ABSTRACT

As a highly prevalent global condition, myopia significantly impacts the ocular health of young individuals in China. Orthokeratology lens, as a rigid corneal contact lens, has demonstrated effective control over the progression of myopia; however, its mechanism of action remains incompletely elucidated. As one of the factors influencing visual acuity, higher-order aberrations will undergo marked changes after orthokeratology, with particular emphasis on the alterations in spherical aberrations and coma. The changes in corneal morphology induced by orthokeratology lead to significant positive increase in both spherical aberration and coma. Furthermore, the elevation of spherical aberration and coma demonstrates a negative correlation with the rate of axial length growth following orthokeratology. The interplay among spherical aberration, coma, defocus, accommodation, astigmatism, and pseudo-accommodation may constitute the underlying mechanism governing the control of myopia through orthokeratology.

ABSTRACT

Aims: In this study, we aimed to investigate the intercorrelations between tear film break up time, measured non-invasively using non-invasive keratographic break-up time (NIKBUT), higher order aberrations (HOA) and quality of vision (QoV) in pseudophakic patients.Study Design: Cross-sectional.Methods: Thirty-five pseudophakic aged patients aged 50 years or older, and 35 control phakic patients aged 17 to 23 years with corrected visual acuity of 20/20 were included in this study. All subjects underwent similar examination including QoV questionnaire, aberrometry to measure HOA, and NIKBUT. HOA was measured with the OPD-Scan/ ARK 10000 corneal analyzer (Nidek CO. Ltd), expressed as Root Mean Square (RMS) HOA and NIKBUT was assessed using non-invasive TF-Scan module Keratograph 5M (K5M), equipped with modified tear film scanning function (Oculus, Wetzlar, Germany). Statistical analysis was performed to find the correlation between NIKBUT, HOA and QoV.Results: Patients in the pseudophakic group were significantly older (median age 66 vs. 20 years; P<0.01), had shorter NIKBUT (10.5 vs. 17.2; P<0.01), lower QoV score (1.63 vs. 0.68; P=0.04), and higher RMS HOA (0.5 vs. 0.26; P<0.01) compared to control group. NIKBUT was inversely correlated with RMS HOA (r = -0.19; p = 0.03) and RMS HOA was significantly correlated with QoV, even after adjustment for age and gender (r = -0.21; P0.04). NIKBUT <9.93s was correlated with lower QoV. The area under the curve was 0.81 (95% CI = 0.67 0.95, p = 0.012), and had 100% sensitivity and 61% specificity. Conclusion: Shorter NIKBUT was correlated with greater HOA and greater HOA was correlated with lower QoV. NIKBUT value of shorter than 9.93s could potentially predict pseudophakic patients who will likely experience visual symptoms leading to decreased QoV; thus, the use of artificial tears might be beneficial.

ABSTRACT

Purpose: Vision is one of the most important senses. The first stage of vision is the creation of the observed object抯 image on the retina. The quality of the retinal image is affected by several factors, such as diffraction, sampling on the retina, chromatic aberration, scattering and higher order aberrations. The measurement of the quality is achieved both with subjective (visual acuity, contrast sensitivity) and objective methods (PSF, MTF, Strehl ratio, RMS). The purpose of this project is the measurement of higher order aberrations of the anterior corneal surface (with a Placido corneal topographer) and of the contrast sensitivity (with a Pelli Robson optotype). Then, we will try to find if there is a correlation between them. Methods and Materials: 20 persons participated in this survey, divided in two groups of 10 persons each. The first group (group 1) included subjects up to 39 years old and the second group (group 2) from 40 years and up. The participants didn抰 have any pathological problems, except lower order refractive errors. Both eyes of each individual were included in the procedure. First, the higher order aberrations of the anterior corneal surface were measured, with the implementation of a Placido corneal topographer. This was followed by a measurement of the contrast sensitivity. The whole procedure took place under two lighting conditions, both photopic and mesopic. The examination presentation of the higher order aberrations is performed with the use of Zernike polynomials. Results: The results of the measuring procedure showed that for the first group, under photopic conditions (luminance 32.70 cd/m2) the mean value ( standard deviation) for the higher order aberrations RMS and decimal logarithm contrast sensitivity was 0.073 0.018?m and 1.54 0,16 (contrast 2.88 1.24%) respectively. For the second group the corresponding values were 0.080 0.036?m and 1.59 0.16 (contrast 2.57 1.24%). Accordingly, under mesopic conditions (luminance 1.14 cd/m2) the values for the first group were 0.252 0.064 ?m and 1.27 0.15 (contrast 5.37 2.06%), while for the second were 0.253 0.069?m and 1.32 0.12 (contrast 4.79 1.35%). Spherical aberration and coma (horizontal and vertical) were also measured for both groups under photopic and mesopic conditions. The results showed that for the first group, under photopic conditions, RMS for coma and spherical aberration is 0.033 0.014 ?m and 0.022 0.011 ?m respectively, while for the second group 0.041 0.027 ?m and 0.024 0.008 ?m. Under mesopic conditions, the relevant results are 0.139 0.065 ?m and 0.124 0.035?m for the first group and 0.149 0.066 ?m and 0.107 0.038 ?m for the second group. Finally, we should mention here that the equivalent defocus error corresponding to the higher order aberration RMS was estimated. The results for all the participants (without age separation), are 0.23 0.09D (photopic conditions) and 0.28 0.07D (mesopic conditions). Conclusions: From the statistical analysis of the results we conclude that there exists a symmetry between left and right eyes regarding higher order aberrations. Furthermore, it seems that age is not a significant factor for differences on the magnitude of higher order aberrations both under photopic and mesopic conditions as well. Similar conclusions are also reached for the contrast sensitivity measurements. In addition, we have observed that a correlation exists between the higher order aberrations of the anterior corneal surface and the contrast sensitivity, for both photopic and mesopic conditions. Correlation also exists between coma and contrast only under photopic conditions. But there is no significant correlation between spherical aberration and contrast. The age difference of these two groups has not an effect on the results. Finally, the equivalent defocus is too small, both for photopic and mesopic conditions and between each other, in order to be clinically significant.

ABSTRACT

AIM: To compare the postoperative efficacy of corneal wavefront-guided femtosecond laser-assisted excimer in situ keratomileusis(FS-LASIK)in the treatment of myopia patients with different degrees of astigmatism and the changes of corneal higher order aberration.METHODS: A total of 133 patients(265 eyes)with myopia and astigmatism were enrolled in this retrospective study. All of them underwent corneal wavefront-guided FS-LASIK surgery for the first time in Aier Eye Hospital(Kunming)from April to October 2020. The patients were divided into three groups according to the different astigmatism: Low astigmatism group: astigmatism ≤1.0D, 62 cases(124 eyes), medium astigmatism group: a total of 54 cases(107 eyes)with astigmatism was 1.25-2.0D, high astigmatism group: a total of 17 cases(34 eyes)with astigmatism ≥2.25D. Visual acuity and refraction were recorded before surgery and 3mo after the surgery and the cornea of the patients was measured by Pentacam three-dimensional corneal topography. Record total higher order aberrations(root mean square), spherical aberration, horizontal coma, vertical coma, horizontal clover and oblique clover in the 6mm diameter range of the cornea. The postoperative effects of the three groups of patients were observed and the changes of corneal high order aberration before and after surgery were compared. RESULTS: The validity index of visual acuity in the three groups of patients was all greater than 1.1 and the residual diopter was all within ±0.30D. The residual diopter in the low astigmatism group was the least than that of the other two groups(P&#x003C;0.05). At 3mo after surgery, the corneal total higher order aberration, spherical aberration and vertical coma of the three groups were all increased compared with those before surgery(P&#x003C;0.05). The spherical aberration increase in the high astigmatism group was less than that in the other two groups(P&#x003C;0.05).CONCLUSION: Corneal wavefront-guided FS-LASIK surgery is safe and effective in the treatment of myopia with different degrees of astigmatism, and the effect is also accurate for patients with high astigmatism. The degree of of preoperative astigmatism is not responsible for increased corneal higher order aberration after the surgery.

ABSTRACT

@#AIM:To analyze satisfaction on night vision and its related factors after FS-LASIK in patients with high and low-to-moderate myopia.<p>METHODS:This was a retrospective study. Totally 75 patients(150 eyes)who underwent FS-LASIK in Xi'an No.1 Hospital from June 2018 to September 2019 were included. All subjects were divided into two groups based on spherical equivalent(SE): high myopia group(SE≥-6.00D)and low-to-moderate myopia group(<-6.00D). Six months after surgery, corneal topography and corneal higher-order aberrations(total HOAs, spherical aberrations, coma and trefoil)under 7mm pupil diameter was investigated by Sirius. Patients were required to complete a questionnaire to evaluate satisfaction and night vision symptoms.<p>RESULTS:For the high myopia group and low-to-moderate myopia group, preoperative SE was -7.85±1.27D and -4.57±1.01D respectively. There were significant differences in total HOAs, spherical aberrations and coma(all <i>P</i><0.001)except for the trefoil(<i>P</i>>0.05)between the two groups. Overall satisfaction was 95.3% and 100.0% in high and low-to-moderate myopia group. 18.8% and 9.3% experienced worsen night vision, 51.2% and 40.6% experienced halo, 41.9% and 43.8% experienced starburst, and 18.6% and 15.6% experienced double vision, respectively. Except that there was significant differences in night vision between the two groups(<i>P</i>=0.010), the occurrence of other night vision symptoms were similar and there were no significant differences in any of them(all <i>P</i>>0.05). For all patients, the satisfaction was correlated with preoperative astigmatism and ablation zone(all<i> P</i><0.05), night vision was correlated with preoperative SE, ablation depth, corneal curvature, total HOAs, spherical aberrations and coma postoperatively(all<i> P</i><0.05), star burst was correlated with preoperative total HOAs, coma and postoperative trefoil(all<i> P</i><0.05), double vision was correlated with preoperative astigmatism(<i>P</i><0.05).<p>CONCLUSION: High myopic patient experienced worsen night vision mainly because of increased total HOAs, spherical aberration and coma. However, symptoms such as halo, starburst and double vision were similar in both groups and did not affect overall satisfaction.

ABSTRACT

@#AIM: To present the visual quality and refractive outcomes obtained in small incision lenticule extraction(SMILE)refractive surgeries, and observe the follow-up of patients over 1a period.<p>METHODS: Retrospective comparative analysis of 85 patients(85 eyes)were treated with SMILE between July and December 2019. Pre- and postoperative uncorrected distance visual acuity(UDVA), corrected distance visual acuity(CDVA), spherical equivalent refraction(SE)were all observed, to evaluate the efficacy, safety and predictability after SMILE refractive surgery. Total corneal higher-order aberrations(HOAs), modulation transfer function cut-off(MTFcutoff), strehl ratio(SR), and objective scattering index(OSI)were evaluated. <p>RESULTS: One year after SMILE refractive surgery, the efficacy index was 1.08, 84 eyes(99%)reached an UDVA as or over CDVA preoperatively, 22 eyes(26%)gained one line of CDVA than preoperatively. The safety index was 1.04, 59 eyes(69%)of the treated eyes had an unchanged CDVA postoperatively, 24 eyes(28%)gained one line of CDVA, 2 eyes(2%)gained two lines of CDVA. 85 eyes(100%)were all within ±0.50D. The pre-correction is highly related with the actual correction of SE(<i>Y</i>=0.9949<i>X</i>-0.0033; <i>R</i>2=0.9977). HOA, spherical aberration and coma were increased under pupil diameters of 6mm(all <i>P</i><0.001). Both MTFcutoff and SR were higher than that of SMILE preoperatively(<i>P</i><0.05).<p>CONCLUSION: SMILE demonstrated to be safe, effective, and predictable in the treatment of myopia and myopic astigmatism, and good accuracy in correcting medium and low astigmatism. The post-quality of retinal image is better than that of SMILE preoperatively.

ABSTRACT

@#AIM: To study the effects of small-incision lenticule extraction(SMILE)and femtosecond laser-assisted excimer laser <i>in situ</i> keratomileusis(FS-LASIK)on early postoperative corneal higher order aberrations(HOAs)and visual quality in patients with high myopia. <p>METHODS:Totally 102 patients(204 eyes)with high myopia who underwent corneal refractive surgery between August 2018 and August 2020 in the hospital were selected as study subjects for the prospective study, and they were randomly divided into SMILE group(51 cases, 102 eyes)and FS-LASIK group(51 cases, 102 eyes). The postoperative visual acuity, corneal HOAs and objective visual quality were compared between the two groups, and the correlation between corneal HOAs and objective visual quality was analyzed. <p>RESULTS: At 1mo after surgery, the spherical equivalent degrees in SMILE group and FS-LASIK group were significantly reduced(<i>P</i><0.05), and there was no statistically significant difference in the proportion of patients with postoperative uncorrected visual acuity≥preoperative best corrected visual acuity between the two groups(95.1% <i>vs</i> 92.2%, <i>P</i>>0.05). The corneal HOAs in the two groups were significantly increased(<i>P</i><0.05), and the overall spherical aberration(SA), trefoil and higher HOAs in SMILE group were lower than those in FS-LASIK group(all <i>P</i><0.05). The objective scattering index(OSI)was significantly increased in the two groups while the MTF cutoff frequency(MTFcutoff), Strehl Ratio(SR)and contrast visual acuity(VA100%, VA20%, VA9%)were significantly decreased(all <i>P</i><0.05). The OSI of SMILE group was lower than that of FS-LASIK group while MTFcutoff and VA9% were higher than those of FS-LASIK group(all <i>P</i><0.05). The corneal HOAs in patients with high myopia were negatively correlated with MTFcutoff and were positively correlated with OSI(<i>P</i><0.05).<p>CONCLUSION:Both SMILE and FS-LASIK can effectively correct high myopia, and SMILE has a smaller increase in early postoperative corneal HOAs, and it is of great significance to relieve visual quality impairment.

ABSTRACT

@#AIM: To study the efficacy and safety of accelerated collagen cross-linking in keratoconus.<p>METHODS: Medical records of keratoconic corneas underwent accelerated collagen cross-linking at King Chulalongkorn Memorial Hospital, Thailand between April 2015 and August 2018 were reviewed. Preoperative and postoperative data at 1a of visual acuity, auto-refraction, corneal topography, higher-order aberrations(HOA), topometric indices and corneal densitometry were evaluated. Age of 24 and 30 years old, maximum keratometry value(Kmax)of 55 D, and baseline best corrected visual acuity(BCVA)of 20/40(or 0.3 in LogMAR unit)were used as cut-off values to highlight the cross-linking effects. The effect of age, preoperative Kmax and BCVA were analyzed. The association between the change of corneal densitometry and other factors including preoperative Kmax, Kmean, manifest refraction spherical equivalent(MRSE), visual acuity, thinnest pachymetry, the change in Kmax, and the change of those parameters were also analyzed. <i>P</i><0.05 was considered statistically significant.<p>RESULTS: One hundred and fifty-five patients(185 eyes)were included. One hundred and nineteen patients were male and thirty-six patients were female. According to Amsler-Krumeich classification, stages 1 and 2 were dominant(37.84% and 35.14% respectively). At 1a, mean LogMAR uncorrected visual acuity(UCVA)improved by 0.1(<i>P</i><0.05). The number of eyes of which postoperative BCVA improved more than 0.2 LogMAR was higher in the worse baseline BCVA group(preoperative BCVA ≥0.3)compared to the better baseline BCVA group(preoperative BCVA < 0.3)(78.26% <i>vs</i> 21.74%, <i>P</i><0.05). Mean Kmax decreased from baseline by 2.36 diopters(D)(<i>P</i><0.05). Seventy-three percent of the eyes of which Kmax reduced more than 2.0 D had preoperative Kmax ≥55 D. Corneal HOA at 6 mm from corneal apex decreased by 0.40(<i>P</i><0.05). Corneal densitometry at 0-6 mm zone increased at 1mo and persisted 1a postoperatively(<i>P</i><0.05). The relationship of the increase in densitometric value and the decrease of thinnest pachymetry at 1a were in linear fashion. Index of surface variance, index of vertical asymmetry, keratoconus index, index of height decentration decreased at 1a(<i>P</i><0.05). Success rate at 1a was 90.24%. Postoperative corneal haze was found 11.35%, 30.27%, 15.67%, 10.27% and 2.16% at 1wk, 1, 3, 6mo and 1a respectively. No eyes developed corneal edema. There was one case of sterile keratitis.<p>CONCLUSION: Accelerated collagen cross-linking in keratoconus was effective to flatten, reshape the cornea, improved visual acuity, HOA and topometric indices. Great Kmax reduction was found in advanced keratoconus. The magnitude of Kmax reduction is also greatest among previous reports.

ABSTRACT

@#AIM: To observe the changes of the vault height and the correlation analysis of visual quality under different pupil diameters after ICL surgery.<p>METHODS: Retrospective case study. Selected 43 patients(86 eyes)who implanted V4c ICL surgery successfully at Shanghai Aier Eye Hospital and followed up regularly to meet the measurement requirements from January 2018 to January 2019, Routine parameters and iTrace were checked before and 1wk, 1mo, and 3mo after surgery. Visante-OCT was used to measure the vault height under the pupils of 3mm and 5mm. <p>RESULTS: The vault height under the 3mm pupil at 1wk, 1mo and 3mo after surgery were(586.06±24.49, 560.16±21.31, 508.19±21.17)μm, the values under the 5mm pupil were(698.13±20.98, 667.69±20.16, 613.28±19.79)μm(<i>F</i>Time=40.160,<i>P</i>Time<0.001; <i>F</i>Groups=56.835,<i>P</i>Groups<0.001). The wave of vaule under the different pupils was(112.06±8.91, 107.53±6.94,101.01±10.64)μm(<i>F</i>=16.875, <i>P</i>>0.05). The total high-order aberration(HOA)and RMS of the whole eye after ICL were significantly reduced(<i>F</i>=29.847, 85.019; <i>P</i><0.001). The MTF and SR value were significantly higher than before surgery(<i>F</i>=47.653, 33.264; <i>P</i><0.001), and there was no statistical difference after ICL. At 3mo after surgery, the RMS and MTF values at 3mm and 5mm pupils were compared. The RMS value of the total HOA under the pupil of 5mm is significantly higher than that at 3mm, and the increase of spherical and trefoil aberration is significant(<i>t</i>=21.812, 17.136; <i>P</i><0.001). No significant difference in coma value(<i>t</i>=2.028, <i>P</i>>0.05). The MTF at 5mm pupils were significantly lower than those at 3mm at spatial frequencies of 5, 10, 15, 20, 25, 30c/d(<i>t</i>=15.138, 9.147, 7.018, 4.693, 3.814 and 3.075, all <i>P</i><0.05).<p>CONCLUSION: The impact of pupil movement on vault height fluctuation is about 110μm. It is recommended that the minimum vault height should be above 300μm. It can significantly reduce the root mean square value of total high-order aberration and aberration of the whole eye, and significantly increase the MTF and SR values after ICL surgery. The pupil size has a significant effect on visual quality after ICL. The spherical aberration and trefoil aberration increase under the condition of large pupil, and the MTF value generally decreases.

Treated eyes were divided into two groups, according to the preoperative spherical equivalent(SE): mild to moderate myopic group(<-6.0D, 76 eyes)and high myopic group(≥-6.0D, 53 eyes). Corneal HOAs of the anterior surface, posterior surface, and total cornea were measured by Pentacam anterior segment analysis system preoperatively and 6mo postoperatively. <p>RESULTS: The tHOAs, spherical aberrations and horizontal coma of the anterior surface and total cornea, significantly increased in both groups 6mo postoperatively(all <i>P</i><0.01). And more tHOAs, spherical aberrations and horizontal coma of the anterior surface and total cornea were induced in high myopic group than mild to moderate myopic group postoperatively(all <i>P</i><0.01). The horizontal coma of the posterior surface, significantly increased in both groups 6mo after operation(all <i>P</i><0.01). And more horizontal coma of the posterior surface were induced in high myopic group than mild to moderate myopic group postoperatively(<i>P</i><0.01). Changes in anterior surface and total corneal tHOAs, spherical aberrations and horizontal coma were related to the SE(all <i>P</i><0.01).<p>CONCLUSION:Anterior and total corneal tHOAs, spherical aberrations and horizontal coma, significantly increased after FS-LASIK, and aberration changes were related to SE. Whereas posterior corneal HOAs remained stable except horizontal coma. The long-term effect should be investigated in the future.

ABSTRACT

Objective:To compare the changes of corneal asphericity and higher-order aberrations after smart pulse technology-assisted transepithelial photorefractive keratectomy (Smart) for low and moderate myopia and to investigate the changes in the shape of the front corneal surface in patients with different diopters. Methods:A non-randomized controlled study design was used.Ninety-eight eyes of 54 patients with moderate or low myopia who underwent Smart surgery in Tianjin Medical University Eye Hospital from November 2018 to March 2019 were included. The 41 eyes of 23 patients with low myopia were set as the low-myopia group, and 57 eyes of 31 patients with moderate myopia were assigned as the moderate-myopia group. The Pentacam anterior segment analysis system was used to measure Q value, index of surface variance (ISV), corneal higher-order aberration (HOA), corneal vertical coma (Z 3-1), corneal horizontal coma (Z 31) and spherical aberration (Z 40) before surgery, 1 month and 3 months after surgery. The anterior surface morphology was compared between the low-myopia and moderate-myopia group.Pearson correlation analysis was used to analyze the correlations between measurement parameters. The study protocol was approved by an Ethics Committee of Tianjin Medical University Eye Hospital (No.2019KY-17). Written informed consent was obtained from each patient before surgery. Results:Corneal Q value, ISV, HOA and Z 40 were 0.445±0.191, 26.973±5.611, 0.671±0.142 and 0.384±0.188, respectively, in the low-myopia group at one month after surgery, which were significantly increased than corresponding preoperative values of -0.273±0.817, 13.784±2.376, 0.433±0.687 and 0.231±0.062 (all at P<0.05). Corneal Q value, ISV, HOA and Z 40 were 0.693±0.203, 34.038±5.773, 0.874±0.216 and 0.520±0.129, respectively, in the moderate-myopia group at one month after surgery, which were significantly increased than corresponding preoperative values of -0.309±0.104, 14.838±3.992, 0.409±0.081 and 0.228±0.089 (all at P<0.05). Corneal Q values, ISV, HOA and Z 40 in the moderate-myopia group were higher than those in the low-myopia group at different time points after surgery, showing significant differences between the two groups (all at P<0.05). There was no significant difference in postoperative 1-month and 3-month corneal Z 3-1 and Z 31 between the two groups (both at P>0.05). The results of correlation analysis showed that there were no significant differences in ΔQ value and ΔISV between the two groups, both of which were negatively correlated with spherical equivalent (ΔQ value: low-myopia group: r=-0.364, P=0.044; moderate-myopia group: r=-0.589, P<0.01; ΔISV: low-myopia group: r=-0.298, P=0.039; moderate-myopia group: r=-0.409, P=0.022). ΔQ value and ΔZ 40 were positively correlated in the moderate-myopia group ( r=0.348, P=0.009); there was no significant correlation between ΔQ value and ΔZ 40 in the low-myopia group ( r=0.180, P=0.266). Conclusions:The corneal high-order aberrations and ISV after Smart are increased in comparison with preoperative values in the low-myopia and moderate-myopia eyes, and the corneal Q values change from negative to positive. The effect of Smart on corneal asphericity is less in the low-myopia eyes.

ABSTRACT

Signaling pathways in innate and adaptive immunity play vital roles in pathogen recognition and the functions of immune cells. Higher-order assemblies have recently emerged as a central principle that governs immune signaling and, by extension, cellular communication in general. There are mainly two types of higher-order assemblies: 1) ordered, solid-like large supramolecular complexes formed by stable and rigid protein-protein interactions, and 2) liquid-like phase-separated condensates formed by weaker and more dynamic intermolecular interactions. This review covers key examples of both types of higher-order assemblies in major immune pathways. By placing emphasis on the molecular structures of the examples provided, we discuss how their structural organization enables elegant mechanisms of signaling regulation.

ABSTRACT

Abstract The Five-Factor Model (ffm) of personality predominates in the scientific literature, and the Big Five Inventory (bfi) is one of its main evaluations. Considering the limitations of previous studies, the aim of the present study was to explore the dimensionality of the bfi assessment instrument, to test higherorder factors and to analyze its relations with self-esteem, age, and sex of the participants. A total of 549 university students participated. The results showed adequate psychometric properties for the bfi and for the two-factor model of higher order personality traits. In addition, significant differences were observed by sex in terms of agreeableness and neuroticism, relationships between age, neuroticism, and conscientiousness, as well as relationships between both higher order factors and self-esteem.

ABSTRACT

Background: Reasoning refers to logical thinking involving problem-solving and decision-making skills. Physical Education teachers need to manage teaching and learning processes efficiently to build reasoning ability among students. The purpose of this study was to identify the students’ reasoning achievement level based on the net and wall category using RSAT in Physical Education. Methods: A quasi-experimental design was used in this study in four schools in the district of Hilir Perak, Perak. The sample of the study consisted of 240 Form 2 students who used RSAT instrument (validity, r = .78; reliability, r = .74) in the pre-test and post-test. The treatment group underwent teaching and learning programs based on the net and wall TGfU model while the control group underwent the badminton technical model. The subjects of the control group received six teaching and learning sessions based on the plans provided. Results: The pre-test result of treatment group showed overall (M = 2.21; SD = 0.30) compared to the control group (M = 2.18; SD = 0.43). The students of the treatment group (M = 3.64; SD = 0.67) showed higher mean score compared to control group (M = 2.88; SD = 0.82) in post-test. The achievement level of the treatment group students showed that the majority of respondents achieved good level (n = 85; 60.70%) while the control group achieved pass level (n = 62; 44.30%). Conclusion: The study concluded that the use of RSAT could improve the reasoning level among students holistically and comprehensively. The implication of the study suggested an RSAT instrument to be used as an alternative instrument for determining the level of the learning domain based on reasoning to achieve higher-order thinking skills.

ABSTRACT

Objective@#To evaluate the relationship between the offset of a laser-assisted flap using the WaveLight FS200 femtosecond laser and the clinical results after femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK).@*Methods@#In this prospective cohort study, 125 patients who underwent FS-LASIK for myopia by WaveLight FS200 femtosecond laser from June 2017 to July 2018 at the Tungwah Ophthalmic Center were divided into two groups according to the offset of the corneal flap from the pupil center: the no-offset group (57 eyes) and the certain-offset group (68 eyes); the baseline data, including age, sex, uncorrected visual acuity(UCVA), spherical degree, and central corneal thickness were matched in the two groups.UCVA, residual astigmatism, spherical degree, corneal curvature and aberration were observed 1 week and 1 month after surgery. The study protocol was approved by the Ethics Committee of Tungwah Hospital of Sun Yat-Sen University (No.2017DHLL004). Written informed consent was obtained from each subject prior to entering the study cohort.@*Results@#UCVA, corneal curvature, spherical degree, spherical and corneal aberration between the two groups were not significantly different (all at P>0.05). One week after surgery, the residual astigmatism was ≤-0.50 D in 50 eyes (87.7%, 50/57)and >-0.50 D in 7 eyes (12.3%, 7/57) in the no-offset group; the residual astigmatism was ≤-0.50 D in 50 eyes (73.5%, 50/68) and >-0.50 D in 18 eyes (26.5%, 18/68) in the no-offset group. The residual astigmatism between the two groups 1 week after surgery was significantly different (χ2=3.902, P=0.048), and there was no significant difference in residual astigmatism 1 month after surgery (χ2=2.031, P=0.068). The trefoil of the no-offset group was statistically less than of the certain-offset group at 1 week and 1 month after surgery (0.05[0.04, 0.08]vs.0.06[0.04, 0.10]; 0.05[0.03, 0.07]vs.0.06[0.05, 0.09])(Z=-2.245, P=0.022; Z=-2.370, P=0.018). The spherical aberration and coma were not significantly different between the two groups at 1 week and 1 month after surgery (both at P>0.05).@*Conclusions@#The offset from femtosecond laser-assisted flap by WaveLight FS200 has no effect on long-term visual acuity or residual astigmatism. Some patients may have different visual experiences because of the residual astigmatism and higher order aberration during the early postoperative stage.

ABSTRACT

@#AIM:To explore the effect of eccentricity of overnight orthokeratology(OK)lenses on 2-year eye axial growth and visual quality.<p>METHODS:Based on the degree of eccentricity of OK lenses, patients were divided into three groups: a low degree of eccentricity group(degree of eccentricity ≤0.5 mm), a group of moderate eccentricity(eccentric degree >0.5-≤1.0 mm)and a group with a high degree of eccentricity(eccentric degree >1-≤1.5 mm). The degree of eccentricity of the OK lens, spherical equivalent(SE), the uncorrected visual acuity after wearing OK lenses(UCVA), axial length before and after wearing OK lenses(AL), total higher-order aberrations(HOA), comas, and spherical aberrations(SA)for 3 mm pupils were analyzed. The difference among the three groups for all parameters was compared using the Kruskal-Wallis H Rank-Sum test.<p>RESULTS:The study retrospectively analyzed 75 cases(139 eyes). In the low eccentricity group(53 eyes), the mean age was 11.4±2.4 years, SE was -3.24±1.48 D, and AL was 24.85±1.01 mm. In the moderate eccentricity group(53 eyes), the mean age was 11.4±2.2 years, SE was -3.22±1.29 D, and AL was 25.15±0.92 mm. In the high eccentricity group(31 eyes), the mean age was 11.5±1.9 years, SE was -3.54±1.43 D, and AL was 24.95±0.84mm. After two years, there was no significant difference in the changes of the axis among the three groups(<i>P</i>=0.089). The HOA, SA, and coma in the high eccentric group were significantly higher than in the middle eccentric group(<i>P</i><0.05). The HOA, SA, and coma in the high eccentric group were also significantly higher than those in the low eccentric group(<i>P</i><0.05).<p>CONCLUSION:For OK lenses, it is unnecessary to strictly require the absolute centralization of the lens position. An unnecessary change of the lenses may delay the eye-axis control. However, the balance between axial control and visual quality should be assessed.

ABSTRACT

Purpose: To evaluate the agreement of selected higher order aberration measurements between aberrometers based on three different wavefront technologies. Methods: Twenty-three eyes of 23 participants were compared between Zywave, OPD-Scan III, and iDesign aberrometers, for total ocular aberrations. Participants were between 19 and 69 years of age, and exclusion criteria were previous ocular surgery or trauma, contact lens wear within the preceding 2 weeks, and ocular or systemic disease. Corneal aberrations were compared between the OPD-Scan III and GALILEI™ G2 aberrometers. Zernike coefficients of vertical and oblique trefoil, vertical and horizontal coma, and spherical aberration were analyzed in R software. Results: In all, 276 scans were captured in total, with a male-to-female ratio of 11:12. Total ocular vertical coma [mean difference (MD) = 0.026 ?m, P < 0.005], vertical trefoil (MD = 0.033 ?m, P < 0.05), and spherical aberration (MD = 0.022 ?m, P < 0.05) differed significantly between the iDesign and OPD-Scan III. Differences in total vertical (MD = 0.072 ?m, P < 0.05) and oblique trefoil (MD = 0.058 ?m, P < 0.05) were demonstrated between the Zywave and OPD-Scan III, and spherical aberration (MD = 0.030 ?m, P < 0.005) between iDesign and Zywave. iDesign corneal horizontal coma (MD = 0.025 ?m, P < 0.05) and spherical aberration (MD = 0.043 ?m, P < 0.005) measurements were significantly different between the GALILEI™ G2 and the OPD-Scan III. Conclusion: Zywave, iDesign, and OPD-Scan III, and GALILEITM G2 and OPD-Scan III may be used interchangeably for their total ocular and corneal wavefront functions, respectively; however, care must be taken if using these devices for guiding ablation or monitoring corneal disease.
