of specific lesions, and the amount of infiltration by various disease entities. The aim of the study is toAims & objective: evaluate the spectrum of haematological disorders reported in bone marrow aspiration and to know the age and sex incidence. This prospective study is an observational study was conducted over a one-yearMaterials and methods: period on 73 patients and the spectrum of hematological disorders was studied on bone marrow aspiration smears. Conclusion: A thorough examination of the bone marrow is essential for diagnosing haematologic disorders. It is a simple and cost effective procedure which can be performed routinely without using any specialized equipment or a need of general anaesthesia

Resumo Introdução O metabólito do benzeno ácido trans, trans-mucônico (t, t-MA) urinário é considerado bom bioindicador de exposição a níveis de benzeno no ar superiores a 0,50 ppm. Objetivo Determinar níveis urinários de ácido trans, trans-mucônico (t, t-MA) e investigar relação com potenciais fontes de exposição ao benzeno e associação com parâmetros hematológicos. Método Estudo seccional, com 150 moradores de Açailândia, Maranhão, Brasil. Utilizou-se questionário estruturado para caracterização sociodemográfica, ocupacional, dentre outras. Coletaram-se amostras de urina e sangue para determinar níveis de t, t-MA e realização de hemograma. As associações foram exploradas mediante regressão logística e linear múltipla. Resultados O t, t-MA foi detectado em 27% dos participantes, sendo a média da concentração 0,15 mg/g de creatinina. As alterações hematológicas mais frequentes foram bastonetes baixos (41%), eosinofilia (33%) e níveis reduzidos de hemoglobina (19%). Não encontrou-se correlação estatisticamente significativa entre t, t-MA e parâmetros do hemograma. Os fatores que influenciaram de forma significativa a excreção de t, t-MA foram cor da pele, prática regular de atividades de lazer com exposição a solventes orgânicos e consumo de refrigerante e refresco nas últimas 24 horas. Conclusão A população parece não estar exposta a concentrações elevadas de benzeno. Entretanto, o perfil hematológico revelou presença de alterações bioquímicas compatíveis com anemia e comprometimento imunológico.


Abstract Background The urine benzene metabolite trans,trans-muconic (t,t-MA) is considered a good bioindicator of exposure to benzene levels in the air above 0.50 ppm. Objective Determine urinary levels of trans, trans-muconic acid (t,t-MA) and investigate the relationship with potential sources of exposure to benzene and its association with hematological parameters. Method A sectional study was carried out with 150 residents of Açailândia, Maranhão, Brazil. A structured questionnaire was used for sociodemographic and occupational characterization, among others. Urine and blood samples were collected to determine t,t-MA levels and blood counts. Associations were explored using multiple linear and logistic regressions. Results The t,t-MA was detected in 27% of the participants, with an average concentration of 0.15 mg/g creatinine. The most frequent hematological changes were low rods (41%), eosinophilia (33%), and reduced hemoglobin levels (19%). No statistically significant correlation was found between t,t-MA and blood count parameters. The factors that significantly influenced the excretion of t, t-MA were skin color, regular practice of leisure activities with exposure to organic solvents, and consumption of soda and soft drinks in the last 24 hours. Conclusion The population does not appear to be exposed to high concentrations of benzene. However, the hematological profile revealed the presence of biochemical changes compatible with anemia and immunological impairment.

ABSTRACT

Homoharringtonine (HHT), a cephalotaxus alkaloid has shown promising results in the treatment of several hematological disorders such as chronic myeloid leukemia, acute myeloid leukemia (AML), and myelodysplastic syndrome. It is known for its unique mechanism of action by which it prevents the initial elongation step of protein biosynthesis. Hence, it is used in hematological malignancies where it synergistically potentiates the action of other drugs and induces apoptosis. The relevant studies published were searched using an electronic database from 2002 to 2019. The articles published in English were only considered. Search engines such as PubMed, MEDLINE, Google Scholar, and Semantic scholar were used. In this review, we have discussed the effect of HHT in combination with other chemotherapeutic agents for AML with or without genetic mutation specification and the future perspective of these regimens. Although standard treatment options exist for most of these diseases, still cure rates are low with reported morbidity and the drug resistance emergence is pervasive. Thus, novel treatment approaches are crucial for better outcome. Alternative regimens together with HHT have not been a standard practice, although they have shown a very good potential in AML patients. Many of the combinations were also proved to be safe and effective with very low toxic potential. All these data outcomes of various combinations under different scenarios exhibit that HHT has promising results in the treatment of AML which may lead to its approval in the upcoming years.

ABSTRACT

Rituximab (anti-CD20 antibody) has been approved as a treatment for B-cell associated hematological disorders. CD20 expression and its complement regulatory proteins and membrane binding structures play a crucial role in rituximab efficacy. Complement-dependent cytotoxicity and antibody-dependent cellular cytotoxicity are the major mechanisms by which rituximab eliminates B cells. The efficacy of anti-CD20 varies in different diseases. Rituximab was approved as a successful treatment in diseases such as non-Hodgkin lymphomas, particularly diffuse large B-cell lymphoma and follicular lymphoma. In addition, rituximab has recently shown promising results with several autoimmune diseases; it was approved for rheumatoid arthritis as well as being used for other diseases such as systemic lupus erythematosus. Likewise, rituximab was successfully used for incompatible ABO organ transplantation instead of the invasive splenectomy procedure. This review will discuss the use of rituximab for different hematological diseases.

ABSTRACT

Background: Bone marrow aspiration and trephine biopsy are two most important techniques which are complementary for diagnosing both neoplastic and non- neoplastic haematological diseases. The present study was conducted in the department of pathology to compare the role of bone marrow aspirate and trephine biopsy for diagnosing the haematological diseases.Methods: The study was conducted in the department of pathology government medical college Srinagar over a period of 2 years from Feb 2016 to Jan 2018. The study was one-year retrospective and one year prospective. Bone marrow aspiration was done in 626 cases out of which biosy was done in 300 cases.Results: Out of the selected 300 cases were both aspiration and trephine biopsy were available, diagnostic material on bone marrow aspirate was adequate in 288 cases. 12 cases were aparticulate. Biopsy was inadequate in 8 cases. Overall megaloblastic anaemia (26.6%) was most common followed by dual deficiency anemia. Multiple myeloma was the most common neoplastic pathology (13%) followed by followed by acute leukaemia’s (6%) and Chronic lymphoproliferative disorders (CLPDS) (5%). 1 case each of visceral leishmaniasis and malarial parasite were also noted. 4 cases (1.3%) of granulomatous pathology and 2 cases (0.6%) of Hodgkins lymphoma were diagnosed exclusively on bone marrow biopsy.Conclusions: Bone marrow aspiration and biopsy are complimentary to each other in diagnosing hematological disorders with biopsy being gold standard in the assessment of cellularity, pattern, extent of tumor infiltration and focal infiltration. It is more helpful in diagnosing granulomatous pathology and metastatic deposits of tumors eliciting fibrotic response.

Introduction: Blood disorders are very common among different age groups. They usually range from anemias to advanced hematological malignancies. Material and Methods: The present observational study was conducted in the Department of Pathology, M.L.B. Medical College, Jhansi to find out the incidence of different hematological disorders. marrow examination was done. SPSS software was used for data analysis. Results: Hematological disorders were more common among males (63.55%) and among those aged Bone between 21-30 yrs (27.1%). Anemia was the most common diagnosis in 49 cases (41.52%) followed by leukemia in 16 cases (13.56%). Thirteen cases (11.02%) were diagnosed as malarial parasite positive in bone marrow examination. Conclusion: Bone marrow examination is a useful test in reaching the final diagnosis.

ABSTRACT

Modern medical oncology has introduced various anti-cancer drugs since the World War I and II. Unlike for the solid tumors, hematological malignancies had been documented some limitations for curing it with chemotherapeutic agents only. In 1960, Dr. Nowell and Dr. Hungerford had discovered elongated chromosome (Philadelphia chromosome) which has documented as a product of translocation between 9th and 22nd chromosome in the patients with chronic myeloid leukemia. In 1970s, immunochemistry technique using monoclonal antibody has spread world widely and from 1990s, flow cytometry method has been available. In appreciation of these evolutions in basic science, the treatment strategy ofhematological malignancies has changed from the chemotherapeutic agents to targeted agents. Among the targeted agents, some drugs are newly developed and others are recreated as anti-cancer drugs after long-time of discard because of their toxicities or teratogenic effects. Nowadays, we are in the middle of flood of targeted agents, for example, tyrosinekinase inhibitors, epidermal growth factor receptor blockers, farnesyl transferase inhibitors, histone deacetylase inhibitors, and etc. In 21st century, the optimal treatment of hematological malignancies should follow a tailor- made strategy according to the patient and disease itself. In the present article, some representative agents will be introduced in accordance with target diseases.
