ABSTRACT

ABSTRACT Objective: Evaluate the initial results and the possible complications of the combination of pelvic fixation using iliac screws in long instrumentations of the lumbar spine in elderly patients. Methods: An analysis of 38 patients who underwent lumbar spine arthrodesis instrumentation with more than 3 levels, in which we included level L5-S1 and extended the fixation to the iliac crest. Radiological assessment of instrumentation through X-ray of the lumbar spine and pelvis, and computed tomography, to investigate the presence of radiological fusion. Clinical evaluation through a questionnaire of buttock pain including the visual analogue pain score (VAS). Complications related to pelvic instrumentation were investigated. Results: All patients had radiological fusion in the lumbosacral transition. A halo was found around the iliac screw on imaging studies in 31% of patients, but without related symptoms. The questionnaire of buttock pain found that 15% of patients had some low intensity buttock pain (VAS 1-2) and no need to or interest in removing the screws. There was infection at the surgical site in 2% of cases, hematoma in the buttocks in 5%, and vertebral body fracture in the cranial level instrumented in 7% of cases. Conclusions: Pelvic fixation through the iliac screws proved to be effective in protecting the S1 screws in long instrumentations including the L5-S1 level in elderly patients, allowing the radiological bone fusion. The overall results for pain were satisfactory, based on a questionnaire of buttock pain. There are no signs of overload or degeneration of the sacroiliac joints in the early years after surgery. RESUMO Objetivo: Avaliar os resultados iniciais e as possíveis complicações da combinação da fixação pélvica através dos parafusos nos ilíacos nas instrumentações longas da coluna lombar em pacientes idosos. Métodos: Análise de 38 pacientes, submetidos a artrodese da coluna lombar com instrumentação maior que três níveis, incluímos o nível L5-S1 e estendemos a fixação ao ilíaco. Avaliação radiológica da instrumentação, através de radiografia da coluna lombar e da bacia, e tomografia computadorizada, pesquisando-se presença de fusão radiológica. Avaliação clínica através de questionário sobre dor glútea, incluindo o escore visual analógico de dor (VAS). Foram pesquisadas complicações relacionadas a instrumentação pélvica. Resultados: Todos os pacientes possuíam fusão radiológica na transição lombossacra. Foi encontrado halo ao redor do parafuso ilíaco nos exames de imagem de 31% dos pacientes, porém sem sintomas relacionados. O questionário sobre dor glútea identificou que 15% dos pacientes possuíam alguma dor glútea de baixa intensidade (VAS 1 a 2) e sem necessidade ou interesse de retirar os parafusos. Houve infecção em sitio cirúrgico em 2% dos casos, hematoma em região glútea em 5% e fratura do corpo vertebral no nível mais cranialmente instrumentado em 7% dos casos. Conclusão: A fixação pélvica, através de parafusos ilíacos, mostrou-se eficaz na proteção dos parafusos de S1 em instrumentações longas, que incluam o nível L5-S1 em pacientes idosos, permitindo a fusão óssea radiológica. Os resultados gerais de dor foram satisfatórios com base em um questionário sobre dor glútea. Não há sinais de sobrecarga ou degeneração das articulações sacro-ilíacas nos primeiros anos após a cirurgia. Nível de Evidência IV; Estudo terapêutico - série de casos.


RESUMEN Objetivo: Evaluar los resultados iniciales y las posibles complicaciones de la combinación de la fijación pélvica con tornillos en los ilíacos en las instrumentaciones largas de la columna lumbar en pacientes ancianos. Métodos: Análisis de 38 pacientes sometidos a la artrodesis de la columna lumbar con instrumentación de más que tres niveles cuando se incluyó L5-S1 y se extendió la fijación al ilíaco. Evaluación radiológica de la instrumentación de la columna lumbar y cadera y tomografía computarizada para investigar la presencia de fusión radiológica. Evaluación clínica con cuestionario sobre dolor glúteo, incluyendo la Escala Visual Analógica (EVA). Se han investigado las complicaciones relacionados con la instrumentación pélvica. Resultados: Todos los pacientes tenían fusión radiológica en la transición lumbosacra. Se encontró halo radiológico alrededor del tornillo ilíaco en los estudios de imagen del 31% de los pacientes, pero sin síntomas relacionados. El cuestionario sobre el dolor glúteo identificó que el 15% de los pacientes tenían algún dolor de baja intensidad (EVA 1-2) y no había necesidad o interés en retirar los tornillos. Se produjo una infección en el sitio quirúrgico en 2% de los casos, hematoma en la región glútea en un 5% y fractura del cuerpo vertebral en el nivel más craneal de instrumentación en el 7% de los casos. Conclusiones: La fijación de la pelvis a través de tornillos en ilíacos se mostró eficaz en la protección de los tornillos de S1 en instrumentaciones largas que incluyan el nivel L5-S1 en pacientes ancianos, permitiendo la fusión ósea radiológica. Los resultados generales del dolor fueron satisfactorios sobre la base de un cuestionario sobre el dolor glúteo. No hay signos de sobrecarga o degeneración de las articulaciones sacroilíacas en los primeros años después de la cirugía. Nivel de Evidencia IV; Estudio terapéutico - Serie de casos.

ABSTRACT

PURPOSE: Catheter direct thrombolysis (CDT) has been shown to be an effective treatment for deep venous thrombosis. The objective of the review is to improve safety and efficacy of the CDT by using ward based protocol, better able to predict complications and treatment outcome through monitoring of haemostatic parameters and clinical observation during thrombolysis procedure. MATERIALS AND METHODS: MEDLINE, EMBASE, CENTRAL and Web of Science were searched for all articles on deep venous thrombosis, thrombolysis and correlations of clinical events (bleeding, successful thrombolysis) during thrombolysis with hemostatic parameters to March 2016. The risk of bias in included studies was assessed by Cochrane Collaboration’s tool and Cochrane Risk of Bias Assessment Tool: for Non-Randomized Studies of Interventions. RESULTS: Twenty-four studies were included in the review and we found that improving safety and efficacy of CDT by using ward based protocol depending on eight factors; strict patient selection criteria, types of fibrinolytic drugs, mode of fibrinolytic drug injection, biochemical markers monitoring (fibrinogen, D-dimer, activated partial thromboplastin time, plasminogen activator inhibitor-1), timing of intervention, usage of intermittent pneumatic calf, ward monitoring and thrombolysis imaging assessment (intravascular ultrasound). These factors may help to improve safety and efficacy by reducing total thrombolytic drug dosage and at the same time ensure successful lysis. There is a marked lack of randomized controlled trials discussing the safety and efficacy of catheter direct thrombolysis. CONCLUSION: CDT can be performed safely and efficiently in clinical ward, providing that careful nursing, biochemical monitoring, proper selection and mode of infusion of fibrinolytic drugs, usage of Intermittent pneumatic calf and adequate thrombolysis imaging assessment are ensured.

ABSTRACT Objective : to evaluate the histologic and morphometric characteristics of bone biopsies of the anterior iliac crest of patients of different age groups. Methods : we studied 30 bone samples from the iliac crest, using brightfield optical microscopy. We divided the samples by donors' age groups in three groups: Group 1 (n = 10), subjects aged between 25 and 39 years; Group 2 (n = 10), subjects aged between 40 and 64 years; Group 3 (n = 10), individuals aged 65 years and over. We randomly divided the samples into two sets with 15 specimens. In the first study segment (n = 15), we used histological to assess the osteogenic property of the graft, through the analysis of cell reserve in the periosteum, the number of osteocytes in the lacunae and the number of Haversian and Volkmann's canals. In the second study segment (n = 15), we investigated the morphology of osteoconductive property of the graft, through quantification of the trabecular meshwork (Vv) and trabecular area (Sv). Results : histologically, we observed degeneration of bone occurring with age, characterized by thinning of the periosteum, with gradual replacement of the steogenic layer by fibrous tissue, small amount of Haversian and Volkmann's canals, osteocyte lacunae voids and fine spongy bone trabeculae, allowing ample medullary space, usually occupied by fat cells and adipocytes. Morphologically, with respect to the quantification of the trabecular meshwork (Vv), we found statistically significant differences between Groups 1 and 3 and between Groups 2 and 3, with reduction of the trabecular meshwork of about 45% in the elderly over 65 years old ; there was no statistically significant difference between Groups 1 and 2. There was also no statistical difference between the Groups regarding Sv. Conclusion : the results of this experiment suggest that, in the elderly (over 65 years old), the osteogenic property of autologous bone graft decreases and the osteoconductive property is compromised.


RESUMO Objetivo: avaliar as características histológicas e morfométricas de biópsias ósseas da região anterior da crista ilíaca de pacientes de diferentes faixas etárias. Métodos: foram estudadas 30 amostras de osso da crista ilíaca, utilizando-se microscopia óptica de campo claro. As amostras foram divididas pela faixa etária dos doadores em três grupos: Grupo 1 (n = 10), indivíduos com idade entre 25 e 39 anos; Grupo 2 (n = 10), indivíduos com idade entre 40 e 64 anos; Grupo 3 (n = 10), indivíduos com idade igual ou superior a 65 anos. As amostras foram separadas aleatoriamente em dois conjuntos com 15 peças. No primeiro segmento do estudo (n = 15), foi avaliada histologicamente a propriedade osteogênica do enxerto, através da análise da reserva celular no periósteo, do número de osteócitos nas lacunas e da quantidade de canais de Havers e de Volkmann. No segundo segmento do estudo (n = 15), investigou-se morfologicamente a propriedade osteocondutora do enxerto, através da quantificação da rede trabecular (Vv) e da área trabecular (Sv). Resultados: histologicamente, observou-se que ocorre degeneração do tecido ósseo com a idade, caracterizada pelo adelgaçamento do periósteo, com substituição gradual da camada osteogênica por tecido fibroso, pequena quantidade de canais de Havers e de Volkmann, osteoplastos vazios e trabéculas finas de osso esponjoso, permitindo amplo espaço medular, em geral ocupado por células lipídicas e adipócitos. Morfologicamente, com relação à quantificação da rede trabecular (Vv), foi observada diferença estatisticamente significante entre os Grupos 1 e 3 e entre os Grupos 2 e 3, com redução da rede trabecular de cerca de 45% no idoso acima de 65 anos de idade; não foi observada diferença estatisticamente significante entre os Grupos 1 e 2. Não foi observada diferença estatística entre os grupos quanto à Sv. Conclusão: os achados do presente experimento sugerem que nos indivíduos idosos (acima de 65 anos de idade), a propriedade osteogênica do enxerto ósseo autólogo diminui e a propriedade osteocondutora está comprometida.

ABSTRACT

Objetivo: comparar el dolor posoperatorio en pacientes sometidos a artroscopia de cadera con la técnica dentro-fuera en comparación con la técnica fuera-dentro. Métodos: se realizó un estudio prospectivo en el que se comparó el dolor posoperatorio en un grupo de pacientes (n = 31) a los que se les realizó artroscopia de cadera mediante la técnica fuera-dentro, con los pacientes (n = 31) a los que se les realizó con la técnica dentro-fuera; ambos grupos de pacientes recibieron la misma analgesia multimodal. Los parámetros principales medidos fueron: la escala visual análoga en diferentes momentos hasta 24 horas después de la operación y la necesidad de opiáceos. Para las variables categóricas se realizó la comparación mediante Chi cuadrado, y para las variables cuantitativas mediante t Student o Mann-Whitney. Se calcularon el OR y el IC 95 por ciento. Para el análisis de los datos se utilizó el programa SPSS versión 21.0 y se consideró un error alfa de 5 por ciento. Resultados: la escala visual análoga posoperatoria fue menor en pacientes con la técnica de fuera-dentro después de 1 hora (diferencia media 1,9 IC 95 por ciento: 0,5 a 3,2, p = 0,01), después de 2 horas (diferencia media 1,29 IC 95 por ciento: 0,3 a 2,2, p = 0,01), y en el momento del alta (diferencia media 0,77 IC 95 por ciento: 0,16 a 1,18; p = 0,01). Sin embargo, 24 horas después del procedimiento, la diferencia no fue significativa (diferencia media 0,4 IC 95 por ciento: 0,02 a 0,84; p = 0,08). Las necesidades de opioides en el posoperatorio fueron significativamente menores en los pacientes con la técnica fuera-dentro ─6,5 por ciento vs. 41,9 por ciento─ (OR crudo 0,09 IC 95 porciento: 0,02 a 0,47; p = 0,004), incluso después de ajustar por edad, sexo y el IMC (OR ajustado 0,09 IC 95 por ciento: 0,016 hasta 0,51, p = 0,006). Conclusiones: los pacientes a los que se les realiza artroscopia de cadera con la técnica fuera-dentro tienen menos dolor posoperatorio y requerimientos de opioides que aquellos a los que se les realiza la técnica dentro-fuera(AU)


Objective: compare postoperative pain in patients undergoing hip arthroscopy with the inside-out technique compared to outside-in technique. Methods: a prospective study was conducted on postoperative pain in a group of patients (n = 31) who underwent hip arthroscopy by outside-in technique, which were compared with patients (n = 31) who received the inside-out technique. Both groups of patients received the same multimodal analgesia. The main parameters were measured: visual analogue scale at different times up to 24 hours after surgery and opioid need. Categorical variables were compared using Chi-square, and quantitative variables using Student t or Mann-Whitney. OR and CI 95 percent were calculated. SPSS version 21.0 was used for data analysis program and an alpha error of 5 percent was considered. Results: postoperative visual analogue scale was lower in patients with outside-in technique after 1 hour (mean difference 1.9, 95 percent CI 0.5 to 3.2, p = 0.01) after 2 hours (mean difference 1.29, 95 percent CI 0.3 to 2.2, p = 0.01) and at discharge (mean difference 0.77, 95 percent CI 0.16 to 1.18; p = 0.01). However, 24 hours after the procedure, the difference was not significant (mean difference 0.4, 95 percent CI 0.02 to 0.84; p = 0.08). Opioid requirements postoperatively were significantly lower in patients with the outside-in technique ─6,5 percent vs. 41.9 percent─ (crude OR 0.09 95 percnt CI 0.02 to 0.47; p = 0.004), even after adjusting age, sex and BMI (adjusted OR 0.09, 95 percent CI: 0.016 to 0.51, p = 0.006). Conclusions: Patients who underwent hip arthroscopy with outside-in technique have less postoperative pain and opioid requirements that those who undergo the inside-out technique(AU)


Objectif: le but de ce travail est de comparer la douleur postopératoire chez des patients opérés par la technique arthroscopique de dedans en dehors avec la technique de dehors en dedans au niveau de la hanche. Méthodes: une étude prospective a été réalisée afin de comparer la douleur postopératoire dans un groupe de patients traités par la technique arthroscopique de dehors en dedans (n = 31) et dans un groupe de patients traités par la technique arthroscopique de dedans en dehors (n = 31) au niveau de la hanche. Tous les deux groupes ont reçu une analgésie multimodale. On a fondamentalement évalué des paramètres tels que l'échelle visuelle analogique (EVA) en différents moments, même 24 heures après l'opération, et le besoin d'opiacés. Pour les variables catégoriques, on a utilisé le test de Chi carré, et pour les variables quantitatives le test du t de Student ou de Mann-Whitney. On a également calculé l'OR et l'IC 95 pourcent. Pour l'analyse des données, on a utilisé le logiciel SPSS v21.0, et une erreur alpha de 5 pourcent a été considérée. Résultats: la valeur EVA en postopératoire a été inférieure chez les patients opérés par la technique de dehors en dedans après 1 h (différence moyenne 1.9 IC 95 pourcent : 0.5 à 3.2, p = 0.01), après 2 h (différence moyenne 1.29 IC 95 pourcent : 0.3 à 2.2, p = 0.01), et à la sortie de l'hôpital (différence moyenne 0,77 IC 95 pourcent : 0.16 à 1.18 ; p = 0.01). Par contre, la différence n'a pas été significative (différence moyenne 0.4 IC 95 pourcent : 0.02 à 0.84 ; p = 0.08) 24 h après le geste opératoire. Le besoin d'opiacés en postopératoire a été significativement inférieur chez les patients traités par la technique de dehors en dedans ─6.5 pourcent vs. 41.9 pourcent─ (OR cru 0.09 IC 95 pourcent : 0.02 à 0.47 ; p = 0.004), même après l'ajustement par âge, sexe et IMC (OR ajusté 0.09 IC 95 pourcent : 0.016 jusqu'à 0.51, p = 0.006). Conclusions: on conclut que les patients traités par la technique arthroscopique de dehors en dedans souffrent moins de douleur postopératoire et n'ont pas besoin d'opiacés que ceux qui ont été traités par la technique arthroscopique de dedans en dehors. ABSTRACT

Introduction: Unstable posterior pelvic ring injuries are best treated with operative methods due to better post-op functional score. Our patient cohort was involved in heavy manual laboring frequently required ground level work in their activities of daily living. There are very few outcome studies dealing exclusively with such patients. Materials & Methods: Forty one patients who were treated with percutaneous sacroiliac screw fixation under fluoroscopic guidance and were followed-up for at least one year were analyzed retrospectively for functional outcome using the Majeed score. Results: Twenty one (51.22%) and thirteen (31.70%) patients were found to be in excellent and good categories respectively and majority of the patients (thirty/73.17%) were able to return to their original occupation with or without minor adjustments. Conclusion: Percutaneous ilio-sacral screw fixation for posterior pelvic unstable injuries is an acceptable mode of treatment in patients involved in heavy manual laboring. laboring

Objectives: The purpose of this study is to compare the efficacy of iliohypogastric/ilioinguinal nerve blocks performed with the ultrasound guided and the anatomical landmark techniques for postoperative pain management in cases of adult inguinal herniorrhaphy. Methods: 40 patients, ASA I-II status were randomized into two groups equally: in Group AN (anatomical landmark technique) and in Group ultrasound (ultrasound guided technique), iliohypogastric/ilioinguinal nerve block was performed with 20 ml of 0.5% levobupivacaine prior to surgery with the specified techniques. Pain score in postoperative assessment, first mobilization time, duration of hospital stay, score of postoperative analgesia satisfaction, opioid induced side effects and complications related to block were assessed for 24 h postoperatively. Results: VAS scores at rest in the recovery room and all the clinical follow-up points were found significantly less in Group ultrasound (p < 0.01 or p < 0.001). VAS scores at movement in the recovery room and all the clinical follow-up points were found significantly less in Group ultrasound (p < 0.001 in all time points). While duration of hospital stay and the first mobilization time were being found significantly shorter, analgesia satisfaction scores were found significantly higher in ultrasound Group (p < 0.05, p < 0.001, p < 0.001 respectively). Conclusion: According to our study, US guided iliohypogastric/ilioinguinal nerve block in adult inguinal herniorrhaphies provides a more effective analgesia and higher satisfaction of analgesia than iliohypogastric/ilioinguinal nerve block with the anatomical landmark technique. Moreover, it may be suggested that the observation of anatomical structures with the US may increase the success of the block, and minimize the block-related complications. .


Objetivo: Comparar a eficácia de bloqueios dos nervos ílio-hipogástrico/ilioinguinal feitos com a técnica guiada por ultrassom e a de marcos anatômicos para o manejo da dor no pós-operatório em casos de herniorrafia inguinal em adultos. Métodos: Foram randomicamente divididos 40 pacientes, estado físico ASA I-II, em dois grupos iguais: nos grupos AN (técnica de marcos anatômicos) e US (técnica guiada por ultrassom), o bloqueio dos nervos ílio-hipogástrico/ilioinguinal foi feito com 20 mL de levobupivacaína a 0,5% antes da cirurgia com as técnicas especificadas. Escore de dor na avaliação pós-operatória, tempo de primeira mobilização, tempo de internação hospitalar, escore de satisfação com a analgesia no pós-operatório, efeitos colaterais induzidos por opiáceos e complicações relacionadas ao bloqueio foram avaliados durante 24 horas de pós-operatório. Resultados: Escores EVAem repouso na sala de recuperação e todos os valores clínicos durante o acompanhamento foram significativamente menores no grupo ultrassom (p < 0,01 ou p < 0,001). Escores EVA em movimento na sala de recuperação e todos os valores clínicos durante o acompanhamento foram significativamente menores no grupo ultrassom (p < 0,001 em todos os tempos avaliados). Enquanto os tempos de internação e da primeira mobilização foram significativa-mente menores, os índices de satisfação com a analgesia foram significativamente maiores no grupo ultrasom (p<0,05, p< 0,001, p< 0,001, respectivamente). Conclusão: De acordo com o nosso estudo, o bloqueio dos nervos ílio-hipogástrico/ilioinguinal guiado por US em herniorrafias inguinais em adultos proporciona uma analgesia mais eficaz e maior satisfação com a analgesia ...


Objetivo: El objetivo de este estudio fue comparar la eficacia de bloqueos de los nervios ileohipogástrico/ilioinguinal realizados con la técnica guiada por ultrasonido y la de marca anatómicas para el manejo del dolor en el postoperatorio en casos de herniorrafia inguinal en adultos. Métodos: 40 pacientes, estado físico ASA I-II, fueron aleatoriamente divididos en 2 grupos iguales: grupos con técnica de marcas anatómicas) y grupo con técnica guiada por ultrasonido. El bloqueo de los nervios iliohipogástrico/ilioinguinal fue realizado con 20 mL de levobupivacaína al 0,5% antes de la cirugía con las técnicas especificadas. La puntuación de dolor en la evaluación postoperatoria, tiempo de primera movilización, tiempo de ingreso hospitalario, puntuación de satisfacción con la analgesia en el postoperatorio, efectos colaterales inducidos por opiáceos y complicaciones relacionadas con el bloqueo fueron evaluados durante 24 h de postoperatorio. Resultados: Las puntuaciones EVA en reposo en la sala de recuperación y todos los valores clínicos durante el seguimiento fueron significativamente menores en el grupo con técnica guiada por ultrasonido (p < 0,01 o p < 0,001). Las puntuaciones EVA en movimiento en la sala de recuperación y todos los valores clínicos durante el seguimiento fueron significativamente menores en el grupo con técnica guiada por ultrasonido (p < 0,001 en todos los tiempos calculados). Mientras que los tiempos de ingreso y de la primera movilización fueron significativamente menores, los índices de satisfacción con la analgesia fueron significativamente mayores en el grupo ultrasonido (p < 0,05; p < 0,001; p < 0,001 respectivamente). ABSTRACT

Avulsion fractures of the apophysis of the iliac crest have rare incidence and are little known. In this article, we report the case of an 11-year-old female patient who presented this injury after indirect trauma. From careful radiographic analysis, an avulsion fracture of the iliac crest was identified. It was decided to use nonsurgical treatment comprising analgesia and load restriction. A fratura avulsão da apófise da crista ilíaca apresenta incidência rara e pouco conhecida. Neste artigo relatamos caso de paciente do sexo feminino, de 11 anos, que apresentou essa lesão após trauma indireto. Após uma análise cuidadosa da radiografia, foi identificada fratura avulsão da crista ilíaca e optou-se pelo tratamento não cirúrgico com analgesia e restrição de carga. ABSTRACT

Background: The internal iliac artery originates from the common iliac artery at the level of sacroiliac joint. The internal iliac artery descends posterior to the greater sciatic foramen thereby dividing into anterior and posterior divisions. The posterior division of the internal iliac artery is known to give rise to three main branches i.e. iliolumbar artery, lateral sacral artery. Accidental haemorrhage is common during erroneous interpretation of anomalous blood vessels. The knowledge of the normal and the abnormal anatomy of the branches of the internal iliac artery is essential for obstetric surgeons. Methods: 50 adult human pelvic halves were procured from embalmed cadavers of J.J.M. Medical College and S.S.I.M.S & R.C, Davangere, Karnataka, India for the study. Results: Out of 50 specimens, ilio-lumbar artery took origin from posterior division, directly in 22 specimens (44%), with obturator artery in 5 specimens (10%) and with superior gluteal artery in 2 specimens (4%). From anterior division, with obturator artery in 6 specimens (12%) and with inferior gluteal artery in 1 specimen (2%). From common trunk, as direct branch in 10 specimens (20%), with lateral sacral artery in 1 specimen (2%), with vertebral branch in 1 specimen (2%), with superior gluteal artery in 1 specimen (2%) and was found to be absent in 1 specimen (2%). Posterior division of internal iliac artery given origin directly to superior gluteal artery in 44 specimens (88%), with ilio-lumbar artery in 1 specimen (2%), with obturator artery in 2 specimens (4%). Lateral sacral artery from posterior division was observed in 38 specimens (76%) and unpaired origin was observed in 7 specimens (14%). Conclusion: Internal iliac artery supplies the pelvic viscera and musculature the knowledge of its branches helpful in pelvis surgeries.

ABSTRACT

Objective To study the feeding arteries of sacral tumors with digital substraction angiography (DSA). Methods A total of 27 patients with sacral tumors, who were encountered at authors’ hospital during the period from January 2006 to December 2012 , were enrolled in this study. DSA of abdominal aorta, bilateral internal iliac arteries, median sacral artery and lumbar arteries was performed in all patients. The origins, branches of the feeding arteries were determined, and the results were analyzed. Results Of the 27 cases with sacral tumors, DSA demonstrated median sacral artery in 20 (20 arteries in total), lateral sacral artery in 22 (36 arteries in total), ilio-lumbar artery in 18 (27 arteries in total), lumbar artery in 10 (15 arteries in total), inferior gluteal artery in 3 (3 arteries in total) and superior gluteal artery in 2 (2 arteries in total). Conclusion In our series, the blood supply of the sacral tumors is mainly from the median sacral artery, lateral sacral artery, ilio-lumbar artery and lumbar artery. Occasionally, superior and inferior gluteal arteries also participate in the blood supply of the sacral tumors. For the evaluation of sacral tumors, attention should be paid to the presence of rare feeding arteries.

ABSTRACT

OBJECTIVE: The overuse injuries in the hip joint occur commonly in sports practitioners and currently due to technical advances in diagnostic imaging, especially magnetic resonance imaging (MRI), are often misdiagnosed. Recently, a group of people were reported, all female, with pain and swelling in the pelvic region.T2-weighted MRI showed increased signal in the enthesis of the iliotibial band (ITB) along the lower border of the iliac tubercle. We report a case of a 34 year old woman, non-professional runner, with pain at the iliac crest with no history of trauma and whose MRI was compatible with the proximal iliotibial band syndrome. .


As lesões por sobrecarga na articulação do quadril ocorrem comumente em praticantes de esporte e atualmente, por causa do avanço das técnicas de diagnóstico por imagem, especialmente a ressonância magnética (RM), são frequentemente diagnosticadas. Recentemente, foi estudado um grupo de pacientes, todos do sexo feminino, com quadro de dor e edema na região do tubérculo ilíaco. A RM ponderada em T2 demonstrava aumento de sinal na êntese da banda iliotibial (BIT) ao longo da margem inferior do tubérculo ilíaco. Relatamos um caso de uma mulher de 34 anos, corredora não profissional, com quadro de dor na crista ilíaca sem história de trauma e cuja RM era compatível com a síndrome da banda iliotibial proximal. .

OBJECTIVE: Posterior pelvic lesions, especially of the sacral-iliac joint, have high mortality and morbidity risks. Definitive fixation is necessary for the joint stabilization, and one option is the sacral percutaneous pinning with screws. Proximity to important structures to this region brings risks to the fixation procedure; therefore, it is important to know the tridimensional anatomy of the pelvis posterior region. Deviations of the surgeon's hand of four degrees may target the screws to those structures; dimorphisms of the upper sacrum and a poor lesion reduction may redound in a screw malpositioning. This study is aimed to evaluate the dimensions of a safe surgical corridor for safe sacroiliac screw insertion and relations with age and sex of the patients. METHOD: One hundred randomly selected pelvis CTs of patients with no pelvic diseases, seen at a tertiary care teaching Hospital. Measurements were made by computer and the safest area for screw insertion was calculated by two methods. The results were expressed in mm (not in degrees), in order to be a further surgical reference. RESULTS: There was a significant size difference in the analyzed sacral vertebra, differing on a wider size in men than in women. There was no significant statistical difference between vertebral size and age. By both methods, a safe area for screw insertion could be defined. CONCLUSION: Age does not influence the width of the surgical corridor. The surgeon has a safe corridor considered narrower when inserting screws in a female pelvis than when in a male one. However, as the smallest vertebra found (feminine) was considered for statics, it was concluded that this corridor is 20 mm wide in any direction, taking as a reference the centrum of the vertebra. .


OBJETIVO: Lesões pélvicas posteriores, especialmente da articulação sacro-ilíaca, têm alta mortalidade e morbidade. Fixação definitiva é necessária para estabilização, parafusos percutâneos são uma opção no sacro. Estruturas nobres próximas à região trazem riscos à fixação. Assim, é importante conhecer a anatomia tridimensional da região posterior da pelve. Desvios da mão do cirurgião da ordem de 4º podem direcionar os parafusos àquelas estruturas; dismorfismos do sacro superior e redução ruim da lesão podem contribuir para mau posicionamento dos parafusos. Este estudo objetiva avaliar as dimensões do corredor de segurança para inserção segura de parafuso iliossacral e relações com sexo e idade dos pacientes. MÉTODOS: Selecionadas randomicamente 100 tomografias computadorizadas de pelve de pacientes sem doenças pélvicas, atendidos em hospital terciário de ensino. Feitas medições por computador, calculada por dois métodos a área mais segura para inserção de parafusos, resultado expresso em mm (não em graus), para ser mais uma referência cirúrgica. RESULTADOS: Houve diferença significativa no tamanho da vértebra sacral analisada, que tem volume maior em homens do que em mulheres. Não houve significância estatística entre tamanho vertebral e idade. Encontrou-se pelos dois métodos área segura para inserção de parafusos. Conclusões: A idade não influencia o tamanho do corredor. O cirurgião tem um corredor de segurança considerado menor ao inserir parafusos em uma pelve feminina do que masculina. ABSTRACT

O osteoma osteoide é um tumor benigno do osso que se apresenta geralmente com dor noturna em adultos jovens, aliviada por repouso e salicilatos. Pode acometer qualquer osso, mas sua ocorrência no ilíaco é infrequente. Os autores descrevem dois casos de osteoma osteoide intramedular, localizado junto à articulação sacroilíaca, com sintomas que simulavam dor ciática e cujo diagnóstico se deu de forma tardia, apesar de o exame radiológico inicial mostrar lesão esclerótica em ambos os casos. O diagnóstico foi ratificado por tomografia computadorizada e o tratamento proposto foi a ressecção em bloco da lesão, com excisão do nidus. O diagnóstico definitivo foi confirmado pelo exame histopatológico. No seguimento de longo prazo, ambos estão assintomáticos e observou-se completa remodelação óssea no sítio cirúrgico. Os autores enfatizam as características típicas do tumor, a localização não usual, o diagnóstico diferencial e o tratamento.


Osteoid osteoma is a benign bone tumor that generally presents with nighttime pain among young adults and is relieved by rest and salicylates. It can affect any bone, but occurrences in the iliac are unusual. The authors describe two cases of intramedullary osteoid osteoma next to the sacroiliac joint, with symptoms that simulated sciatic pain. The cases were diagnosed late, although the initial radiographs showed sclerotic lesions in both cases. The diagnosis was confirmed by means of CT scan and the nidus was excised surgically through en bloc resection. The definitive diagnosis was given by means of histopathological examination. Over long-term follow-up, both cases remained asymptomatic and complete bone remodeling at the surgical site was observed. The authors highlight the typical characteristics of the tumor, the unusual location, the differential diagnosis and the treatment.

ABSTRACT

PURPOSE: This study was performed to evaluate the effect of aspiration thrombectomy for the treatment of ileofemoral deep vein thrombosis. METHODS: We reviewed the records of sixty-two patients who were treated with catheter-directed thrombolysis (CDT) at our institution between November 2001 and October 2007. The patients were divided into two groups: those who were treated by CDT with aspiration thrombectomy (AT) (the aspiration group; 33 patients) or those who were treated with using CDT alone (the CDT alone group; 29 patients). The obtained data included the demographics, the procedural details, the periprocedural evaluation and the thrombus resolution. RESULTS: The treatment time with using CDT only was 40.93+/-15.59 hours compared with 23.18+/-8.22 hours with using AT (P<0.001). The mean dose of urokinase with using CDT only was 2.48+/-0.82 million IU as compared with 1.60+/-0.54 million IU with using AT (P<0.001). The use of AT did not improve the overall lytic success (P=0.084), but more patients had complete thrombus resolution (13 vs. 22 patients, respectively). There was no difference in symptom improvement or the long term patency rate between the two groups. There was no major morbidity or mortality. The prophylactic IVC filters were inserted in 31 patients and entrapped thrombi were founded in 3 patients of the aspiration group. Evidence of venous reflux was identified in the CDT only group (3 patients; 10.4%). CONCLUSION: These results suggest that the use of AT offers more effective thrombus removal in less time and with using a lower dose of thrombolytic agents. When performing AT treatment, prophylactic IVC filter insertion should be considered for preventing floating thrombi.

ABSTRACT

PURPOSE: Deep vein thrombosis is usually managed conservatively or with radiologic intervention. Yet deep vein reconstruction is of value as one of the treatments for a small group of patients who present with significant chronic venous insufficiency due to hypoplastic, absent or occluded deep venous outflow such as occurs because of chronic ilio-femoral vein thrombosis. Therefore, we evaluate the improvement of symptoms and graft patency after a Palma-Dale operation as a treatment modality for chronic ilio-femoral vein thrombosis. METHODS: From January 2001 to August 2008, a retrospective study was performed on 15 patients with chronic ilio-femoral vein thrombosis that was treated with a Palma-Dale operation. RESULTS: The mean age of the patients was 51.1+/-15.9 years. The ratio of males to females was 8 to 7. All the patients had lower limb swelling. A Palma-Dale operation was performed on all the patients. Simultaneous thrombectomy was done for 1 patient and arteriovenous fistulas were used to improve graft patency in four patients. The mean follow-up period was 36.3+/-24.0 months. Postoperatively, there was improvement of symptoms in all the patients. However, there was recurrence of symptoms in three patients after two or three months. The rate of graft patency was 84.1% at one year after surgery. CONCLUSION: The Palma-Dale operation has the possibility of recurrence of deep vein thrombosis and postoperative complications. Yet in patients with significant chronic venous insufficiency due to chronic ilio-femoral vein thrombosis, this type of surgery is obviously of value with respect to improvement of symptoms and an acceptable graft patency rate.

ABSTRACT

PURPOSE: To evaluate the effectiveness of iliosacral screw fixation with anterior plating in the management of an unstable pelvic ring injury. MATERIALS AND METHODS: Nineteen patients with an unstable pelvic ring injury were enrolled in this retrospective study. All patients were followed up for at least 1 year. The mean age of the patients was 43 years. According to the AO-OTA classification, there were five B2 injuries, 11 C1 injuries, and 3 C2 injuries. After anterior fixation by plating, ilio-sacral screw fixation was performed percutaneously under the C-arm guide. RESULTS: All cases united except for 1 case of nonunion at the pubic ramus. The radiology results showed 9, 7, 2 and 1 case of anatomic, nearly anatomic, moderate and poor reduction, respectively. Sixteen out of 19 patients had a good or excellent functional result. Two moderate and one poor result were from an unsatisfactory reduction in a type C injury with the residual neurological signs. Screw misplacement with neurological compromise occurred in one patient but there were no adverse sequelae after its removal. Regarding the complications, there were two cases of screw loosening, two cases of anterior metal failures, and 1 case of a deep infection. CONCLUSION: Percutaneous ilio-sacral screwing with anterior plating may be a useful method for treating unstable pelvic ring injuries, and the reduction quality and neurological signs are important.

ABSTRACT

Iliolumbar artery injury is a rare but well-known complication of abdominal trauma and is usually associated with pelvic bone fracture. If a pseudoaneurysm develops and ruptures, it is a serious condition due to the enduing massive intraperitoneal or retroperitoneal bleeding. Superselective embolization has become the most effective treatment for pelvic hemorrhage caused by iliolumbar artery injury in which early detection and treatment are very important. We report a case of successful transarterial embolization of a pseudoaneurysm following blunt trauma in 32-year-old female.
