ABSTRACT

Background:Overfilled and under filled tube are most important cause of pre-analytical error and failure to complete requested examination in sample. The study was performed to analyse relationship betweentime of receiving of sample, location of sample, type of vacuum tube, method of collection, total volume required in a patient and volume incompleteness.Material And Methods:The study involved collection and patient samples data, i.e. receiving time of sample, place of collection, type of vacuum tube, blood collection method, total volume required in a patient, measurement of volume before processing. Average incompleteness of vacuum tube was calculated and average relationship between incompleteness and these parameters done.Result:Average incompleteness in OPD samples is better than Non-OPD samples. Averageincompleteness is better during 9 am to 12 am than early morning hour s. Incompleteness with relation to vacuum tube type is in general Plain > (EDTA/Fluoride) > Citrate. Average incompleteness better in sample collected with vacuum tube needle with holder than use of syringe. Average incompleteness increases as total volume required in a patient increases. Conclusion:Dedicated phlebotomist and use of vacuum tube needle with holder is required to bring overall improvement in completeness of blood collection.

ABSTRACT

This paper aims to reflect on the issue of masculinity and femininity as both men's and women's mental functions. We take into account the fact that masculinity and femininity coexist inside our psychic life, creating, preserving, and denying aspects of each particular experience. The author proposes some extended thoughts as she points out that, if these functions are not integrated inside our psychic life, they express themselves in rivalries inside ourselves and in our loving, fraternal, and professional relationships. It harms the reciprocal and complementary encounter that underlies the human incompleteness in its search for creative relationships.

ABSTRACT

This article particularizes the confrontation between the public policy of social protection and the values of the capital society. Due to necessities related to dependence, fragility and victimization, its preventive and restorative action finds obstacles to complete attention, to exercise social control and to assign whole public responsibility. The social worker moves around this complex field that paradoxically operates barriers of access for social protection rights.

ABSTRACT

Intake and Output (I/O) records in hospitals were often found to be incomplete and illegible. The form used to record I/O is not user-friendly — i.e., they feature miniscule boxes, ‘total’ lines that do not correspond with shift changes and lack of instructions. Complaints often received from Specialists & Doctors regarding calculation errors or no totalling of I/O. Moreover, Nursing Sisters objective rounds often saw incompleteness of I/O chart. This study aims to identify the types of mistakes in recording the existing I/O chart. The second aim is to find out whether shift totalling of I/O chart helps in reducing mistakes. We try to determine whether the identified mistakes were repeated in the new I/O Chart. This study was conducted from October till December 2010 in 9 selected wards in Sibu Hospital. Data collection was divided into 3 phases. A pre-implementation audit using a checklist was carried out. The compliance rate of completeness of documentation of I/O Chart was 63%. A one month trial of new I/O chart was being done in the selected 9 wards. Post implementation audit showed a significant improvement of compliance rate (88%). Feedback from health care workers (N=110) showed that, 89% of doctors (n=17) and 60% of nurses (n=93) in the sample prefer to use the new format as more practical and relevant to the changing shift of nurses and doctors’ ward round. It is suggested to implement the new format to increase compliance rate of documentation of I/O charting. Briefing should be given to nurses periodically and the new format should be introduced to nursing students in nursing colleges.
