ABSTRACT

As lesões na face decorrentes de mordedura humana estão entre os tipos de trauma causados por violência interpessoal. Injúrias causadas nesta região podem gerar uma série de morbidades como perdas teciduais importantes, infecções, além de cicatrizes irreversíveis quando a ferida é passível de reparo, portanto, os cuidados pós-trauma devem visar à minimização de cura, além da funcionalidade, buscando o máximo de estética possível. Este trabalho tem como objetivo relatar um caso clínico de mordida humana em região de dorso nasal, com avulsão da ponta nasal, atendida no Hospital da Restauração Governador Paulo Guerra, Recife-PE, pelo serviço de Cirurgia Buco-Maxilo-Facial. Procedeu-se com reparo primário e antibioticoterapia profilática, no qual o resultado do tratamento foi considerado satisfatório, boa cicatrização tecidual e sem queixas funcionais e estéticas... Injuries to the face resulting from human bite are among the types of trauma caused by interpersonal violence. The injuries caused in this region, can generate a series of morbidities such as major tissue loss, infection, and irreversible scarring when the wound is repairable, therefore, posttrauma care should aim to minimize healing, in addition to functionality, seeking maximum aesthetics as possible. This paper aims to report a clinical case of human bite in the nasal dorsum region, with avulsion of the nasal tip, attended at the Hospital da Restauração Governador Paulo Guerra, Recife-PE, by the service of Oral and Maxillofacial Surgery. Las lesiones faciales producidas por mordeduras humanas se encuentran entre los tipos de traumatismos causados por la violencia interpersonal. Las lesiones causadas en esta región pueden generar una serie de morbilidades como la pérdida de tejidos importantes, la infección y la cicatrización irreversible cuando la herida es reparable, por lo tanto, los cuidados posttrauma deben tener como objetivo minimizar la cicatrización, además de la funcionalidad, buscando la máxima estética posible. Human bite injuries of the eyelids are particularly rare. In most reported cases of human bites to the eyelids associated with tissue loss, the avulsed tissue was viable and available for reconstruction. ABSTRACT

ABSTRACT Animal bites are a common problem in the emergency room. There are many reports of isolated cases (mainly of domestic mammals); however, texts with more comprehensive and general guidance on these kinds of bite injuries are necessary, including those caused by wild mammals, reptiles, and even fish. This review aims to update knowledge on this problem, which includes human and domestic and wild animal bites. Health teams in emergency care centers should be aware of the initial and late measures required to control this problem.

Introdução: As marcas de mordida são lesões que contribuem para a identificação de agressores, vítimas e criminosos, possuindo valor probatório em processos judiciais por apresentarem características peculiares de cada indivíduo, embora não apresentem imutabilidade e perenidade, salvo quando registradas fotograficamente. Objetivo: Verificar o potencial de autoimagens de marcas de mordida a partir de aparelhos celulares, tanto para identificação de agressores, bem como estimar o respectivo tempo de percepção da unicidade. Metodologia: A pesquisa consistiu na participação de 6 (seis) voluntários, sendo definido por sorteio que um destes seria a suposta vítima e outro o agressor. Após sorteio, o agressor desferiria uma mordida na porção medial do antebraço esquerdo da vítima. Foram feitas fotografias desta mordida com a câmera do celular e registrados os tempos com intervalos de 5 minutos até 40 minutos. Após esta etapa, foram realizadas fotografias dos arcos dentais superiores e inferiores dos 5 (cinco) suspeitos. O confronto dessas imagens com a autoimagem do agredido foi feito por comparação direta e sobreposição, utilizando um software livre. Resultados: Após a comparação dos arcos dentais dos suspeitos com as lesões, foi possível identificar o agressor. Conclusão: No presente estudo piloto, observou-se que as imagens fotográficas feitas até quinze minutos após a mordida mostraram características anatômicas suficientes para realizar o confronto e identificar o ofensor. Passados 20 minutos da agressão, não mais havia características lesivas na pele que embasassem o requisito da unicidade.


Introduction: Bite marks are injuries that contribute to victims and criminals identification, having probative value in lawsuits, since they present characteristics of each individual, even though they don't show immutability and permanence, except when photographed. Objective: Verify the potential of self-images of bite marks from mobile devices, for identification of aggressors and to estimate the respective time of perception of the unicity. Methodology: This research consists of the participation of 6 (six) volunteers, being defined by raffle that one will be the victim and another the aggressor. After this definition, the offender will bite on the medial portion of the victim's left forearm. Photographs of this bite were taken with the camera of the cell phone and the times registered with time spaces of 5 minutes to 40 minutes. After this step, photographs of the upper and lower arches of the 5 (five) suspects were taken. The confrontation of these images with the self-image of the assaulter was done by direct comparison and overlap using Image J (free software). Results: After comparing the arches of the 5 suspects, it was possible to identify the aggressor. Conclusion: In the present pilot study, it was observed that the photographical images taken up to fifteen minutes after the bite showed sufficient anatomical characteristics to compare and identify the offender. After 20 minutes of the picture being taken, there were no more harmful skin characteristics that would support the uniqueness requirement

Introduction: Physical violence became an important global problem in the last decades. This scenario is reflected in the increasing jurisprudence. In specific situations, perpetrators and victims of physical violence may inflict an especial patterned injury named bitemark. Knowing the case characteristics in lawsuits involving human bites and bitemarks is important to understand and encourage the role of Forensic Dentists as expert witness for bitemark analysis in Court. Objectives: The present study aimed to assess the jurisprudence to screen lawsuits that involve human bites and bitemarks in order to extract characteristics that could support research and practice of bitemark analysis. Material and methods: A systematic search was conducted in an open access jurisprudence system with the keywords "bite" and "bitemark". Full-texts of lawsuits were selected and underwent data extraction. Results: A total of 2488 lawsuits were detected, from which 1125 were eligible in the present study. The lawsuits dated between 1997 (n=13, 1.15%) and 2015 (n=202, 17.95%). Forensic expertises were performed in 641 (56.97%) lawsuits, but the identity of suspects was investigated only in 3 (0.26%) cases. Conclusions: An increasing trend of lawsuits involving human bites and bitemarks was observed. Forensic Dentists must be aware of the current bitemark scenario as well the limitations inherent to bitemark analysis to face casework in the field. The present study provides evidence to encourage major education in bitemark analysis.


Introdução: Ao longo dos anos a violência física vem se tornando um grande problema global que tem mostrado seu reflexo no aumento do número de jurisprudências envolvendo o assunto. Em alguns casos o agressor e a vítima podem produzir o mesmo padrão de injúria física, como a marca de mordida. Sabendo que as marcas de mordida e mordeduras humanas podem estar presentes em casos que envolvam ações judiciais é muito importante entender e incentivar a atuação do perito odontolegista para análise das marcas de mordida em processos judiciais. Objetivo: Analisar acórdãos que estivessem relacionados às marcas de mordidas e que pudessem subsidiar a rotina de marcas de mordida e a prática de análise dessas marcas. Materiais e métodos: Foi realizada uma busca sistematizada de acórdãos no Tribunal de Justiça de São Paulo utilizando as seguintes palavras-chave: mordida, dentada e mordedura. Apenas os acórdãos que estavam disponíveis em inteiro teor foram avaliados para captação dos dados que foram tabulados em uma planilha própria da pesquisa. Resultados: Foram detectados 2488 acórdãos, dos quais 1125 foram compatíveis com os critérios de elegibilidade, e datados entre os anos de 1997 a 2015. A análise pericial foi realizada em 641 (56,97%) casos, sendo o agressor identificado pela mordida em apenas 3 (0,26%) casos. Conclusão: Houve um significativo aumento em processos judiciais envolvendo marcas de mordida. Os odontolegistas devem estar cientes da ocorrência das marcas de mordida no cenário das agressões físicas bem como as limitações inerentes à análise dessas marcas frente ao campo de atuação dos peritos odontolegistas. O presente estudo fornece evidências para incentivar o aumento do estudo da análise das marcas de mordida.

ABSTRACT

Human bite facial injuries pose a challenge to the reconstructive surgeons. The site of defect, the three dimensional nature of defect and the need for restoration of the cosmesis of facial unit involved is technically demanding. We describe our experience of 26 patients of human bites to the face with soft tissue loss requiring reconstruction using one or the other modality over a period of 4 years presenting to our hospital.

Human bite injuries are one of the commonest bite injuries seen in the emergency room in a developing country. This bite injury can occasionally lead to devastating complications but yet, one of the most neglected injuries by both the victims and inexperienced attending Clinician. We present a case of fatal and extensive necrotising fasciitis by Eschcerecia coli following human bite by a schizophrenic patient.

ABSTRACT

A 22-year-old man underwent an operation for posterolateral fusion of the lumbar spine at L3-5. He was ventilated via a tracheostomy site in a prone position for 210 minutes. Ventilator function and eyeballs were checked periodically. After changing his position to supine for the wake-up test, it was noticed that his tongue was self-inflicted and looked to be cut unless immediate decompression was applied. After several manual attempts to open the mouth failed, anesthesia depth was deepened with thiopental sodium and neuromuscular blocker to decompress and reposition the tongue into the intraoral cavity. Minimal teeth marks and scarring remained after seven months without any complications.

As mordeduras representam uma lesão comum geralmente vista nas emergências dos hospitais, correspondendo a cerca de 1% dos atendimentos. Os autores conduziram um estudo para avaliar a conduta de fechamento primário das mordeduras na face dos pacientes atendidos na emergência do Serviço de Cirurgia Plástica do Hospital Regional da Asa Norte (HRAN, Brasília-DF) de janeiro a dezembro de 1999. O estudo compreendeu 42 pacientes, com média de idade de 17 anos (variação de um a 50 anos) e 29 (69,0%) dos pacientes eram homens. As mordeduras caninas foram as mais freqüentes (71,4%), seguidas pelas mordeduras humanas (26,2%). O tempo médio entre a agressão e o atendimento no Serviço de Cirurgia Plástica do HRAN foi de 16 horas. A orelha foi o sítio principal (40,0%), seguido pelo lábio. O tratamento utilizado foi a sutura direta em 28 (66,7%) pacientes, retalhos locais em 12 (28,6%) pacientes e enxerto em dois casos. Não houve infecção nos casos estudados. Os achados sugerem que o fechamento imediato das lesões na face é seguro, até em casos após várias horas da lesão.


Mammalian bite wounds represent a common type of injury usually seen in the emergency room, constitutingapproximately 1% of visits to emergency departments. The aim of this study was to evaluate the management of the bites injuries to the face presenting to Department of Plastic Surgery, Regional Hospital of North Wing, Brasília, Brazil, from January to December, 1999. There were 42 patients, with a mean age of 17 years, ranging between 1 and 50 years: 29 (69,0%) patients were males. Dog bites were responsible for 71,4% of all bite wounds, following by human bites (26,2%). The mean duration of injury before presentation was 16 hours. The ear was the most common site of injury (40,0%), followed by the lips. Surgical treatment consisted of primary closure, either by direct suturing (66,7%), local flap (28,6%) or skin grafting (4,7%). There was no infection in this study. The results indicate that immediate closure ofbite injuries is safe, even with older injuries.
