ABSTRACT

Objective@#To investigate the awareness and training needs of incontinence-associated dermatitis (IAD) among clinical nurses, so as to provide insights into the standardization of clinical nursing of IAD. @*Methods@#Clinical nurses with more than one year of service in Lanxi People's Hospital were selected as study subjects in October 2021. The demographics, awareness of IAD knowledge and training needs were collected using a self-designed questionnaire. The age-, duration of service-, academic degree-, professional title- and department-specific awareness of IAD knowledge was analyzed.@*Results@#A total of 420 questionnaires were allocated, and 409 valid questionnaires were recovered, with an effective recovery rate of 97.38%. All respondents were females, with a mean age of (37.68±4.82) years, and a mean service duration of (18.72±3.46) years. There were 199 nurses with a primary title (48.66%), 169 nurses with a intermediate title (41.32%), and 41 nurses with a senior title (10.02%). There were 217 nurses with a bachelor degree or above (53.06%), and 208 nurses from internal medicine and surgery departments (50.86%). The overall awareness of IAD knowledge was 74.57% among the respondents, and the awareness rates of IAD definition, the difference between IAD and pressure ulcers and influencing factors of IAD were 86.06%, 83.37% and 82.15%, respectively. The nurses at ages of 41 to 53 years (89.01%), having a senior title (94.24%), with a nursing duration of 21 years and longer (92.97%) and participating in IAD training during the recent one year (77.41%) had a high awareness rate of IAD-related knowledge. There was a high proportion of needs for training on basic IAD-related knowledge (396 nurses, 96.82%).@*Conclusions@#The awareness of IAD knowledge requires to be improved among clinical nurses. The training on IAD knowledge is recommended to be improved.

ABSTRACT

Objective To investigate the effects of two intervention methods, hydroglue dressing and skin powder combined with skin protective film, in severe tumor patients with moderate and severe incontinence dermatitis. Methods A total of 68 patients with moderate and severe incontinence dermatitis were selected from ICU in Shanghai Oncology Hospital, and randomly divided into 2 groups by random digit table, each group included 34 cases, on the basis of the consistency of skin care, the experimental group used water adhesive dressing protection, every 1-3 days replacement, the control group used skin care powder joint skin protective film, three times a day, two groups of intervention for 10 days in a row. The healing effect, healing time, nursing time and nursing cost of IAD were observed. Results Of the 68 patients, 2 were transferred to the local hospital for treatment and 1 died. Finally, 65 patients were completed, 32 in the control group and 33 in the experimental group. In the average nursing time, the difference was statistically significant in the experimental group (1.83 ± 0.78) min and the control group (4.45 ± 0.52) min (t = 15.908, P<0.01). The experimental group took less time to care for IAD than the control group, which were (45.78 ± 25.45)min and (110.97 ± 27.22) min. The difference was statistically significant (t=9.967, P<0.01). The cost of care in the experimental group was lower than that in the control group, which was (164.06 ± 60.32)yuan and (280 ± 0.00)yuan. The difference was statistically significant (t=11.041, P<0.01). Conclusions The water glue dressing can be used for the nursing of moderate and severe incontinence dermatitis, and it can effectively reduce the nursing time of nursing staff and reduce the economic cost.

Objective@#To analyze the characteristics of the literatures and research hotspots of incontinence-associated dermatitis (IAD), in order to provide references for clinical research.@*Methods@#Bibliometrics method was conducted in Chinese National Knowledge Infrastructure (CNKI) and Wanfang database, VIP database, China Biology Medicine disc (CBMdisc), "PubMed", "Web of Science" from January1, 1983 to October 30, 2018. The distribution of literature, author, document type and fund were analyzed.@*Results@#A total of 1 044 articles were included in 277 journals, including 861 articles in China and 183 articles abroad. The total amount of literatures showed an upward trend with the fastest growth rate in 2018. Most of the literatures (24.71%, 211/861) were published in Jiangsu Province while the U.S. was the first among the published countries. And English was the main language. The institutions of publication were from hospitals(95.82%, 825/861), and then the universities (3.84%, 30/861). The type of literatures was mainly research. And funds had a great influence on articles, but the current support rate was low.@*Conclusions@#IAD is one of the research booms both at home and abroad in recent years. The amount of literatures has increased substantially, but the breadth and depth of research in China still needs to be improved, especially about narrow research content and the population samples.

Objective To improve the effectiveness of incontinence-associated dermatitis (IAD) prevention and management by establishment skin care quality indicators and controlling the implementation process of incontinence care. Methods The implementation rate of nursing measures (the treatment rate of incontinence, the effective collection of fecal implementation rate, and the rate of skin protection implementation) were used as the process indicators , and the incidence of IAD was used as the outcome indicators. 2747 incontinence patients from July to December 2016 before the implementation of the process control were used as the control group, and 3, 069 incontinence patients from July to December 2017 after the implementation of the process control were used as observation groups. Comparing the difference between the two groups of process indicators and outcome indicators. Results The control group's implementation rate of incontinence, effective collection of fecal implementation rate, and skin protection implementation rate were 54.2% , 63.8% , and 27.7% , respectively. The observation group was 91.8%, 94.8%, and 92.8%, respectively. The difference between the two groups was statistically significant. (χ2=5.032, P=0.025; χ2=6.574, P=0.010; χ2=20.038, P=0.000;). The incidence of IAD in the control group was 28.9%, and the incidence in the observation group was 11.2% . The difference between the two groups was statistically significant (χ2=288.402, P=0.000). Conclusion The establishment of incontinence nursing quality indicators and the implementation of process control will help improve the level of incontinence care and reduce the incidence of IAD.

ABSTRACT

Objective Incontinence-associated dermatitis(IAD) is a common skin disease, but there is still lack of evidence on which kind of skin proctectant can provide effective prevention and cost reduction. The article aimed to explore the clinical effect and cost-effectiveness of two skin protectants based on structured skin care regimens in the nursing care of incontinent patients and provide evidence for effective prevention of IAD.Methods A total of 124 patients with incontinence were randomly selected and divided into observation group (62 cases) and control group (62 cases). On the basis of the structured skin care program which focused on incontinence management, cleansing and protection, the patients in the observation group were treated with the composite zinc oxide ointment for 7 days, while patients in control group were treated with ostomy powder combined with skin barrier film for 7 days. The main observation indicators were the incidence of IAD and the cost of nursing materials.Results All 62 patients in the observation group completed the study, while in the control group 2 patients fell off because of death and discharge from hospital and 60 patients completed the study. The per-protocol analysis showed that the incidences of IAD in two groups were 6.45% and 20.00%, the intention-to-treat analysis showed 6.45% and 22.58%, and there was significant difference between groups (P<0.05). The nursing cost of the observation group (239.71±48.55) was lower than that of control group (264.35±61.55), and the difference was statistically significant (P<0.05).Conclusion The use of the composite zinc oxide ointment based on a structured skin care regimen can improve the preventive effect of IAD and reducing costs.

ABSTRACT

Objective To investigate the effects of OB built-in tampon indwelled in anus combined with acupoint massage in ICU patients with incontinence-associated dermatitis (IAD). Methods 100 patients with incontinence hospitalized in ICU were randomly assigned to control group (n=50) versus combined therapy group (n=50). The patients in the control group accepted routine nursing while the patients in the combined therapy group were treated with OB built-in tampon indwelled in anus combined with acupoint massage besides the routine nursing care. The two groups were compared in terms of incidence rate of IAD, IAD occurrence time, severity of IAD and incontinence-related nursing workload. Results The incidence rate of IAD and the severity of IAD in the combined therapy group were lower than those of the control group.The IAD in the combined therapy group occurred later than in the control group. Thetime for incontinence-related nursing and the frequency of perineum washing in the combined therapy group were shorter and lower than in the control group. Conclusions The application of OB built-in tampon indwelled in anus combined with acupoint massage in ICU patients with incontinence can not only reduce the incidence rate and severity of IAD but also lower the workload of nursing.The therapy is worthy of clinical application.

Objective To explore how to carry out the evidence-based nursing care on multiple myeloma(MM)patients with incontinence associated dermatitis(IAD)by combining with a case of MM in clinic.Methods According to the patient′s situation, the clinical questions were pertinently put forward and converted according to the principle of PICO.And then some databases such as NGC,CDSR,Medline and PubMed were retrieved from January 1,2006 to December 31,2016 for collecting high-quality clinical evidences of the prevention and nursing care of IAD.Results Three clinical practice guidelines,1 expert opinion,3 systematic re-views and 3 RCT were included.According to the final retrieval results and combining with the patient′s condition and wish,the ab-sorbent products were appropriately used,and the contaminated skin was cleaned in time;at the same time the disposable non-woven fabric coordinated with pH balanced skin cleanser was selected to clean skin gently.Based on the principles of cleaning,preserving moisture and skin protection,some superior products,such as 3-in-1 incontinence care wipes or Cavilon transparent protective film, were given priority to select and regularly evaluated.Conclusion Applying the evidence-based evidence can pertinently provide the individualized nursing scheme for the patients,thus reduces the incidence rate of IAD.

ABSTRACT

Objective To explore the influencing factors of incontinence-associated dermatitis (IAD) complication to severe stroke patients, and to provide basis for prevention of IAD.Methods Clinical data of 125 severe stroke patients was collected, and was grouped into IAD group and non-IAD group according to with or without complicated with IAD.Correlations of IAD and gender, age, obfuscation, temperature≥38 ℃, BMI, complicated with chronic disease, NIHSS score, stroke type, nutrition, immunity were analyzed.Results There were 30 of 125 severe stroke patients complicated with IAD, and the incidence of IAD was 24.00%,with 20 mild cases,7 moderate cases and 3 severe cases.Logistic aggression suggested that age (OR=7.515, 95%CI: 2.369-21.454), temperature (OR=5.225, 95%CI: 2.612-10.774), NIHSS score (OR=3.467, 95%CI: 1.455-7.767), stroke type (OR=0.596, 95%CI: 0.387-0.895), hypoproteinenia (OR=8.120, 95%CI: 3.574-27.644) were influencing factors of IAD.Conclusion Senility, fever≥38 ℃, NIHSS score≥12, cerebral arterial thrombosis, hypoproteinemia could be risk factors of complication with IAD among severe stroke patients, and preventive interventions should be taken to reduce the risk of IAD.

Objective To compare the reliability and validity of 2 incontinence- associated dermatitis (IAD) risk assessment tools (IAD risk assessment scale and PAT) in patients with incontinence, and to search for the best risk assessment tool. Methods A total of 101 IAD cases were chosen from the Affiliated Hospital of Nantong University.Reliability was evaluated by the inter- rater reliability,internal consistency,item sensitivity analysis and retest reliability; validity was evaluated by the structure validity and predictive validity. Results Cronbach α was 0.313 (IAD risk assessment scale) and 0.421(PAT);Cronbach α coefficient was the highest after the items that Change sheets and mats and relevant factors were removed respectively in two tools,which were 0.431, 0.428; The correlation between the total scores were 0.711 (P<0.01)(IAD risk assessment scale)and 0.498 (PAT)(P<0.01);Area under curve were 0.661 (IAD risk assessment scale),0.864 (PAT); the best cut-off value to forecast a poor prognosis was 53.5 (IAD risk assessment scale),6.5 (PAT). Conclusions The reliability and validity of IAD risk assessment scale and PAT are low. The change of sheets and mats and Related influencing factors are needed to be adjusted and revised.

Objective To explore the effect of stoma bag for preventing and treating incontinence-associated dermatitis (IAD) in the patients with fecal incontinence .Methods All randomized controlled trials (RCTs) on the effect of stoma bag for preventing and treating IAD in critical patients with fecal incontinence were collected by retrieving the Chinese and foreign databases such as the Cochrane Library ,Web of Knowledge ,OVID ,Pubmed ,CNKI ,VIP and Wanfang Data .The meta analysis was performed by a-dopting RevMan 5 .3 software .Results A total of 22 RCT involving 1993 patients were included .The analysis results showed that the application simple stoma bag nursing in the patients with fecal incontinence was conducive to reduce the incidence rate of IAD [OR=0 .06 ,95% CI(0 .02-0 .13) ,P<0 .01] ,stoma bag combined with vacuum aspiration could reduce the incidence rate of IAD [OR=0 .09 ,95% CI(0 .04-0 .21) ,P<0 .01] ,stoma bag combined with skin protective agent could decrease the incidence rate of IAD [OR=0 .08 ,95% CI(0 .03-0 .22) ,P<0 .01] ,stoma bag simple application could improve the cure rate of IAD [OR=12 .97 , 95% CI(5 .92-28 .41) ,P<0 .01] ,stoma bag combined with vacuum aspiration could increase the cure rate of IAD [OR=3 .68 , 95% CI(2 .15-6 .30) ,P<0 .01] ,stoma bag combined with skin protective agent could improve the cure rate of IAD [OR=7 .01 , 95% CI(3 .95 -12 .43) ,P<0 .01] .Conclusion Stoma bags simple application ,combined with vacuum aspiration device and skin protective agent in preventing and treating fecal IAD is better than the routine skin care measures .However ,the quality of included studies is lower ,so a large sample and high-quality RCTs are needed for further study .

Objective To explore the effect of quality control circle on the prevalence of incontinence-associated dermatitis.Methods We set up a quality control circle group,analyzing the current situations,set the goals and analysed the causes of the high incidence of incontinence-associated dermatitis in the ICU after deciding a specific subject and then we developed strategies.The two groups were compared in terms of knowledge on the dermatitis,assessment of the dermatitis risks,the dermatitis-related management and the incidence of incontinence-associated dermatitis.Results After manipulation of the quality control circle,the aims of incontinence dermatitis related knowledge,risks assessment and management process were all achieved.The scores on nurses knowledge on the dermatitis,ability in solving problems,quality control operation,confidence,ability in coordination,and team work spirits were all significantly higher than those before the manipulation of the circle (P<0.01) and the incidence of incontinenceassociated dermatitis was statistically significantly lower (P<0.001).Conclusion The management based on the quality control in the ICU of neurology department can effectively reduce the neurology incidence of incontinence-associated dermatitis and improve the nurses professional skills.

ABSTRACT

A Unidade de Terapia Intensiva (UTI) é o local destinado ao atendimento de pacientes graves e durante a internação esses estão expostos a diversos fatores que podem levar a ocorrência de danos na pele. Dentre esses danos, a dermatite associada à incontinência (DAI). O objetivo geral do estudo é analisar o problema da DAI em pacientes adultos internados em uma UTI de um hospital público de Belo Horizonte - MG. Já os objetivos principais foram:identificar na literatura os fatores de risco para DAI em pacientes adultos,caracterizar os pacientes que desenvolveram a DAI, estimar a incidência,determinar os fatores de risco de DAI na UTI e propor um modelo de predição de risco. Trata-se de um estudo realizado em três etapas: revisão integrativa(RI), grau de concordância e estudo de coorte concorrente. Os fatores de risco para DAI identificados na RI foram: idade, comorbidades, nutrição, oxigenação,perfusão, temperatura, incontinência fecal e/ou urinária, atrito mecânico, uso agentes irritantes, capacidade cognitiva. Obteve-se uma incidência global de20,4% e uma densidade de incidência de 2,5 casos/100 pessoas -dia. Foram encontrados 19 fatores de risco que apresentaram associação significativa coma DAI. No modelo final de predição de risco encontrou-se: sexo masculino,trauma, hipnótico/sedativos, lactulona, suporte nutricional, fezes pastosas e ardência. Os resultados mostraram que a DAI é um achado comum em pacientes internados em UTI adulto e por isso merece atenção especial para que se tenha melhoria na qualidade da assistência de enfermagem prestada a esses pacientes.


The Intensive Care Unit (ICU) is the place for the care of critically ill patients during hospitalization and these are exposed to several factors that can lead to damage to the skin. Among such damage, dermatitis associated with incontinence (DAI). The overall objective of the study is to analyze the problemof DAI in adult patients admitted to the ICU of a public hospital in Belo Horizonte- MG. Since the main objectives were to identify the literature the risk factors forDAI in adult patients, to characterize patients who developed DAI estimate theincidence, determine the DAI risk factors in ICU and propose a risk prediction model. This is a study carried out in three stages: an integrative review (RI),degree of concordance and study concurrent cohort. The risk factors identifiedin DAI RI were age, comorbidities, nutrition, oxygenation, perfusion,temperature, fecal and / or urinary incontinence, mechanical friction, use irritants, cognitive ability. This yields an overall incidence of 20.4% and an incidence density of 2.5 cases / 100 person-days. They found 19 risk factors significantly associated with DAI. In the final model of risk prediction met: male,trauma, hypnotic / sedative, lactulona, nutritional support, loose stools and burning. The results showed that the DAI is a common finding in patients admitted to adult ICUs and therefore deserves special attention in order to have better quality of nursing care provided to these patients.

Objective To discuss the risk factors for incontinence-associated dermatitis (IAD) in elderly patients to provide the basis for the scientific research and formulation of effective precautions. Methods Totally 170 elderly patients were collected age, gender and other 10 data. The study allocated patients into case group and control group based on the end whether get incontinent dermatitis. Univarate and multivariate Logistic regression analysis were conducted to compare the 11 indicators mentioned above between the two groups to find out the risk factors. Results The types of incontinence, diarrhea, damp frequency, diabetes, and antimicrobials species were significantly different between the two groups (χ2=4.317-17.404, P 3 times per day and/or stool is thin and watery then patients, diarrhea and hemorrhoids patients, diabetes prevalence > 10 years or the level of blood glucose control with poor patients attention,and take effective intervention measures, focusing on prevention and cure together.

ABSTRACT

Objective To identify the risk factors associated with incontinence-associated dermatitis (IAD) in ICU patients with enteral nutrition associated diarrhea (ENAD), and to provide information for taking evidence-based preventive measures. Methods We used a case-control study to collect data. Information of the patients (n=55) and controls (n=55) were retrieved from the medical records of 2 intensive care units (ICUs). The patients with IAD and the controls without IAD were matched with age, gender and ward. Information on disease-related information, perineal environment, drug use and nutritional status were collected. Results The simple factor analysis showed that the risks of IAD were related to coronary heart disease (CHD )(χ2=3.96), composite variable for perfusion (χ2=7.33), APACHE II score (χ2=9.87), FiO2 (Z=-2.96), artificial airway (χ2=93.91), ENAD days (Z=-5.45), stool property (χ2=8.46), antibacterial drug use (χ2=5.55), steroid medicine (χ2=11.34) and enteral nutrition days(Z=-3.33), P<0.05. Logistic regression analysis showed that FiO2(P<0.05,OR=1.03, OR95%CI=1.01-1.06), APACHE Ⅱscore(P<0.05,OR=3.12, OR95%CI=1.01-9.64)and ENAD days(P<0.01,OR=1.64, OR95%CI=1.29-2.10)was independent risk factors for IAD. Conclusions The higher of FiO2, APACHE II score over 15 points and the longer of ENAD days, patients are more likely to occur IAD. Early monitoring and prevention of IAD are recommended to promote skin health.

Methods A total of 114 patients who were admitted into ICU and NICU of Hebei Xingtai People′s Hospital from December 2013 to November 2014 were selected. These subjects were divided into 3 groups randomly. All three groups received routine skin care, on this basis, skin protective membrane group (34 cases) used skin protective membrane for skin protection, semi-transparent dressing group (40 cases) adopted semi-permeable dressing for skin protection, while the combination group (40 cases) used skin protective membrane combined with semi-transparent dressing for skin protection. The incidence, occurrence and severity of IAD in the three groups were compared. Results The incidence rate of IAD in the combination group was 20.0%(8/40), 20.6%(7/34) in the skin protective membrane group, and significantly lower than 47.5% (19/40) in the semi-transparent dressing group, the difference was significant ( χ2=9.201, P<0.05). The occurrence time of IAD in the combination group was (4.75±0.46) days, which was significantly longer than those of the skin protective membrane group [(3.86±1.35) days] and the semi-transparent dressing group [(2.74±0.73) days], the difference was significant (F=17.120, P < 0.05). The IAD severity of the combination group scored 3.63 ±0.92, which was significantly lower than those of the skin protective membrane group (5.29±1.11) and the semi-transparent dressing group (6.74±1.79), the difference was significant (F=12.240, P<0.05). Conclusions Skin protective membrane combined with semi-transparent dressing have priority in decreasing IAD incidence rate, and delaying the occurrence time of IAD, and relieving the severity of IAD, which is better than either of the two.

ABSTRACT

Objective To investigate ICU nurses' perceptions towards incontinence-associated dermatitis.Methods Totally 300 ICU nurses in Suzhou were investigated with a self-designed questionnaire.Results College education was considered to be their main source of knowledge about incontinence-associated dermatitis in 53.2 percent of the nurses.99.8 percent of nurses thought college education was far enough for clinical practice.68.5 percent of nurses thought it was very important to train for knowledge of incontinence-associated dermatitis.Conclusions ICU nurses in Suzhou have less knowledge on incontinence associated dermatitis.It can not meet clinical needs.It is recommended that more incontinence-associated dermatitis knowledge should be added in the school education and continuing education in order to improve nurses' professional competency and to reduce the incidence rate of incontinence-associated dermatitis

ABSTRACT

Objective To examine the reliability and validity of the Chinese version of the perineal assessment tool (PAT),and explore its applicability in domestic patients with incontinence.Methods A convenience sample of 100 patients were recruited for evaluation.The patients with incontinence were assessed for lAD risk using the Chinese version of PAT.The reliability tests were conducted on internal consistency,interrater,test-retest,and the validity tests were conducted on content validity index(CVI),exploratory factor analysis,receiver operator characteristic curve (ROC).Results The Chinese version of PAT comprised 4 items,Cronbach's α was 0.512,and items related to the total score was appropriate; inter-rater reliability was 0.889; test-retest reliability was 0.791.CVI was 0.95.Exploratory analysis showed that the most interpretable solution consisted of 2 factors.The accumulative variance contribution explained 67.397% of variance of the total scale.Predictive validity was moderate,the best cut-off point was determined at 6.5 points.Conclusions Due to the special design,its internal consistency reliability was low; content validity was relatively sufficient,but it still needs further revision and test.Inter-rater reliability,test-retest reliability,construct validity,predictive validity was generally good,and it could be used as the screening tool of IAD risk.The results prompt PAT could be used as a benchmark,and we can adjust the entry of PAT to provide a more accurate IAD predicting tool by studying the IAD risk factors.
