ABSTRACT

@#Noise is a common occupational hazardous factor in the workplace. In addition to the specific damage to the auditory , system of workers it can also harm the cardiovascular system and cause a serious disease burden. The mechanism of , occupational noise on the cardiovascular system of workers is mainly oxidative stress inflammation and vascular endothelial , - - - damage. As a stressor noise mainly leads to the changes of sympathoadrenal medullary system and hypothalamic pituitary , , , - , adrenal axis. The substances that play an important role include catecholamines cortisol angiotensin Ⅱ endothelin 1 - endothelial nitric oxide synthase and interleukin 6. The population epidemiological studies have shown that occupational noise , exposure can lead to elevated blood pressure abnormal electrocardiogram and elevated blood lipids in workers. The influencing ( , , ) factors include noise characteristics intensity cumulative noise exposure and frequency and noise combined with other ( , , , - , occupational hazards such as high temperature welding fumes organic solvents hand transmitted vibration and work ) , , , shifts . However due to the influence of research conditions experimental design and other factors some research conclusions still have limitations. More prospective and comprehensive studies are needed to verify the relevant conclusions in the future.

ABSTRACT

Objective@#To investigate the impact of informal care satisfaction on quality of life on the elderly by establishing a structural equation model between them and introducing two mediating variables of frailty and depressive factors.@*Methods@#A total of 210 elderly over 60 years old from 6 communities of 2 urban areas in Qingdao were surveyed by general information questionnaire, the informal care satisfaction scale, frailty phenotype, depression scale and the SF-36 brief health scale.@*Results@#There was a direct effect on quality of life of daily life care dimension in informal nursing satisfaction (β=0.160, P<0.01) and an indirect effect of debilitating as intermediary (β=0.055, P<0.01). There was a direct effect on quality of life (β=0.130, P<0.01) and an indirect effect mediated by depression (β=0.132, P<0.01). There was a direct effect on quality of life (β=0.150, P<0.01) and an indirect effect mediated by debilitating and depression (β=0.147, P<0.01).@*Conclusions@#We should pay more attention to the impact of informal care on the quality of life on the elderly. Taking corresponding measures to promote the use of informal nursing and improving the quality of informal care is of great significance to improve the quality of life of the elderly and promote healthy aging.

ABSTRACT

This study reviewed the history and major issues of the reimbursement system reform on public hospitals in China,and analyzed the impact mechanism of specific reform approaches including price adjustment of health service and government special appropriation on the health care services provided by hospitals using qualitative analysis methods.This study has the following findings:if the public hospital is allowed to operate independently,the compensation by health service price adjustment will encourage hospitals to increase their service provision;the compensation method of government special appropriation will decrease the amount of service,and probably cut back the amount of drugs sold;with both reforms combined,hospitals will be encouraged to increase their medical service provision to make up for the revenue lost caused by canceling drug mark-ups.Finally,there were several policy suggestions.
