ABSTRACT

Resumo O presente artigo aborda o tema da ética no contexto das organizações públicas, edificada em dois grandes pilares: a Constituição Federal Brasileira e o marco legal do Sistema de Gestão da Ética do Poder Executivo Federal. Para o exercício das funções, os servidores, além da qualificação técnica, necessitam de competências políticas e éticas. Este trabalho é resultante de pesquisa que teve como objetivos: compreender os sentidos da ética e da ética pública para servidores públicos e identificar a importância e as dificuldades para o exercício da ética no processo de trabalho desenvolvido. Trata-se de uma pesquisa exploratória com utilização de abordagem qualitativa. A coleta de dados foi realizada por meio de entrevista semiestruturada e os dados coletados foram analisados por meio da análise de conteúdo, na modalidade de análise temática. Os resultados da pesquisa subsidiaram reflexões para auxiliar futuras decisões relacionadas ao tema estudado, tendo em vista o intuito de disseminar a importância do cumprimento dos preceitos éticos que devem alicerçar a atuação dos servidores. Os argumentos conclusivos reforçam a necessidade de uma ampla discussão do tema "Ética" dentro da esfera pública e a urgência em fomentar posturas que visem à melhoria das relações, cimentando pensamentos críticos e reflexivos no processo de trabalho com base em imperativos éticos.


Abstract This article aims to discuss the subject of ethics in the context of public organizations, built on two main pillars: the Brazilian Federal Constitution and the legal framework of the Ethics Management System of the Federal Executive Power. To carry out their duties, public servants, in addition to technical qualifications, need political and ethical skills. This article is the result of a research that had the following objectives: to understand the meanings of ethics and public ethics for public servants and to identify the importance and difficulties for the exercise of ethics in the work process developed. This is an exploratory research using a qualitative approach. Information collection was carried out with semi-structured interviews, and the data collected analyzed by content analysis, in the thematic analysis mode. The research results supported reflections to help future decisions related to the studied topic, considering the objective of disseminating the importance of complying with the ethical precepts that should underpin the performance of public servants. The conclusive arguments support the need for a broad discussion of the topic of "Ethics" within the public reach and the urgency to promote postures aimed at improving relationships, reinforcing critical and reflective thoughts in the work process based on ethical imperatives.

Introducción: En los hospitales públicos, las dificultades surgidas por el déficit de recursos logísticos en el laboratorio clínico, dificultan aún más una atención de calidad. La carencia de una gestión por procesos y los conflictos de interés surgidos por la proliferación de laboratorios no certificados ni acreditados, hacen necesaria una nueva visión sobre este tema. La Medicina del Laboratorio Basado en la Evidencia, es una herramienta de la cual clínicos, tecnólogos y biólogos pueden beneficiarse. Por otro lado, es necesario que, a partir del estado, se consensuen los esfuerzos realizados por los distintos actores de este problema: UNAGESP, IETSI, SIS, INACAL, SUSALUD, Defensoría del Pueblo, Contraloría General de la República, Colegios profesionales y Empresa privada para mejorar la gestión del laboratorio clínico.


Background: In public hospitals, the difficulties arising from the lack of logistical resources in the clinical laboratory make quality care even more difficult. The lack of management by processes and the conflicts of interest arising from the proliferation of laboratories that are not certified or accredited make a new vision on this subject necessary. Evidence-Based Laboratory Medicine is a tool from which clinicians, technologists and biologists can benefit. On the other hand, it is necessary that, from the state, the efforts made by the UNAGESP, IETSI, SIS, INACAL, SUSALUD, the Ombudsman's Office, the Comptroller General's Office of the Republic, professional associations and private companies to improve the management of the clinical laboratory be agreed upon.

ABSTRACT

Background: Population policies, family planning and welfare programmes undertaken by the Government of India, have led to continuous decrease in the fertility rate but sustainability and target of net reproduction rate 1 is not yet achieved. With increase trend of adopting the permanent method of contraception by eligible couples, the preferences of tubectomy over vasectomy is still remarkable. Hence, the present study is taken up to find Knowledge, Attitude and Practice regarding sterilization and factors influencing it.Methods: A cross-sectional study is conducted in the year 2017 among eligible couples who have completed their family size or pregnant women who came for second confinement to Vydehi Institute of Medical Sciences and Research Centre and Shreyas Maternity Home, Hoskote and Shreyas maternity home are included. An institutional ethics committee (IEC) is given on vasectomy, the better method of sterilization and responses are recorded.Results: A total 92.6% couples were aware of permanent family planning methods. Among them only 30.6% couples are aware of vasectomy. There is a significant association between number of children in a family and to undergo sterilization (p=0.02) method. Reason for not choosing vasectomy are unawareness (49.3%) and social stigma (38%) mainly. 11 couples (7.3%) changed their opinion to undergo vasectomy after giving an IEC.Conclusions: Many couples were knowledgeable about tubectomies and opted for it. Only a small number of couples changed their opinion from tubectomy to vasectomy after being given an IEC.

ABSTRACT

Objetivo: realizar uma reflexão sobre a aplicação dos princípios bioéticos na atuação do cirurgião-dentista em âmbito hospitalar. Material e Métodos: uma revisão crítica da literatura foi realizada acessando-se as bases de dados PubMed, Scielo e Biblioteca Virtual em Saúde, utilizando-se os descritores "Bioética", "Equipe Hospitalar de Odontologia", "Ética Odontológica" e "Ética Institucional". Resultados: não foram encontrados estudos específicos sobre o tema proposto. Há escassez de trabalhos na literatura científica específicos sobre a aplicação dos princípios bioéticos ao dia a dia dos profissionais de Odontologia em âmbito hospitalar. Conclusão: a observância e o respeito aos pilares bioéticos da autonomia, justiça, beneficência e não maleficência devem estar incluídos nas atitudes do cirurgião-dentista e da equipe multiprofissional, minimizando os desconfortos ocasionados por problemas de ordem geral e bucal.


Objective: to reflect on the application of the basic principles of bioethics in the performance of the dentist in a hospital environment. Materials and Methods: a critical review of the scientific literature was performed by searching the PubMed, Scielo, and Virtual Health Library databases using the descriptors "Bioethics," "Dental Staff, Hospital," "Ethics, Dental" and "Ethics, Institutional." Results: there were no specific studies on the proposed theme. There was a lack of studies in the scientific literature on the specific application of the bioethical principles in the routine of dental professionals at hospitals. Conclusion: the observance and respect for the bioethical pillars of autonomy, justice, beneficence and non-maleficence must be included in the attitudes of the dentist and the multidisciplinary team, minimizing the discomforts caused by problems of general and oral disorders.

ABSTRACT

The field of Global Health has been under construction since the last decades of the past century. It focuses on health issues that extrapolate national borders, as well as their determinants and possible solutions. Global health conceptions carry ethical values. This essay aims to reflect on values involved in global health: social justice, equity and solidarity. To this end, we reviewed scientific papers and multilateral agencies' documents. We identified the defense of global health as a universal public good, and we analyzed justice and equity values with a focus on the allocation and distribution of resources, within a tendency to prioritize the most disadvantaged ones. Solidarity concepts are presented in an attempt to explain whether there is a moral responsibility for being supportive of people from other countries, which would justify international cooperation in health.


Desde as últimas décadas do século passado vem sendo construído o campo da saúde global, enfocando questões de saúde supraterritoriais, que extrapolam as fronteiras nacionais, assim como seus determinantes e suas possíveis soluções. Este ensaio objetiva refletir sobre os valores éticos envolvidos na saúde global: justiça social, equidade e solidariedade. Procedeu-se à revisão de artigos científicos e documentos de agências multilaterais. Identificou-se a defesa da saúde global como um bem universal público, analisando os valores da justiça e da equidade com enfoque na alocação e distribuição de recursos, com tendência a priorizar os mais desfavorecidos. São apresentados conceitos de solidariedade, buscando compreender se há a responsabilidade moral de ser solidário com pessoas de outros países, o que justificaria a cooperação internacional na saúde.

O conceito de humanização é amplo e abrange um conjunto de conhecimentos, práticas, atitudes e relações. Envolve a efetiva participação de profissionais, gestores, usuários e movimentos sociais para que as experiências compartilhadas possam proporcionar melhorias na qualidade do atendimento de saúde. O objetivo deste estudo foi analisar a percepção de médicos e enfermeiros de unidades de assistência médica ambulatorial (AMA) sobre humanização nos serviços de saúde. Trata-se de pesquisa qualitativa, de corte transversal, realizada mediante entrevistas semiestruturadas, de outubro de 2010 a maio de 2011, e o critério de representatividade da amostra para o encerramento da coleta de dados foi a saturação do discurso. Constatou-se que a percepção dos profissionais sobre humanização tem distintos sentidos e significados variados, possibilitando a construção de categorias empíricas: a preocupação com o outro – alteridade, direitos dos usuários, informação e autonomia dos usuários e avaliação de risco. Situações comuns e particulares vivenciadas no cotidiano das unidades AMA, como o contato direto com problemas de saúde que envolvem maiores riscos aos usuários, parecem influenciar a percepção dos profissionais sobre humanização, podendo afetar a preservação dos direitos e as relações estabelecidas entre os profissionais e os usuários...


The concept of humanization is broad and it encompasses a set of knowledge, practices, attitudes, and relations. It involves the effective participation of professionals, managers, users, and social movements so that shared experiences can provide an improved quality of healthcare. This study aimed to analyze the perception of physicians and nurses from outpatient medical care units (AMA) facilities regarding humanization in health services. This is a qualitative research, with a cross-sectional design, carried out by means of semi-structured interviews, from October 2010 to May 2011, and the sample representativeness criterion to stop collecting data was discourse saturation. It was found out that the professionals’ perception regarding humanization has various senses and meanings, allowing the construction of empirical categories: concern for the other – otherness, users’ rights, users’ information and autonomy, and risk assessment. Usual and particular situations experienced on a daily basis in outpatient medical care units facilities, such as direct contact to health issues that involve greater risks to users, seem to influence the professionals’ perception regarding humanization, and they can affect the preservation of rights and relations established between professionals and users...

The objective of this article is to present and discuss the process of implementation and functioning of clinical bioethics committees in the hospital complex of a Brazilian public university, with four of its hospitals having these commissions in different structuring stages: a general hospital; a psychiatric hospital; a children's hospital and a maternity ward. For this, alternatives, difficulties, as well as the viability, are discussed for their implementation, in addition to practical recommendations in the form of steps, with accounts of experiences in Brazil and around the world. ABSTRACT

Con el fin de aportar a la construcción del hospital universitario de la Universidad Nacional de Colombia, se presenta una revisión acerca de la importancia de este tipo de instituciones para la prestación de servicios para la enseñanza de las ciencias de la salud, como propósitos importantes de cualquier sistema sanitario. Igualmente se discuten los principales propósitos que en un contexto como el actual debe tener el desarrollo de este tipo de instituciones. Se destaca que el hospital debe ser el aula más importante durante el entrenamiento en salud, para lo cual debe coincidir el uso planificado y programado de los recursos hospitalarios con la atención de los pacientes que de forma circunstancial acceden al servicio, garantizando que el ejercicio docente se vuelque en todas las actividades de la institución y permitiendo confluencia de otras disciplinas y saberes que permitan abordar de manera amplia el proceso salud-enfermedad, con un ejercicio ético de los procesos de enseñanza y atención que allí concurren. Se concluye que la construcción del hospital universitario no es simplemente la adecuación y dotación de una infraestructura, sino el conocimiento profundo de las dos actividades que allí concurren, con el fin de armonizarlos para garantizar la mejor calidad, en un entorno social que justificadamente demanda la sostenibilidad y continuidad de estos procesos.


In order to contribute to the construction of the university hospital of the Universidad Nacional de Colombia, is a review about the importance of these institutions for the provision of services and for the teaching of health sciences as important purposes of any health system. It also discusses the main purposes in a context such as this must be the development of these institutions. It is noted that the hospital should be the most important classroom during training in health, to which must match planned and scheduled use of hospital resources to the care of patients who circumstantially access the service, ensuring that the teaching practice overturning in all activities of the institution and allowing confluence of other disciplines and allowing comprehensively address the health-disease process, with an ethical practice of teaching and care that converge there. We conclude that the construction of the university hospital is not simply the adequacy and provision of infrastructure, but the knowledge of the two activities that converge there, in order to harmonize them to ensure the best quality in a social environment that justifiably demand the sustainability and continuity of these processes.

ABSTRACT

Os relacionamentos dos estudantes entre si e destes com seus professores podem se tornar difíceis em diferentes momentos. O ingresso na universidade parece ser um período especialmente tenso em função dos trotes sofridos nesta fase da vida acadêmica. Este estudo apresenta uma revisão da literatura a respeito do trote, em especial aquele realizado nas escolas médicas, tentando identificar as características ritualísticas e os fatores mantenedores deste. Paralelamente, constrói uma metáfora com o mito do vampiro, propondo esta lenda como um paradigma para compreender as relações humanas nas faculdades de Medicina. São discutidas as características comuns entre vampiros e vítimas, encontradas no mito e nas inter-relações presentes nas escolas médicas, abordando-se possíveis estratégias de manejo.


The students' relationships with one another and their teachers can become difficult at different times. However, entrance into the university seems to be a particularly tense moment, due to the hazing suffered this stage of their academic lives. This study presents a review of the literature on the hazing, especially that have happen in medical schools, trying to identify the characteristics and maintaining factors of those ritual. In parallel, constructs a metaphor of the vampire myth, proposing this legend as a paradigm for understanding the human relationships in the medical schools. The common characteristics between vampires and victims, found in myth, and these interrelationships are discussed in medical schools, approaching at possible management strategies.

ABSTRACT

OBJETIVO: Identificar e analisar, à luz de referencial ético, as escolhas e justificativas de profissionais de saúde pública em situações hipotéticas de priorização de pacientes em casos de limitações de recursos no atendimento de emergências médicas. MÉTODOS: Pesquisa qualiquantitativa, mediante entrevistas com 80 profissionais de saúde pública, pósgraduandos (mestrado e doutorado) em Saúde Pública, aos quais foram apresentadas situações hipotéticas, envolvendo os critérios de sexo, idade e responsabilidade, sendo requerido que escolhessem entre alternativas que se referiam à existência de pessoas, correndo igual risco de vida, que necessitavam de atendimento em um serviço de emergência. RESULTADOS: As escolhas priorizaram crianças, jovens, mulheres e casadas, com a tomada de decisão invocando os princípios éticos de vulnerabilidade, utilidade social e equidade. CONCLUSÃO: A pesquisa mostra clara tendência de justificativas das escolhas feitas, orientadas pela ética utilitarista.


OBJECTIVE: To identify and analyze, in the light of ethical considerations, the choices and justifications of public health professionals in hypothetical situations of patient prioritization in circumstances of limited resources during emergency medical care. METHODS: Qualitative and quantitative study, carried out through interviews with 80 public health professionals, graduate students (MSc and PhD students) in public health who were faced with hypothetical situations involving the criteria of gender, age, and responsibility, asked to choose between alternatives that referred to the existence of people, equally submitted to life-threatening situations, who needed care in an emergency department. RESULTS: The choices prioritized children, young individuals, women, and married women, with decision-making invoking the ethical principles of vulnerability, social utility, and equity. CONCLUSION: The study shows a clear tendency to justify the choices that were made, guided by utilitarian ethics.

ABSTRACT

O termo bullying vem sendo utilizado para descrever um tipo de violência insidiosa e devastadora, que compromete negativamente a saúde e o ambiente. Pesquisas conduzidas sobre o tema em estudantes de Medicina revelam que ela ocorre especialmente sob a forma de abusos repetidos, mas permanece pouco explorada durante a formação médica. O objetivo deste trabalho é analisar o tema do trote como aparece nas falas de docentes e estudantes quando são estimulados a relatar situações de violência entre alunos durante a graduação em Medicina. Foi feita pesquisa de caráter exploratório, qualitativa, de corte transversal, sob a forma de estudo de caso. Foram entrevistados alunos em cargo de representação e professores em cargo de chefia. Os resultados evidenciaram a ocorrência de diversas situações de abuso, especificamente durante o trote universitário. É necessário um amplo debate sobre a violência na universidade. Para que se construa um ambiente de respeito e cooperação, adequado ao desenvolvimento de pessoas, a violência precisa ser banida das universidades.


The term "bullying" is used to characterize a kind of insidious and devastating violence that negatively affects health and the school setting. Research on the issue in medical students suggests that it occurs in the form of repeated abuse, but the issue is still insufficiently explored during medical training. The current study aims to analyze the issue of hazing as it appears in the discourse of faculty and students when they are encouraged to report on situations of violence between undergraduate medical students. An exploratory, qualitative, cross-sectional case study was performed. Student representatives and faculty department heads were interviewed. The findings included various situations of abuse, specifically hazing. A wide debate on violence in the university is needed. In order to establish a school environment of respect and cooperation, adequate for personal development, violence must be banned from the universities.

INTRODUÇÃO: O hospital é uma organização com responsabilidades éticas especiais que não podem ser comprometidas quando da incorporação de traços empresariais em sua estruturação. Este artigo traz resultados de pesquisa documental, qualitativa, exploratória, descritiva que teve como objeto valores institucionais dos hospitais. OBJETIVOS: Identificar, na literatura, valores esperados para hospitais; identificar valores declarados pelos hospitais em suas páginas na internet e compará-los com a literatura. MÉTODOS: Leitura crítica de artigos, teses e livros e busca dos valores declarados pelos hospitais em suas páginas na internet. Os dados foram organizados por análise de conteúdo. RESULTADOS: Os valores encontrados na literatura e nas páginas dos hospitais foram agrupados nas categorias: assistência, administração e compromisso social. Os resultados revelam que os hospitais reconhecem, nos valores declarados em suas páginas, a expectativa social em relação a uma instituição que, embora possa ter estrutura burocrática e organizacional com muitas semelhanças para com as empresas, diferencia-se destas por lidar com a saúde e vida das pessoas, que são, ao mesmo tempo, bens e direitos. Expressam o zelo que o hospital tem de dedicar a estes bens e direitos, por intermédio dos administradores e demais profissionais da instituição para que, assim, a organização hospitalar imprima sua marca e compromisso social e transmita confiança aos pacientes. Resta o desafio de sua prática.


INTRODUCTION: The hospital, as a unique type of social organization requires elevated values for management. This paper shows the result of a documental, qualitative, exploratory and descriptive survey about hospitals and their value statements. OBJECTIVE: Identify values expected for hospitals by a search of literature; identify values expressed by hospitals on their web pages and compare results of both. METHODS: Critical reading of theses, books and articles. A bibliographic search was carried out on BVS (Health Virtual Library) using keywords such as ethics and healthcare management. The values stated by hospitals on web pages were found in sections such as social responsibility, mission, view, principles, and our values. The categories care, healthcare management and accountability were defined after content analysis of empirical data. RESULTS: Values stated by hospitals on web pages express social expectations for an organization that deals with issues as elevated as health and life. Although hospitals have a bureaucratic and organizational structure that resembles those of business enterprises, they are different due to their `duties to patients' rights and life. Healthcare managers, as well as health professionals, must imprint an ethical attitude on their job and daily work. Only such an attitude will permit patients to trust the hospital and its services.

ABSTRACT

It is the prerequisite and foundation to set up the institutional ethics committee of organ transplantation that holds a proper structure for ethics committee to play a key role effectively.In terms of our reality and based on the successful practices and guidelines of international organizations of ethics committee,some suggestions and guidelines on reasonably standardizing the structure of ethics committee for organ transplantation in China are given to guarantee ethics committee to fulfill its target functions.
