ABSTRACT
The American Academy of Neurology (AAN) has recently updated its practise advisory on antiseizure medication withdrawal. The recommendations have been formulated for pediatric as well as adult epilepsy, with emphasis on the risk factors for seizure relapse, occurrence of status epilepticus or death on drug withdrawal, and effect on quality of life in both age groups. We herein list the important aspects of the updated recommendations in pediatric epilepsy for the benefit of the general pediatricians. The full update is available at the AAN website.
ABSTRACT
Many medical postgraduate teaching programs have residents with professional and personal problems that may limit theirperformance. A Problem resident is the one who does not meet the expectations of the training program owing to deficits in knowledge,skill or attitude. Medical administration and faculty of every institution must have a system that is sensitized to handle a difficult learner.Problems need to be addressed before they escalate or result in compromise of patient care. The present review discusses a broadapproach to recognizing a Problem resident and provides suggestions on remedial measures

In the use of antiepileptic drugs in patients with epilepsy,1/3 of the patients with epilepsy can not get effective control,showing resistance to antiepileptic drugs.Now the mechanism of drug resistance is considered to be caused by multiple factors work together.The concept and connotation between drug-resistant epilepsy and refractory epilepsy are different.Since 2010,the international union of antiepileptic proposeddrug-resistant epilepsy expert consensus,different medical centers use the new defition to study different populations and confirm that the new definition is effective and reliable.However,there are some controversier about the drug dosage,drug kinds,judge for determining evaluation index and the treatment judgement in that definition.
