ABSTRACT

Objective:To develop a scientific and comprehensive job competency evaluation scale for pre hospital emergency nurses, and to test its reliability and validity.Methods:Based on the theory of nurse medical quality responsibility model, the first draft of the scale was formed through literature review, Delphi method, and other research methods. A questionnaire survey was conducted among 563 pre hospital nurses from 6 tertiary A hospitals in Shandong Province. SPSS 27.0 statistical software was used for factor analysis to test the reliability and validity of the scale.Results:The final competency evaluation scale for pre hospital emergency nurses included 6 dimensions and 40 items. Cronbach′s α coefficient of the total scale was 0.921, and the Spearman Brown split half reliability coefficient was 0.756. Conclusion:The job competency evaluation scale for pre hospital emergency nurses constructed in this study has good reliability and validity, and can be used to evaluate the job competency of pre hospital emergency nurses.

ABSTRACT

<p><b>Objective: </b>This study aimed to clarify the circumstances and challenges in home care-oriented hospital nurse training on the basis of results of hospital nurse self-evaluations of nursing activities implemented in preparation for home care.</p><p><b>Methods: </b>Group interviews were conducted with 13 nurses at 9 different hospitals, and the results were qualitatively and inductively analyzed.</p><p><b>Results and Discussion: </b>Six categories for actual nursing activities implemented in preparation for home care were extracted. These categories were "I leave home care to other medical professionals," "No form of inter-hospital cooperation has been established," "There is disparity in cooperation with home-visiting nurses," "Social resources are not being used due to a lack of knowledge," "Able to develop a perspective of home care through workshop participation, years of experience, and interest," and "Competency is lacking despite knowledge of home care." Home care was apparently not being utilized by hospital nurses due to a lack of knowledge of social resources and a lack of interdisciplinary cooperation.</p><p><b>Conclusions: </b>Challenges in nurse training implemented in preparation for home care included compensating for the lack of practical knowledge of social resources and devising a duty system and inter-facility exchanges to allow nurses to build experience in other areas and departments. The results of this study suggested the need for organizational training initiatives and for improvement of the quality of individuals in order to put acquired knowledge into practice.</p>

ABSTRACT

<p><b>Introduction:</b> High-quality, community-based dementia care requires a comprehensive, holistic approach. This study aimed to identify the barriers to achieving efficient cooperation and coordination among medical professionals, care managers, and medical social workers, and to improve the management model of community-based, integrated dementia care.</p><p><b>Methods:</b> We collected qualitative data through three focus group discussions at JA Konan Kosei Hospital. Thirteen participants (four directors of nursing service departments, three chief nurses, four medical social workers, and two care managers) were recruited for the discussions. The data were analyzed using an inductive, multi-step approach referred to as the qualitative content analysis.</p><p><b>Results:</b> Nine themes arose as follows: little attention given to patient wishes, lack of time and space to provide high-quality care, disturbing hospital environment, poor compensation for staff members, refusing to visit outpatient clinics, declined admission, daily life support by family members and caregivers, dementia care team, and community bonding.</p><p><b>Conclusion:</b> The participants wanted to launch a dementia care team in their hospital to improve the care environment and the quality of dementia-specific care. The study also suggested that advance care planning could be systematically implemented in clinical practice as a way to honor the decisions made by dementia patients.</p>

ABSTRACT

PURPOSE: This study was conducted to identify the effects of organizational health and patient safety culture on nursing activities for patient safety as perceived by hospital nurses. METHODS: A self-report survey was administered to staff nurses of one advanced general hospital and two general hospitals in South Korea. Of the questionnaires, 188 were analyzed. RESULTS: Organizational health had a significantly positive correlation with patient safety culture (r=.52, p<.001) and patient safety nursing activities (r=.31, p<.001). CONCLUSION: The findings in this study indicate that among the factors influencing patient safety nursing activities, organizational factors were more important than individual factors, and organizational health had a big effect on patient safety nursing activities.

ABSTRACT

PURPOSE: This study was to examine the relationships between nurse's perception of patient safety culture and performance for safety nursing activities at rehabilitation hospitals. METHODS: This study applied a descriptive research design. Participants were 194 nurses who have provided nursing services for more than 6 months at 4 rehabilitation hospitals located in B metropolitan city. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffé test, Pearson's correlation coefficients and multiple regression. RESULTS: There was a positive correlation between the awareness of patient safety culture and safety nursing activity. Multivariate analysis showed that hospital work environment, experience of education, hospital climate, frequency of reported events, and marital status were significantly associated with the safety nursing activity. Overall, approximately 23.1% of total variability in the safety nursing activity could be explained by the 5 variables (R²=0.231, p<.005). CONCLUSION: Nurses at rehabilitation hospitals are relatively positive about patient safety culture. Therefore, we need to develop safety education programs at the level of organization in order to improve patient safety through performing effective safety nursing activities in addition to increase awareness of patient safety culture among nurses. Furthermore, we need hospital's strategies at the system level for open communication and outcome reports regarding patient safety.

ABSTRACT

PURPOSE: The purpose of this study is to examine the effective factors of organizational commitment among hospital nurses. Method: The subjects were 250 registered nurses working in 3 hospitals in Busan. Data were obtained by self-reported questionnaires from the 10th of June to 30th of June, 2009. Data were analyzed by frequency and percentage, t-test, ANOVA and Sheffe's test and stepwise multiple regression. Result: Mean of job satisfaction was 60.56+/-9.25 (range of scale: 20-100), internal marketing was 69.49 (range of scale: 25-125), job stress obtained 100.60+/-17.93 (range of scale: 45-180), and organizational commitment got 47.78+/-6.84 (range of scale: 15-75). The extent of the organizational commitment had differences according to the age (F=3.300, p=.039), educational level (F=3.21, p=.042), religion (F=3.30, p=.021), position (F=5.837, p=.003), and career length (F=2.642, p=.035). The influencing factors in organizational commitment of hospital nurses were job satisfaction, internal marketing, and age. R2=.503. The job satisfaction has the highest significant predictor of organizational commitment. CONCLUSION: The influencing factors on organizational commitment were job satisfaction, internal marketing and age. Thus it is needed to improve the job satisfaction and internal marketing of hospitals' nurses and to provide care and support to younger nurses.

ABSTRACT

PURPOSE: The present study was intended to measure fatigue symptoms according to the quality of sleep among nurses working for an university hospital and to reveal its related factors. METHODS: The self-administered questionnaires were given to 397 nurses employed in an university hospital located in Daejeon City during the period from June 1st to July 31st, 2007. RESULTS: The group with higher quality of sleep accounted for 78.1% of all the subjects and the one with lower quality, 21.9%. According to qualities of sleep, the fatigue symptom was determined to be significantly higher in the group with lower quality of sleep than its counterpart. On adjustment for quality of sleep, the level of fatigue symptom was significantly higher in the groups, than in each respective counterparts, who don't take regular exercises, who have few leisure time, who have lower level of subjective health status, who range> or = 1 and < 10 years in job careers, who work in shifts, who reported that it is hard to get physical burden of work, who are satisfied with the job, who find themselves unfit to the job, who are considering quitting the job, who have higher job demand, who have lower job control, who have higher level of supervisor support, who have type A behavior pattern. CONCLUSION: The study results may explain that the fatigue symptom of nurses is highly associated with the quality of sleep independently of other variables and also with job-related characteristics independently of the sleep quality. In conclusion, to reduce the fatigue symptom of nurses, strategic efforts should be directed for programs to improve the quality of sleep and a proper job analysis.
