ABSTRACT

La literatura ha hecho hincapié en la centralidad que cumplen los entornos alimentarios en las elecciones alimentarias y en el estado nutricional de la población. Objetivo: identificar las percepciones de padres, madres y apoderados, profesionales de establecimientos educacionales, funcionarios municipales, y feriantes pertenecientes a la zona sur de Santiago de Chile, respecto a las barreras y limitaciones de la puesta en práctica de una alimentación saludable en el ambiente alimentario escolar. Método: se realizaron seis grupos focales agrupando a 50 actores claves vinculados a los establecimientos educacionales de Ciudad Sur utilizando una muestra no probabilística. Se aplicó un análisis de contenido mediante la técnica de codificación temática buscando relevar los universos semánticos emergentes. Resultados: se identificaron nueve barreras, o semánticas, que caracterizan limitantes a la realización de la alimentación saludable en los entornos escolares desde la perspectiva de los participantes: la familia, kioscos escolares, la salida de los establecimientos escolares, gusto, determinantes socioeconómicos, política pública, falta de conocimiento, publicidad y disponibilidad de productos sin sellos. Conclusiones: las limitaciones asociadas al ambiente alimentario doméstico tienen una representación significativamente alta (47,9%) y contienen un carácter de responsabilización individual en su enunciación. Esta cultura explicativa es opuesta a la evidencia científica y académica respecto al funcionamiento de la conducta alimentaria, y a la centralidad de los ambientes alimentarios respecto a la facilitación u obstaculización del consumo de alimentos saludables o adecuados.


Literature has emphasized on food environments centrality in food choices and nutritional status. Objective: identify social perceptions of fathers, mothers and guardians, professionals from educational establishments, municipal officials, and stallholders belonging to the southern area of Santiago de Chile, regarding limitations of healthy diet implementation on school environments. Method: six focus groups were carried out grouping 50 key actors linked to educational establishments using a non-probabilistic sample. A content analysis was applied through thematic coding technique seeking to reveal emerging semantic universes. Results: Nine barriers, or semantics, were identified characterizing limitations to healthy eating habits in school environments: family, school kiosks, leaving school establishments, taste, socioeconomic determinants, public policy, lack of knowledge, publicity, and availability of products without seals. Conclusions: limitations associated with domestic food environment have a significantly high representation (47,9%) and contain an individualized responsibility feature. This explanatory culture is opposed to scientific and academic evidence regarding the functioning of eating behavior, and the centrality of food environments facilitating or hindering healthy food consumption.

ABSTRACT

AIM: To analyze the distribution frequency of gene polymorphisms of β receptor blockers, angiotensin receptor antagonists, angiotensin converting enzyme inhibitors, calcium antagonists, and diuretics in hypertensive patients from southern Anhui province, and provide a theoretical basis for gene detection of hypertension drugs and personalized medication. METHODS: Drug gene testing information from 839 hospitalized patients with hypertension at Yijishan Hospital of Wannan Medical College from July 2021 to April 2023 were collected, and the distribution frequency of each gene locus were analyzed. RESULTS: The genotype frequencies of ACE (I/D) I/I, I/D, and D/D were 42.1%, 46.0%, and 11.9%, respectively. the genotype frequencies of ADRB1 (1165G>C) G/G, G/C, and C/C were 8.3%, 40.0%, and 51.6%, respectively. The genotype frequencies of AGTR1 (1166A>C) A/A, A/C, and C/C were 90.2%, 9.8%, and 0.0%. The genotype frequencies of CYP2C9*3 (1075A>C) *1/*1, *1/*3, and *3/*3 were 91.3%, 8.7%, and 0.0%, respectively; the genotype frequencies of CYP2D6* 10 (100C > T) *1/*1, *1/*10, and *10/*10 were 25.0%, 36.6%, and 38.4%, respectively. The genotype frequencies of CYP3A5*3 (6986A>G) *1/*1, *1/*3, and *3/*3 were 7.0%, 39.0%, and 54.0%, respectively. The frequencies of NPPA (2238T>C) T/T, T / C, and C / C genotypes were 97.9%, 2.1%, and 0.0%, respectively. In addition, there was a significant difference in the genotype distribution frequency of multiple drug related gene loci in southern Anhui compared to other regions in China (P< 0.05). CONCLUSION: The genotype distribution frequency of hypertensive drug related gene loci had certain bias in southern Anhui, and were significant different from other regions in China, indicating that conducting genetic polymorphism testing of hypertensive drugs had certain guiding significance for the individualized application of hypertensive drugs in southern Anhui.

ABSTRACT

@#Objective To summarize and explore the individualized surgical treatment strategy and prognosis of anomalous aortic origin of coronary artery (AAOCA). Methods The clinical data of children with AAOCA admitted to Shanghai Children's Medical Center from March 2018 to August 2021 were retrospectively analyzed. Results A total of 17 children were enrolled, including 13 males and 4 females, with a median age of 88 (44, 138) months and a median weight of 25 (18, 29) kg. All patients received operations. The methods of coronary artery management included coronary artery decapitation in 9 patients, coronary artery transplantation in 5 patients and coronary artery perforation in 3 patients. One patient with severe cardiac insufficiency (left ventricular ejection fraction 15%) received mechanical circulatory assistance after the operation for 12 days. No death occurred in the early postoperative period, the average ICU stay time was 4.3±3.0 d, and the total hospital stay was 14.4±6.1 d. All the children received regular anticoagulation therapy for 3 months after discharge. The median follow-up time was 15 (13, 24) months. All patients received regular anticoagulation therapy for 3 months after discharge. No clinical symptoms such as chest pain and syncope occurred again. The cardiac function grade was significantly improved compared with that before operation. Imaging examination showed that the coronary artery blood flow on the operation side was unobstructed, and no restenosis occurred. Conclusion AAOCA is easy to induce myocardial ischemia and even sudden cardiac death. Once diagnosed, operation should be carried out as soon as possible. According to the anatomic characteristics of coronary artery, the early effect of individualized surgery is satisfactory, and the symptoms of the children are significantly improved and the cardiac function recovers well in the mid-term follow-up.

ABSTRACT

@#OBJECTIVE To provide reference for guiding the individualized drug therapy management of imatinib for gastrointestinal stromal tumor (GIST), with the goal of enhancing patient survival rates and improving their quality of life. METHODS Using a nominal group technique, a multidisciplinary (clinical, pharmaceutical and evidence-based) expert panel was formed to create the Consensus of Chinese Experts on Individualized Medication Management of Imatinib for Gastrointestinal Stromal Tumors outline through joint discussions. The expert panel conducted systematic retrieval, analysis, and summarization of the outline’s content, and reached relevant consensus based on China’s current situation, clinical needs, and research evidence. An external expert panel was also formed, comprising experienced multidisciplinary experts in clinical practice. Delphi method questionnaire was employed to openly collect the external experts’ opinions, which were then organized, summarized, analyzed, provided with feedback, revised, and finally formed into a consensus. RESULTS & CONCLUSIONS The drafting of this consensus included the clinical application of imatinib in neoadjuvant therapy for GIST patients, adjuvant therapy for adult patients with significant risk of recurrence after surgical resection, and drug therapy for patients with recurrent, metastatic, or unresectable tumors; pharmaceutical monitoring and long-term medication management. This consensus provides standardized processes and methods for medical institutions in individualized drug therapy management for GIST patients and holds significant importance in improving the clinical efficacy of imatinib and ensuring drug safety.

ABSTRACT

AIM: To analyze the recurrence factors of patients with retinal vein occlusion(RVO)induced macular edema(ME)and construct a nomogram model.METHODS: Retrospective study. A total of 306 patients with RVO induced ME admitted to our hospital from January 2019 to June 2022 were included as study objects, and they were divided into modeling group with 214 cases(214 eyes)and 92 cases(92 eyes)in the verification group by 7:3. All patients were followed up for 1 a after receiving anti-vascular endothelial growth factor(VEGF)treatment, and patients in the modeling group were separated into a recurrence group(n=66)and a non recurrence group(n=148)based on whether they had recurrence. Clinical data were collected and multivariate Logistic regression was applied to analyze and determine the factors affecting recurrence in patients with RVO induced ME; R3.6.3 software was applied to construct a nomogram model for predicting the recurrence risk of patients with RVO induced ME; ROC curve and calibration curve were applied to evaluate the discrimination and consistency of nomogram model in predicting the recurrence risk of patients with RVO induced ME.RESULTS: There were statistically significant differences in central retinal thickness(CRT), course of disease, hyperreflective foci(HF), disorder of retinal inner layer structure, and injection frequency between the non recurrence group and the recurrence group before treatment(all P&#x0026;#x003C;0.05). The multivariate Logistic regression analysis showed that pre-treatment CRT(OR=1.011), course of disease(OR=1.104), HF(OR=5.074), retinal inner layer structural disorder(OR=4.640), and injection frequency(OR=4.036)were influencing factors for recurrence in patients with RVO induced ME(all P&#x0026;#x003C;0.01). The area under the ROC curve of the modeling group was 0.924(95%CI: 0.882-0.966), the slope of the calibration curve was close to 1, and the results of the Hosmer-Lemeshow goodness of fit test showed that χ2=11.817, P=0.160; the area under the ROC curve of the verification group was 0.939(95%CI: 0.892-0.985), the slope of the calibration curve was close to 1, and the results of the Hosmer-Lemeshow goodness of fit test showed χ2=6.082, P=0.638.CONCLUSION: Pre-treatment CRT, course of disease, HF, disorder of retinal inner layer structure, and injection frequency are independent risk factors for recurrence in patients with RVO induced ME. The nomogram model constructed based on this has a high discrimination and consistency in predicting the recurrence risk of patients with RVO induced ME.

ABSTRACT

OBJECTIVE To investigate the effects of individualized dosing regimen on blood trough concentration of vancomycin and renal function in critically ill patients. METHODS According to relevant guidelines and the results of Vancomycin Calculator, clinical pharmacists formulated an individualized dosing regimen of vancomycin including loading dose and maintenance dose for critically ill patients based on the two independent variables of body weight and creatinine clearance rate. Using the method of retrospective study, patients who were admitted to the department of intensive care unit (ICU) of the Second Affiliated Hospital of Guangzhou Medical University and used the regimen from July 2018 to December 2021 were selected as the trial group, and patients who were treated with vancomycin and received blood drug concentration monitoring in ICU from January 2015 to June 2018 were recruited in the control group. The difference in trough concentration distribution and the incidence of acute kidney injury (AKI) after medication were compared between the two groups, the change of serum creatinine before and after medication in the trial group was analyzed. RESULTS Totally 197 patients were included in the trial group and 144 patients were in the control group. There was no significant difference between the two groups in the clinical information (gender, age, body weight, acute physiology and chronic health evaluation Ⅱ score, the proportion of patients with renal insufficiency, etc.) (P>0.05). The proportions of major infection sites (including lung, urinary, abdominal, blood and central nervous system) and treatment type (target or empirical treatment) also had no significant difference between the two groups (P>0.05). There was no significant difference in the attainment rate of ideal trough concentration (15-20 μg/mL) and the proportion of patients with trough concentration >20 μg/mL between the two groups (P>0.05), while the attainment rate of target trough concentration (10-20 μg/mL) and the proportion of patients with trough concentration <10 μg/mL were significantly different between the two groups (P<0.05). The attainment rate of target trough concentration in patients with chronic renal insufficiency in trial group was significantly higher than that in control group (P<0.05). There was no significant difference in the incidence of AKI and vancomycin-associated AKI between the two groups (P>0.05). In the trial group with medication duration ≥7 days , the level of serum creatinine on the 7th day of treatment was increased significantly, compared with that on the 3rd day of treatment (P<0.05). CONCLUSIONS This individualized dosing regimen can improve the attainment rate of target trough concentration of vancomycin in critically ill patients, especially those with chronic renal insufficiency, during the first standardized monitoring, and not increase the risk of renal injury compared with previous empirical medication.

Objective @#To discuss the effectiveness and mechanism for movement of maxillary buccally transposed canines by using a door-shaped individualized dental archwire mechanic and to provide a reference for clinicians.@*Methods@#Eight patients with unilateral maxillary transposed canines were enrolled. All patients were treated with door-shaped individualized archwires. Before treatment (T1) and after the crowns of the transposed canines were moved to the right buccal positions in the dental arch during the treatment (T2), orthopantomograms were taken both at T1 and T2 to compare the linear changes (distance changes of the crown and root apex) and angular changes to study the mechanisms of tooth movement. The probing depth and buccal crown height were measured using a periodontal probe to compare periodontal changes before treatment (T1) and after treatment (T3) between the transposed canines and contralateral canines. @*Results@# All eight transposed canines were successfully brought back to their normal dental arch position but were made more buccal by using the door-shaped individualized dental archwire, with a mean of (11.5 ± 2.7) months. The average overall duration was (28.3 ± 4.7) months. The crown distance changes of the canines from T1 to T2 (8.1 mm) were greater than those of the root apexes (1.5 mm) (P<0.05). The mean angulation changes of the long axes of the canines were 17.5°. There was no significant difference in the depth of periodontal measurement and buccal crown height measurement between T1 and T3 (P>0.05). @*Conclusion @# The buccal movement of maxillary transposed canines under a door-shaped individualized dental archwire was effective and feasible. The movement pattern under this mechanism was controlled tipping.

ABSTRACT

The main clinical manifestation of dyslipidemia is hyperlipidemia, which is an important risk factor leading to the occurrence of cardiovascular and cerebrovascular diseases such as atherosclerosis, coronary heart disease and stroke. In clinical practice, lipid-lowering drugs are mainly used for treatment, but there are issues such as individual differences and genetic effects. Therefore, it is necessary to perform gene detection on patients, so as to guide individualized application of lipid-lowering drugs. This review mainly previews the definition of gene detection and the individualized treatment of lipid-lowering drugs, and introduces the application of gene detection in the individualized treatment of lipid-lowering drugs (statins, fibrates, nicotinic acid and ezetimibe). Among them, the gene polymorphisms of APOE, SLCO1B1 and CYP450 family play a key role in the efficacy and safety of statins; the gene polymorphisms of APOA/B/C family have a significant impact on the efficacy of fenofibrate; the gene polymorphisms of HCAR2 and DGAT2 have an important impact on the efficacy of niacin; the gene polymorphisms of NPC1L1 have a significant impact on the efficacy of ezetimibe. It is suggested to conduct genotype detection for patients with dyslipidemia to select appropriate treatment strategies, so as to provide individualized medication guidance.

ABSTRACT

OBJECTIVE To investigate the effect of gene polymorphism on opioid-induced constipation. METHODS The target genes related to opioid-induced constipation were screened out through searching guidelines, databases and evidence-based medical data, and then 100 cancer pain patients who received opioid drugs for analgesia were included as the study subjects. According to whether there were adverse effects of constipation after medication or not, they were divided into test group and control group, with 50 cases in each group. The target gene was detected by PCR or fluorescence in situ hybridization. The SNPStats program was used to carry out Hardy-Weinberg balance test and correlation analysis between gene polymorphism and opioid-induced constipation. The multivariate Logistic regression analysis was used to explore the relevant predictive factors of opioid-induced constipation, and receiver operating characteristic (ROC) curve of subjects was drawn to analyze the effectiveness of each predictive factor in predicting opioid-induced constipation. RESULTS CYP2D6, CYP3A5*3, ABCB1 and OPRM1 were selected as target genes for detection. The results of genotype detection showed that the frequency distribution of CYP2D6 (rs1065852, rs1135822, rs16947, rs28371725, rs28371735), CYP3A5*3 (058rs776746), ABCB1 (062rs1045642), OPRM1 (047rs1799971) alleles were consistent with Hardy-Weinbergbalance test. The correlation analysis results showed that the proportion of genotype GG and AG in CYP3A5*3 (058rs776746, 163.com A>G) and genotype AA and AG in OPRM1 (047rs1799971, A>G) of patients was significantly higher in test group than that in the control group (P<0.05). Multivariate Logistic regression analysis showed that medication duration, CYP3A5*3 and OPRM1 gene polymorphism could be used as predictors of opioid- induced constipation in patients (P<0.05). The ROC curve analysis results showed that the areas under the ROC curves for medication duration and CYP3A5*3, OPRM1 gene polymorphism were 0.648, 0.640 and 0.670, respectively, with the optimal cutoff values of 124.0, 0.5 and 0.5, respectively. CONCLUSIONS Genotype GG and AG in CYP3A5*3 (058rs776746,A>G) and genotype AA and AG in OPRM1 (047rs1799971,A>G) are associated with opioid-induced constipation, which are expected to become clinical predictors of opioid-induced constipation, and more attention should be paid to the occurrence of constipation in patients who have been taking opioids for a long time.

ABSTRACT

Objective:To determine the effectiveness of individualized voice therapy in persistent pediatric voice disorders. Methods:Thirty-eight children who were admitted to the Department of Pediatric Otolaryngology Shenzhen Hospital, Southern Medical University due to persistent voice disorder from November 2021 to October 2022 were included. All children were evaluated by dynamic laryngoscopy before voice therapy. Two voice doctors performed GRBAS score and acoustic analysis on the children's voice samples to obtain the relevant parameters including F0, Jitter, Shimmer, and MPT; All children were given personalized voice therapy for 8 weeks. Results:Among 38 children with voice disorders, 75.8%(29 cases) were diagnosed with vocal nodules, 20.6%(8 cases) were vocal polyps, and 3.4%(1 case) were vocal cysts. And in all children. And 51.7%(20 cases) had the sign of supraglottic extrusion under dynamic laryngoscopy. GRBAS scores decreased from 1.93 ± 0.62, 1.82 ± 0.55, 0.98 ± 0.54, 0.65 ± 0.48, 1.05 ± 0.52 to 0.62 ± 0.60, 0.58 ± 0.53, 0.32 ± 0.40, 0.22 ± 0.36, 0.37 ± 0.36. F0, Jitter, Shimmer decreased from(243.11±39.73) Hz, (0.85±0.99)%, (9.96±3.78)% to(225.43±43.20) Hz, (0.33±0.57)%, (7.72±4.32)%, respectively MPT was prolonged from(5.82±2.30) s to(7.87±3.21) s after treatment. All parameters changes had statistical significance. Conclusion:Voice therapy can solve children's voice problems, improve their voice quality and effectively treat children's voice disorders.

ABSTRACT

Objective:To explore any effect of following an individualized exercise program on the cardio-pulmonary health of elderly persons with stable chronic obstructive pulmonary disease (COPD).Methods:A total of 120 elderly COPD patients were randomly divided into an experimental group and a control group, each of 60. Both groups were given medication and routine rehabilitation treatment for 3 months, but the experimental group also followed an individualized exercise program based on the cardiopulmonary exercise test. Before and after the treatment, Borg scoring was used to assess the subjects′ degree of dyspnea. Forced expiratory volume (FEV 1), forced vital capacity (FVC), maximum oxygen uptake (VO 2max), anaerobic threshold (AT), heart rate (HR) and a heart index (CI) were also recorded. The 6-minute walk test (6 MWD) was administered to evaluate the rehabilitation effect. Results:There were no significant differences in the average Borg scores, cardiopulmonary function indexes or 6MWD distances between the two groups before treatment. After the treatment significant improvement was observed in all of the measurements in both groups, but at that point all of the experimental group′s averages were better than those of the control group.Conclusion:Individualized exercise based on the cardiopulmonary exercise test can alleviate the symptoms of dyspnea in elderly COPD patients, improve their cardiopulmonary functioning and exercise endurance and improve the rehabilitation effect.

ABSTRACT

Objective:To investigate the feasibility of individualized primary clinical target volume (CTV) delineation in intensity-modulated radiotherapy for nasopharyngeal carcinoma (NPC).Methods:Clinical data of 87 consecutive patients newly diagnosed with lateralized NPC in Jiangsu Cancer Hospital between October 2016 and February 2018 were retrospectively analyzed. Lateralized NPC is defined as tumor invasion not exceeding the contralateral wall. According to the tumor spread, the primary CTV was optimized as follows: CTV2 only covered the medial part of the contralateral pterygopalatine fossa, whereas the contralateral foramen oval was not included; on the level of parapharyngeal space, the contralateral side of CTV only covered the posterior lateral lymph nodes, whereas the contralateral internal jugular vein was not regularly covered. Failure patterns and 5-year survival [local control rate (LCR), progression-free survival (PFS) and overall survival (OS)] were evaluated by Kaplan-Meier method. Paired t-test and rank-sum test were used to analyze the dose variation in the optimized region and adverse reactions. Results:The median follow-up time was 59.5 months. The 5-year LCR, PFS, and OS were 98.9%, 86.5% and 92.1%, respectively. There was no local recurrence in the optimized area of CTV. Dosimetric comparison results showed that the doses of parotid gland, temporal lobe, cochlea and middle ear on the contralateral side were reduced by 13.45%, 9.14%, 38.83%, and 29.36%, respectively. Four cases (4.6%) developed grade 3 hearing loss, all on the ipsilateral side. The optimized scheme significantly alleviated the hearing loss on the contralateral side compared to that on the ipsilateral side ( P<0.001). Other grade 3 late adverse reactions included cranial nerve injury, subcutaneous fibrosis in the neck and visual impairment, with 1 case each. Conclusion:Individualized primary CTV for lateralized NPC is feasible and safe, with obvious dosimetric advantages and reduced adverse reaction rate, which is worthy of clinical promotion.

ABSTRACT

Objective:To explore the application of step-by-step mentorship combined with individualized assessment in practical nursing skill training in higher vocational colleges.Methods:A total of 219 nursing students of Class 2020 were selected from a higher vocational college; they were randomly divided into control group (107 students) and experimental group (112 students), both of which were offered nursing training courses in the first semester. The control group was offered conventional teaching, while the experimental group was offered step-by-step mentorship combined with individualized assessment. The two groups were compared for post-training assessment scores of theoretical knowledge and practical skills and pre-training and post-training comprehensive nursing ability as well as their recognition of the teaching methods after training. The t-test and chi-square test were performed using SPSS 22.0. Results:The post-training assessment scores of theoretical knowledge and practical skills were significantly higher in both subitem score [(89.68±3.58) vs. (82.56±3.35)] and total score [(91.75±3.01) vs. (85.36±2.58)] in the experimental group than in the control group ( P<0.05). There were no significant differences in pre-training comprehensive nursing ability between the two groups, while the post-training comprehensive nursing ability scores were significantly higher in both subitem score and total score [(86.53±2.61) vs. (80.32±2.31)] in the experimental group than in the control group ( P<0.05). After the training, students in the experimental group showed increased recognition of the teaching methods in stimulating their learning interest and initiative, improving their self-learning ability, improving their comprehensive literacy, improving their ability to apply theoretical knowledge, and improving their clinical thinking ability compared with the control group ( P<0.05). Conclusion:The application of step-by-step mentorship combined with individualized assessment in practical nursing skill training in higher vocational colleges can improve students' academic performance and comprehensive nursing ability and increase their recognition of the teaching methods, thus making it worthwhile to be widely adopted.

ABSTRACT

Septic shock is the most critical stage of sepsis, and the core of the treatment is improving tissue hypoperfusion.In addition to the improvement of large circulation by fluid resuscitation, the microcirculation also need to be improved, then proper vasoactive drugs should be applied to different types of shock to meet the needs of the body for perfusion.The condition of septic shock progresses rapidly, and the microcirculation in different pathophysiological stages is different, which increases the difficulty of rational use of vasoactive drugs.Monitoring and accurate evaluation of peripheral microcirculation may provide guidance for individualized treatment of septic shock.

ABSTRACT

AIM: To establish individualized drug therapy strategy for patients with rejection and hyperglycemia after liver transplantation. METHODS: Clinical pharmacist collaborated with the surgeons and participated in the diagnosis and treatment of rejection and hyperglycemia after liver transplantation. Taking together liver function, therapeutic drug monitoring, drug-drug interactions between tacrolimus and wuzhi capsule, individualized drug therapy was adapted to improve the prognosis. RESULTS: The patient recovered well and survived in good health till now. CONCLUSION: It is highly suggested that clinical pharmacists actively involved in treatment of more severe and difficult-to-treat disease and design the individualized dosing regimens. This will largely contribute in reduced adverse drug reaction, improved safety and effectiveness in drug use as well as the quality of life in the "post-transplantation era".

Spatialomics is another research hotspot of biotechnology after single-cell sequencing technology, which can make up for the defect that single-cell sequencing technology can not obtain cell spatial distribution information. Spatialomics mainly studies the relative position of cells in tissue samples to reveal the effect of cell spatial distribution on diseases. In recent years, spatialomics has made new progress in the pathogenesis, target exploration, drug development and many other aspects of head and neck tumors. This paper summarizes the latest progress of spatialomics in the diagnosis and treatment of head and neck cancer.

ABSTRACT

Linezolid is an antibacterial agent for the treatment of multi-resistant Gram-positive bacterial infections, which is widely used in clinical practice. However, there are large individual differences in the pharmacokinetic characteristics of the drug in patients, and it is difficult to obtain the optimal therapeutic effect when the drug is administered according to the conventional dose in the instructions. Therefore, it is necessary to carry out therapeutic drug monitoring (TDM) for linezolid, and guide and optimize its antibacterial treatment plan by using population pharmacokinetics (PPK) and pharmacodynamics principles. This paper summarizes the PPK changes and the research progress of individualized administration of linezolid in various populations, and recommends that the patient’s steady-state blood concentration is kept at 2-8 mg/mL through TDM when using linezolid clinically. It is recommended to appropriately reduce the dosage for patients with liver and kidney dysfunction, appropriately increase the dosage for obese, burned and children patients, and provide pharmaceutical monitoring during the medication process to promote rational drug use.

ABSTRACT

OBJECTIVE To compare the effects of individualized parenteral nutrition versus pre-mixed parenteral nutrition on liver function of patients with acute kidney injury (AKI). METHODS Totally 97 AKI patients in the intensive care unit of our hospital from January 2021 to March 2022 were collected and randomly divided into pre-mixed multi-chamber bag (MCB) group (48 cases) and compounded parenteral nutrition (COM) group (49 cases). The patients in both groups were given routine treatment to correct the reversible cause in time, and parenteral nutrition support treatment was started within 48 hours after the fluid resuscitation was successful or the hemodynamics of low-dose vasoactive drugs were stable. MCB group was given one bag of Fat emulsion amino acid (17) glucose (11%) injection, intravenous infusion, once a day; COM group was given Medium/long chain Fat emulsion injection (C8-24Ve) 0.5-0.8 g/kg+Compound amino acid 18AA-Ⅶ 1.0-1.2 g/kg+Glucose injection 1.5-2.5 g/kg+one Water soluble vitamin injection+Fat-soluble vitamin injection (Ⅱ) 10 mL+Multiple trace element injection (Ⅱ) 10 mL+ individualized supplement of sodium chloride and potassium chloride, with a ratio of glucose to lipid of 5∶5 and a ratio of heat to nitrogen of 100∶1. The treatment course of both groups lasted for 7 days. The percentage of abnormal liver function, the levels of liver function indexes [alanine aminotransferase (ALT), total bilirubin (TBIL), aspartate transaminase (AST)], albumin (ALB), interleukin-6 (IL-6) and C-reactive protein (CRP) were observed in 2 groups before and after treatment. RESULTS After treatment, the ratio of liver dysfunction, the levels of ALT, AST and CRP in MCB group were significantly higher than before treatment; the ratio of liver dysfunction, the levels of ALT and CRP in MCB group were significantly higher than COM group (P<0.05). There were no statistical significance in the ratio of liver dysfunction, the levels of ALT, AST, TBIL and CRP in COM group before and after treatment (P>0.05). CONCLUSIONS Individualized parenteral nutrition support treatment can reduce the occurrence of liver injury and improve the nutritional status of AKI patients.
