ABSTRACT

Aims: To report a case of a rare giant eyelid eccrine hidrocystoma confirmed on histopathological examination and successful surgical excision with no recurrence.Presentation of Case: A young male presented with a mass over his eyelid which was gradually increasing in size, leading to drooping of the upper eyelid with no other associated symptoms. The mass 20x15x10 mm was present over the upper lid, non-mobile, non-tender, and firm. On the CT scan, a well-defined round to oval hypodense cystic mass was seen to be originating from the left upper eyelid. The cyst was excised and sent for histopathological analysis. The histological appearance of cystic spaces lined by cuboidal epithelium and containing mucinous fluid was suggestive of eccrine hidrocystoma. There was no recurrence till 2 months of follow-up.Conclusion: Eccrine hidrocystomas are 1-6 mm in size, generally 4 mm, and cysts larger than 10 mm are rare. Larger cysts have been described as “giant eccrine hidrocystoma.” Giant eyelid eccrine hidrocystoma is an unusual condition of the eyelid that must be considered in the differential diagnosis of eyelid tumours. They can be managed by complete surgical excision although recurrences are found to be common.

ABSTRACT

Accessory breast tissue occurs due to failure of resolution of embryonic mammary ridges present from axilla to groin bilaterally. It may be functional or non-functional. This condition is more commonly seen in the females and very few cases have been reported in males. Development of ectopic breast tissue is dependent on hormonal stimulation similar to normal breast.It remains asymptomatic usually and increases in size after a hormonal stimulation at the time or after puberty. Majority of the occurrences are completely benign, but this condition also has the potential to convert into a malignant lesion. Apocrine hidrocystoma is regarded as cystic retention and adenomatous proliferation of apocrine sweat glands. It commonly occurs on the head & neck, but can infrequently be present in other locations like axilla, vulva, penis. This is the report of case of apocrine hidrocystoma with accessory breast tissue in a 30 year old male presented with left axillary swelling. Cytomorphological assessment revealed benign epithelial lesion and a diagnosis was established only after a histopathological examination. It emphasises the role of histopathology in diagnosis of axillary breast lesions

Introdução: Hidrocistomas são cistos cor da pele ou translúcidos, únicos ou múltiplos, que aparecem na face. Têm origem na glândula sudorípara écrina ou apócrina e, às vezes, são desfigurantes. Podem ser tratados com cirurgia, eletrodissecção ou cáusticos, mas costumam ser recidivantes e os tratamentos podem deixar cicatrizes ou discromias. A toxina botulínica A (BoNT-A) foi sugerida como terapia em poucos casos na literatura. Objetivo: Analisar os efeitos da toxina botulínica em casos de hidrocistoma da face. Métodos: Estudo observacional retrospectivo de série de casos. Foram avaliados pacientes portadores de hidrocistoma na face, tratados com BoNT-A. Resultados: De 13 pacientes avaliados, cinco foram incluídos, a maior casuística descrita até o momento. Destes, todas as lesões tratadas apresentaram melhora clínica durante o período avaliado. Quatro pacientes mostraram regressão parcial e um, regressão total das lesões. Conclusões: Nesta série de casos, a BoNT-A mostrou resultados positivos e duradouros no tratamento de hidrocistomas faciais, sem efeitos adversos.


Introduction: Hidrocystomas are skin-colored or translucent cysts, single or multiple, appearing on the face. They originate in the eccrine or apocrine sweat gland and are sometimes disfiguring. They can be excised by surgery, electrodissection, or by caustics, but are usually recurrent and the treatments can leave scars or dyschromia. Botulinum toxin A (BoNT-A) has been suggested as therapy in few cases in the literature. Objective: To analyze the effects of BoNT-A in cases of facial hidrocystomas. Methods: Retrospective observational study of case series. Patients with facial hidrocystomas treated with BoNT-A at the Cosmiatry Outpatient Dermatology Clinic of the Hospital do Servidor Público Municipal de São Paulo, were evaluated. Results: Of the 13 patients evaluated, 5 were included, the largest case series described so far. Of these, all lesions treated showed clinical improvement during the period evaluated. Four patients showed partial regression and one, total regression of the lesions. Conclusion: In this series of cases, BoNT-A showed positive and long-lasting results in the treatment of facial hidrocytsomas, with no adverse effects.

ABSTRACT

Apocrine hidrocystoma, also called apocrine cystadenoma, is a benign cystic tumor-like lesion that arises from the proliferation of apocrine glands. Clinically, it usually occurs singly as a unilocular or multilocular, dome-shaped translucent cyst. Histologically, it appears as unilocular or multilocular cysts composed of an inner layer of single or double layer of secretory columnar epithelium with decapitation secretion lying above an outer myoepithelial cell layer. Apocrine hidrocystomas mostly occur within the head and neck region and involvement of genitalia is extremely rare. This paper emphasizes the importance of considering the differential diagnosis of a genital cystic lesion. Herein, we report a case of apocrine hidrocystoma occurring in the penis and compare the clinicopathological characteristics of apocrine hidrocystoma in genitalia with the previous cases.

ABSTRACT

Apocrine hidrocystoma is a benign tumor arising from apocrine sweat gland. It presents as a solitary translucent nodule, usually on the face, head, and neck. However it is extremely rare for the apocrine hidrocystoma to arise on the extremities, and no case of apocrine hidrocystoma on lower extremity has been reported in Korean dermatologic literature. Herein, we report a case of 63-year-old male apocrine hidrocystoma that developed in ankle area with a review of the literature.

ABSTRACT

Hidrocystomas, eccrine or apocrine are rare ductal cystic benign lesions of the sweat gland. These uncommon cystic lesions are extremely rare in the external ear. Here, we discuss the clinical features and management of eccrine hydrocystoma in the external ear by describing the clinical, radiographic, pathologic features and management of an external auditory canal eccrine hidrocystoma in a 73-year-old Korean man. To the best of our knowledge, this tumor has not been reported previously in this site. Five month after surgery, there has been no evidence of local recurrence.

ABSTRACT

INTRODUÇÃO: O hidrocistoma palpebral écrino corresponde a lesão cística rara, usualmente de pequenas dimensões, não ultrapassando o diâmetro de 10 mm. Sua abordagem usualmente é por excisão simples e biópsia. É alto o índice de recidiva local. MÉTODOS: Paciente com múltiplos hidrocistomas bilaterais, vários maiores que 10 mm. Sob anestesia local, foi realizada incisão infraciliar tradicionalmente usada para blefaroplastia inferior e retirada dos hidrocistomas com cápsula íntegra. RESULTADOS: Paciente com excelentes resultados estéticos e sem recidivas até o presente momento, mais de seis meses após o procedimento. CONCLUSÃO: A incisão infraciliar de blefaroplastia inferior é uma técnica promissora para exérese de hidrocistomas maiores que 10 mm. Mais estudos devem ser realizados para confirmação do bom resultado estético-funcional desta técnica.


INTRODUCTION: Eccrine eyelid hidrocystoma is a rare cystic lesion, usually small and not exceeding 10 mm in diameter. Its treatment is usually by simple excision and biopsy. The rate of local recurrence is high. METHODS: A patient presented with multiple bilateral hidrocystomas, several larger than 10 mm. Under local anesthesia, an infraciliary incision traditionally used for lower blepharoplasty was performed, and hidrocystomas with intact capsules were withdrawn. RESULTS: The patient had excellent esthetic results, with no recurrence for more than six months after the procedure. CONCLUSION: The infraciliary lower blepharoplasty incision is a promising technique for excising hidrocystomas larger than 10 mm. More studies should be conducted to confirm the good esthetic and functional outcomes of this technique.

ABSTRACT

A 38-year-old man complained of multiple papules and nodules in the left retroauricular region for 38 years. On physical examination, there were tens of irregularly sized, skin-colored or pink papules and nodules in the left retroauricular region. Pathological examination showed a large irregularly shaped unilocular cystic space surrounded by a fibrous pseudocapsule in the dermis. The cystic space was lined by a double layer of epithelial cells, including an outer layer of flattened vacuolated myoepithelial cells and an inner layer of cuboidal or columnar secretory cells with eosinophilic cytoplasm. Apocrine secretion was present. Numerous hyperplastic apocrine glands were seen in the deep dermis. A final diagnosis of multiple apocrine hidrocystomas accompanied by apocrine hyperplasia was rendered based on the clinical and pathological presentations.

ABSTRACT

Apocrine hidrocystomas (AHC) are uncommon cystic lesions of apocrine glands and are most often solitary. They are usually found in the head and neck region. Multiple AHC have been rarely reported in the literature usually in the eyelids and face. On extensive search of literature, we came across only a single case report of multiple axillary AHC. We report a case of a 31-year-old female who presented with fullness and discomfort in bilateral axillae, so a clinical diagnosis of bilateral accessory breast was made. However on histopathological examination, it turned out to be bilateral multiple AHC associated with apocrine hyperplasia.

ABSTRACT

Os hidrocistomas écrinos são lesões raras, císticas e benignas, que resultam em deformidades nas regiões palpebrais bilateralmente. Vários tratamentos são citados na literatura, porém nenhum deles é considerado padrão de referência. Deve-se somar as impressões obtidas à avaliação clínica e aos recursos disponíveis. Paciente de 73 anos, feminino, apresentando tumorações em regiões palpebrais bilaterais, de crescimento lento, com obstrução parcial do campo de visão e ectrópio. Foi submetida a dois procedimentos cirúrgicos para ressecção, em 2007 e em 2008. Apresentou melhora significativa do contorno palpebral bilateral, com boa simetria entre as regiões. O exame histopatológico concluiu: hidrocistoma écrino. O tratamento cirúrgico para a polipose palpebral bilateral relacionada ao hidrocistoma écrino mostrou-se uma modalidade que pode apresentar bons resultados estéticos e funcionais, sendo reprodutível, e sem causar maiores morbidades pós-operatórias ao paciente.


Eccrine hidrocystomas are rare lesions, cysts, or benign tumors, which lead to bilateral deformities in the eyelid areas. Several treatments are described in the literature. However, none of them has been established as the gold standard. Hence, it becomes necessary to consider the contribution of different opinions and available resources to clinical evaluation. A 73-year-old female patient presented with slow-growing tumors on both eyelids, which consequently led to partial visual field obstruction and ectropion. She underwent 2 surgical resections, one in 2007 and the other in 2008. As a result, she showed significant improvement of the bilateral eyelid contours and satisfactory symmetry between the areas. Histopathological diagnosis indicated eccrine hidrocystoma. The surgical treatment of bilateral eyelid polyposis associated with eccrine hidrocystoma proved to be a reproducible procedure that may ensure satisfactory aesthetic and functional results, without causing major postoperative morbidities to the patient.

ABSTRACT

An apocrine hydrocystoma is a translucent cystic nodule that looks like adenomatous cystic proliferation of an apocrine gland, which originated from a hair germ. Dermoid cyst is a cystic teratoma that contains developmentally mature skin appendages, such as hair follicles, sweat glands, and other tissues derived from the ectoderm. Many investigators believe that dermoid cysts re originated from the ectoderm, specifically the hair germ cells. A 58-year-old woman was presented with a purple, asymptomatic, dome-shaped nodule on her right lower eyelid that had been slowly increasing in size for several years. Histopathological examination revealed two well-demarcated cysts: one was lined with stratified squamous epithelium with mature hair and sebaceous glands and the other contained large cystic spaces, which papillary projections extended. We report a rare case of a dermoid cyst, combined with an apocrine hydrocystoma, occurring in the right lower eyelid.

ABSTRACT

Relato de caso de hidrocistoma palpebral e apresentação de técnica cirúrgica na conduta terapêutica dos tumores císticos benignos da pálpebra. Os hidrocistomas são tumores benignos relativamente frequentes nas pálpebras, principalmente, na pálpebra inferior, com maior prevalência no sexo feminino e a partir da quarta década de vida. O diagnóstico é clínico e, em casos de lesão única, a conduta cirúrgica é o tratamento de escolha. Deve-se descrever melhor a técnica cirúrgica utilizada, pois traz bons resultados estéticos e menor risco de recidiva.


This report describes the case of a hidrocystoma of the eyelid and the surgical technique used in the therapeutical management of benign cystic lesions of the eyelids. Hidrocystomas are relatively common benign lesions of the eyelids, principally the lower eyelid. They are more common in females over thirty years of age. Diagnosis is clinical and when there is a single lesion, surgery is the treatment of choice. The surgical technique used should be described in greater detail, since it offers good esthetic results and a low risk of recurrence.

ABSTRACT

This is a case report followed by a short review of literature of apocrine hidrocystomas. A 67 year old lady came to this clinic with multiple translucent, asymptomatic papulocystic lesions on the face which got worse in summer. A biopsy showed a single cystic cavity lined by two layers of small cuboial epithelial cells was seen. Apocrine differentiation characterised by decapitation secretion was seen. A diagnosis of multiple apocrine hidrocystomas was made. Treatment can be attempted with individual excision of lesions or lasers. Topical atropine and scopolamine too have been tried with variable success. Our patient did not wish any treatment once the benign nature of the condition was confirmed.


Este é um relato de caso seguido por uma breve revisão da literatura sobre hidrocistomas apócrinos. Uma senhora de 67 anos de idade foi à clínica anteriormente citada apresentando lesões papulocísticas múltiplas, translúcidas e assintomáticas na face que pioravam durante o verão. Uma biopsia mostrou uma cavidade cística única contornada por duas camadas de pequenas células epiteliais cubóides. Diferenciação apócrina caracterizada por remoção de secreção foi observada. Foi feito um diagnóstico de hidrocistomas múltiplos apócrinos. Tratamento pode ser tentado com excisão individual de lesões ou laser. Atropina tópica e escopolamina também foram tentadas com sucesso variável. Nossa paciente não quis nenhum tratamento uma vez que a natureza benigna da condição foi confirmada.

ABSTRACT

Eccrine hidrocystomas are small benign cystic lesions that normally affect only adult females. Eccrine hidrocystoma is characterized by tense vesicles that are predominately located on the face and particularly on the periorbital areas. Histologically, eccrine hidrocystoma consists of one or more partially collapsed unilocular cysts in the dermis, and these cysts are often situated adjacent to normal eccrine ducts. An 8-year-old boy presented with multiple skin-colored to bluish tiny papules on his nose. Histopathologically, the dilated partial cystic structures in the middermis were lined by one or two layers of flattened or cuboidal epithelial cells. We report here on an atypical presentation of multiple eccrine hidrocystomas that were localized on the nose of a child.

ABSTRACT

Eccrine hidrocystomas are benign cystic dilatations with eccrine differentiation. The clinical presentation usually occurs as a solitary tense vesicle or translucent papule located predominantly on the face, whereas multiple eccrine hidrocystomas are unusual. Histologically, eccrine hidrocystoma consists of one or more partially collapsed unilocular cysts in the dermis, which show flattened epithelium that is partially lined by two layer of cuboidal epithelium. We report here on a case of the patient with multiple eccrine hidrocystomas that showed an especially generalized presentation that included the face, neck, anterior chest and both arms.

ABSTRACT

A 36-year-old female patient with a mass in the coccygeal region underwent surgical removal of the mass, and this revealed a hybrid cyst in coexistence with a glomus coccygeum. This unusual cutaneous cyst had an epithelial lining composed of keratinizing, stratified squamous epithelium with an intact granular layer immediately adjacent to apocrine cells, and the apocrine cells showed the characteristic features of "decapitation secretion". The glomus coccygeum, which is a minor finding in specimens from the sacral area and it may represent a diagnostic challenge to the unaware observer, was incidentally identified in the dermis. The glomus coccygeum was located beneath the epithelial transition area of the hybrid cyst. Immunohistochemical analysis revealed that the cytoplasm of the epithelioid glomus cells was positive for smooth muscle actin, and these epithelioid glomus cells were arranged in concentric layers around blood vessels, and the cellular stroma surrounding the glomus bodies were positive for S-100 protein.
