ABSTRACT

Background: Clinically suspicious oral lesions are usually first dealt with an incisional biopsy. The management and treatment plan of these entities depends mainly on this report. The disparity in incisional and excisional biopsy report is an important area of concern affecting the patient’s management. Objective: Aim of the study was to compare retrospectively the incisional (presurgical) and excisional (post-surgical) biopsy reports of proven cases of carcinomas. Material and Methods: A total of 98 excisional biopsy cases of proved oral carcinoma were selected. Both incisional and excisional biopsy reports were retrieved. Sex predilection, Frequency of site of tumor, correlation between incisional and excisional biopsy, type of biopsy and change in the status were studied and statistically analyzed. Significance between Pre-biopsy and post biopsy was statistically analyzed using Chi-Square Tests. Results: There was concordance of 66.3% in incisional and excisional biopsy report. 33.7% cases showed disparity. Upgrading in the excisional biopsy report was noted in 15.30% cases. Downgrading was noted in 14.26% cases. When statistically analyzed the difference in pre-and post-biopsy was statistically insignificant. Conclusion: Incisional biopsy was found to have certain restriction in the valuation of Oral lesions. Clinicians should be vigilant the possibility of under diagnosis from incisional biopsy and even undetected carcinoma in the incisional specimen, especially of nonhomogeneous lesions.

Se presenta el caso clínico de un paciente de 58 años de edad, quien fue atendido en el Servicio de Ortopedia y Traumatología del Hospital General Docente "Dr. Juan Bruno Zayas Alfonso" de Santiago de Cuba por presentar aumento de volumen en la cara posterior del muslo izquierdo, de consistencia pétrea y adherida a planos profundos, no doloroso a la palpación superficial y profunda, además de parestesia en muslo y pierna izquierdos. Los resultados de los estudios clínicos, imagenológicos e histopatológicos efectuados permitieron diagnosticar un rabdomiosarcoma pleomórfico, para lo cual se indicó tratamiento médico y quirúrgico, así como seguimiento por oncología.


The case report of a 58 years patient is presented, who was assisted in the Orthopedics and Traumatology Service from "Dr. Juan Bruno Zayas Alfonso" Teaching General Hospital in Santiago de Cuba, for presenting an increase of volume in the posterior face of the left thigh, of stony consistency and adhered to deep planes, not painful to the superficial or deep palpation, besides paresthesia in left thigh and leg. The results of the clinical, imagenological and histopathological studies allowed to diagnose a pleomorphic rhabdomyosarcoma, for which medical and surgical treatment was indicated, as well as oncology follow-up.

Se presenta el caso clínico de una paciente con antecedentes de gastritis, que acudió al consultorio médico de su área de salud por presentar aumento de volumen en paladar, con crecimiento progresivo, acompañado de molestias para masticar y deglutir, la cual fue remitida al Servicio de Cirugía Maxilofacial del Hospital Clinicoquirúrgico Docente "Dr. Joaquín Castillo Duany" de Santiago de Cuba. Los resultados de los exámenes complementarios, incluida la biopsia incisional en paladar, confirmaron que se trataba de un carcinoma ameloblástico. Se indicó seguimiento clínico en el Hospital Oncológico Docente "Conrado Benítez" de la mencionada ciudad.


The case report of a patient with history of gastritis that visited the doctor's office of her health area due to an increase of volume in palate, with progressive growth, accompanied by problems to chew and to swallow is presented. She was referred to the Maxillofacial Surgery Service of "Dr. Joaquín Castillo Duany" Teaching Clinical Surgical Hospital in Santiago de Cuba. The results of the additional tests, including the incisional biopsy in palate, confirmed that it was an ameloblastic carcinoma. Clinical follow-up in "Conrado Benítez" Teaching Oncological Hospital in Santiago de Cuba was suggested.

Introdução e revisão da literatura: o carcinoma espinocelular da cavidade bucal (CECB) corresponde à maior parte dos cânceres de boca, acometendo em maior escala indivíduos do sexo masculino, acima dos 45 anos de idade e que tenham o hábito do tabagismo e/ou etilismo. Um dos aspectos mais importantes acerca do CECB é seu diagnóstico precoce, o qual é melhor realizado por meio de biópsia incisional seguida de exame histopatológico. Descrição do caso: o presente trabalho apresenta o caso clínico de uma lesão de CECB extensa em paciente do sexo masculino, leucoderma, com 54 anos de idade, agricultor, que teve seu diagnóstico realizado por meio de biópsia incisional e exame histopatológico, com posterior encaminhamento para tratamento médico.


Introduction and literature review: squamous cell carcinoma of the oral cavity (SCCOC) corresponds to the majority of oral cancers, affecting to a greater extent among males above 45 years old and that have the habit of smoking and / or alcohol. One of the most important aspects about the SCCOC is early diagnosis, which is more appropriately performed by incisional biopsy followed by histopathological examination. Case description: this paper presents a case study of a extensive SCCOC in a male patient, caucasian, with 54-year-old farmer, who had their diagnosis made by biopsy and histopathology, with subsequent referral to medical treatment.

ABSTRACT

Pseudocyst of the auricle is an uncommon condition, which typically presents as an asymptomatic, noninflammatory swelling of the external ear. Most of the patients are usually young, healthy males without trauma. Histological examination is characterized by the intracartilaginous accumulation of serous fluid without an epithelial lining. The pathogenesis is unknown. Some theories about the release of lysosomal enzymes or inflammatory mediators and embryologic dysplasia of the auricle cartilage have been suggested. Treatment of pseudocyst of the auricle has usually involved a simple aspiration and compressive dressing, but this may often cause the fluid to reaccumulate. Treatment by surgical excision may result in a permanent deformity and scar of the auricle. Therefore, various treatments have been suggested. We report a case of pseudocyst of the auricle successfully treated by incisional biopsy only.
