ABSTRACT

Objective:To establish a standardized information management system (IMS) for preserving, managing, querying, and performing statistics on biospecimens and their clinical data, which is conducive to improving the utilization of biobank.Methods:Under the premise of ensuring operating environment and data security, a database-based data logic relationship model is created and applied to the IMS to manage and analyze biospecimens and their supporting clinical information of patients enrolled in the biobank of our center.Results:To ensure the establishment of the follow-up cohort, biospecimens and clinical information of inpatients and outpatients were continuously collected in the biobank of our center. Since December 2014, more than 270 000 biospecimens from inpatient, outpatient, and scientific research have been preserved. The IMS optimized by this model efficiently completes the basic work of the biobank. At the same time, the data can be queried jointly and in batches, and then converted into a report format for statistical analysis.Conclusions:The IMS of our center is suitable for application and popularization as a construction and management model for the hospital-level biobank, which meets the daily work of the biobank and diverse research needs, and provides a convenient platform and rich resources for the development of precision medicine.

ABSTRACT

Objective:To assess the psychometric properties of the Chinese version of 12-item immediate mood scale(IMS-12) in patients with depression.Methods:From January to June 2018, a total of 459 patients with depression recruited from an outpatient clinic by convenient sampling approach.All the subjects were assessed by the Chinese version of the IMS-12, and 43 of them were assessed again at the end of the first week.The 16-item quick inventory of depressive symptomatology (QIDS-SR16), and the generalized anxiety disorder scale-7(GAD-7) were used as validity indicator.The factor structure of the scale was evaluated by exploratory and confirmatory factor analyses.The internal consistency of the Chinese version of the IMS-12 scale was evaluated by Cronbach’s alpha coefficient.The intraclass correlation coefficient (ICC) was used to evaluate test-retest reliability.Pearson’s correlation coefficient was used to evaluate calibration validity.The softwares of SAS 9.4 and Mplus 8.5 were used for statistical analysis.Results:The exploratory factor analysis indicated that the fitting result of the two-factor model was good(including depression and anxiety factors). The results of the confirmatory factor analysis indicated that the factor model fit well and met the reference standard ( χ2/ df=2.82, CFI=0.936, TLI=0.920, RMSEA=0.088). The Cronbach’s alpha coefficient of the Chinese version of the IMS-12 was 0.95, and the ICC for test-retest reliability was 0.85.The correlation coefficients of the total IMS-12 score with the QIDS-SR16 score and with the GAD-7 score were 0.69 and 0.70, respectively. Conclusion:The Chinese version of the IMS-12 has good reliability and validity and is suitable for the clinical assessment of depressive and anxiety symptoms in patients with depression.

ABSTRACT

ObjectiveTo compare the effects of different drying methods on volatile components of Pseudostellariae Radix. MethodThe samples were dried by different methods, including air drying, sun drying, hot air drying (40, 60, 80 ℃) and vacuum freeze drying. Gas chromatography-ion mobility spectrometry (GC-IMS) was used to compare the changes of volatile components in the samples after different treatments. The samples were incubated at 80 ℃ and 500 r·min-1 for 15 min, the injection temperature was 85 ℃, the injection volume was 200 μL, the flow rate of carrier gas was from 2 mL to 150 mL during 20 min, and the temperature of IMS detector was 60 ℃. SE-54 capillary column (0.32 mm×30 m, 0.25 μm) was used, the column temperature was 60 ℃, and the analysis time was 35 min. The differential spectra of volatile components were constructed and analyzed by principal component analysis (PCA). ResultA total of 37 volatile components were identified from dried Pseudostellariae Radix. The number of compounds in descending order was ketones, aldehydes and alcohols. There were some differences in the volatile components in samples dried by different methods. And the volatile components in samples with sun drying, air drying and hot air drying at 40 ℃ were similar, compared with other drying methods, vacuum freeze drying and hot air drying at 80 ℃ had great effects on the volatile components of Pseudostellariae Radix, and the compounds in the samples with vacuum freeze drying were the least. ConclusionIn this study, GC-IMS for the detection and analysis of volatile components in Pseudostellariae Radix is established, which has the characteristics of high efficiency, nondestructive inspection and simple sample processing. This method can be used for the distinction of Pseudostellariae Radix dried by different methods. And hot air drying at 40 ℃ can effectively retain the volatile components of Pseudostellariae Radix, and achieve similar flavor to samples with sun drying and air drying.

ABSTRACT

Objective:The volatile components of Rhododendri Mollis Flos were determined and the differences of volatile components at different flowering stages were compared and analyzed. Method:Gas chromatography-ion mobility spectrometry (GC-IMS) was used to detect the volatile components in Rhododendri Mollis Flos at different flowering stages (bud stage, initial flowering stage, half-flowering stage, blooming stage and late blooming stage). GC-IMS spectra combined with cluster analysis, principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA) were used to compare the differences and similarities of volatile components in different flowering stages. Result:A total of 70 volatile components in Rhododendri Mollis Flos at different flowering stages were detected, among which 67 were common components, and 47 were identified qualitatively, mainly alcohols, esters and aldehydes. Carveol was a special component at the late blooming stage. The content of alpha-terpineol is the highest at the initial flowering stage, but not at the blooming stage and late blooming stage. The relative contents of the active ingredients [6-methyl-5-hepten-2-one, nonanal, alpha-terpineol, 1,8-cineole, linalool oxide, 1-octen-3-ol, (<italic>E</italic>)-3-hexenol] showed a decreasing trend during flowering stages. GC-IMS spectra showed that the samples at different flowering stages had their own characteristic peak regions, and also had common regions. The results of cluster analysis, PCA and OPLS-DA all showed that the samples at different flowering stages were distinguishable. OPLS-DA was used to screen 19 different components to distinguish different flowering stages, including <italic>γ</italic>-butyrolactone, 1,8-cineole, ethyl hexanoate, etc. Conclusion:Rhododendri Mollis Flos samples at different flowering stages can be distinguished obviously, and the active substances in the volatile components are gradually dissipated with the degree of flower opening, which can provide reference for the improvement of material basis and the study of different flowering stages of Rhododendri Mollis Flos.

ABSTRACT

Objective:To compare the effects of different drying methods on the chemical constituents of Trichosanthis Fructus. Method:Trichosanthis Fructus was dried by means of air drying, sun drying, hot air drying (40, 60, 80 ℃) and variable temperature drying (50-80, 80-50 ℃). The contents of nucleosides and flavonoids in Trichosanthis Fructus peels and seeds treated by different methods were compared by high performance liquid chromatography (HPLC), mobile phase was acetonitrile-0.2% acetic acid aqueous solution (3∶7) (A)-acetonitrile (B) for gradient elution (0-15 min, 97-95%B; 15-30 min, 95%-90%B; 30-35 min, 90%-87%B; 35-40 min, 87%-86.5%B; 40-48 min, 86.5%-97%B; 48-50 min, 97%B), the detection wavelength was 260 nm, and the flow rate was 0.4 mL·min<sup>-1</sup>. Gas chromatography-ion mobility spectrometry (GC-IMS) was used to compare the changes of volatile components in the samples treated by different treatments. The volatile components were incubated on a SE-54 capillary column (0.32 mm×30 m, 0.25 μm) at 80 ℃ and 500 r·min<sup>-1</sup> for 15 min, the injection temperature was 85 ℃, the injection volume was 400 μL, the analysis time was 35 min, carrier gas was high purity nitrogen, the flow rate of carrier gas was 2.0 mL·min<sup>-1</sup>, the flow rate of drift gas was 150 mL·min<sup>-1</sup>, and the temperature of IMS detector was 45 ℃. Result:The contents of uridine, adenosine and adenine were higher after hot air drying at >50 ℃. Low temperature drying was conducive to maintaining the stability of cytidine, cytosine, rutin, luteolin and 2ʹ-deoxyadenosine. GC-IMS technology could realize the analysis and identification of Trichosanthis Fructus samples after different treatments. There were more volatile components after hot air drying at 80 ℃ and variable temperature drying. Conclusion:Hot air drying at 40 ℃ and 60 ℃ can retain nucleosides and flavonoids, and the volatile components are similar to those in traditional drying methods, which has the advantages of high efficient, controllable and suitable for industrial production.

ABSTRACT

Specific microbiological parameters and the presence of Salmonella spp. were investigated in 72 chicken meat samples (36 wings and 36 drumsticks) collected from markets and butcher shops. The specific microbiological parameters were determined using a conventional cultural method and the presence of Salmonella spp. in chicken samples was determined using conventional and immunomagnetic separation (IMS)-polymerase chain reaction (PCR) methods. In addition, antimicrobial susceptibility of the isolates was revealed using the Kirby-Bauer disc diffusion method. The results indicated that 30 of the 72 samples were positive for Salmonella spp. by the conventional method, and 42 of the 72 were positive by the IMS-PCR method. However, 30 of the 72 samples were positive for Salmonella spp. by both methods. The Salmonella spp. isolates were confirmed by the VITEK2 Compact System and PCR. The susceptibilities of the isolates against 10 antibiotics were determined. The results indicated that isolates (27/30) showed the highest susceptibility to gentamycin (90.00%), while the highest resistance was to nalidixic acid and tetracycline at the 100 and 93.34% levels, respectively. These results indicate a high prevalence of Salmonella spp. in poultry meat from Erzurum city, Turkey, and the antimicrobial resistance profile of these isolates should be considered for public health. The results also show that the IMS-PCR technique was superior to the conventional method for detecting Salmonella in poultry meat.KEYWORDS:Chicken meat. Salmonella. IMS. PCR. Antimicrobial. INTRODUCTION Chicken is one of the most popular food products worldwide, because of nutritional, sensorial and economic factors. Chicken is widely consumed in homes and fast-food establishments, but can become contaminated during processing. The contamination of poultry products with Salmonella and other microorganismsis due to unhygienic conditions during the production, processing, distribution, marketing and preparationstages (DOOKERAN et al., 2012). The genus Salmonella includes short rod-shaped, facultative anaerobe, Gram-negative bacteria. Warm-blooded animals and humans are natural hosts for Salmonella spp. Detecting Salmonella spp. during production and before consumption is important to prevent food-borne salmonellosis. A Salmonella infection in humans is usually caused by consuming undercooked meat or other cross-contaminated foods, such as vegetables,milk and eggs (HASSANEIN et al., 2011). According to a report published by the Centres for Disease Control and Prevention (CDC), it is estimated that about 1.2 million people in the US have been exposed to Salmonella infections, and that an average of 23.000 hospitalisations and 450 deaths occur from these infections. The prevalence rates of Salmonella spp. in chicken meat sold in Turkey are 34-68.75%. Not only in Turkey, but in most developing countries, the absence of an epidemiological surveillance system for salmonellosis cases makes it difficult to effectively assess prevalence (KÄFERSTEIN, 2003). Parâmetros microbiológicos específicos e a presença de Salmonella spp. foram investigados em 72 amostras de carne de frango (36 asas e 36 baquetas) coletadas em mercados e açougues. Os parâmetros microbiológicos específicos foram determinados utilizando um método cultural convencional e a presença de Salmonella spp. em amostras de frango foi determinada utilizando métodos de reação em cadeia da polimerase (PCR) por separação convencional e imunomagnética (IMS). Além disso, a suscetibilidade antimicrobiana dos isolados foi revelada pelo método de difusão do disco de Kirby-Bauer. Os resultadosindicaram que 30 das 72 amostras foram positivas para Salmonella spp. pelo método convencional, e 42 das 72 foram positivas pelo método IMS-PCR. No entanto, 30 das 72 amostras foram positivas para Salmonella spp. por ambos os métodos. Os isolados de Salmonella spp. foram confirmados pelo sistema VITEK2 Compact e PCR. As susceptibilidades dos isolados a 10 antibióticos foram determinadas. Os resultados indicaram que os isolados (27/30) apresentaram maior suscetibilidade à gentamicina (90,00%), enquanto a maior resistência foi ao ácido nalidíxico e à tetraciclina nos níveis de 100 e 93,34%, respectivamente. Estes resultados indicam uma alta prevalência de Salmonella spp. em carne de frango da cidade de Erzurum, Turquia, e o perfil de resistência antimicrobiana desses isolados deve ser considerado para a saúde pública. Os resultados também demonstram que a técnica de IMS-PCR foi superior ao método convencional para detecção de Salmonella em carne de frango.

ABSTRACT

Recently, several scoring systems have been proposed to predict remission from type 2 diabetes mellitus (T2DM) after metabolic surgery. The ABCD score was compared to the individualized metabolic surgery (IMS) score in terms of the prediction of long-term T2DM remission; however, which of the two scoring systems is better remains controversial. Thus, Three East Asian countries Metabolic Surgery (TEAMS), which has been organized as a study group since 2016, is conducting a retrospective, international, multi-institutional study to compare the two scoring systems in East Asian obese patients after metabolic surgery. The primary study objective is to compare the ABCD score with the IMS score at 3 and 5 years after sleeve gastrectomy (SG), Roux-en-Y gastric bypass, one anastomosis gastric bypass and SG with duodenojejunal bypass. The secondary objectives include evaluating patients who were good candidates for SG, and adjusting the IMS scoring system for East Asian patients.

Abstract Introduction: The scales to measure functional mobility in critically ill patients were developed and validated in English, there is a need for these tools in Spanish speaking countries. Objective: To perform translation, cultural adaptation and inter-rater reliability of the Spanish versions of the Perme Intensive Care Unit Mobility Score and IMS tools in ICU patients. Methods: Translation and validation study between November 2016 and July 2017, following the COSMIN Protocol's recommendations. Two couples of physiotherapists with the role of observer/rater applied both scales in 150 patients upon admission and discharge of a medical-surgical ICU from a private hospital in Colombia. The sample size was defined taking into account the lowest proportion of reported agreement (68.57%), a Kappa index of 0.2784 or higher to ensure that the calculated n was adequate, and a confidence level of 95% Results: Translation and cultural adaptation were performed, the final version of both scales in Spanish was approved by the authors. The sample was 150 patients, 52% were men, the average age was 58 ± 17 years, invasive mechanical ventilation was present in 63 (42%) of the cases. Inter-rater reliability of the ICU Mobility Scale was between 0.97 and 1.00, and for the Perme Intensive Care Unit the Mobility Score it was between 0.99 and 1 in the two moments of the measurements. Conclusions: Both scales were translated and culturally adapted and presented excellent inter-rater reliability in the two pairs of raters (rater/observer).


Resumen Introducción: Las escalas para medir la movilidad funcional en el paciente crítico han sido desarrolladas y validadas en lengua inglesa, existe una necesidad de contar con estas escalas en nuestros países hispanoparlantes. Objetivo: Realizar traducción, adaptación cultural y determinar confiabilidad inter evaluador de la versión en español del Perme Intensive Care Unit Mobility Score y del ICU Mobility Scale (IMS). Métodos: Estudio de traducción y validación entre noviembre de 2016 y Julio de 2017 siguiendo las recomendaciones del Protocolo COSMIN. Dos parejas de fisioterapeutas con el rol de observador/evaluador aplicaron ambas escalas en 150 pacientes al ingreso y egreso de una UCI médico-quirúrgica de una clínica privada en Colombia. Se definió el tamaño de muestra teniendo en cuenta la menor proporción de concordancia reportada (68.6%), un índice Kappa 0.2784 o superior para garantizar que el n calculado fuera adecuado, y un nivel de confianza de 95% Resultados: Se realizó la traducción y adaptación cultural, la versión final de ambas escalas en idioma español fue aprobada por los autores. La muestra fue de 150 pacientes, 52% fueron hombres, la edad promedio fue de 58 ±17 años, la ventilación mecánica invasiva estuvo presente en 63 (42.0%) de los casos. Se encontró confiabilidad inter-evaluador del ICU Mobility Scale entre 0.97 y 1 y para Perme Intensive Care Unit Mobility Score estuvo entre 0.99 y 1.00 en los dos momentos de mediciones. Conclusiones Ambas escalas fueron traducidas y adaptadas culturalmente y presentaron excelente confiabilidad inter-evaluador en las dos parejas de evaluadores (evaluador/observador).

Objective To compare PCR-SBT to IMS-ELISA in the HLA-B27 detection in the ankylosing spondylitis (AS)pa-tients.Methods Simultaneously,PCR-SBT and IMS-ELISA were used to detect the HLA-B27 expression in peripheral blood samples which were suspected patients with AS from 120 cases.Chisquare test of paired design and the area under curve of receiver operating characteristics of SPSS17.0 software were used to evaluate the value of PCR-SBT and IMS-ELISA in HLA-B27 detection of AS patients.Results Among 120 cases of suspected patients with AS,the positive rates of HLA-B27 detected by PCR-SBT and IMS-ELISA were 45.83%(55/120)and 37.50% (45/120),respectively.There was statistical difference between the two methods in the HLA-B27 detection (χ2 =59.455,P =0.000).The sensibility and spe-cificity of PCR-SBT were 96.36% and 96.92%,respectively.While the sensibility and the specificity of IMS-ELISA were 69.09% and 89.23%,respectively.Area under the curve of two methods were 0.966 and 0.792,respectively.Conclusion In comparison with IMS-ELISA,the sensibility and the specificity of PCR-SBT in HLA-B27 detection were higher in AS diag-nosis,that is to say,PCR-SBT is better in HLA-B27 detection and AS diagnosis.

Objective: To investigate the inhibited effect of epigallocatechin-3-gallate (EGCG) on the expression of NPM1 in IMS-M2 cells harboring the NPM1 mutations.Methods:Cell proliferation assay was performed to test the effects of EGCG on cell growth of IMS-M2 cells harboring the NPM1 mutations. Western blot analysis were performed to test the protein expression of NPM1, AKT, those associated with apoptosis. Results: EGCG can down-regulate the expression of NPM1 in IMS-M2 cells harboring the NPM1 mutations. Moreover, EGCG also suppressed the cell proliferation and induced apoptosis in IMS-M2 cells.Conclusions:The results suggested that EGCG could be considered as a reagent for treatment of AML patients with NPM1 mutations.

ABSTRACT

Diagnosis of leptospirosis by PCR is hampered due to the presence of substances on biological fluids. Here, we report an immunomagnetic separation step prior to PCR which improved the detection of Leptospira spp. in blood and urine samples from dogs. It resulted in a significant improvement on sensitivity for diagnosis of canine leptospirosis.

ABSTRACT

Justification: The first National Guidelines on Infant and Young Child Feeding (IYCF) were formulated by Ministry of Women and Child Development (Food & Nutrition Board) in 2004, and the same guidelines were revised in 2006. India is committed to halving the prevalence of under weight children by 2015 as one of the key indicators of progress towards the Millennium Development Goals (MDG). By the end of 2009 nutritional achievement goals did not make for happy reading. So there was need to revise the existing guidelines and to have more viable and scientifically accepted national guidelines on Infant and Young child feeding. Process: A National Consultative Meet was organized by Indian Academy of Pediatrics at Gurgaon in 2009 where members of IYCF and Nutrition Chapters of IAP, BPNI, WHO, UNICEF, USAID, WFP were present. Each group made detailed presentations after reviewing recent literature on the subject. After extensive discussions a consensus was reached and the guidelines were formulated. Objectives: To formulate, endorse, adopt and disseminate guidelines related to Infant & Young Child feeding from an Indian perspective (including infant feeding in the context of HIV infection). Recommendations: Optimal infant and young child feeding: Early initiation of breastfeeding, exclusive breastfeeding for the first six month of life followed by continued breastfeeding for up to two years and beyond with adequate complementary foods is the most appropriate feeding strategy for infants and young children. Adequate nutrition and anemia control for adolescent girls, pregnant and lactating mother is also advocated.

PURPOSE: Intramuscular stimulation(IMS) shows good results in the treatment of chronic pain patients who did not respond to other treatments such as oral analgesics, trigger point injection, nerve block and epidural steroid injection. But, IMS procedure especially, patients with diabetes mellitus(DM) has sometimes serious problem. So, we present a very rare case of intramuscular abscess in the sternocleiomastoid muscle after IMS with literature review. METHODS: A 66 year old male visited our department 7 days after IMS in the neck. His premorbid conditions and risk factors of deep neck infection was DM and old age. Computed tomographic scans of the head and neck region were performed in this patient: signs of deep neck infection, were seen enhanced abscess in the sternocleidomastoid muscle, cellulitis overlying tissue of the neck, and air bubbles involved muscle. Necrotic wound was excised serially and we treated this with the Vacuum-assisted closure(VAC, Kinetics Concepts International, San Antonio, Texas) system device. After appropriately shaping the sponge in the pockets, continuous negative pressure of 125mmHg was applied. The VAC therapy was utilized for a period of 12 days. RESULTS: We obtained satisfactory results from wide excision, drainage of the abscess with the VAC system, and then primary closure. The postoperative course was uneventful. CONCLUSION: We suggest that many of the infectious complications may be preventable by strict adherence to aseptic techniques and that some of the other complications may be minimized by refining the techniques with a clear understanding of the medical disorders of patients. And, the refined technique using the VAC system can provide a means of simple and effective management for the cervical intramuscular abscess, with better cosmetic and functional results.

ABSTRACT

Herpes zoster is the consequence of reactivation of latent varicella zoster virus from dorsal root ganglia. Postherpetic neuralgia (PHN) may be diagnosed when pain persists in a dermatomal pattern long after the vesicular erruption has healed. PHN is a kind of neuropathic pain. The pathophysiology of PHN is uncertain, but neuropathic pain due to denervation supersensitivity may be important to understand the pathophysiology of PHN. Numerous treatment have been introduced for the management of PHN, but no methods that results in complete remission. Gunn's intramuscular stimulation (IMS) is one of the best treatment of chronic pain, especially neuropathic pain. We tried Gunn's IMS for treatment of PHN patients affecting thoracic dermatomes. As a result, the visual analogue scale (VAS) was decreased from 7-8 to 2-3 and the result were satisfactory. The purpose of this case report is to introduce the Gunn's IMS and review our experience for the treatment of PHN.

ABSTRACT

Intramuscular Stimulation (IMS) is a refined technique of traditional oriental acupuncture, and IMS has proved effective for relieving chronic pain of a neuropathic origin. IMS is currently seen to be quite a safe procedure with minimal complications having been reported to date. Various complications have been documented for acupuncture, but few complications or adverse effects have been reported in relation to the relatively new technique of IMS. We report here on a case of cervical spinal epidural hematoma that manifested as a cause of delayed hemiparesis. The safety of this procedure in unpracticed hands seems to be questionable.

ABSTRACT

IMS is more than a needling technique. It has a background based on physiology, and needling depends upon a detailed knowledge of anatomy. The rationale conforms to Canon's law of denervation supersensitivity. Thus, IMS is a theoretical model that has the support of western physiologists. IMS worked by desensitizing supersensitive body segments. Therefore the technique needs detailed physical examination on segmental innervation area and needling skill to find accurate needling point. IMNS (Interventional Microadhesiolysis and Nerve Stimulation) works for hypersensitive joint soft tissue and FIMS (Fluoroscopy guided Interventional Microadhesiolysis and nerve Stimulation) works for hypersensitive nerve root, whereas classical IMS works for nerve stimulation due to muscle needling. In the future, we can adopt these techniques for brain disease according by Canon's law.

ABSTRACT

Recently, intra-Muscular Stimulation (IMS) therapy is being increasingly used for musculoskeletal pain. This procedure is generally regarded as a safe procedure for the general public. Some cases of iatrogenic pneumothorax caused by acupuncture have been reported in the medical literature. However, a case of an IMS therapy associated pneumothorax has not reported. We experienced two cases of iatrogenic pneumothorax after IMS therapy. A 62 year-old man received IMS therapy on the right shoulder due to posterior neck pain. After IMS therapy, acute dyspnea and chest discomfort developed. The other patient was a 74 year-old woman who also received IMS therapy. This patient experienced a nonproductive cough and acute dyspnea after the treatment. As the popularity of this form of alternative medicine increases, we might expect to see more cases of iatrogenic pneumothorax. Physicians should be aware of the adverse events associated with IMS therapy.

ABSTRACT

Chronic pain can cause disability, mild to severe suffering and high medical costs. Some unfortunate patient do not improve despite administering conservative treatment and then the various interventional therapies, including oriental medical treatment and/or surgery, and they find themselves in search of a more effective pain relief. Deep dry needling is one of the newer treatment modalities for these patients. The last 10 years have seen a lot of progress in understanding the neural pathways and the type and extent of tissue involvement during chronic pain. This in turn has stimulated the development of new treatment techniques, and deep dry needling is one of them. So, the authors of this paper discuss the individual theories, the characteristics and future directions of several deep dry needling techniques, and we examine the new dry needling technique that has been recently developed in Korea.
