ABSTRACT

Objective:To explore the application effect of problem-based learning (PBL) combined with evidence-based medicine (EBM) teaching in clinical practice of hematology department.Methods:The 68 medical students practicing in the hematology department from July 15 to November 8, 2019 were set as the experimental group, and 65 medical students from December 29, 2018 to June 28, 2019 were set as the control group. Patients with pancytopenia were selected in the first week of admission for PBL combined with EBM teaching practice in the experimental group, but in the control group, these students did not take this practice. Theoretical knowledge, case analysis and assessment, were conducted before the students leaving the department. SPSS 22.0 was used for t test. Results:Compared with the control group, the scores of theoretical knowledge and case analysis of medical students in the experimental group were improved to a certain extent, with statistical significance ( P = 0.001, P < 0.001). And the higher the score of teaching practice, the higher the score of theoretical knowledge and case analysis ( P < 0.001). Conclusion:PBL combined with EBM teaching practice can effectively improve students' learning enthusiasm and initiative, also can enhance their clinical thinking ability, then can help improve the teaching quality of the hematology.

ABSTRACT

Objective@#To analyze the status quo and influencing factors of psychological collectivism of nurses in hematology department of tertiary care hospital, so as to provide guidance for improving the cohesion of nurses in hematology department.@*Methods@#A total of 249 hematological nurses from 6 tertiary care hospitals in Wuhan and Jingzhou were selected by convenience sampling. The basic information and psychological collectivism scale (PCS) were used to investigate psychological collectivism.@*Results@#The score of psychological collectivism of nurses in hematology department was 53.21±6.84. Univariate analysis showed that the scores of psychological collectivism of nurses in hematology department had significant differences in age, educational background, professional title, hospital grade, children or not, working hours, advanced studies, monthly income and employment methods (F=5.125-76.674, P<0.05). The occupational expectation score was 75.06±6.25 and the occupational values score was 92.32±5.84 in the nurses from Hematology Department of tertiary hospitals, the Pearson correlation analysis showed that career expectation, career values and psychological collectivism were significantly positively correlated (r=0.510, 0.624, P<0.01). Multivariate linear regression equation showed that educational background, professional title, children, working hours, advanced education, hospital level, monthly income, career expectations and professional values were significant influencing factors of psychological collectivism of hematological nurses (P<0.01).@*Conclusions@#The level of psychological collectivism of nurses in hematology department is relatively high. We should fully realize the importance of psychological collectivism of nurses and improve the psychological tendency of nurses in hematology department towards collective.

ABSTRACT

In order to improve the quality of medical records written by interns of clinical medicine in hematology department,we evaluated and analyzed 100 medical records written by the interns.The main reasons that affect the quality of medical record written by interns are the writing defects that are history of present illness,past history,personal history,the record of the clinical operation,blood transfusion and chemotherapy.There are other reasons for affecting the quality of medical records,such as students in clinical practice not paying attention to the writing of medical records,the relative lag of teaching management,the particularity of the diseases in the department of hematology and the weak legal consciousness.Therefore,we should strengthen the teaching management,improve the comprehensive quality of the clinical teaching teachers,carry out the pre job training of the medical records for the interns,train the interns' inquiries and the ability to communicate with the doctors and patients,and make use of the typical medical records of the Department of Hematology to combine the discussion and other ways,so as to strengthen the interns' clinical disease writing skills and legal consciousness as well as the quality of their writing medical records.

ABSTRACT

The current situation is that the study of blood system diseases is difficult, and the students lack both the initiative and enthusiasm to learn. Clinical teachers' energy is limited because of other clini-cal, management and research roles, so the quality of teaching varies greatly. Meanwhile, teaching quality has nothing to do with personal reviews. Furthermore, the relationship between doctors and patients is in trouble in current medical environment. All of these above affect the quality of clinical practice in hematology department. Aiming at these problems, we improve the teaching quality significantly by raising the students' interests, enhancing their initiative, improving teachers' teaching level and enthusiasm, carrying out the in-ternship system, and strictly enforce the elimination system.

ABSTRACT

The clinical probation is an important part of medical education, which is also a bridge from theoretical learning to clinic practice. To improve the quality of clinical probation teaching of hematology department, multiple teaching activities have been carried out, including full preparation before teaching, creating better medical environment, arousing enthusiasm, tralning the physician-patient communication ability and strengthening operating skill of the students. Good study effect has been achieved.

ABSTRACT

OBJECTIVE To investigate the clinical distribution characteristics and drug resistance of pathogens in hematology department ward during recent two years,and instruct clinical application of antibiotics reasonably.METHODS The bacteria were identified by VITEK-60 with GNI+,GPI and YBC.Drug sensitivity tests of bacteria were performed by K-B method.The results were analyzed by WHONET 5.3 software.RESULTS The results showed that 370 strains of pathogens were isolated from all detected samples.The composition ratio of Gram-positive bacteria,Gram-negative bacteria and fungi was 59.7%,32.4% and 7.8%,respectively.Coagulase negative Staphylococcus,Escherichia coli,Pseudomonas aeruginosa,Klebsiella pneumoniae and Acinetobacter baumannii were the most frequently isolated pathogens.The ESBLs producing strains in E.coli and Klebsiella spp were 59.3% and 24.2%,respectively.The MRSA in S.aureus and MRCNS in coagulase negative Staphylococcus were 18.2% and 87.8%,respectively.CONCLUSIONS Inspecting pathogens and studying the trend of their drug resistance are important to rationally select antibiotics,reduce infection incidence and mortality rate,and decrease the occurrence of drug resistant strains.
