ABSTRACT

Bronchopulmonary dysplasia(BPD)is a common chronic respiratory disease, with high mortality and poor long-term prognosis in surviving children, and there is currently no effective intervention.Breastfeeding reduces the incidence of BPD.These depend on a variety of bioactive substances in human breast milk, such as immunoglobulins, oligosaccharides, exosomes, etc.Exosomes are nano-like vesicle bodies secreted by cells, which participate in information transmission and functional regulation between cells.They have high affinity for the body and can tolerate digestion of gastrointestinal enzymes and stomach acid.These enable them to target lung, liver and other organs smoothly.They are expected to become a new method for the prevention and treatment of BPD.

ABSTRACT

PURPOSE: We report a case of toxic keratoconjunctivitis resulting from the self-application of human breast milk as a traditional folk remedy for allergic conjunctivitis. CASE SUMMARY: An 82-year-old woman presented with pain and conjunctival hyperemia in the right eye that had been worsening for three days. Two months previously, she was treated with antiallergic eye drops for allergic conjunctivitis at another eye clinic. However, the symptoms did not improve. She applied her daughter-in-law's breast milk into her right eye as a folk remedy for three days. The pain and conjunctival hyperemia worsened. At the initial visit, her corrected visual acuity was 0.3 in the right eye. Slit lamp examination demonstrated conjunctival hyperemia, punctate epithelial erosion at the central cornea, corneal keratic precipitates and white-colored deposits in the peripheral cornea combined with irregularly shaped small nodules. There was no anterior chamber inflammation. There was no medical history of rheumatoid arthritis or tuberculosis. Blood tests for serum and other infectious and inflammatory levels for infection and inflammatory markers were performed followed by application of topical steroids and antibiotics with artificial tears. After 3 weeks of treatment, conjunctival hyperemia and corneal deposits had almost resolved and best-corrected visual acuity improved to 1.0. CONCLUSIONS: The self-application of human breast milk may cause toxic keratoconjunctivitis. Therefore, efforts should be made, actively, to inform and educate the elderly in rural areas not to use human breast milk as a folk remedy.

ABSTRACT

Human breast milk stem cells (hBSCs) contain a population of cells with the ability to differentiate into various cell lineages for cell therapy applications. The current study examined the differentiation potential of hBSCs into hepatocytes- like cells. The cells were isolated from the breast milk and were treated with hepatogenic medium containing hepatocyte growth factor, insulin-like growth factor and dexamethasone for 7 days subsequently; Oncostatin M was added to the culture media. RT-PCR and immunocytochemistry were performed to detect the hepatogenic markers. The glycogen storage and the ability of the cells to absorb and release indocynanin green were also tested. The data showed that most of the differentiated cells formed cell aggregates after the 30th day, with more cells accumulated to form spheroids. RT-PCR revealed the expression of the hepatic nuclear factor, albumin, cytokeratin 18 and 19, cytochrome P2B6, glucose-6-phospahtase and claudin. The functional assays also showed glycogen storage and omission of indicynine green. Our study demonstrated hBSCs are novel population that can differentiate into hepatocyte-like cells.

ABSTRACT

PURPOSE: We aimed to compare the clinical findings, including morbidity, duration of parenteral nutrition, and length of hospital stay between very low birth weight infants (VLBWIs) fed bovine milk-based formula (BOV) and VLBWIs fed human breast milk (HBM) in a neonatal intensive care unit (NICU). METHODS: VLBWIs admitted to the NICU of Dongsan Medical Center, Keimyung University, were enrolled. Infants born from March to August 2014 (n=28) were fed BOV (the BOV group), and those born from September to December 2014 (n=18) were fed HBM (the HBM group). Pasteurized (heating at 62.5degrees C for 30 minutes) donor human milk was used if the mother's own milk was not available because of insufficient breast milk production. RESULTS: The gestational age (28.0+/-1.7 weeks vs. 27.8+/-1.4 weeks) and birth weight (1,055+/-265 g vs. 1,175+/-187 g), of the infants in the BOV and HBM, groups were similar. In addition, perinatal characteristics were similar between the groups. The duration of parenteral nutrition (36.4 days vs. 24.1 days, P=0.038), length of hospital stay (74.3 days vs.61.1 days, P=0.037), and incidence of nosocomial sepsis (53.6% vs. 22.2%, P=0.035), significantly differed between the BOV and HBM groups. Furthermore, the frequency of feeding intolerance was higher in the BOV group than in the HBM group, but this difference was not significant. Perinatal cytomegalovirus infection was not detected in any of the infants fed pasteurized donor human milk. CONCLUSION: Human-breast-milk-based diet for VLBWIs significantly reduces the incidence of nosocomial sepsis, duration of parenteral nutrition, and length of hospital stay.

ABSTRACT

OBJETIVOS: Avaliar a composição de ácidos graxos do leite humano maduro de mulheres residentes em área distante da costa litorânea brasileira. MÉTODOS: Amostras de leite materno maduro foram obtidas de 47 mulheres lactantes com idade entre 18 e 35 anos, que tiveram partos a termo e em aleitamento exclusivo ou predominante. A coleta de leite se deu a partir da 5ª semana pós-parto, por meio de ordenha manual. A composição de ácidos graxos do leite foi determinada por cromatografia gasosa. RESULTADOS: Verificou-se que a concentração de eicosapentaenoico (0,08%) foi superior ao observado em estudos brasileiros prévios. Entretanto, o teor de docosahexaenoico (0,09%) encontrado no leite humano foi um dos menores já verificados no mundo. O teor de ácidos graxos trans (2,05%) foi similar ao relatado em estudos nacionais prévios à obrigatoriedade de declaração do teor deste em rótulos de alimentos, sugerindo que esta medida não surtiu efeito na redução de seu teor na dieta habitual das mulheres. CONCLUSÕES: Baixo teor de docosahexaenoico e elevada concentração de ácidos graxos trans foram verificados no leite materno maduro de mulheres residentes em área distante da costa litorânea brasileira.


OBJECTIVES: To evaluate the fatty acid composition of mature human milk of women living far from the coastal area of Brazil. METHODS: Mature breast milk samples were obtained from 47 lactating women aged between 18 and 35 years, who delivered their babies at term and who exclusively or predominantly breastfed. Milk collection took place after the fifth week postpartum by hand expression. The fatty acid composition of the milk was determined by gas chromatography. RESULTS: It was observed that the concentration of eicosapentaenoic acid (0.08%) was higher than that observed in previous studies in Brazil. However, the content of docosahexaenoic acid (0.09%) found in human milk was one of the lowest verified in the world. The content of trans fatty acids (2.05%) was similar to that reported in national studies previous to the mandatory declaration of this fatty acid content in food labels, suggesting that this measure had no effect on reducing the content of this fatty acid in the usual diet of women. CONCLUSIONS: Low levels of docosahexaenoic acid and high concentrations of trans fatty acids were observed in mature breast milk of women living far from the coastal area in Brazil.

ABSTRACT

El objetivo del estudio fue evaluar la capacidad inmunomoduladora de dos cepas de L. paracasei (66 y 71) y una de L. rhamnosus (75) aisladas de leche materna y heces de lactante, en Mérida, Venezuela, caracterizadas como potencialmente probióticas en estudios previos. Las bacterias fueron administradas a ratones BALB/c en el agua de bebida durante 7 días. Con los fluidos intestinales se realizaron pruebas de ELISA para determinar los valores de las citoquinas IFN-γ e IL-10. Se realizó la coloración de hematoxilina-eosina a cortes histológicos del intestino delgado para observar posibles reacciones inflamatorias. Para el análisis estadístico, se utilizó la prueba de ANOVA de una vía, seguido del test post hoc de Tukey, considerando a p<0,05 como significativo. Se observaron incrementos significativos, por encima del grupo control, en las concentraciones de IFN-γ e IL-10 en el fluido intestinal de los ratones alimentados con las cepas 71 y 75. No se observaron reacciones inflamatorias en el intestino delgado. Al inducir un aumento en la producción de las citoquinas, estas cepas fueron capaces de estimular el sistema inmune manteniendo la homeostasis, por lo que pueden ser utilizadas como adyuvantes del sistema inmune intestinal, sin producir efectos secundarios indeseables.


The objective of this study was to evaluate the immunomodulating capacity of two L. paracasei strains (66 and 71), and one L. rhamnosus (75) isolated from breast milk and lactating children feces in Mérida, Venezuela, characterized as potentially probiotic in previous studies. The bacteria were administered to BALB/c mice in their drinking water during 7 days. Intestinal fluids were tested by ELISA to determine IFN-γ and IL-10 cytokines values. Histological sections of the small intestine were stained with hematoxilin-eosin to detect possible inflammatory reactions. The statistical analysis was done by a one way ANOVA, followed by the Tukey post hoc test, considering p<0.05 as significant. There were significant increases, as compared with the control group, of INF-γ and IL.10 concentrations in the intestinal fluid of mice fed with strains 71 and 75. There were no inflammatory reactions of the small intestine tissue. By inducing an increase in cytokine production, these strains were capable of stimulating the immune system maintaining homeostasis, due to which they can be used as adjuvant of the immune intestinal system, without producing undesirable side effects.

ABSTRACT

BACKGROUND: Lactobacillus gasseri BNR17 is a type of probiotic strain isolated from human breast milk. A study was reported regarding the fact that BNR17 was an inhibitor of obesity and diabetic activities in the human body through previous animal experiments. This study was furthered to investigate the effect of BNR17, a probiotic strain isolated from human breast milk, on obese and overweight adults. METHODS: Sixty-two obese volunteers aged 19 to 60 with body mass index > or = 23 kg/m2 and fasting blood sugar > or = 100 mg/dL participated in a placebo controlled, randomized, and double-blind trial. For 12 weeks, 57 participants were given either placebo or BNR17 and were tested by measuring body fat, body weight, various biochemical parameters, vital signs, and computed tomography at the start of the study and at weeks 4, 8, and 12. The subjects assumed usual daily activities without having to make behavioral or dietary modifications during the course of the study. RESULTS: At the 12th week, a slight reduction in body weight was noted in the BNR17 group, but there were no significant weight changes between groups. Decrease of waist and hip circumferences in the BNR17 group was more pronounced than those in the placebo group. The two groups had no special or severe adverse reactions. CONCLUSION: Despite there being no change in behavior or diet, administration of only the supplement of BNR17 reduced weight and waist and hip circumference. However, there were no significant differences between the two groups. These findings warrant a subsequent longer-term prospective clinical investigation with a large population.

Objective Since there are significant variation of the dietary structure recent years in China,it is necessary to re-investigate the fatty acid composition of human breast milk for the presentation of the latest data of fatty acid composition in China. Methods Using a gas chromatography GC-2010,the composition of fatty acids was detected in the human colostrums and the mature breast milk(consecutively from postnatal day 1 to day 7 and from postnatal day 42)obtained from 62 healthy postpartum women in Shanghai and Chongqing,two big cities of China,from Jan to July,2008. Results The level of total fatty acid(TFA)tended to increase significantly from the colostrums to the mature breast milk. No significant difference in the level of TFA was found between two cities. The significantly higher monounsaturates(MUFA)level(44.06% vs. 33.85%,P < 0.01)and lower linoleic acid(LA,C18 : 2n-6)level(18.43% vs. 27.62%,P < 0.01)of the mature breast milk were observed in Chongqing women compared with those in Shanghai women. The docosahexenoic acid(DHA)level of the mature breast milk in Shanghai women was significantly higher than that in Chongqing women(0.41% vs. 0.29%,P < 0.01). There was no significant difference in the level of arachidonic acid(AA,C20 : 4n-6)between two cities. Conclusions The fatty acid composition in human breast milk tends to vary with the extension of the lactation. There is significant difference in the fatty acid compositions in human breast milk between Shanghai and Chongqing owing to different dietary habits in the different regions of China.

Objective: Because the analysis of polychlorinated biphenyls (PCBs) in 1970’s was performed using a packed column gas chromatograph with an electron capture detector to determine total-PCB concentration, analytical data obtained by this analysis do not meet the requirement for the risk assessment of PCB congeners. In this context, the present study was carried out to reevaluate the congeneric analysis data by analyzing breast milk fat specimens that have been kept frozen. Methods: PCB congeners in human breast milk were analyzed by high-resolution gas chromatography/high-resolution mass spectrometry (HRGC/HRMS) using a capillary column for the selected ion monitoring (SIM) of PCBs. Results: Twelve major PCB congeners were detected in breast milk. The concentrations of all congeners of PCBs in breast milk were found to decrease annually, and their changes over time differed greatly for each congener. Between 1973 and 2000, almost no changes in the proportions of the 12 congeners of heptachlorinated biphenyls (HpCBs) and hexachlorinated biphenyls (HxCBs) were found whereas a marked and a slight decrease in the proportion of pentachlorinated biphenyls (PeCBs) and tetrachlorinated biphenyls (TeCBs), respectively, were observed. Conclusions: The PCB contamination of the Japanese population is estimated to be largely caused by seafood intake, but the proportions of PCB congeners in fish commodities were different from those in breast milk. The absorption, metabolism and therefore the accumulation of PCBs in the human body differ greatly depending on the congener.
