ABSTRACT

Objective To observe the clinical efficacy of suspended moxibustion plus Chinese medication in treating icteric hepatitis. Method Ninety-two patients with icteric hepatitis were randomized into a treatment group and a control group, 46 in each group. The treatment group was intervened by suspended moxibustion plus oral administration of Chinese medication for supplementing yang, tonifying spleen, expelling dampness, and activating blood circulation;the control group was intervened only by the same Chinese medication. The symptom and sign score, and liver function items including TBil and ALT were observed, and the clinical efficacy was compared. Result The symptom and sign scores were significantly changed in both groups after intervention (P<0.05). After intervention, there was a significant difference in comparing the symptom and sign score between the two groups (P<0.05). The TBil and ALT levels were significantly changed after intervention in both groups (P<0.05). The recovery rate and total effective rate were respectively 87.0%and 95.7%in the treatment group versus 76.1%and 82.6%in the control group, and the inter-group differences were statistically significant (P<0.05). Conclusion Suspended moxibustion plus Chinese medication is an effective method in treating icteric hepatitis.

ABSTRACT

Invasion of the bile duct by hepatocellular carcinoma (HCC), which is called intrahepatic bile duct HCC, is rare and has a poor prognosis. Early diagnosis and surgical resection is important for treatment. A 58-year-old man who underwent hepatic resection for HCC 4 years ago and received transarterial chemoembolization (TACE) 2 years after the operation for recurred HCC presented with jaundice. CT scan revealed a tumor in the common bile duct without intrahepatic lesion. Therefore, ERCP was done to perform biopsy and biliary drainage. Histological examination was compatible with hepatocellular carcinoma. However, the tumor could not be visualized at angiography and thus, only transarterial chemoinfusion was performed without embolization. The tumor had disappeared on follow-up CT scan, and the patient has been disease free for 23 months without evidence of recurrence. Herein, we report a case of intrahepatic bile duct HCC which disappeared after ERCP.

ABSTRACT

Leptospirosis is an important cause of acute febrile illness in the monsoon season in India. It is a zoonotic disease that is spread primarily by rodents. There exist two clinical types: anicteric and icteric leptospirosis. Both have an initial septicemic phase followed by an immune phase. The clinical manifestations vary and the disease manifestations may range from a nonspecific febrile illness to one with severe multiorgan failure. Weil's disease is the severe form of the infection; which occurs in less than 10% of the patients and is associated with high mortality. The methods available for diagnosis and treatment of leptospirosis are discussed in this review. Crystalline penicillin is the drug of choice for treatment of leptospirosis in children. Avoidance of contact with flood waters and rodent control are vital for prevention of the disease. We also discuss the differences between childhood leptospirosis and adult disease. We used two methods to garner the information presented in this article: i) we searched the PubMed database (http://www.ncbi.nlm.nih.gov/pubmed/) using the keywords 'leptospirosis' and 'children,' with special emphasis given to articles from the Indian literature; and ii) we reviewed the chapters on leptospirosis in the standard textbooks of pediatric and infectious diseases.

ABSTRACT

Hepatocellular Carcinoma (HCC) is still a leading health problem worldwide, due to its correlation with HBV and HCV infection and its management which is strongly dependent on patient’s condition and tumor extension. Surgery, with liver resection or liver transplantation offer a good survival rate as a primary management of such cancer. But since liver resection must consider some aspect of liver function and tumor size, and liver transplantation was not a choice in Indonesia, many treatment modalities has been developed which can be used to overcome this problem, such as tumor ablation, transarterial chemo embolization (TACE), chemotherapy and radiotherapy. With the development of conformal radiotherapy, the hepatitis induced radiation therapy could be minimized. This paper present a case of conformal radiation therapy utilization in icteric type HCC in Dr. Cipto Mangunkusumo Hospital, Jakarta. Hepatic resection was planned for this patient.

ABSTRACT

The most causes of an icteric hepatoma are a late stage hepatocellular carcinoma or intrahepatic cholangiocarcinoma. A hepatocellular carcinoma, causing an obstrucution of the bile duct, rarely results in jaundice. With a late stage hepatocellular carcinoma, the accurate diagnosis and treatment may be difficult. Herein, we report a case of a hepatocellular carcinoma and obstructive jaundice, due to hilar tumor emboli, with a review of the literature.
