ABSTRACT

Objective: To explore the imaging features of intramedullary guide rod and its influence on the alignment of the femoral prosthesis in unicompartmental knee arthroplasty (UKA). Methods: Between August 2016 and November 2016, 50 patients (50 knees) with primary anteromedial osteoarthritis were treated with UKA by Oxford MicroPlasty minimally invasive replacement system. There were 10 males and 40 females. The age ranged from 62 to 77 years with an average of 68.8 years. Preoperative varus and flexion deformity angles were (5.22±3.46)° and (7.42±2.65)°, respectively. The knee range of motion (ROM) was (106.85±7.62)°. The Hospital for Special Surgery (HSS) score was 68.26±4.65. The angles between the femoral intramedullary guide rod and the anatomical axis of femur on the coronal and sagittal planes, the femoral component valgus/varus angle (FCVA), the femoral component posterior slope angle (FCPSA), knee varus deformity angle, and knee flexion deformity angle were measured by intra- and post-operative X-ray films. The postoperative ROM and HSS score were measured. Results: Intraoperative X-ray films measurement showed that the lateral side angles between femoral intramedullary guide rod and femoral anatomical axis were observed on coronal plane, and the angles ranged from 0.28 to 2.06° with an average of 0.96°. While the posterior side angles were observed on sagittal plane, and the angles ranged from 0.09 to 0.48° with an average of 0.23°. The angulations (>1°) between femoral intramedullary part guide rod and outside part of the rod were confirmed in 12 cases (24%) on coronal plane. Postoperative femoral prosthesis were mild varus in 38 patients (76%). The FCVA ranged from -1.76 to 4.08° with an average of 2.21°. The FCPSA ranged from 7.12 to 13.86° with an average of 9.16°. All patients were followed up 22-26 months, with an average of 24.5 months. The incisions healed by first intention. At last follow-up, the varus and flexion deformity angles were (1.82±1.05) and (2.54 ± 1.86)°, respectively. ROM was (124.62±5.85)° and HSS score was 91.58±3.65. There were significant differences between pre- and post-operative parameters ( P<0.05). No complication such as dislocation or aseptic loosening of the prosthesis occurred during the follow-up. Conclusion: UKA by Oxford MicroPlasty minimally invasive replacement system can obtain accurate femoral prosthesis position with the help of intramedullary guide system, and the effectiveness is excellent.

ABSTRACT

<p><b>Background:</b>Identification of the proper femoral intramedullary (IM) access point is an important determinant of final implant position in IM-guided total knee arthroplasty (TKA). The aim of this study was to identify the optimal entry point in Chinese participants using a new three-dimensional method.</p><p><b>Methods:</b>A series of computed tomography scans of 44 femurs in Chinese participants from October 2014 to October 2015 were imported into Mimics 17.0 software to identify the optimal entry point. The apex of the intercondylar notch (AIN) was used as the reference bony anatomical landmark to identify the proper entry point to insert the IM rod. The statistical significance was calculated on the basis of a 5% level (P < 0.05) using the Student's t-test.</p><p><b>Results:</b>For the males, the average ideal entry point was 1.49 mm medial and 13.39 mm anterior to the AIN. The values were 1.77 mm medial and 15.29 mm anterior to the AIN in females. A significant difference was present between males and females (13.39 ± 2.46 mm vs. 15.29 ± 3.44 mm, t = 2.124, P = 0.040). When using the recommended location as the entry point for the IM rod, the mean potential error differed significantly from the femoral trochlear groove (the potential error of IM in males in coronal plane: 0.93° ± 0.24° vs. 1.27° ± 0.32°, t = -4.166, P <0.001; the potential error of IM in males in sagittal plane: 1.40° ± 0.42° vs. 2.79° ± 0.70°, t = 7.155, P < 0.001; the potential error of IM in females in coronal plane: 0.73° ± 0.28° vs. 1.15° ± 0.35°, t = 3.940, P < 0.001; and the potential error of IM in females in sagittal plane: 1.48° ± 0.47° vs. 2.76° ± 0.83°, t =5.574, P < 0.001). A significant difference was present between the recommended point and the point 10 mm anterior to the origin of the posterior cruciate ligament (the potential error of IM in males in coronal plane: 0.93° ± 0.24° vs. 1.53° ± 0.43°, t = 5.948, P < 0.001; the potential error of IM in males in sagittal plane: 1.40° ± 0.42° vs. 2.15° ± 0.75°, t = 3.152, P = 0.003; the potential error of IM in females in coronal plane: 0.73° ± 0.28° vs. 1.28° ± 0.42°, t = -4.632, P < 0.001; and the potential error of IM in females in sagittal plane: 1.48° ± 0.47° vs. 2.40° ± 0.93°, t = 3.763, P = 0.001).</p><p><b>Conclusions</b>The technique described here is an innovative method for swift, easy, and accurate access to the medullary canal during TKA, and it can optimize the position and orientation of the prosthetic components in knee arthroplasty.</p>

ABSTRACT

PURPOSE: The purpose of this study was to evaluate clinical results and accuracy of femoral cutting in the coronal plane in total knee arthroplasty (TKA) using a fixed length intramedullary guide. MATERIALS AND METHODS: From 2005 to 2008, 101 patients (154 knees) underwent TKA (NexGen LPS implant). The minimal follow-up period was 3 years (mean, 4.4 years). The patients were divided into two groups (group 1, 94alpha, 98 or =2degrees MAD was 65 in group 1 and 24 in group 2. The mean PTA, KSKS, and KSFS were 10.17degrees, 96.0, and 96.6, respectively, in group 1 and 11.58degrees, 84.5, and 85.5, respectively, in group 2. CONCLUSIONS: The percentage of coronal alignment outliers was relatively high (34 in 154 cases, 22%) after TKA using a fixed length intramedullary guide. However, there was no statistically significant intergroup difference in clinical results (KSKS, p=0.67; KSFS, p=0.56).
