ABSTRACT

Aim To study the application of HL-60 monocyte activation test in the pyrogen detection of freeze-dried rabies vaccine for human use.Methods The established HL-60 mononuclear cell activation test (MAT) was transferred between laboratories and the method was verified; referring to the interference test in the photometric method of the bacterial endotoxin test method of the Chinese Pharmacopoeia, HL-60- IL- 6 MAT was used to detect the recovery and pyrogen content of 13 batches of freeze-dried rabies vaccine for human use.Results The linearity of the amount of 1L-6 secreted by HL-60 cells, which stimulated by dif¬ferent concentrations of endotoxin standards was above 0.95; the calculated minimum detection limit was not more than 0.125 EU • mL"1 ; the recovery experiment with a solution containing 0.5 and 1.0 EU • mL"1 of endotoxin was performed to cheek the accuracy of the method.HL-60-IL-6 was used to detect 13 hatches of Freeze-dried rahies vaccine for human use, and the re¬covery of endotoxin was between 50 % to 200%.It was consistent that HL-60-IL-6 with pyrogens and en¬dotoxin test for 4 batches of freeze-dried rabies vaccine for human use which pyrogens and endotoxin test failed and the 3 batches of water for injection.Conclusion The HL-60 MAT using IL-6 as a detection indicator is suitable for the detection of pyrogenic substances in freeze-dried rabies vaccine for human use.

ABSTRACT

Objective To investigate the efficacy,safety of alprostadil combined with Tripterygium wilfordii polyglycosides in the treatment of patients with diabetic nephropathy and its influence on inflammatory factors . Methods From December 2015 to June 2018, 100 patients with diabetic nephropathy admitted to Dajiangdong Hospital were randomly divided into two groups according to the digital table ,with 50 cases in each group.The control group received oral administration of Tripterygium wilfordii glycosides ,the observation group received combined use of alprostadil intravenous infusion.The efficacy,safety and expression of serum TNF -and IL-6 were observed in the two groups.Results Before treatment, the fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin of the control group were (8.87 ±0.65)mmol/L,(12.26 ±1.57)mmol/L,(8.15 ±0.56)%,respectively, which of the observation group were (8.69 ±0.72)mmol/L,(12.41 ±1.64)mmol/L,(8.12 ±0.49)%,respectively, and there were no statistically significant differences between the two groups (t=0.78,0.69,0.72,all P>0.05).After treatment,the fasting blood glucose ,2 h postprandial blood glucose and glycosylated hemoglobin of the control group were (7.34 ±0.61) mmol/L,(10.04 ±1.40) mmol/L,(7.28 ±0.53)%,respectively,which of the observation group were (6.93 ±0.39)mmol/L,(8.57 ±1.33)mmol/L,(6.21 ±0.44)%,respectively.The blood glucose of the two groups was significantly improved (t=4.13,5.75,3.69,5.25,all P<0.05),and the blood glucose indicators of the observation group were significantly improved compared with the control group ( t =3.56,4.28,4.12,all P<0.05).Before treatment,the 24-hour urinary protein quantification ,urine beta 2-microglobulin in the control group were (0.27 ±0.05)g/L and (0.12 ±0.05)mg/L,respectively,which in the observation group were (0.26 ±0.06)g/L, (0.12 ±0.04)mg/L,respectively,there were no statistically significant differences between the two groups (t=0.58, 0.63,all P>0.05).After treatment,the 24-hour urinary protein quantification ,urine beta 2-microglobulin in the control group were (0.19 ±0.04)g/L,(0.08 ±0.04)mg/L,respectively,which in the observation group were (0.14 ± 0.03)g/L,(0.06 ±0.03) mg/L, respectively, the indicators of proteinuria in both two groups were significantly improved (t=3.97,4.38,all P<0.05),and the indicators of proteinuria in the observation group were significantly improved compared with the control group (t=3.84,3.99,all P<0.05).Before treatment,the serum creatinine and urea nitrogen levels in the control group were (150.97 ±23.82)μmol/L,(13.57 ±2.24) mmol/L,respectively,which in the observation group were (162.73 ±21.75) g/L,(14.05 ±2.31) mmol/L,respectively,there were no statistically significant differences in renal function between the two groups (t=1.02,0.93,all P>0.05).After treatment,the levels of serum creatinine and urea nitrogen in the control group were ( 122.38 ±18.34 ) μmol/L, ( 8.72 ± 0.71)mmol/L,respectively,which in the observation group were (101.41 ±15.64) g/L,(5.11 ±0.68) mmol/L, respectively,and the indicators of renal function in both two groups were significantly improved (t=5.31,6.47,4.92, 6.33,all P<0.05).The indicators of renal function in the observation group were significantly improved compared with the control group ( t=4.96,5.14,all P<0.05).Before treatment,the levels of TNF -αand IL -6 in the control group were (28.28 ±4.75) ng/L,(17.13 ±4.46) ng/L,respectively,which in the observation group were (27.87 ±4.81)ng/L,(16.98 ±4.27) ng/L,respectively,there were no statistically significant differences in IL -6 and TNF-αlevels between the two groups (t=0.86,0.97,all P>0.05).After treatment,the levels of TNF-αand IL-6 in the control group were (19.72 ±4.21)ng/L,(14.35 ±3.25) ng/L,respectively,which in the observation group were (14.61 ±3.18)ng/L,IL-6 (11.28 ±3.09)ng/L,respectively,the levels of TNF-αand IL-6 in the two groups were significantly improved (t=5.83,8.24,4.66,7.38,all P<0.05).The levels of TNF-αand IL-6 in the observation group were significantly improved compared with the control group (t=4.32,3.89,all P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups ( P>0.05).Conclusion Alprostadil combined with Tripterygium wilfordii polyglycosides can significantly improve the clinical efficacy of diabetic nephropathy ,with fewer adverse reactions and high safety ,which may be related to its ability to regulate the expression of serum TNF -αand IL-6.

ABSTRACT

Objective To discuss the clinical value of serum brain natriuretic peptide (BNP) ,tumor necrosis factor‐α(TNF‐α) , matrix metalloproteinase‐9(MMP‐9) and interleukin‐6(IL‐6) detection in chronic heart failure (CHF) patients with different heart function grades and their correlation with CHF .Methods Totally 67 cases of CHF in our hospital from Jan .2013 to Jun .2015 were selected as the observation group and the NYHA classification was performed :gradeⅠ in 15 cases ,gradeⅡ in 21 cases ,gradeⅢ in 23 cases and grade Ⅳ in 9 cases .Other 30 individuals undergoing the healthy physical examination were chosen as the control group at the same period .The venous blood samples were collected for detecting serum BNP ,TNF‐α,MMP‐9 and IL‐6 .LVEDD ,LVESD and LVEF were measured by echocardiography .Then the obtained data were analyzed by using the SPSS 21 .0 software .Results The serum BNP ,TNF‐α,MMP‐9 and IL‐6 levels in the observation group and various cardiac function groups were higher ,the differences between them were statistically significant (P< 0 .05);LVEF in the observation group and various cardiac function groups were obviously declined ,while LVEDD and LVESD were obviously increased ,the differences were statistically significant (P<0 .05) .The linear regression was adopted to analyze the correlation between serum BNP ,TNF‐α,MMP‐9 and IL‐6 with the left ventricular echocardiographic parameters in the CHF patients ,the results showed that serum BNP ,TNF‐α,MMP‐9 and IL‐6 levels were negatively correlated with LVEF (P<0 .05) ,while positively correlated with LVEDD and LVESD (P<0 .05) .Conclusion The serum BNP ,TNF‐α,MMP‐9 and IL‐6 levels are the good indexes for diagnosing CHF ,moreover can accurately reflect different heart function status in the CHF patients with different cardiac function grades ,which are closely related to the occurrence and de‐velopment process of CHF .

ABSTRACT

Objective A prospective randomized controlled trial was carried out to explore the best time of In-domethacin suppositories administration for the prevention of post-ERCP pancreatitis in high-risk groups. Methods 81 patients were enrolled in the study finally. Patients were randomized into group A (100 mg rectal Indomethacin suppositories was administrated immediately after ERCP), group B (100 mg rectal Indomethacin suppositories was administrated half an hour after ERCP) and group C (ERCP alone group, which did not give Indomethacin supposito-ries). The level of serum amylase, urine amylase, serum CRP, serum IL-6, serum TNF-α were measured before, 3 h, 24 h and 48 h after ERCP, and the incidence of PEP and hyperamylasemia were analyzed. Results There was 1 case (4.00%) of PEP in group A, 2 cases (5.41%) of PEP in group B and 5 cases (26.31%) of PEP in group C;the incidences of PEP of group A and group B were significant lower than that in group C (P < 0.05). There was 1 case (4.00%) of hyperamylasemia in group A, 5 cases (13.51 %) of hyperamylasemia in group B and 6 cases (31.6 %) of hyperamylasemia in group C, and the incidences of hyperamylasemia of group A and group B were significant lower than that in group C ( P< 0.05). Conclusion Administration of 100 mg Indomethacin suppositories immediately or half an hour after ERCP can effectively reduce the incidence of PEP and hyperamylasemia.

ABSTRACT

Objective To investigate the value of the E-selectin,IL-6 and TNF-α for the diagnosis of myocardial damage in children.Methods The E-selectin,IL-6 and TNF-α levels were detected in 62 patients with myocardial damage and 62 children without myocardial damage.The values of the E-selectin,IL-6 and TNF-α for the diagnosis of myocardial damage were analyzed by the receiver operating characteristic (ROC) curve.Results E-selectin levels were (45.16 ± 5.34) and (15.37 ± 4.65) μg/L,respectively in the patients with myocardial damage and the control group,and the difference between the two groups was significant(P < 0.001).IL-6 levels were (21.31 ± 5.28) ng/L and (17.09 ± 4.62) ng/L,TNF-α levels were (27.22 ± 8.56) ng/L and(21.46 ± 8.08) ng/L,showing no significant difference (P > 0.05).ROC curve analysis showed that AOC of E-selectin in the diagnosis of myocardial damage was 0.945 with 95% CI 0.899 ~0.991 (sensitivity:0.801,specificity:0.999,Youden Index:0.800),and the cutoff value was 29.93 μg/L.Conclusion E-selectin can be used as one of the diagnostic indices for myocardial damage in children,which has great diagnostic value.IL-6 and TNF-α are of limited value in the diagnosis of myocardial damage.
