ABSTRACT

@#Objective    To explore the effect of body mass index (BMI) on the outcomes of laparoscopic surgery for esophageal hiatal hernia. Methods    We divided the patients who underwent hiatal hernioraphy and fundoplication surgery in our hospital between July 2013 and June 2018 into two groups according to the BMI: a group A, BMI ≥24 kg/m2, 77 patients, 41 males, 36 females, with an average age of 42 years; a group B: BMI<24 kg/m2, 63 patients, 38 males, 25 females, with an average age of 67 years, and the age, gender, type of hiatal hernia, score of subjective feeling of symptoms, level of reflux esophagitis were analyzed with the propensity score matching method. Fifty one patients were successfully matched in each group, and the curative effect of surgery was compared between the two groups. Results    There was no statistical difference in the type of surgery, intraoperative complications, postoperative complications, and hospital stay between the two groups (P>0.05). The operative time of the group A was significantly longer than that of the group B (P=0.023). There was no statistical difference between the two groups in postoperative recurrence (P=0.741). Conclusion    The operative time in overweight patients is significantly longer than that in the non-overweight patients, but it has no effect on the surgical outcomes and complications.

ABSTRACT

Objetivos: Describir la técnica transumbilical de reparo como una alternativa quirúrgicapara la corrección de la hernia umbilical no complicada, y evaluar su aplicabilidad y beneficio estético. Métodos: Estudio retrospectivo y descriptivo realizado en el Hospital Manuel Uribe Ángel (Envigado-Antioquia-), desde enero de 2005 hasta enero de 2007. Luego de hacer el diagnóstico se realizó la corrección de la hernia umbilical a través de una incisión transumbilical, en 21 pacientes con herniaumbilical no complicada y con defectos menores de 5 cm. Se evaluaron el tiempo de cirugía, la satisfacción estética, las complicaciones y la recidiva de la hernia. Resultados: El tiempo quirúrgico promedio fue 35 minutos.Se obtuvieron buenos resultados estéticos en 20 de 21 casos (95,2 %). Doce casos (57,1 %) presentaron dolor con valor de 3 en una escala de 1 a 10. No se presentaron otras complicaciones. No existió recidiva de la hernia. Conclusiones: La técnica transumbilical de reparo puede ser una excelente opción para la reparación de hernias umbilicales; se obtienen buenos resultados estéticos sin evidencia de incrementar las complicaciones o la recidiva de la hernia.


Objectives: To describe the transumbilical hernia repair technique as a surgical alternative for the correction of uncomplicated umbilical hernias, and to evaluate it's applicability and aesthetic benefit. Methods: Retrospective descriptive study of 21 patients with uncomplicated umbilical hernias; performed at the Manuel Uribe Angel Hospital (Envigado -Antioquia), from January 2005 to January 2007. Correction of the umbilical hernia was performed through a transumbilical incision. Surgical time, aesthetic satisfaction, complications and hernia recurrence were evaluated. Results: Mean surgical time was 35 minutes. Good aesthetic results were obtained in 20 of 21 cases (95.2%). Twelve cases (57.1%) presented a pain score of 3 out of 10 on the pain scale. No further complications occurred. No recurrence of the hernia was documented. Conclusion: The transumbilical repair technique could be an excellent option for the repair of umbilical hernias.
