ABSTRACT

Objective:To observe the curative effect of local application of minocycline-HCl ointments and the influence on high sensitivity C reaction protein (hs-CRP) and interleukin-10 (IL-10) levels in gingival crevicular fluid of patients with chronic periodon-titis. Methods:Totally 72 cases of patients with chronic periodontitis (96 teeth) were selected and divided into the observation group (36 cases, 47 teeth) and the control group (36 cases, 49 teeth) by a random number table. The patients in the two groups were given the routine periodental non-surgical treatment, such as supragingival scaling, subgingival scaling, scaling and root planning etc. The patients in the observation group were given minocycline-HCl ointments filled in the periodontal pockets, and the patients in the control group were given compound iodine glycerol liquids filled in the periodontal pockets once a week for 4 weeks. The changes in hs-CRP and IL-10 levels in gingival crevicular fluid of the patients before and after the medical treatment were observed, and the clinical cura-tive effect was compared as well. Results:After the 4-week medical treatment, the hs-CRP levels in gingival crevicular fluid of the pa-tients in the two groups were obviously declined, while the IL-10 levels were obviously increased(P<0. 01 or P<0. 05), and the de-clining or increasing rate in the observation group was much higher than that in the control group (P<0. 05). The total clinical effi-ciency in the observation group was 94. 44%, which was much higher than that in the control group (77. 78%, P<0. 05). Conclu-sion:The local application of minocycline-HCl has favorable curative effect on chronic periodontitis, which can reduce the hs-CRP lev-els in gingival crevicular fluid, increase the IL-10 levels, control the inflammatory damage in periodontium effectively and improve the local inflammatory reaction in periodontium.

ABSTRACT

Objective To investigate the carotid arterial stiffness in patients with coronary slow flow ( CSF) .Methods forty-five patients with CSF and Forty -five persons having normal coronary arteries ( NCA) detected by coronary angiography with a similar distri-bution of risk factors were recruited .Stiffness parameter (β), pressure-strain elastic modulus (Ep), arterial compliance (AC) and lo-cal pulse-wave velocity (PWV) were obtained at the level of bilateral carotid artery by a real time echo -tracking system.Serum levels of high-sensitivity C-reactive protein ( hs-CRP) were measured in two groups of subjects .Linear regression analysis were performed to evaluate the correlation between hs -CRP and the parameters of the carotid artery stiffness .Results We found that stiffness parameter (β), Ep and PWV were significantly higher in CSF group those that of control group (β:11.80 ±3.19 vs 9.70 ±3.76,P<0.01;Ep:149.90 ±44.47 vs 130.10 ±41.56,P<0.05;PWV:7.40 ±0.84 vs 7.00 ±1.08,P<0.05), AC was lower than that of control group (0.640 ±0.180 vs 0.760 ±0.192 ,P<0.01).The levels of high-sensitivity C-reactive protein (hs-CRP) was significantly higher in CSF group than that of control group (13.90 ±10.66 vs 9.30 ±6.33,P<0.05).The levels of hs-CRP was positively correlated with theβ(r=0.272,P=0.005), Ep(r=0.411,P=0.003), and PWV(r=0.452,P=0.001), but negatively correlated with AC (r=-0.293,P=0.025).Conclusion Echo-tracking technology is a simple practical method to evaluate carotid artery stiffness in patients with CSF and correlation well with coronary slow flow and artery stiffness .

ABSTRACT

Objective To investigate the change of high sensitivity c-reaction protein( hs-CRP)on carotid artery plaque. Methods A total of 5 852 individuals were selected as our subjects who randomly stratified from the health examination survey participants in Tangshan Kailuan Company community during 2006 - 2007. A total of 5 440 people(age above 40 years old, free of stroke, transient ischemic attack(TIA)and myocardial infarction ) were qualified and enrolled in the final analysis. A questionnaire survey, blood biochemical analysis and carotid artery ultrasound examination were finished by trained medical staff. According to the hs-CRP 3 mg measurements at the first and third times health examinations, all subjects were divided into four groups, including continued decreasing group(hs-CRP < 3 mg/ L),decreasing group(hs-CRP decreased from≥3 mg/ L to < 3 mg/ L ),increasing group((hs-CRP increased from < 3 mg/ L to≥3 mg/ L)and continued high group((hs-CRP ≥3 mg/ L). Multiple logistic regression analysis was used to test the impact of change of hs-CRP on carotid artery plaque. Results Incidence of carotid artery plaques in continued decreasing, decreasing, increasing and continued high groups were 33. 6%(1 308 / 3 577),44. 7%(178 / 398),45. 1%(186 / 412)and 54. 9%(151 / 275),respectively. The dependent variable was named based on carotid artery plaque(1 = plaque and 0 = no plaque),and in dependent variable was named based on hs-CRP value change (0 = continued decreasing group;1 = decreasing group;2 = increasing group;3 = continued high group). Multiply factors logistic regression analysis showed that the risk of carotid artery plaques for women increased compared to decreasing group( RR = 1. 17,95% CI:1. 01 - 1. 35,P = 0. 035). After adjusting gender, age, smoking, low density lipoprotein cholesterol, fasting blood-glucose,baseline lg( hs-CRP)and body mass index, the risks of developing carotid artery plague in increasing group and continued high group were increased compared to decreasing group among the women and whole population(for all subject:RR = 1. 33,95% CI:1. 04 - 1. 70,P =0. 023;for women subject:RR = 1. 63,95% CI:1. 08 - 2. 46,P = 0. 021). Conclusion Increasing hs-CRP is a risk factors of incidence of carotid artery plaque possibly.
