ABSTRACT

Objective To analyze and minimize iatrogenic hospital acquired conditions (HACs). Methods " Admission conditions" in homepage of medical records was studied to collect iatrogenic problems between 2014 and 2015. Self-inspection of physicians was made to identify causes of iatrogenic HACs for improvement measures. The post-improvement HACs data ( 2015 ) were compared with that of before (2014). Results Physicians came to learn following the self-examination the importance of pre-admission communications, doctor-patient communications and cooperation between the departments in less HACs. Occurrence of HACs in 2015 was significantly lower than 2014. Conclusions Enhanced analysis and management of HACs, especially for iatrogenic problems, significantly contribute to less HACs.

ABSTRACT

Hospital-acquired conditions (HACs)are key to patient safety.This paper introduced a new classification system of HACs,the CHADx model,and described the principle and method of establishing the model as well as the problems found during its use.The model can provide a basis for studying the problem of incidence,causes and influence factors of hospital acquired conditions,so as to help medical institutions to explore the causes of HACs and to ensure patient safety and improve the quality of health care.

ABSTRACT

Hospital acquired conditions(HACs)are a common headache for hospitals,which often give rise to waste of medical resources and additional payment of medical insurance funds,as well as deterioration of patient safety and doctor-patient relations.These aftermaths have made HACs a high priority in hospitals management worldwide,as they are tightening control of HACs,reducing medical resource waste,improving quality of care,and enhancing patient safety.That is why HACs are so important for us.
