ABSTRACT

Objective:To explore the application research of continuity nursing based on the interactive reaching standard theory after heart transplantation.Methods:The clinical data of 104 patients after heart transplantation in Nanjing Hospital Affiliated to Nanjing Medical University (Nanjing First Hospital) from June 2012 to June 2020 were collected for retrospective study. According to the file order, they were divided into the control group and the experimental group with 52 cases in each group. The control group took regular nursing care, and the experimental group took continuation nursing based on the interactive standard theory on the basis of the control group. The intervention continued for 24 weeks. The compliance behavior, psychological state and other indexes were compared between two groups.Results:The scove of Positive and Negative Affect Scale: after 12 weeks and 24 weeks of nursing, the experimental group′s positive emotion scores (27.71 ± 4.42, 35.11 ± 4.19) were higher than those of the control group (24.45 ± 4.03, 28.87 ± 4.36). The negative emotional scores (24.52 ± 3.71, 16.66 ± 2.28) were lower than those of the control group (28.84 ± 4.14, 23.31 ± 3.46), the differences were statistically significant ( t=5.60, 11.57, P<0.05). After 12-week and 24-week nursing, the experimental group′s health knowledge level score (44.12 ± 4.43, 53.64 ± 4.55) , self-care skills score(35.35 ± 4.01, 44.78 ± 4.22) , self-responsibility score (21.15 ± 2.38, 26.11 ± 1.44) , and self-concept score (18.56 ± 6.25, 23.58 ± 2.58) were higher than the control group (37.78 ± 4.52 and 45.56 ± 5.13, 31.11 ± 3.64 and 36.65 ± 3.91, 18.82 ± 2.46 and 22.35 ± 1.29, 15.96 ± 4.10 and 18.12 ± 3.10), the differences were statistically significant ( t values were 2.51-14.03, P<0.05). Conclusions:Continuing care led by the interactive standard theory can improve the health literacy of patients after heart transplantation, relieve negative emotions, promote post-traumatic growth, enhance self-management capabilities, establish compliance behaviors, and improve heart function.

El objetivo del presente trabajo es relatar la creación de un equipo de desarrollo infantil en la ciudad de Tandil. Introduciremos la experiencia humana y la emoción entendiendo que lo que esto despierta es tan valioso como la teoría que sostiene la práctica. Se narra la experiencia de seis años de trabajo dentro del ámbito de la salud pública, con el fin de poder compartir algunas formas de abordaje y un modelo de intervención, que permita sistematizar a su vez la formación de otros equipos de desarrollo, los cuales a su vez puedan encontrar y realizar abordajes regionales, atravesados por el contexto y cultura del lugar


The objective of this work is to report the creation of a child development team in the city of Tandil. We will introduce human experience and emotion, understanding that what this awakens is as valuable as the theory that sustains the practice. The experience of six years of work within the field of public health is narrated, in order to be able to share some forms of approach and an intervention model, which allows systematizing the formation of other development teams, which in turn they can find and carry out regional approaches, crossed by the context and culture of the place

ABSTRACT

Objective To explore the application effect of Interactive continuing nursing intervention model with three subjects and two tracks,which is based on internet and applied to chronic heart failure patients.Methods During January 1 to December 30,2017,110 chronic heart failure patients,who were going to discharge from the Second People's Hospital of Guangdong Province,were selected for this research.Participants were equally divided into control group and observation group with random number table with 55 cases each.Participants were adopted conventional medication and traditional care,while patients in control group were given extra interactive continuing nursing intervention with three subjects and two tracks,which was relied on internet-based cardiovascular disease management platform.Comparing patients' scores of life quality,self-care ability,anxiety and depression degree before intervention and that in 6 months after intervention with Minnesota Living with Heart Failure Questionnaire (MLHFQ),Self Care of Heart Failure Index (SCHFI),Self-rating Anxiety Scale(SAS)and Hamilton Depression Scale (HAMD) respectively.Moreover,6 minute walk test was conducted,simultaneously,the incidence of cardiovascular events was calculated respectively before intervention,3 months after intervention and 6 months after intervention.Results It showed that total scores of MLHFQ,SCHFI,SAS and HAMD for patients in observation group before intervention were (49.05 ± 4.39),(101.57 ±21.04),(58.3 ± 5.2) and (40.1 ± 3.5) points respectively while that for patients in observation group after 6 months intervention were (43.03 ± 6.85),(123.17 ± 18.23),(42.3 ± 4.2) and (24.4 ± 4.3) points respectively.The difference was statistically significant (t =5.49-20.60,all P <0.01).In comparison,intervention effect of observation group was better than that of control group,although,the SAS scores of the control group were statistically significant.Furthermore,after intervention,the difference of MLHFQ,SCHFI,SAS and HAMD scores were statistically significant(t =3.48-17.79,all P <0.01).Moreover,6 months after intervention,6 minute walk test evaluation showed that there was statistical significance between observation group and control group (~=8.11,P <0.05).Besides,The death rate in observation group was lower than that in control group after 6 months intervention (~=6.30,P <0.05).Meanwhile,readmission rate after 3 months and 6 months indicates that there was statistical significance between observation group and control group(x2=5.25,6.30,P <0.05).Conclusions Interactive nursing intervention model with three subjects and two tracks which based on internet is able to enhance chronic heart failure patients' self-care ability and life quality,decrease the incidence of cardiovascular events and negative emotion of patients.

ABSTRACT

Objective:To explore the effect of comprehensive intervention on the diabetes-relevant behaviors in aging patients with prediabetes in rural areas of Yiyang.Methods:The aging patients with prediabetes in rural areas of Yiyang were chosen and asked for the questionnaire.The patients were randomly divided into two groups:the control group and the intervention group.The control group received the routine health education,while the intervention group underwent comprehensive intervention for one year.6 months and 12 months before the intervention or after the intervention,the behavior characteristics of diabetes,levels of related physiological and biochemical indexes were evaluated.Results:The relative behavior characteristics of diabetes,levels of related physiological and biochemical indexes between the two groups were comparable before the intervention (P>0.05).6 months or 12 months after the intervention,it showed improvements in diabetes-related behaviors (P<0.05) and the scores of diabetes-related behaviors compared with the control group (P<0.001).Based on the related physiological and biochemical indexes,the intervention group showed significant reductions in the levels of waist-to-hip ratio,BMI,fasting plasma glucose and 2 h postprandial plasma glucose (P<0.01).Conclusion:The comprehensive intervention can improve the diabetes-relevant behaviors,physiological and biochemical indexes in the older patients with pre-diabetes.

ABSTRACT

Objective · To explore family medical intervention model of senile dementia with behavioral and psychological symptoms. Methods · Four streets of Changning District in Shanghai were randomly selected and subjects were enrolled according to the inclusion criteria, who were randomly divided into the intervention group (n=71) and control group (n=70). The intervention group received door-to-door service from psychiatric doctors, given drug treatment and psychological intervention. Subjects were evaluated by several scales, including Behavioral Pathology in Alzheimer's Disease Rating Scale (BEHAVE-AD), Mini-Mental State of Examination (MMSE), Activity of Daily Living Scale (ADL), Quality of Life-Alzheimer's Disease (QOL-AD), and Generic Quality of Life Inventory-74 (GQOLI-74), at baseline and by the end of 6 months and 12 months. Results · ① There was no significant difference in the total scores and all factor scores of BEHAVE-AD between the two groups before intervention (P>0.05). Repeated measures analysis of variance revealed a significant main effect of time (P0.05). Repeated measures analysis of variance revealed a significant main effect of time (P<0.001). The between-group effect was significant in the scores of MMSE and QOL-AD (P<0.001). The interactive effect of time×group was significant in the scores of MMSE, ADL, QOL-AD, and GQOLI-74 (P<0.05). ④ Inter-group comparison of MMSE, ADL, QOL-AD, and GQOLI-74 scores indicated that by the end of 6 months, scores of MMSE of the intervention group were remarkably better than those of the control group and the differences were statistically significant (P<0.05). By the end of 12 months, scores of MMSE, ADL, QOL-AD, and GQOLI-74 of the intervention group were remarkably better than those of the control group and the differences were statistically significant (P<0.05). Conclusion · The family medical intervention model of door-to-door services from psychiatrists integrating multidisciplinary team is effective to attenuate the mental and behavioral symptoms of senile dementia patients, and can improve the quality of life of patients and caregivers. The effect of persistent implementation will be more remarkable.

ABSTRACT

To explore the new model of community-based hypertension self-management, through literature review and qualitative interviews with experts and stakeholders.Among 15 000 patients with informed consents and voluntary participation, 1 080 patients were randomly selected as the intervention group, and 588 patients were randomly selected from hypertension management database of community health service centers as the control group.Analysis of covariance based on rank scores was employed to compare the score changes in two survey questionnaires, before and after six months of the intervention project, and between the intervention group and the control group, on self-assessment of overall health status, and qual-ity of health care services received in the community.Analysis of covariance was employed to compare the changes of blood pressure. Results The self-assessment of overall health status significantly improved for the intervention group, compared with the control group (P<0.05).The blood pressure of the inter-vention group remained stable, while the diastolic pressure for the control group increased as compared with that before intervention, the difference was statistically significant (P<0.05).During six months of the intervention project, as to the incremental value of scores in dimensions like initiative and design of health care service system, the difference between the intervention group and the control group was statistically significant ( P <0 .05 ) . Conclusion The intervention based on the new model of community-based hypertension self-management has a positive role in improving the patients'overall health status, stabilizing the blood pressure, and optimizing hypertension health care services in community.

ABSTRACT

Objective To observe the influence of hospital -community -family -individual 4 -level intervention model on schizophrenia patients in compliance,recurrence rate,psychotic symptom,social function,quali-ty of life and provide guidance for disease management.Methods 198 schizophrenia patients were divided into 4 -level intervention model group and conventional intervention group.After 6 moths follow -up,the effect was evaluated by compliance rate,recurrence rate,BPRS,WHO -DASII,SQLS.Results After the intervention for 6 months,the compliance,recurrence rate,the difference of severity of psychotic symptom,the difference of social function of the 4 -level intervention model group were 96.74%,13.04%,(7.57 ±2.83)points,[understanding and communication (6.32 ±3.64)points,physical exercise (5.16 ±3.28)points,self care (2.87 ±1.38)points,get on well with others (9.05 ±3.51)points,life activity (5.42 ±2.49)points,social participation (7.85 ±3.28)points],which of conven-tional intervention group were 74.74%,28.42%,(4.86 ±1.24)points,[understanding and communication (3.89 ± 1.86)points,physical exercise (2.80 ±1.238)points,self care (2.19 ±1.16)points,get on well with others (5.83 ± 3.04)points,life activity (3.41 ±1.96)points,social participation (4.27 ±2.17)points],the differences between the two groups were significant (χ2 =9.020,P =0.003;χ2 =6.932,P =0.009;t =8.527,P =0.000;t =5.775,P =0.000;t =6.554,P =0.000;t =3.652,P =0.000;t =6.712,P =0.000;t =6.144,P =0.000;t =8.829,P =0.000).The differences of before intervention and after intervention in the 4 -level intervention model group were psychological society (16.03 ±6.75)points,power and energy (15.79 ±6.41 )points,symptoms and side effects (19.37 ±7.28 )points,which in the conventional intervention group were psychological society (12.65 ± 5.46)points,power and energy (11.81 ±4.71 )points,symptoms and side effects (15.18 ±6.29 )points,the differences were statistically significant between the two groups (t =3.770,P =0.000;t =4.849,P =0.000;t =4.216,P =0.000).Conclusion Hospital -community -family -individual 4 -level intervention model has a positive effect on schizophrenia patients,which is worthy of clinical promotion.

ABSTRACT

Objective The purpose of this study was to investigate the effect of a dietary modification intervention model applied by ward nurse on change of dietary behavior among patients with type 2 diabetes mellitus (T2DM).Methods A total of 80 participants were divided into intervention patients (n=40) and control subjects (n=40) by random number table.Except lecture-based diabetes educational which was applied for control subjects,a dietary modification intervention model was conducted in intervention patients for a period of two weeks.The intervention program consisted of evaluating an individual's stage of change after being provided dietary information regarding kind of food and portions,discussion with a role model,and keeping a food diary record.Body mass index (BMI),waist-hip ratio (WHR),fasting plasma glucose (FPG),postprandial 2-h plasma glucose (2hPG),glycosylated hemoglobin (HbAlc) and score of healthy eating behavior were measured at initial and six months later.Results Compared with control group,BMI,WHR,FPG,2hPG,HbA1c in intervention group were significantly decreased,P < 0.01 or 0.05.After six months intervention,FPG,2hPG and HbA1c in both groups were significantly decreased compared with baseline levels,P< 0.01.Compared with control group,the scores of healthy eating behavior in intervention group were significantly decreased,P< 0.05.After six months intervention,the scores of healthy eating behavior in both groups were significantly elevated,P < 0.01,compared with baseline levels.Conclusions This study yielded evidence for the benefits of using the dietary modification intervention model as a framework in healthy eating behavior among patients with T2DM.

ABSTRACT

Objective To study the effect of application of comprehensive intervention model in the three-grade prevention for metabolic syndrome(MS). Methods 180 cases of patients diagnosed as MS were chosen in our hospital from January to June 2005,and divided randomly into the experimental group and the control group with 90 cases in each group. The control group received routine general health education intervention, but not related strengthened management and intervention, the experimental group adopted an integrated management approach, to establish a model of co- management included health care workers, patients and their families. Two groups were observed in patients' knowledge, self-management level, medical compliance and various indicators, then analyzed the incidence of cardio - cerebral vascular events after intervention. Results Knowledge, self-management level, medical compliance and related indicators of patients, and the incidence of cardio-cerebral vascular events after intervention were significant between the two groups. Conclusions Comprehensive intervention model can improve the patients' health knowledge attitudes, self-management level and the metabolic function. It can reduce the cardio-cerebral vascular accidents, improve the quality of life, and has a high economic and social value, so is worth promoting.
