ABSTRACT

Objective To investigate the application values of S100-β protein, homocysteine (Hcy) and high-sensitive C-reactive protein (hs-CRP) in the diagnosis of acute ischemic stroke(AIS).Methods A total of 73 patients with AIS were enrolled in this study. The Non-stroke patients with similar symptoms were selected as control group (n =50).The relationship between levels of S100-β protein,Hcy and hs-CRP and infarct volume was evaluated. The receiver operating characteristic (ROC) curve was used to evaluate the diagnostic values of single and combined detection of these indicators in AIS.Results The levels of S100-β protein,Hcy and hs-CRP were higher in AIS patients than those in control group after adjusting the major vascular risk factors (P < 0.05).There were significant differences in S100-β protein,Hcy and hs-CRP between different groups according to the infarct volume (P < 0.05).S100-β protein,Hcy and hs-CRP positively were correlated with infarct volume (r =0.625,P =0.000;r =291,P =0.012;r =0.396,P =0.001 respectively).ROC curves showed that the diagnostic performances for AIS were as follows:S100-β protein (AUC =0.809,sensitivity:65.8%,specificity:82.0%),Hcy (AUC =0.775,sensitivity:54.8%,specificity:84.0%),hs-CRP (AUC =0.698,sensitivity:58.9%,specificity:78.0%) and Combined detection (AUC =0.878,the sensitivity:68.5 %,specificity:94%).Conclusions S100-β protein,Hcy and hs-CRP protein can be used as biomarkers for AIS,which can help the diagnosis of AIS.S100-β protein can be used as a biomarker to evaluate the severity of AIS,Hcy and hs-CRP can also reflect the severity of AIS to some extent.Single detections of S100-β protein,Hcy and hs-CRP show limited value in the diagnosis of AIS.Combined detection gains a high specificity, but sensitivity is not very high.

ABSTRACT

Objective To investigate whether apathy during the acute stage of stroke was related to glycated hemoglobin and homocysteine(Hcy) and functional disability, and to elucidate possible factors that are related to poststroke apathy (PSA).Methods Totally 298 patients with acute cerebral infarction who met the inclusion and exclusion criteria were recruited from the Fourth Affiliated Hospital of China Medical University. The control group of 298 volunteers was recruited from medical center in our hospital during the same period. The apathy scale (AS) was used to assess PSA within 2 weeks after stroke.Patients with an AS score≥ 14 points were defined as PSA,and the patients were divided into apathy group (A group) and non-apathy group (NA group) according to AS score.Modified Rankin Scale (mRS) was used to assess functional disability.Functional disability in daily living activities was assessed with the Barthel Index(BI).Serum levels of Hcy,glycated hemoglobin A1C (HbA1 c),lipid series and fibrinogen of patients were determined. Results The prevalence of PSA in patients was significantly higher than the control group (P < 0.01).In the case group,A group had an older age, higher mRS score, higher Hcy level, lower educational level, lower BI score, and lower HbA1C level than the NA group(P <0.05).In the A group, the mRS score was positively correlated with the AS score(B =1.508,P =0.003),serum Hcy level was positively correlated with the AS score(B =0.170,P =0.013),the HbA1C level was negatively correlated with the AS score(B =-0.872,P =0.031).Conclusions The serum Hcy level,HbA1C level and functional disability are closely related to apathy during the acute stage of ischemic stroke.

ABSTRACT

Objective To investigate the changes of serum ferritin and homocysteine of diabetic patients,to explore its correlation with diabetic gastroparesis. Methods Pure 50 patients with diabetes,50 patients with diabetic gastroparesis, and 50 cases of normal control group were chosen. The serum ferritin levels were measured with chemiluminescence method, the serum homocysteine levels were measured with enzyme linked immunosorbent assay (ELISA) method. The changes of serum ferritin and serum homocysteine in each groups were observed. Its relationship with diabetic gastroparesis was studied. Results For all diabetic patients, the serum ferritin and homocysteine levels were significantly higher than those in normal control group (P < 0.01).Patients with diabetic gastroparesis was higher than those of pure diabetes (P < 0.01).Serum ferritin levels was positively correlated with glycosylated hemoglobin and homocysteine (r =0.62,0.78,P < 0.01).Multi-factor analysis showed that serum ferritin, and homocysteine level was positively associated with diabetic gastroparesis (P < 0.05).Conclusions The increases of serum ferritin and homocysteine levels were closely related to the occurrence and development of diabetic gastroparesis. They can be used as one of parameters of diabetic gastroparesis, and provide theoretical basis for clinical intervention therapy.

ABSTRACT

Objective To investigate the correlation between serum homocysteine (HCY) and chronic heart failure (CHF) hypercoagulable state in patients. Methods A total of 105 cases of patients with CHF was divided into three groups according to the New York Heart Association (NYHA) classification standard functions:heart functional grade Ⅱ group (42cases),cardiac function grade Ⅲ group (35 cases) and,NYHA class Ⅳ group (28cases).At the same time,40 healthy individuals were regard as the control group.HCY,fibrinogen (Fbg),D-dimer (DDI),HCY,N-terminal pro-brain natriuretic peptide (NT-proBNP) were detected by fasting venous blood samples which were collected within 24 hours after admission. Results Compared to the control group, the expression of Fbg,DDI,HCY and NT-proBNP increased, whereas, antithrombin Ⅲ (AT-Ⅲ) was reduced.Fbg,DDI,HCY,NT-proBNP,and AT-Ⅲ were found in all patient cases.Four groups were compared with each other, except for cardiac function Ⅱ group and the normal group had no significant difference between them (P > 0.05),the difference between both other groups was significantly different (P < 0.05),HCY had a positive correlation with Fbg,DDI,and NT-proBNP (r =0.268,0.295,and 0.404,P < 0.05),and negative correlation with AT-Ⅲ (r =-0.240,P < 0.05).Conclusions HCY might be a reliable indicator as a judge of CHF patients with hypercoagulable state,to detect HCY,FBG,DDI,and AT-Ⅲ in CHF patients. It benefits for judging thrombosis risk and determining the severity of the diseases.Anticoagulant therapy might be beneficial to reduce the long-term adverse events.
