ABSTRACT

El hibernoma es un tumor limítrofe, benigno de comportamiento agresivo, de tejidos blandos, raro en frecuencia, que se origina a partir de los adipocitos pardos residuales, afectando principalmente a mujeres entre 10 a 20 años. Clínicamente se presenta como tumoración, más frecuente en los muslos, de crecimiento lento, móvil, firme e indolora. En los estudios de imagen, puede resultar difícil realizar un diagnóstico diferencial por su similitud con neoplasias malignas. El tratamiento ideal es la resección quirúrgica con márgenes amplios para evitar cualquier tipo de recurrencia local. Se presenta el caso de un hibernoma típico, manejo y tratamiento quirúrgico realizado.


Hibernoma is a borderline tumor, benign with aggressive behavior, of soft tissues, rare in frequency, which originates from residual brown adipocytes, mainly affecting women between 10 and 20 years of age. Clinically it presents as a slow-growing, mobile, firm and painless tumor, more frequent on the thighs. In imaging studies, it can be difficult to make a differential diagnosis due to its similarity to malignant neoplasms. The ideal treatment is surgical resection with wide margins to avoid any type of local recurrence. The case of a typical hibernoma, management and surgical treatment performed is presented.

ABSTRACT

Se describe el caso clínico de una paciente de 32 años de edad, con antecedentes de esquizofrenia paranoide, atendida en la consulta de Ortopedia y Traumatología del Hospital General Docente Dr. Juan Bruno Zayas Alfonso de Santiago de Cuba, por presentar aumento de volumen en la cara interna del muslo izquierdo que le dificultaba la marcha. Al examen físico se observó un tumor de aproximadamente 20 cm, no doloroso a la palpación, así como blando y pastoso, con límites imprecisos y adherido a planos profundos. Según los estudios imagenológicos efectuados se trataba de tejido graso, por lo que no existía correlación con los elementos aportados al examen físico. Teniendo en cuenta lo anterior se discutió el caso con los imagenólogos y se decidió realizar la exéresis del tumor. Se tomó muestra para biopsia cuyo resultado informó la presencia de un hibernoma. La fémina evolucionó satisfactoriamente y se reincorporó a sus actividades habituales a los 2 meses.


The case report of a 32 years patient with history of paranoid schizophrenia is described. She was assisted in the Orthopedics and Traumatology Service of Dr. Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba, due to an increase of volume in the internal face of the left thigh that difficulted her from walking. A tumor of approximately 20 cm was observed with the physical exam, that was not painful to the palpation, but soft and doughy, with imprecise limits and adhered to deep planes. According to the imaging studies it was an adipose tissue, reason why there was no correlation with the elements obtained with the physical exam. Taking into account the above-mentioned the case was discussed with the imaging specialists and the decision was removing the tumor. A sample for biopsy was taken whose result informed the presence of a lipoma. The woman had a favorable clinical course and she returned to her usual activities 2 months later.

ABSTRACT

Hibernoma is a rare, benign soft tissue tumor described in <200 case reports/case series. It is slow-growing, painless, and commonly mistaken for lipoma or liposarcoma. Histopathological diagnosis is must for confirmation. Total excision is the treatment of choice. We present a rare case of hibernoma of thigh, which was initially misdiagnosed as atypical lipomatous tumor/well-differentiated liposarcoma on imaging, which turned out to be a hibernoma on histopathological examination.

ABSTRACT

Abstract Mammary hibernomas are extremely rare benign tumors composed of brown fat cells, with only five cases previously reported in the literature. We report the case of a 42- year-old female patient with a painless growing mass in her right breast. A partial mastectomy was performed, and the diagnosis of hibernoma was confirmed by the histological features and the immunohistochemical profile. Although hibernoma is a benign tumor, its main differential diagnoses include aggressive lesions, making the accurate diagnosis essential to provide adequate care to the patient.


Resumo Hibernomas mamários são tumores benignos extremamente raros compostos por gordura marrom, com apenas cinco casos previamente relatados na literatura. Relatamos o caso de uma paciente do sexo feminino, de 42 anos de idade, apresentando- se com uma massa indolor em sua mama direita. Realizou-se uma mastectomia parcial e o diagnóstico de hibernomamamário foi confirmado pelo padrãomorfológico e pelo perfil imuno-histoquímico. Embora hibernomas constituamneoplasias benignas, seus principais diagnósticos diferenciais incluem lesões agressivas, sendo o diagnóstico acurado extremamente importante para o correto manejo clínico do paciente.

BACKGROUND: Hibernoma is a rare benign tumor of adults that is composed of multivacuolated adipocytes resembling brown fat cells. Hibernoma typically occurs in soft tissue, and intraosseous examples are very rare. Intraosseous hibernomas can radiologically mimic metastatic carcinoma and other tumorous conditions. METHODS: To collect the intraosseous hibernomas, we searched the pathologic database and reviewed the hematoxylin and eosin (H&E)–stained slides of bone biopsy samples performed to differentiate radiologically abnormal bone lesions from 2006 to 2016. A total of six intraosseous hibernoma cases were collected, and clinical and radiological information was verified from electronic medical records. H&E slide review and immunohistochemical staining for CD68, pan-cytokeratin, and S-100 protein were performed. RESULTS: Magnetic resonance imaging of intraosseous hibernomas showed low signal intensity with slightly hyperintense foci on T1 and intermediate to high signal intensity on T2 weighted images. Intraosseous hibernomas appeared as heterogeneous sclerotic lesions with trabecular thickening on computed tomography scans and revealed mild hypermetabolism on positron emission tomography scans. Histopathologically, the bone marrow space was replaced by sheets of multivacuolated, foamy adipocytes resembling brown fat cells, without destruction of bone trabeculae. In immunohistochemical analysis, the tumor cells were negative for CD68 and pan-cytokeratin and positive for S-100 protein. CONCLUSIONS: Intraosseous hibernoma is very rare. This tumor can be overlooked due to its rarity and resemblance to bone marrow fat. Pathologists need to be aware of this entity to avoid misdiagnosis of this rare lesion.

ABSTRACT

El hibernoma o lipoma de grasa parda, es un tumor benigno extremadamente infrecuente. Presentamos el caso, de un paciente femenino de 12 años de edad con una tumoración gigante en región dorsal, de origen congénito, con una extensión desde la región cervical baja hasta el sacro, presentaba con ausencia de los músculos de la región glútea, se realizó estudios de imagen, así como química sanguínea, donde se evidencio anemia severa y desnutrición proteico energética, se organizó un equipo multidisciplinario centrado al tratamiento definitivo de la lesión, la cual provocaba además seria deformidad de la columna lumbar (cifoescoliosis), se logró la resección completa de la lesión, con un peso total de 12 Kg, con reconstrucción de los bordes anales sin afectar el esfínter anal, se mantuvo bajo seguimiento durante 1 años sin presentar ninguna recidiva de la lesión.


The hibernoma or fetal brown fat, is an uncommon benign tumor. We report a case with an unusual mass in a 12 years old female, in dorsal region, congenital origin, with extension from down cervical region until sacral region, absence of gluteal region muscle. We did image study and blood test, with diagnosis of severe anemia and malnutrition. We organized multidisciplinary team to do final treatment for the lesion that caused important deformity to lumbar spine (kyphoscoliosis), the surgical team achieved gross total resection of tumor, with 12 kg of total weight and reconstruction of anal edge without damage of sphincter, follow up during one year without recurrence.

ABSTRACT

Hibernoma is a slow growing, rare benign tumour, which derived from brown adipose tissue. This tumour is usually found in the area where foetal fat persists such as back, axilla, retro peritoneum and thorax. Hibernoma rarely occurs in the retro pharynx. We report a case of retropharyngeal hibernoma in a 44-year-old male. He presented with obstructive symptoms for six months and a retropharyngeal mass upon examination. His CT scan findings showed a mass in the prevertebral region from level of C2 until C5 causing narrowing of upper aero digestive tract. Histopathological examination reported as hibernoma.

ABSTRACT

Hibernomas are uncommon benign tumours that arise from the remnants of fetal brown adipose tissue. They are usually asymptomatic and have a slow growth pattern. Intrathoracic and pleural locations are exceptional for localization of hibernoma. A review of the English language medical literature revealed more than 110 cases, 20 of which were intrathoracic. In the article below, we discuss a 40-year old male patient who had pleural involvement and was treated by surgical resection. Following resection, the patient has remained problem-free for nine years.


Los hibernomas son tumores benignos poco frecuentes que surgen de restos del tejido adiposo marrón fetal. Son generalmente asintomáticos y tienen un patrón de crecimiento lento. Las localizaciones intratorácicas y pleurales son excepcionales para la localización del hibernoma. Una revisión de la literatura médica en lengua inglesa reveló más de 110 casos, 20 de los cuales se trataban de hibernomas intratorácicos. En el siguiente artículo, discutimos el caso de un paciente de 40 años de edad, que tenía una afección pleural y fue tratado con una resección quirúrgica. Después de la resección, el paciente ha permanecido sin problemas durante nueve años.

ABSTRACT

Hibernoma is a very rare, benign soft fatty tumor that derived from remnants of fetal brown tissue. The following case of hibernoma break out occurred from a patient who had received surgical resection of neurilemmoma. There has never been a case that reported about hibernoma accompanied by neurilemmoma. It is anticipated that this experience would be helpful to other physicians who are interested in this rare type of tumor.

El hibernoma es un raro tumor benigno de los tejidos blandos, compuesto por grasa parda, tejido que predomina en animales que hibernan y de allí su nombre. Debido a su infrecuencia en Pediatría y a su diagnóstico complejo, presentamos un paciente de 3 meses de edad, con diagnóstico de tumor abdominal. Los exámenes por ecografía y tomografía computada demostraron un tumor retroperitoneal de carácter infltrativo, vascularizado y con características lipomatosas. La exéresis del tumor permitió arribar al diagnóstico histopatológico de hibernoma o tumor de grasa parda. En esta presentación se describen las características del hibernoma, como tumor infrecuente en niños, y se revisan los tumores de tejido graso, según su frecuencia en Pediatría.


Hibernoma is a rare benign tumor of soft tissue, composed of brown fat. This tissue is predominant in hibernating animals and hence its name. Because of its rarity in Pediatrics and diffcult diagnosis, we report a 3 month-old patient with a diagnosis consistent with an abdominal tumor. Ultrasound and computed tomography exams showed an infltrative retroperitoneal tumor, with hypervascular and lipomatous features. After tumor excision, histopathological exam confrmed the diagnosis of hibernoma or brown fat tumor. This presentation describes the characteristics of this type of tumor, rare in children, and reviews the fatty tumors, according to their frequency in pediatric patients.

ABSTRACT

A 35 year old woman presented with 18 months history of a painless, soft, well-circumscribed, vulvar subcutaneous lump which measured approximately 7 cm. The lesion was completely excised and microscopically was consistent with a hibernoma. According to the available literature in English, this is the first reported case of hibernoma of the vulva.

ABSTRACT

PURPOSE: Hibernoma is a very rare benign soft tissue tumor of the hypervascularized brown fat tissue with no malignant potential. MATERIALS AND METHODS: However, it is difficult to differentiate a hibernoma from other malignant tumors, such as liposarcoma using computed tomography and magnetic resonance imaging, and a surgical resection with histological confirmation is the treatment of choice. RESULTS: Histopathologically, hibernoma is composed of brown adipose cells that are polygonal and multivacuolated with a centrally located nucleus and granular cytoplasm, unlike white adipose cells. CONCLUSION: This article describes a patient with a histologically-confirmed hibernoma of the right shoulder.

ABSTRACT

Exponer nuestra experiencia en el manejo de 1 caso de hibernoma, en el Servicio de Cirugía 2 del Hospital Universitario de Caracas (HUC). Revisión de literatura. Se realizó el estudio clínico, imagenológico y anatomopatológico de una tumoración de partes blandas en el muslo derecho de un paciente masculino de 23 años de edad, de aproximadamente 10 meses de evolución. Ante los hallazgos de estudios por imágenes (imágenes por resonancia magnética y ultrasonido con efecto Doppler), así como el resultado de la biopsia percutánea, por trucut, se decidió realizar su exéresis. El estudio anatomopatológico de la pieza quirúrgica reportó hibernoma. Una lesión benigna, poco frecuente, sin proceso de malignización reportado hasta la fecha. La evaluación clínica, imagenológica y la realización de biopsias percutáneas previo al acto quirúrgico son de suma importancia para las lesiones de partes blandas. El reporte de casos de presentación inusual, como el hibernoma, nutre nuestra gama de diagnósticos diferenciales a tener en cuenta al momento de evaluar lesiones de este tipo.

ABSTRACT

Hibernoma is a rare, benign adipose tumor composed of brown fat cells with eosinophilic granular or multivacuolated cytoplasm. The cytologic features of hibernoma have been rarely reported and may mimic other polygonal cell neoplasms. We report the imprint cytologic features of a case of hibernoma in the left thigh of a 68-year-old woman. Microscopic examination showed large, round, or polygonal brown fat cells. The cells were arranged in fragments or clusters. The nuclei were uniformly round with finely granular chromatin. The cytoplasm was multivacuolated or univacuolated. The abundant eosinophilic granular cytoplasm was also present. No nuclear atypia were present. Immunohistochemical staining showed that cells were positive for S-100 protein.

AIM: To investigate the clinicopathological features of orbit primary liposarcoma. METHODS: By means of histological, ultrastructural and immunohistochemical studies, the clinical and pathological features of two cases of orbit primary liposarcoma were analyzed and compared with 6 cases of primary retroperitoneum liposarcoma. RESULTS: Two cases of orbit primary liposarcoma were young persons. The majority of cases of primary retroperitoneum liposarcoma were aged over 40 years. The tumors were located at the retrobulbar region, and involved the wall of eyeball and optic nerve. The tumor cells of case 1 were round, with abundant eosinophilic granules in the cytoplasm, and there were lipoblasts in the tumor cells of two cases. Local recurrence following excision occurred in case 2.Immunohistochemical expression of S100 protein was positive in all cases, and CK (AE1/AE3), desmin, MyoD1, melanoma were negative. Ultrastructure observation showed lipid drops in the tumor cells in case 2 of orbit primary liposarcoma. CONCLUSION: Primary liposarcoma of the orbit may recur.The diagnosis was based on histological,ultrastruc-tural observation and immunohistochemical staining.

ABSTRACT

Hibernoma is a rare benign tumor of a brown fat origin with hypervascularity. Although the magnetic resonance imaging features resemble a liposarcoma, its malignant potential is unknown. A complete local excision with meticulous hemostasis is the treatment of choice. We present a case of hibernoma in the psoas muscle with a review of the relevant literature.

ABSTRACT

Hibernoma is a rare slowly growing painless subcutaneous neoplasm, and predominates in the scapular and interscapular region, mediastinum, and upper thorax. Microscopically, hibernomas display a distinct lobular pattern and are composed of cells with varying degrees of differentiation, ranging from uniform, round to ovoid, granular eosinophilic cells to multivacuolated cells with multiple, small lipid droplets and centrally placed nuclei. We report a case of hibernoma occurred on the right scapular region in a 33-year-old man.
