ABSTRACT

ABSTRACT BACKGROUND: Developmental dysplasia of the hip (DDH) encompasses a broad spectrum of hip pathologies, including femoral or acetabular dysplasia, hip instability, or both. According to the medical literature, ultrasonography is the most reliable diagnostic method for DDH. Several techniques for the assessment of hips in newborns and infants, using ultrasonography, have been described. OBJECTIVE: To compare the accuracy of the Graf technique and other diagnostic techniques for DDH. DESIGN AND SETTING: A systematic review of studies that analyzed ultrasound techniques for the diagnosis of DDH within an evidence-based health program of a federal university in São Paulo (SP), Brazil. METHODS: A systematic search of relevant literature was conducted in the PubMed, EMBASE, Cochrane Library, CINAHL, and LILACS databases for articles published up to May 5, 2020, relating to studies evaluating the diagnostic accuracy of different ultrasound techniques for diagnosing DDH. The QUADAS 2 tool was used for methodological quality evaluation. RESULTS: All hips were analyzed using the Graf method as a reference standard. The Morin technique had the highest rate of sensitivity, at 81.12-89.47%. The Suzuki and Stress tests showed 100% specificity. The Harcke technique showed a sensibility of 18.21% and specificity of 99.32%. CONCLUSION: All the techniques demonstrated at least one rate (sensibility and specificity) lower than 90.00% when compared to the Graf method. The Morin technique, as evaluated in this systematic review, is recommended after the Graf method because it has the highest sensitivity, especially with the three-pattern classification of 89.47%. ABSTRACT

Australian Cattle Dogs (ACD) are medium-sized animals widely used in fieldwork for managing cattle and sheep. There needs to be more information about the conditions these dogs can develop despite being well-characterized animals since the beginning of the 20th century. Hip dysplasia (HD) is a developmental abnormality between the femoral head and the acetabular fossa, which can be debilitating. However, the available literature has no studies on the prevalence of this condition in dogs of this breed. This study aimed to evaluate radiographs of ACD qualitatively and quantitatively. For this purpose, 49 dogs considered healthy without clinical signs of HD were radiographically assessed, and the animals were classified as dysplastic (D) and non-dysplastic (ND). A frequency of 46.9% of dysplastic dogs was observed, with males being more affected. The cortical index (CI) and angle of inclination (AI) could not differentiate D from ND animals; only the Norberg angle (NA) was effective in this differentiation. No correlation was observed between AI, CI, and AN.(AU)


Os cães da raça Autralian Cattle Dog (ACD) são animais de porte médio, muito utilizados no trabalho de campo para manejo de gado e ovelha. Apesar de serem animais bem caracterizados desde o início do século XX, há poucas informações sobre afecções que esses cães podem desenvolver. A displasia coxofemoral (DCF) é uma anormalidade do desenvolvimento entre a cabeça do fêmur e a fossa acetabular podendo ser debilitante. Contudo, não há estudos, na literatura disponível, sobre a prevalência desta afecção em cães dessa raça. O objetivo deste trabalho foi avaliar qualitativamente e quantitativamente radiografias de cães da raça ACD. Para tanto, foram avaliados radiograficamente 49 cães considerados hígidos e sem sinais clínicos de DCF. Os animais foram classificados em displásicos (D) e não displásicos (ND). Observou-se a frequência de 46,9% de cães displásicos, sendo os machos mais acometidos. O índice cortical (IC) e o ângulo de inclinação (AI) não foram capazes de diferenciar os animais D dos ND, apenas o ângulo de Norberg (AN) foi eficaz nessa diferenciação. Não houve correlação entre AI, IC e AN.(AU)

ABSTRACT Objective: This study aimed to identify the relevance of hospitalizations for congenital hip deformities in Bahia. Methods: This is a retrospective epidemiological study in public databases. Descriptors in health sciences: "congenital hip dysplasia", "congenital hip dislocation", and "congenital dislocation hip". This is qualitative-quantitative research with the analysis of secondary data and cross-sectional typologies in the databases of the Ministry of Health - Health Information (TABNET), made available by the Department of Informatics of the Unified Health System (DATASUS). Results: Bahia was the third Brazilian state with the highest number of hospitalizations, registering 1481 cases. The municipalities in Bahia with the highest prevalence were Itanhém, Salvador, and Barreiras, with 912, 445, and 20 cases, respectively. Conclusions: The elevated number of congenital hip deformities reflects a public health problem, requiring investments in public policies.


RESUMO Objetivo: Identificar a relevância das internações por deformidades congênitas do quadril na Bahia. Métodos: Este é um estudo epidemiológico retrospectivo, feito em bases de dados públicas. Descritores em ciências da saúde: "displasia congênita de quadril", "Hip Dislocation Congenital" AND "Luxação congênita de quadril". Trata-se de uma pesquisa qualiquantitativa, com análise de dados secundários e tipologia transversal nas bases de dados do Ministério da Saúde — Informações de Saúde (TABNET), disponibilizados pelo Departamento de Informática do Sistema Único de Saúde (DATASUS). Resultados: A Bahia foi o terceiro estado brasileiro com maior número de internações, registrando 1.481 casos. Os municípios baianos com maiores prevalências foram Itanhém, Salvador e Barreiras, com 912, 445 e 20 casos, respectivamente. Conclusão: O elevado número de deformidades congênitas do quadril reflete um problema de saúde pública, necessitando de investimentos em políticas públicas.

RESUMEN La displasia de cadera canina o displasia coxo-femoral (DCF) es un desorden progresivo e incapacitante en perros de razas grandes, como el Ovejero Alemán. La selección de reproductores libres de displasia es la única forma de reducir su incidencia. Se han desarrollado varios métodos de diagnóstico basados en el examen radiográfico, en base a los cuales se seleccionan los reproductores para la cría. La DCF tiene una base hereditaria poligénica e influencia ambiental, con una heredabilidad media a baja (alrededor de 0,20 a 0,40), por lo que el progreso de la selección fenotípica ha sido lento. En Argentina la prevalencia de la displasia en la raza sigue siendo alta (>25%) y es imposible prever su incidencia en la progenie del plantel de cría. Algunos países han implementado la selección basada en el valor estimado de cría, obteniendo un importante avance. Los estudios de asociación del genoma completo han revelado numerosos marcadores asociados a la DCF y se han encontrado varios genes candidatos que señalan la posibilidad de implementar una selección genómica en un futuro cercano.


ABSTRACT Canine hip dysplasia (CHD) is a progressive and disabling disorder in large dog breeds, such as the German Shepherd dog. Breeding sires and dams free of dysplasia is the only way to reduce its incidence. Several diagnostic methods have been developed based on radiographic examination, on the basis of which dogs are selected for breeding. CHD has a polygenic hereditary basis and environmental influence, with a median to low heritability (ca. 0,20 to 0,40), so the progress in phenotypic selection has been slow. In Argentina, the prevalence of dysplasia in German Shepherd dogs remains high (> 25%) and it is impossible to predict its incidence in the offspring of the breeding stock. Some countries have implemented a selection based on the estimated breeding value, obtaining an important advance. Genomewide association studies have revealed numerous CHD-associated markers and several candidate genes have been found that point to the possibility of implementing genomic selection in the near future.

SUMMARY: The aim of this study was to evaluate the relation between acetabulum morphological measurements and present the reference values of the acetabulum. The study had a retrospective design and was conducted with 234 healthy subjects (108 females; 126 males) aged 18-53 years over a period of 4 years from 2018 to 2022. Eleven measurements including the center- edge angle (CEA), acetabular angle (AA), acetabular depth (AD), acetabular width (AW), dept to width ratio (ADWR), Extrusion A (EA)-B (EB), Extrusion index (EI), the lateral subluxation (LS), peak to edge distance (PED), and roof obliquity (RO) were taken. The p<0.05 value was considered significant. A significant difference was found in CEA, AA, EB, LS, and RO values, while there was no significance in the AD, AW, ADWR, EI, and PED measurements in comparison with acetabular morphometry according to gender. Also, in the evaluation of acetabulum to age-related changes, there was a significant difference in values of the CEA, AA, AD, AW, ADWR, LS, and PED from decades 1 to 5. The knowledge of radiological acetabulum findings is paramount for the diagnosis of hip dysplasia and may be useful for prosthesis, orthopedic and forensic experts. Also, the most interesting finding was that ADWR increased based on age in a directly proportional trend. The most apparent change based on age was seen in CEA (between decades 3-4), LS (decades 1-5), PED (decades 2-4), AD, and AW (decades 2-5).


El objetivo de este estudio fue evaluar la relación entre las medidas morfológicas del acetábulo y presentar sus valores de referencia. El estudio tuvo un diseño retrospectivo y se realizó con 234 sujetos sanos (108 mujeres; 126 hombres) de 18 a 53 años de edad durante un período de 4 años, desde 2018 hasta 2022. Once mediciones que incluyeron el ángulo centro-margen (ACM), ángulo acetabular (AA), profundidad acetabular (PA), ancho acetabular (AC), relación de profundidad y ancho (RPAC), extrusión A (EA)-B (EB), índice de extrusión (IE), subluxación lateral (SL). Se midió la distancia al margen (DAM) y la oblicuidad del techo (OT). Se consideró significativo el valor de p<0,05. Se encontró una diferencia significativa en los valores de ACM, AA, EB, SL y OT, mientras que no hubo significación en las medidas de AA, AC, RPAC, IE y DAM en comparación con la morfometría acetabular según el sexo. Además, en la evaluación del acetábulo respecto a los cambios relacionados con la edad, hubo una diferencia significativa en los valores de ACM, AA, PA, AC, RPAC, SL y DAM de las décadas 1 a 5. El conocimiento de los hallazgos radiológicos del acetábulo es primordial para el diagnóstico de displasia de cadera y puede ser útil para expertos en prótesis, ortopedia y medicina forense. Además, el hallazgo más interesante fue que RPAC aumentó según la edad en una tendencia directamente proporcional. El cambio más aparente según la edad se observó en ACM (entre las décadas 3 y 4), LS (décadas 1 a 5), DAM (décadas 2 a 4), PA y AC (décadas 2 a 5).

Resumen: Introducción: La displasia del desarrollo de cadera (DDC) es el trastorno más común que afecta la cadera pediátrica; hacer tamizaje a todos los neonatos en forma clínica y utilizar ultrasonografía en forma selectiva para aquellos bebés que se encuentran con alto riesgo es una recomendación muy difundida. Nuestro objetivo es evaluar el impacto que ha tenido el diagnóstico por ultrasonografía (USG) y tratamiento temprano de la DDC en la población infantil de nuestra unidad. Material y métodos: Estudio retrospectivo, descriptivo y transversal. Se revisaron expedientes de aquellos niños de uno a seis meses de edad, con diagnóstico de DDC, sin distinción de sexo, sometidos a rastreo ultrasonográfico en el período de Enero de 2018 a Diciembre de 2019. Se realizó un seguimiento de seis meses en todos los pacientes, a partir del momento del diagnóstico y del inicio de tratamiento con arnés, visitas semanales para recolocación, así como realización de rastreos ultrasonográficos cada cuatro semanas para monitorización del tratamiento. Resultados: Se reportaron 19 casos del lado izquierdo (47.5%), 10 casos del lado derecho (25%) y 11 casos bilaterales (27.5%). Los principales factores de riesgo asociados fueron: producto de la primera gesta, antecedentes familiares de DDC, presentación pélvica, sexo femenino. Los resultados fueron favorables con un uso continuo de arnés de 23 horas, se observó una evolución satisfactoria en 99.2% de los pacientes. Conclusión: Con los resultados obtenidos podemos analizar la tasa de éxito considerable de la clínica de cadera de nuestro hospital con la realización del ultrasonido, encontramos una menor incidencia de pacientes con dolor, limitación de la función, así como patrones de marcha satisfactorios.


Abstract: Introduction: Developmental hip dysplasia (DHD) is the most common disorder affecting pediatric hip; screening all neonates clinically, and using ultrasonography selectively for those babies who are at high risk is a widespread recommendation. our goal is to evaluate the impact that USG diagnosis and early treatment of DHD has had on the child population of our unit. Material and methods: Retrospective, descriptive and cross-sectional study. Records of those children from one to six months of age, with a diagnosis of DHD, without distinction of sex, subjected to ultrasonographic tracking in the period from January 2018 to December 2019 were reviewed. A follow-up of six months was carried out in all patients, from the moment of diagnosis and the start of treatment with harness, weekly visits for relocation, as well as ultrasonographic revision every four weeks to monitor the treatment. Results: 19 cases were reported from the left side (47.5%), 10 cases from the right side (25%) and 11 bilateral cases (27.5%). The main associated risk factors were: product of the first pregnancy, family history of DHD, pelvic presentation, female sex. The results were favorable with a continuous use of harness of 23 hours observing a satisfactory evolution in 99.2% of the patients. Conclusion: With the results obtained we can analyze the considerable success rate of the hip clinic of our hospital with the realization of the ultrasound, we find a lower incidence of patients with pain, limitation of function, as well as satisfactory gait patterns.

ABSTRACT

INTRODUCCIÓN: la displasia de cadera es una de las causas más importantes y prevenibles de discapacidad durante la primera infancia. Es una enfermedad que se presenta en niños menores de 1 año, pero puede tener consecuencias importantes en la edad adulta, incluso puede desencadenar una osteoartrosis de cadera y/o invalidez en adultos jóvenes. OBJETIVO: determinar la correlación entre displasia de cadera con sus factores de riesgo en lactantes de 2 a 12 meses, evaluados en consultorio externo de Traumatología del Hospital Municipal Achacachi en el periodo de mayo ­ 2017 hasta abril ­ 2018. MATERIAL Y MÉTODOS: el tipo de investigación es descriptivo, diseño de tipo caso ­ control. Se estudiaron 128 pacientes lactantes de ambos sexos, que asistieron a su control en el consultorio de traumatología. La correlación entre factores de riesgo y displasia de cadera se determinó mediante la fórmula de Odds Ratio. RESULTADOS: el género femenino es el factor de riesgo con mayor asociación con la displasia de cadera, con un Odds Ratio de 139.04 veces más de padecer de displasia de cadera, seguida de la presentación podálica o transversa (12.58), las gestantes primíparas (4.52), los productos gemelares (3.71), los antecedentes familiares (2.54) y el Oligohidramnios (2.06). CONCLUSIÓN: existe relación importante entre los factores de riesgo (clínicos, obstétricos y fetales), con el desarrollo de displasia de cadera.(AU)


INTRODUCTION: hip dysplasia is one of the most important and preventable causes of disability during early childhood. It is a disease that occurs in children younger than 1 year, but it can have important consequences in adulthood, it can even trigger hip osteoarthritis and / or disability in young adults. OBJECTIVE: to determine the correlation between Hip Dysplasia with its risk factors in infants from 2 to 12 months, evaluated in an external Traumatology office of the Hospital Municipal Achacachi in the period from May - 2017 to April - 2018. MATERIAL AND METHODS: the type of research is descriptive, case control type design. 128 lactating patients of both sexes were studied, who attended their control in the traumatology office. The correlation between risk factors and hip dysplasia was determined using the Odds Ratio formula. RESULTS: female gender is the risk factor with the highest association with hip dysplasia, with an Odds Ratio of 139.04 times more than having Hip dysplasia, followed by breech or transverse presentation (12.58), primiparous pregnant women (4.52), twin products (3.71), Family History (2.54) and Oligohydramnios (2.06). CONCLUSIONS: there is an important relationship between risk factors (clinical, obstetric and fetal), with the development of Hip Dysplasia.(AU)

ABSTRACT

La displasia del desarrollo de la cadera (DDC) es una anomalía de la articulación coxofemoral caracterizada por una laxitud o posicionamiento anormal de la cabeza femoral con respecto al acetábulo. Es la patología ósea perinatal más frecuente, e incluye alteraciones que van desde el aplanamiento o la displasia acetabular hasta una luxación completa de la cabeza femoral fuera de la articulación, que puede comprometer el desarrollo y la estabilidad articular. Nuestro objetivo es transmitir una sistemática de estudio en la valoración de la cadera del recién nacido, haciendo énfasis en un examen físico correcto como pilar fundamental en la detección de la DDC, y orientar al pediatra en la selección adecuada del método diagnóstico complementario acorde a edad del paciente, con el fin de optimizar la detección y reducir el número de caderas luxadas en etapas tempranas y tardías, y así disminuir la incidencia de patologías asociadas desarrolladas a partir de este trastorno


Developmental dysplasia of the hip (DDH) is a hip joint anomaly that is characterized by a laxity or abnormal positioning of the femoral head with respect to the acetabulum. It is the most common perinatal pathology of the skeleton and includes a spectrum of alterations ranging from flattening or acetabular dysplasia, to a complete dislocation of the femoral head outside the joint that can compromise joint development and stability. The purpose of this presentation is to transmit a systematic study in the evaluation of the newborn's hip, emphasizing a correct physical examination as primordial in the detection of DDH. Also, guide the pediatriciain the proper selection of the complementary diagnostic method to be used according to the age of the patient, in order to optimize detection and reduce the number of dislocated hips in early and late stages as well as the incidence of related pathologies.

ABSTRACT: This study determined the canal flare index (CFI) of four dog breeds using two distinct femoral regions as a reference. Thirty-five radiographs of the hip joints of Golden Retrievers (GRG), German Shepherds (GSG), Labrador Retrievers (LRG), and Rottweilers (RG) of both sexes were used. Seventy experimental units were submitted to CFI calculation. Objective (CFIob) and subjective (CFIsub) values of the CFI of each experimental unit were determined according to the anatomical reference used for the calculation. A significant difference in the CFIob between the Golden Retriever and German Shepherd breeds (1.68 ± 0.16 and 1.49 ± 0.08), and in the CFIsub between Golden Retriever, German Shepherd, and Rottweiler breeds (2.09 ± 0.31, 1.86 ± 0.11, and 1.84 ± 0.18) was reported. The subjective form of measurement showed higher values than the objective form (GRG: 2.09 ± 0.31; GSG: 1.86 ± 0.11; LRG: 2.07 ± 0.12; RG: 1.84 ± 0.18). The CFI values of each breed were similar, suggesting a certain racial pattern. A significant difference in the interobserver assessment for both CFIsub and CFIob, in all races was observed. The CFI analysis identified morphological patterns of the proximal femur in the different races. Results indicated the need for standardization of the anatomical references used to calculate the CFI because there were statistical differences among the measurements among the observers.


RESUMO: Objetivou-se determinar o canal flare index (CFI) de quatro raças específicas de cães, utilizando-se duas regiões femorais distintas como referência. Foram analisadas 35 radiografias de articulações coxofemorais de cães das raças Golden Retriever (GGR), Pastor Alemão (GPA), Labrador Retriever (GLR) e Rottweiler (GR), de ambos os sexos, configurando 70 unidades experimentais submetidas ao cálculo do CFI. Determinaram-se os valores objetivo (CFIob) e subjetivo (CFIsub) do CFI de cada unidade experimental de acordo com a referência anatômica utilizada para o cálculo. Houve diferença significativa do CFIob entre as raças Golden Retriever e Pastor Alemão (1,68 ± 0,16 e 1,49 ± 0,08), e do CFIsub entre as raças Golden Retriever, Pastor Alemão e Rottweiler (2,09 ± 0,31, 1,86 ± 0,11 e 1,84 ± 0,18). A forma subjetiva de mensuração apresentou valores maiores que a forma objetiva (GGR: 2,09 ± 0,31; GPA: 1,86 ± 0,11; GLR: 2,07 ± 0,12; GR: 1,84 ± 0,18). Os valores de CFI de cada raça se apresentaram similares, sugerindo haver determinado padrão racial. Houve diferença significativa na avaliação interobservadores, tanto do CFIsub quanto CFIob, em todas as raças. A análise do CFI identificou padrões morfológicos do fêmur proximal nas diferentes raças. Os resultados indicaram a necessidade de padronização dos referenciais anatômicos utilizados para o cálculo do CFI, uma vez que houve relevância estatística entre as diferentes mensurações entre os observadores.

ABSTRACT

The aim of this study was to test the accuracy of a new automated computer software tool for the assessment of passive hip laxity. The hip laxity was estimated using the dedicated computer software by two blinded evaluators, one previously trained and one without specific training for distraction index measurement, in two independent sessions using 230 hip joints from 115 dogs that underwent screening for passive hip laxity using the distraction view. Previously, all of these radiographs were sent to PennHIP Analysis Center for an official distraction index record. The measurement repeatability of the two sessions was adequate for both evaluators. The reproducibility of the official distraction index measurement, mean distraction index±standard deviation 0.44±0.15, was adequate (P>0.05) for the trained evaluator, 0.44±0.15, and non-adequate (P<0.05), for the non-trained evaluator 0.47±0.17. The distraction index measurement tool proposed can be used with confidence for hip laxity evaluation by trained evaluators, as it provided good repeatability and reproducibility of official reports. The simplicity of the process described leads to a less time-consuming and more affordable procedure.(AU)


O objetivo deste estudo foi testar a viabilidade de uma nova ferramenta de software informático para avaliação da lassitude articular passiva da articulação coxofemoral. A lassitude articular da articulação coxofemoral foi estimada usando-se um programa informático especial, desenvolvido para o efeito, por dois avaliadores, um com treino prévio e outro não treinado, na medição do índice de distração, em duas sessões independentes, utilizando-se 230 articulações coxofemorais de 115 cães, as quais efetuaram o rastreio de displasia coxofemoral realizando a projeção de distração da articulação coxofemoral. Previamente, todas as radiografias foram enviadas para o PennHIP Analysis Center, para se obter uma medida oficial do índice de distração. A repetibilidade das medições das duas sessões foi adequada para ambos os avaliadores. A reprodutibilidade do índice de distração oficial, média±desvio-padrão 0,44±0,15, foi adequada (P>0,05) para o avaliador treinado, 0,44±0,15, e não adequada (P>0,05) para o avaliador não treinado, 0,47±0,17. A ferramenta de medição do índice de distração proposta pode ser usada com segurança na medição do índice de distração por avaliadores treinados, uma vez que mostra uma adequada repetibilidade e reprodutibilidade das medições oficiais do índice de distração. A simplicidade do processo apresentado torna-o menos demorado e mais econômico.(AU)

ABSTRACT

Abstract Objective The significance of pelvic osteotomies in developed coxarthrosis is still disputable. Some authors believe that incongruence and early osteoarthritis of the articular surfaces are contraindications for joint-preserving surgery and will stimulate further progression. The opposite view is that triple pelvic osteotomy can be an alternative to early joint replacement. The present study reports the mid to long term results of adolescent patients with developed coxarthrosis treated by proximal femoral and triple pelvic osteotomies and fixed by the Ilizarov technique. Methods A retrospective review between 2002 and 2014 of the treatment of 26 patients with coxarthrosis due to developmental dysplasia of the hip (DDH). The sample was composed of 22 female and 4 male subjects with a mean age at operation of 14.7 years (range: 12-18 years) and mean follow-up of 5.9 years (range: 3-13 years). Results The initial functional results according to the Merle d'Aubigné and Postel criteria were: pain - 4.3 ± 0.05 points; range of motion - 3.6 ± 0.3 points; and gait - 4 ± 0.15 points. The average index of the weight bearing zone (WBZ) was of 38.7º ± 2.721º. The acetabular coefficient (AC) was of 162 ± 6.8, the center-edge angle (CEA) of Wiberg was of 3º ± 0.2º. The outcomes were followed up from 3 to 13 years. At the final follow-up, the radiographic outcomes showed that the value of the WBZ decreased to 8.2º ± 1.293º (0-15º), and that the AC increased to 249 ± 12.05. The average neck-shaft angle (NSA) was of 115º ± 4º, the articulo-trochanteric distance (ATD) was of 8,5 ± 1,5 mm, and the CEA of Wiberg was of 28º ± 1.6º) at the final follow-up. The distribution of the joints according to Tönnis et al was: grade I - 17 joints; grade II - 8 joints; and grade III - 1 joint. The outcomes were good for 14 patients (54%), fair for 10 patients (34.5%), and poor for 2 (11.5%) patients. Conclusion The treatment of adolescent hip dysplasia requires a proper assessment of the degree of dysplasia and the surgery needed to redirect pelvic components to achieve suitable conditions for hip remodeling, and our mid- to long-term results showed very good outcomes when applying these principles using the Ilizarov technique.


Resumo Objetivo A importância das osteotomias pélvicas na coxartrose desenvolvida ainda é discutível. Alguns autores acreditam que a incongruência e a osteoartrite inicial das superfícies articulares são contraindicação para a cirurgia de preservação articular e estimularão maior progressão. A visão oposta é a de que a osteotomia pélvica tripla pode ser uma alternativa à substituição articular precoce. Este estudo relata resultados de médio a longo prazo de pacientes adolescentes com coxartrose desenvolvida, tratados por osteotomias proximais do fêmur e pélvica tripla, fixados pela técnica de Ilizarov. Métodos Revisão retrospectiva entre 2002 e 2014 do tratamento de 26 pacientes com coxartrose devido a displasia do desenvolvimento do quadril. A amostra continha 22 mulheres e 4 homens. A idade média na operação foi de 14,7 anos (variação: 12-18 anos), e o acompanhamento médio foi de 5,9 anos (variação: 3-13). Resultados Os resultados funcionais iniciais de acordo com o método de Merle d'Aubigné e Postel foram: dor - 4,3 ± 0,05 pontos; mobilidade - 3,6 ± 0,3 pontos; e marcha - 4 ± 0,15 pontos. O valor médio do índice da zona de carga (ZC) foi 38,7º ± 2.721º. O coeficiente acetabular (CA) foi de 162 ± 6,8, e o ângulo centro-borda (ACB) de Wiberg foi de 3º ± 0,2º. Os resultados foram acompanhados por 3 a 13 anos. No acompanhamento final, os resultados radiográficos mostraram que o valor do índice da ZC diminuiu para 8.2 ± 1.293º (0º-15º), e o CA aumentou para 249 ± 12,05. A média do ângulo cervicodiafisário (ACD) foi 115º ± 4º, a distância articulotrocantérica (DAT) foi de 8,5 ± 1,5 mm, e o ACB de Wiberg foi 28º ± 1,6º no acompanhamento final. A distribuição das articulações segundo Tönnis et al foi: grau I - 17 articulações; grau II - 8 articulações; e grau III - 1 articulação. O resultado foi bom em 14 pacientes (54%), razoável em 10 pacientes (34,5%), e ruim em 2 (11.5%). Conclusão O tratamento da displasia do quadril na adolescência requer uma avaliação adequada do grau de displasia e a cirurgia necessária para reorientar os componentes pélvicos com o objetivo de alcançar condições adequadas para a remodelação do quadril. Nossos resultados de médio a longo prazo mostraram desfechos muito bons ao aplicarmos esses princípios usando a técnica de Ilizarov.

Objective To investigate the clinical effect of preoperative computed tomography (CT) measurement of acetabulum femoral neck combined anteversion in primary total hip arthroplasty (THA).Methods From March 2016 to may 2018,48 patients (90 hips) with developmental dysplasia of the hip (DDH) who were treated with THA in the orthopaedic department of Jiangmen Central Hospital were measured by X-ray and spiral CT three-dimensional reconstruction technology before operation,and the best combined anteversion angle of acetabulum was determined before implementing THA,which was CT group;the remaining 42 patients were not measured by CT before operation,which was X-ray group.The difference of acetabulum cup anteversion angle,acetabulum femoral neck combined anteversion angle and preoperation angle were evaluated by three-dimensional computed tomography (3D-CT) in CT group.Results There was no significant difference in operative time,intraoperative blood loss,time of getting out of bed and length of lower limbs between the two groups (P > 0.05).In CT group,the acetabulum cup anteversion and acetabulum femoral neck anteversion were close to the pre-operative angle at 2 and 3 months postoperatively (P > 0.05).The Harris hip score of CT group was significantly higher than that of X-ray group at 2 and 3 months after operation,and the incidence of deep vein thrombosis was 6.25%,significantly lower than that of X-ray group (21.43%) (P <0.05).The incidence of dislocation,infection and periprosthetic fracture was slightly lower than that of X-ray group,but the difference was not statistically significant (P > 0.05).Conclusions For DDH patients who received THA for the first time,preoperative CT measurement of acetabulum femoral neck combined with anteversion is conducive to the formulation of the best combined anteversion adjustment scheme,which can obtain the maximum initial stability,improve the surgical effect and reduce the occurrence of dislocation of prosthesis.

ABSTRACT

Objective To explore the influence of different model scopes on acetabular stress distribution and optimal osteotomy result in preoperative planning of Bernese acetabular osteotomy. Methods Two patients with hip dysplasia were established according to different model ranges. Model 1 included the affected hip and femur, and Model 2 included the complete pelvis and affected femur. Compare and analyze the acetabular cartilage contact pressure, area and distribution of the two models under single-leg standing conditions, and simulate osteotomy. Results Compared with Model 1 before surgery, Model 2 had higher contact pressure, smaller contact area and closer distribution to the meniscus edge of acetabulum. Compared with 11 postoperative plans, the variation amplitude of contact pressure and the optimal osteotomy angle for Model 1 were all smaller than those of Model 2. Conclusions The preoperative analysis result of the model with affected hip bone and femur would underestimate the degree of stress concentration and misjudge the location of stress concentration, and the obtained optimal osteotomy rotation angle would be relatively small. The research findings provide certain theoretical basis for preoperative planning and modeling of osteotomy.

ABSTRACT

Abstract Mucopolysaccharidoses (MPS) are inborn errors of metabolism caused by deficient lysosomal enzymes, leading to organomegaly, hip osteonecrosis, coarse facial features, bone deformities, joint stiffness, cardiac and pulmonary symptoms (MPS VI) or hypermobility (MPS IVA). Some patients may present with non-classical forms of the disease in which osteoarticular abnormalities are the initial symptoms of non-classical forms. As orthopedists and surgeons are the specialists most frequently consulted before the diagnosis, it is critical that MPS may be considered as a differential diagnosis for patients with bone dysplasia. Experts in Latin America reviewed medical records focusing on disease onset, first symptoms and the follow-up clinical and surgical outcomes of non-classical MPS VI and IVA patients. All patients displayed orthopedic issues, which worsened over time, followed by cardiac and ophthalmological abnormalities. Our findings enlighten the necessity of including non-classical MPS as possible diagnosis for patients who report osteoarticular abnormalities in absence of inflammation.

ABSTRACT

The acetabulum is the large cup-shaped cavity on the lateral surface of the hip bone, which articulates with the head of the femur to form the hip joint. The morphometry and morphology of the acetabulum is important in the diagnosis and treatment of hip joint injuries and disease. Population specific differences exist regarding the acetabulum, therefore, this study aimed to document the morphometry and morphology of the acetabulum within a Black African population in Kwa-Zulu Natal, South Africa. A total of 100 dry hip bones (R= 44; L= 56) from the University of KwaZulu-Natal (Westville and NRMSOM campuses) were examined for this study. The hip bones, with documented sex, were all from the Black African population. A sliding digital caliper was used to measure the acetabular diameter, depth and acetabular notch width. The morphology of the acetabular margin was document in accordance with Govsa et al. (2005). Statistical analysis was conducted using SPSS version 24 software. A p-value of less than 0.05 was considered statistically significant. The means and standard deviations of the morphometric measurements for the entire population were as follows: (a) acetabular diameter of 54.84 ± 4.18 mm, (b) acetabular depth of 31.30 ± 3.18 mm and (c) acetabular notch width of 21.72 ± 2.98 mm. The observed shapes of the acetabular margin that were angular (41 %), curved (22 %), irregular (23 %) and straight (14 %). Knowledge of the anatomy of the acetabulum is useful to surgeons, prosthetists, anthropologists and forensic anthropology experts. The morphometry and morphology of the Black African population group of KwaZulu-Natal presented with differences when compared to other population groups studied in literature.


El acetábulo es una cavidad grande, en forma de copa, ubicada en la superficie lateral del hueso coxal, que se articula con la cabeza del fémur para formar la articulación coxofemoral. La morfometría y morfología del acetábulo son importantes en el diagnóstico y tratamiento de las lesiones y enfermedades de las articulaciones de la cadera. Existen diferencias específicas de la población con respecto al acetábulo, por lo tanto, este estudio tuvo como objetivo documentar la morfometría y la morfología del acetábulo en una población Africana negra, perteneciente a Kwa-Zulu Natal, Sudáfrica. Para este estudio se examinaron un total de 100 huesos coxales secos (Right = 44; Left = 56) de la Universidad de KwaZulu-Natal (campus de Westville y NRMSOM). Los huesos coxales, con sexo documentado, eran todos de población africana negra. Se utilizó un calibrador digital deslizante para medir el diámetro acetabular, la profundidad y el ancho de la incisura acetabular. La morfología del margen acetabular anterior se documentó de acuerdo con Govsa et al. (2005). El análisis estadístico se realizó utilizando el software SPSS versión 24. Un valor de p inferior a 0,05 se consideró estadísticamente significativo. Las medias y desviaciones estándar de las medidas morfométricas para toda la población fueron las siguientes: (a) diámetro acetabular de 54,84 ± 4.18 mm, (b) profundidad acetabular de 31,30 ± 3,18 mm y (c) ancho de la incisura acetabular de 21,72 ± 2,98 mm. Las formas observadas del margen acetabular fueron angulares (41 %), curvas (22 %), irregulares (23 %) y rectas (14 %). El conocimiento de la anatomía del acetábulo es útil para cirujanos, antropólogos y expertos en antropología forense. La morfometría y morfología del grupo poblacional de África negra de KwaZulu-Natal presentaron diferencias cuando se compararon con otros grupos poblacionales estudiados en la literatura.

ABSTRACT

Canine hip dysplasia (CHD) is a common condition observed in the surgical clinics for small animals. Among the surgical techniques for management of CHD, triple pelvic osteotomy and sacroiliac wedge promote acetabular lateral axial rotation (ventroversion), increasing acetabular coverage and joint stability. The present study aimed to evaluate radiographically, by measuring the Norberg angle (NA) and the acetabular coverage percentage (ACP), the acetabular ventroversion induced by the sacroiliac wedge technique, with or without pelvic osteotomies; we also checked the feasibility of wedges made of polyamide with an angulation of 20° and 30°. The software used to measure NA and ACP was AutoCAD® 2009. Pelves from 10 canine corpses were evaluated radiographically at four time-points: M0 (Control Group), M1 (wedges of 20° and 30°), M2 and M3 (wedges associated with bilateral pubis and ischium osteotomies, respectively). There was no significant increase in the acetabular ventroversion at M1, M2, and M3. The polyamide sacroiliac wedge technique proved to be feasible, stable, and easy to apply. Further, the software proved to be efficient and easy to use for NA and ACP measurements. In the present study, even in the cases of non-dysplasic adult canine corpses, it was concluded that the sacroiliac wedge technique does not require to be accompanied by pubis and ischial osteotomies because they did not significantly increase the NA and ACP.(AU)


A displasia coxofemoral (DCF) é afecção comum na clínica cirúrgica de pequenos animais. Entre as técnicas cirúrgicas para controle da DCF, a osteotomia pélvica tripla (OPT) e a cunha sacroilíaca (CSI), promovem rotação lateral acetabular no eixo axial (ventroversão), aumentando a cobertura acetabular e a estabilidade da articulação. Desta forma, o presente estudo objetivou avaliar radiograficamente, por meio da aferição do ângulo de Norberg (NA) e da porcentagem de cobertura acetabular (PCA), a ventroversão acetabular induzida pela técnica da cunha sacroilíaca, associada ou não às osteotomias pélvicas, além de verificar a exequibilidade das cunhas confeccionadas de poliamida com angulação de 20ο e 30ο. O software utilizado para aferir o AN e o PCA foi o AutoCAD® 2009. Dez pelves de cadáveres caninos foram avaliadas radiograficamente em quatro momentos: MO (Grupo Controle), M1 (cunhas de 20ο e 30ο), M2 e M3 (cunhas associadas à osteotomia bilateral do púbis e ísquio, respectivamente). Não houve aumento significativo da ventroversão em M1, M2 e M3. A técnica de cunha sacroilíaca de poliamida mostrou-se exequível, estável e de fácil aplicação. Não obstante, o software utilizado mostrou-se eficiente e de fácil utilização nas aferições do AN e PCA. Neste estudo, mesmo tratando-se de cadáveres de cães adultos e de maioria não displásicos, concluiu-se que a utilização da técnica de cunha sacroilíaca não necessita de associação à ostectomia púbica e a osteotomia do ísquio por não promoverem aumento significativo do AN e da PCA.(AU)

PURPOSE: The current study aimed to evaluate the results of ultrasound screening for developmental dysplasia of the hips (DDH) done at various weeks of life, to determine the earliest time that ultrasound screening can be performed reliably. METHODS: In this 17-year cohort study, all neonates who underwent ultrasound screening prior to the 12th week of life with subsequent follow-up radiography done at 1 year of life were included. The ultrasound images were evaluated according to the Graf classification, Harcke’s dynamic ultrasound screening method, and Terjesen’s femoral head coverage method. The radiographic images were evaluated according to the acetabular index and the femoral head position. The accuracy and correlation between the ultrasound findings from various weeks of life with the radiographic findings at 1 year of life were evaluated. RESULTS: A total of 348 neonates were included in the study, of whom 92 had abnormal ultrasound findings and 42 had abnormal radiographic findings at 1 year. Significant differences were identified between the findings of ultrasound screening examinations performed prior to the fourth week of life (day 21 and before) and the radiographic findings at 1 year of life (P0.05). The accuracy of ultrasound screening was 79.2% or higher when performed during or after the fourth week of life (day 22 and beyond). CONCLUSION: The earliest that ultrasound screening for DDH can be performed reliably is during the fourth week of life (day 22 and beyond).

ABSTRACT: The purpose of this study was to describe our initial experience with double pelvic osteotomy (DPO) in young dogs affected by hip dysplasia (HD) and to report their postoperative outcome. Seven dogs (four females and three males) were included in our study with mean age 8.3 (±1.7) months, and mean body weight 29.5 (±7.4)Kg. Breeds involved were: Rottweiler (n = 1), Labrador Retriever (n = 3), Golden Retriever (n = 1), Great Dane (n=1) and São Miguel Cattle Dog (n = 1). The most common history complaints were: pelvic limb lameness and pain at hip extension and hip abduction. All surgical procedures consisted of osteotomy of the ilium and pubis, acetabular ventroversion and iliac stabilization with a customized bone plate with seven screws, four screws placed at the cranial fragment and the remaining three in the caudal aspect. Average surgical time was 65.8 (±10.4) minutes and median follow-up assessment was 68 (±15) days. Fracture healing was observed within mean period of 26.3 (±8.9) days. Six patients (86%) had satisfactory outcome and one patient didn't improve after surgery and had to undergo a total hip replacement. Our results showed that DPO is an effective treatment for HD due to the preservation of pelvic geometry and low postoperative morbidity. Since it is a recent technique, further studies are recommended.


RESUMO: O objetivo do estudo é a descrição da osteotomia dupla da pelve (ODP) em pacientes com displasia coxofemoral (DCF) e relatar a evolução pós-operatória. As raças acometidas foram: Rottweiler (n=1), Labrador (n=3), Golden Retriver (n=1), Dogue alemão (n=1) e Fila de São Miguel (n=1). O total foi de sete animais, sendo três machos e quatro fêmeas; a média de idade e peso dos cães foi de 8,3±1,7 meses e 29,5±7,4Kg, respectivamente. As queixas iniciais mais comuns antes da cirurgia foram: claudicação em membro pélvico e dor à extensão e abdução do quadril. A cirurgia constou em realizar a osteotomia do ílio e púbis, rotacão axial do acetábulo sobre a cabeça do fêmur e osteossíntese do ílio com a utilização de placa fixada com sete parafusos, quatro destes no segmento cranial e três no segmento caudal. Após a cirurgia um animal não apresentou melhora do quadro, caracterizado por agravamento da subluxação coxofemoral, sendo submetido à prótese total de quadril. O tempo médio de cirurgia foi de 65,8±10,4 minutos e o período de acompanhamento médio pós operatório foi de 68,0±15,0dias, visibilizando a reação osteoproliferativa no foco de osteotomia ilíaca em um período médio de 26,3±8,9 dias. Dos sete animais envolvidos no estudo, seis tiveram recuperação satisfatória após o primeiro procedimento no período avaliado. Os resultados apontam a técnica de OPD como procedimento eficiente no tratamento de DCF devido à preservação da geometria pélvica e baixa morbidade pós-operatória. Por se tratar de uma recente técnica, ainda pouco difundida, é interessante a realização de maiores estudos.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the clinical effect of ceramic on ceramic total hip arthroplasty(THA)in Crowe IV developmental dysplasia of the hip(DDH).</p><p><b>METHODS</b>From April 2008 to December 2015, 137 hips of 111 Crowe IV DDH patients received THA using Forte or Delta ceramic on ceramic by one senior surgeon, which consists of 85 unilateral hips and 26 bilateral hips. The average age of the patients was(38.88±10.83) years old ranging from 18 to 68 years old. The mean follow-up was(41.16±21.50) months ranging from 12 to 96 months. All the patients were evaluated by Harris Hip Score. Radiographic evaluations were made preoperatively and during follow-up. Harris scores, the incidence of complications such as ceramic fracture, squeaking, dislocation were observed.</p><p><b>RESULTS</b>The mean preoperative Harris score was 56.54±15.67, the mean postoperative Harris score was 88.30±6.86(=0.017). Periprosthetic osteolysis was not deteced around any cup. No ceramic fracture occurred. There were 3 cases of revision surgery due to infection, losening of the stem and limb length discrepancy, respectively; 3 cases of dislocation occurred. Seventy-seven patients were recorded the gait and the hip mobility, the hip flexion of 69 patients were above 120 degrees.</p><p><b>CONCLUSIONS</b>Ceramic on ceramic bearing showed an encouraging result in Crowe IV DDH total hip arthroplasty.</p>
