ABSTRACT
Objective:To observe the effects of self-designed Yiqi Huoxue Huayu Decoction on cardiac function and serum endothelin (ET-1) and matrix metalloproteinase-9 (MMP-9) levels in patients with chronic heart failure (GHF) complicated with atrial fibrillation.Methods:A total of 100 patients with GHF complicated with atrial fibrillation treated in our hospital from January 2019 to June 2021 were selected as the study subjects, and divided into experimental group and control group according to random number table method, with 50 patients in each group. Patients in both groups were given cardiotonic, diuretic, vasodilator and other conventional treatment, and patients in the experimental group added self-designed Yiqi Huoxue Huayu Decoction. TCM syndrome score, left ventricular end diastolic diameter (LVEDD), left ventricular end systolic diameter (LVESD) and left ventricular ejection fraction (LVEF), serum ET-1 and MMP-9 levels, clinical efficacy, safety and incidence of adverse reactions were observed and compared between 2 groups before and after treatment.Results:The total response rate was 82.0% (41/50) in the observation group and 62.0% (31/50) in the control group, and the difference was statistically significant ( χ 2=4.96, P=0.026). After treatment, the symptoms scores of palpitation, shortness of breath, fatigue, chest and hypochasm pain in the observation group were significantly significantly lower than those in the control group ( t=5.28, 5.29, 5.62, 5.42, P<0.01). After treatment, the LVEDD[(51.23±6.59)mm vs. (55.65±6.17)mm, t=3.46], LVESD[(43.10±4.76)mm vs. (45.99±5.31)mm, t=2.87], serum ET-1[(65.79±8.29)μg/L vs. (79.83±10.08)μg/L, t=7.61], MMP-9 [(175.86±24.81)ng/L vs. (189.49±26.13)ng/L, t=2.68] in experimental group were significantly lower than those in the control group ( P<0.05), while LVEF [(50.01±7.6)% vs. (46.25±6.96)%, t=2.57] was significantly higher than that of the control group ( P<0.05). There were no significant differences in GPT, GOT, UA, SCr levels between both groups before and after treatment ( P>0.05). There were no obvious adverse reactions betewwn both groups. Conclusion:The self-designed Yiqi Huoxue Huayu Decoction can relieve the clinical symptoms, improve the heart function and serum ET-1 and MMP-9 levels in patients with GHF complicated with atrial fibrillation safely.
ABSTRACT
Objective To investigate the effect of the promoting blood circulation to Huoxue-Huayu decoction combined with carbogen for diabetic retinopathy (DR). Methods A total of 76 DR patients who met the inclusion criteria were divided into 2 groups by random number table method, 38 cases in each group. Both groups were given the routine hypoglycemic treatment, and the control group added the treatment of calcium oxybenzosulfonate tablets, wihle the treatment group added the Huoxue-Huayu decoction. Both groups were treated for 2 months. The FPG, 2 hPG and HbAlc were detected by automatic biochemical analyzer. The serum levels of hypersensitive c-reactive protein (hs-CRP) and Serum cystatin C (Cys C) were detected by immuno-transmission turbidimetry. The serum homocysteine (Hcy) was determined by radioimmunoassay. The serum transforming growth factor β1 (TGF-β1) and il-6 were detected by the ELISA. The serum glutathione peroxidase (GSH-Px) was detected by colorimetry. The Fluorescence fundus angiography was used to detect the area of macular edema, the number of microangiomas and the area of bleeding focus. The Visual field defect was examined under normal pupils to evaluate the clinical efficacy. Results The total effective rate was 84.2％(32/38) in the treatment group and 60.5％ (23/38) in the control group, with statistically significant difference (χ2=5.331, P=0.021). After treatment, the serum levels of FPG and HbA1c in the treatment group were significantly lower than those in the control group (t values were 2.059 and 2.081, respectively, all Ps<0.05). The Serum Hcy, Cys C, hs-CRP, TGF-β1, IL-6 levels in the treatment group were significantly lower than those in the control group (t values were 4.575, 2.799, 19.520, 6.614, 10.644, P<0.05 or P<0.01), while the GSH-Px levels were significantly higher than the control group (t=10.483, P<0.01). Compared with the control group after treatment, the number of microangiomas in the treatment group decreased, the area of bleeding focus decreased, the gray scale value of the visual field decreased, and the area of macular edema decreased (t values were 7.543, 3.923, 4.957 and 6.691, all Ps<0.01). Conclusions The Huoxue-Huayu decoction combined with carbogen can reduce the degree of visual field defect , reduce the inflammatory reaction and improve the clinical effect for DR patients.

AIM To investigate the clinical effects of Huoxue Huayu Decoction (Angelicae sinensis Radix,Saposhnikoviae Radix,Schizonepetae Herba,etc.) combined with musculus pterygoideus lateralis closure in managing patients with temporomandibular joint dysfunction syndrome (TMJ) due to wind-cold stasis.METHODS From January 2015 to December 2015,68 cases of patients with TMJ due to wind-cold stasis identified and recruited by the department of stomatology of Hebei Provincial Hospital of Traditional Chinese Medicine were randomly assigned to treatment group and control group,each with 34 cases and given the musculus pterygoideus lateralis closure.But the treatment group was administered with an extra two-week adjuvant therapy of Huoxue Huayu Decoction,and generally one week such medication was taken as a therapeutic course.RESULTS After the intervention,the treatment group presented an overall better performance than the control group in terms of the open limit,NRS score,and the differences were statistically significant (P ＜ 0.05);and the values of CMI,PI,DI (P ＜0.05),in contrast to the not significantly varied values between the groups before the intervention (P ＞ 0.05).The treatment group also displayed its advantage if evaluated by TCM criteria,extent of the mandibular movementabnormalities,facial pain,temporomandibular joint sound or noise,irritability,sleeping difficulty,dark purple lips,tongue scores and wind-cold stasis scores (P ＜ 0.05).And generally a remarkably higher total effective rate was observed in the treatment group,and its difference to the control group was statistically significant (P ＜0.05).CONCLUSION Huoxue Huayu Decoction combined with conventional western medicinal approach in the management of TMJ due to wind-cold stasis can achieve a better therapeutic efficacy.

Background Hypertension,ischemia and hypoxia induce the increase of glutamate level in eye tissue and furthermore produce excitatory damage and apoptosis of retinal ganglion cells(RGCs).At present,the study on the protection of traditional Chinese medicine on glutamate-induced retinal excitatory damage is lack.objective Present study was to explore the protective effects of Huoxue huayu decoction,a Chinese herbal recipe,on RGCs in acute ocular hypertension model. Methods Fony-five healthy clean Wistar rats were divided into three groups randomly.The acute ocular hypertension models were established in 40 Wistar rats by injecting the normal saline solution into the anterior chamber to elevate the intraocular pressure for 60 minutes.Huoxue huayu decoction of 4 ml was administered intragastrically once per day in 20 model rats.Other matched 5 normal Wistar rats served as normal control group.The rats were sacrificed on 1,3,7 and 14 days after modeling and the retinas were isolated for the histopathologieal and uhrastructure examination.Expression of glutamate transporter in the retina of acute ocular hypertension and normal rat retina were detected using quantitative real-time polymerase chain reaction.The utilization of animals followed the Association for Research in vision and Ophthalmology. Results After acute ocular hypertension.the retina thickness attenuated and the numbers of RGCs decreased under the light microscope in 1,3,7 and 14 days after modeling in comparison with normal control rats.The degradation of the organelle and edema as well as changes of cell nuclei were seen in model rats under the transmission electron microscopy.The expression of glutamate transporter mRNA in model group and glutamate transporter group was rapidly elevated in the first day (P<0.05),descended at the third days(P<0.01)and returned to the normal level 7 days later(P>0.05).Conclusion Huoxue huayu decoction can protect the retina against RGCs damage in the acute ocular hypertension by elevating the expression of glutamate transporters in retina.

Objective To observe the clinical effect of Yiqi Huoxue Huayu Decoction in the treatment of unstable angina pectoris.Methods 74 cases of unstable angina pectoris were randomly recruited into a treatment group and a control group.The control group was treated with conventional therapy and the treatment group was treated with Yiqi Huoxue Huayu Decoction in addition to convention therapy.Both groups were treated for one course of treatment of 30 days,Symptoms of angina pectoris and electrocardiogram were observed before and after the treatment.Results The total effective rate of symptoms and electrocardiogram improvement were 94.44%and 80.56%respectively in the treatment group,compared with 76.32%and 52.63%in the control group,showing a significant difference(P＜0.05).Conclusion Yiqi Huoxue Huayu Decoction is effective in Creating unstable angina pectoris.
