ABSTRACT

Introducción: El embarazo ectópico es la implantación del producto de la concepción fuera de la cavidad endometrial. Objetivo: Reportar un caso de embarazo ectópico intraligamentario por lo infrecuente que es esta localización. Métodos: Presentación de caso de paciente de 25 años, con antecedentes de VIH e infecciones vaginales a repetición. Asiste a emergencias con dolor intenso en bajo vientre e historia de amenorrea de 12 semanas. Se hace el diagnóstico de posible embarazo ectópico abdominal y se decide laparotomía de urgencia. Resultados: Durante el transoperatorio se diagnostica un embarazo ectópico intraligamentario izquierdo, se realiza salpingectomía con resección de todo el ligamento ancho. La paciente evolucionó satisfactoriamente. Conclusiones: El embarazo ectópico continúa siendo una de las principales urgencias dentro de las especialidades quirúrgicas. Sigue elevando la mortalidad materna a nivel mundial y siempre que las condiciones lo permitan se debe pensar en él para hacer un diagnóstico oportuno(AU)


Introduction: Ectopic pregnancy is the implantation of the product of conception outside the endometrial cavity. Objective: To report a case of intraligamentary ectopic pregnancy because of the infrequent nature of this location. Case Report: This is a 25-year-old patient with a history of HIV and repeated vaginal infections. She comes to the emergency room with severe pain in the lower abdomen and history of amenorrhea for 12 weeks. Possible abdominal ectopic pregnancy is diagnosed and the emergency laparotomy is decided. During the transoperative period, a left intraligamentary ectopic pregnancy was diagnosed. Salpingectomy was performed with resection of the entire broad ligament. Result: The patient evolved satisfactorily. Conclusions: Ectopic pregnancy continues to be one of the main emergencies in surgical specialties. Maternal mortality continues raising worldwide and whenever the conditions suggest so, we must think about it to make a timely diagnosis(AU)

ABSTRACT

Pain control by means of local anesthesia is an intrinsic part of clinical practice in dentistry. Several studies evaluated intraligamental anesthesia using a computer-controlled anesthetic device in children. There is a need to provide a clinical guide for the use of computerized intraligamental anesthesia in children. Intraligamental anesthesia using a computer-controlled anesthetic device was found to cause significantly lower pain perception scores and lower pain-related behavior than traditional techniques. This device proven to be effective in restorative and pulp treatment in children; however, its effectiveness in primary teeth extraction is controversial. It is important to withdraw recommendations necessity of future studies concerning the side effects of computerized intraligamental anesthesia in children. The present study aims to review different clinical aspects of computerized intraligamental anesthesia in children along with the side-effects, type of local anesthesia and postoperative pain of this technique. This study provides dentists with a clinical guide for the use of computerized intraligamental anesthesia.

ABSTRACT

ANECEDENTES El embarazo ectópico abdominal es raro; con baja incidencia y ausencia de sintomatología especifica que dificulta el diagnóstico y no existan criterios para el diagnóstico y tratamiento. OBJETIVO se presenta un caso de embarazo intra-ligamentario de 16 semanas de gestación y se hacer una revisión de la literatura sobre el manejo y comparación de los resultados. CASO CLÍNICO Mujer de 32 años de edad, gestas 3 partos 2; desconoce fecha de ultima menstruación; y control de la fertilidad con condón; acude por presentar dolor abdominal, sin datos de irritación peritoneal; se le realiza ultrasonido y se diagnostica embarazo de 16 semanas con muerte fetal; con sospecha embarazo abdominal; se realiza laparotomía exploradora para extracción fetal, dejando la placenta in situ, y manejo con metotrexate dejando la placenta in situ y egresado al tercer día sin complicaciones. CONCLUSIÓN el embarazo abdominal es raro, de difícil diagnostico; que requiere la extracción quirúrgica y dejar la placenta in situ y manejo adyuvante con metrotexate; reportándose buenos resultados; pero, existe escasa información sobre la fertilidad futura.


BACKGROUND The abdominal ectopic pregnancy is rare; with low incidence and absence of specific symptomatology diagnosis difficult and there are no criteria for the diagnosis and treatment. OBJECTIVE a case of intra-ligament 16 weeks of gestation pregnancy occurs and a review of literature on the handling and comparison of results is done. CASE REPORT Female 32 years old, deeds 3 births 2; known last menstrual period; and fertility control with a condom; comes because of abdominal pain without peritoneal irritation; Ultrasound was performed 16 weeks pregnant and diagnosed with fetal death; abdominal suspected pregnancy; laparotomy for fetal extraction is performed, leaving the placenta in situ, and leaving management methotrexate placental site and discharged on the third day uncomplicated. CONCLUSION abdominal pregnancy is rare, difficult to diagnose; requiring surgical removal and leave the spot and adjuvant use with methotrexate placenta; good results being reported; but there is little information on future fertility.

ABSTRACT

OBJECTIVE: The aim of this study was to investigate and compare the in vivo effects of prostaglandin E2 (PGE2) administered by different methods on orthodontic tooth movement and bone metabolism macroscopically, histopatologically, and biochemically. METHODS: Forty-five young adult New Zealand rabbits were randomly divided into 3 experimental groups (n = 10/group), 1 positive control group (n = 10), and 1 negative control group (n = 5). The experimental rabbits were fitted with springs exerting 20-g reciprocal force on the maxillary incisors and PGE2 (10 microg/mL) was administered by the intravenous, submucosal, or intraligamentous route after appliance insertion and on days 1, 3, 7, and 14 thereafter. All rabbits were sacrificed on day 21 and their premaxillae were resected for histologic evaluation. RESULTS: Tooth movement was observed in the experimental and positive control groups, but the intraligamentous PGE2 group had the highest values of all analyzed parameters, including serum calcium and phosphorus levels and osteoclastic and osteoblastic populations (p < 0.001). CONCLUSIONS: Submucosal and intraligamentous PGE2 administration significantly increases orthodontic tooth movement and bone metabolism, but the intraligamentous route seems to be more effective.

Laparoscopic management is the ideal form of treatment for tubal pregnancy, most common type of ectopic pregnancy. Not only abdominal pregnancy including intraligamentary pregnancy is an rare form of ectopic pregnancy, but intraligamentary pregnancies are usually diagnosed and managed by emergency explorative laparotomy. We present the case of an intraligamentary pregnancy of a 28-year-old woman managed by laparoscopic treament without any complication.

ABSTRACT

Cornual pregnancy, which is a rare form of ectopic pregnancy, is an implantation of embryo in the intramural portion of a fallopian tubes. The incidence of cornual pregnancy is reported to be between 2-4% of all tubal pregnancy, about 1% of ectopic pregnancy, and occurs once for every 2,500 to 5,000 live births. Because the greater distensibility and vascularlity of this site lead to rupture at a latter gestational age and rapid intraperitoneal hemorrhage, cornual pregnancy causes serious maternal mortality and morbidity. Intraligamentary pregnancy, also a form of ectopic pregnancy, is one of the most unusual accident so that the diagnosis of intraligamentary pregnancy is hardly ever made prior to laparotomy. The incidence of intraligamentary pregnancy is reported to be about once for every 245 all ectopic pregnancy, and about once for every 49,000 to 184,000 all pregnancy. We have experienced an extremely rare case of right cornual pregnancy complicated with right intraligamentary pregnancy which was proven by histopathologic diagnoses in a 34-year-old woman and report it with review of literatures.

ABSTRACT

A case of intraperitoneal hemorrhage of intraligamentary pregnancy is one of the most unusual accidents in obstetrics and carries a high degree of maternal and fetal morbity and mortality. The diagnosis of intraligamentary pregnancy is rarely ever made prior to laparotomy. We report a case of it with a brief review of literatures concerned.

The incidence of uterine myoma with cystic degeneration is 4% and symptomatic uterine myoma requires surgical management. We experienced a case of intraligamentary leiomyoma with huge cystic degeneration with its total weight 2526gm in 48 years old woman and brief review of the case and its literatures are presented.

ABSTRACT

Uterine leiomyomas are the most common benign uterine tumors. They are estimated to be present in at least 20% of all women of reproductive age and may be asymptomatic in 40-50% of women older than 40 years of age. They may be subserosal, intramural, or submucosal in location within the uterus or located in the cervix, in the broad ligament, or on a pedicle. But it seems to be rare to experience a intraligamentary huge leiomyoma in the broad ligament.Recently we have experienced a case of intraligamentary huge leiomyoma with its weight of 3146 gm in 45 year-old primiparous woman. We present this case with a brief review of the literature.
