ABSTRACT

Authors present a case of vaginal implantation metastasis following a diagnosis of endometrial cancer. A 58-year-old lady presented with postmenopausal bleeding and was later diagnosed to have endometrial cancer. She underwent an extra-fascial hysterectomy and bilateral salpingo-oophrorectomy with pelvic lymphadenectomy for Stage 3A endometrioid adenocarcinoma of the endometrium. The lymph nodes and cervix were free from the disease and no lymphovascular invasion was seen on the pathological specimen. She defaulted adjuvant radiotherapy and was lost to follow up. Six months later she had a tumour recurrence at the vaginal introitus just below the urethral orifice and awal from the vaginal vault with similar histopathological findings as previous cancer. This case highlights the rare occurrence of implantation metastasis of endometrial cancer.

ABSTRACT

Preoperative biliary drainage (PBD) is an important part of preoperative management of patients with hilar cholangiocarcinoma which could reduce serum total bilirubin,remove jaundice,improve liver function,and reduce the mortality and morbidity.Although PBD is widely used in biliary surgery now,there are still several controversial issues in clinical applications about the indication of PBD,the best way of PBD,implantation metastasis of PBD and so on.With the development of medical image and surgical technology,we had a better understanding of PBD now.This review summarizes the recent scenario and current advancement about the above-mentioned controversy.

ABSTRACT

The occurrence of metastatic cancer from the gallbladder to the skin is uncommon and only less than ten cases of distant cutaneous metastases of gallbladder cancer have been reported in literatures. However, cutaneous implantation metastases of gallbladder cancer following laparoscopic cholecystectomy or percutaneous biliary catheter have been described more commonly than distant metastasis. We present herein a case of cutaneous implantation metastasis of gallbladder adenocarcinoma at the sites of percutaneous transhepatic biliary drainage (PTBD).

ABSTRACT

The implatnation of malignant cells along the needle tract is an extremely rare complication after a percutaneous fine-needle aspiration biopsy(FNAB). However, it is very serious and may result in a change in the prognosis of lung cancer, especially in the curable early stage(T1-2,N0,M0). Recently, we experienced two cases of such complications. A 43 years old female underwent a fine needle aspiration biopsy and a right middle lobectomy with adjuvant chemotherapy due to an adenocarcinoma(T2N0M0). Two years later, a new tumor developed at the site of the needle aspiration biopsy. It had the same pathological findings as the previous lung cancer. Therefore, it was concluded to be an implantation metastasis, and she was treated successfully by a right pneumonectomy and a resection of the chest wall mall with adjuvant radiotherapy. In another case, a 62 years old man was diagnosed with squamous cell lung cancer by a fine needle aspiration biopsy and underwent a right upper lobectomy(T2N0M0) with adjuvant chemotherapy. eight months later, a protruding chest wall mass developed at the aspiration site. It showed the same pathological findings as the previous lung cancer. Consequently, a total excision of the mass with adjuvant radiotherapy was done. Two years after the second operation, although the right lung was intace a metachronous squamous cell lung cancer was found at the left lower lobe. The two patients were still alive 15 and 37 months after thenresection of the chest wall mass, respectively.

ABSTRACT

Percutaneous transhepatic biliary catheterization (PTC) has not only been increasingly used in the localization of benign and malignant lesions, but it is also considered rapid, safe and effective method to establish internal and external drainage for decompression of biliary obstructions. Infections and granulomas at the catheter entry site can occasionally develop, but are easily managed. However, such technique, when used both for obtaining malignant cells for diagnosis and for percutaneous decompression of the biliary tract can exposure the patient at risk for dissemination of the tumor along the catheter tract. This rare complication has been observed in a patient who underwent percutaneous transhepatic biliary drainage (PTBD) for malignant biliary tract obstruction. Although tumor cell seeding along the catheter tract is a very rare complication, we think that PTBD should be avoided when curative resection is planned.
