ABSTRACT

Background Maternal atmospheric pollution during pregnancy may alter fetal intrauterine development programming, thereby increasing the risk of childhood obesity in the future. Objective To investigate the effects of atmospheric pollution exposure during pregnancy on the incidence of childhood obesity in offspring. Methods English databases (PubMed, Web of Science, and Medline) and Chinese databases (Wanfang Data Knowledge Service Platform, China National Knowledge Infrastructure, and VIP Information Chinese Journal Service Platform) were searched for literature reporting exposure to atmospheric pollution during pregnancy and childhood obesity published from 1 January 2000 to 31 August 2023. The quality of the included literature was evaluated using the quality assessment tools for observational cohort and cross-sectional studies recommended by the US National Institutes of Health. Results Twenty-four studies meeting the inclusion criteria were identified and the associated atmospheric pollutants included particulate matter, ozone, nitrogen oxide, carbon oxide, and sulfur oxide. In comparison to the non-exposed group, prenatal exposure to various common atmospheric pollutants were significantly associated with an elevated risk of childhood obesity in offspring. Conclusion Maternal exposure to atmospheric pollution during pregnancy is associated with an elevated risk of childhood obesity in subsequent years. Future studies should pay more attention to the effects of atmospheric pollution on the distribution of children's body fat and metabolic development, and further identify potential mechanisms of atmospheric pollutant exposure leading to childhood obesity.

ABSTRACT

ObjectiveTo introduce a fixation technique with the modified levonorgestrel-releasing intrauterine system (LNG-IUS) and evaluate its efficacy in the treatment of adenomyosis patients with previous LNG-IUS expulsion. MethodsA retrospective analysis was done on 22 adenomyosis patients who underwent modified LNG-IUS fixation due to LNG-IUS expulsion at three hospitals from June 2022 to June 2023. The baseline clinical characteristics, operative and postoperative details were collected and analyzed. The Visual analogu scale (VAS) scores and pictorial blood loss assessment chart (PBAC) scores were measured and compared before, 3 and 6 months after the LNG-IUS fixation. ResultsThe mean operative time was (19.51±7.41) min and intraoperative bleeding was (6.71±5.30) mL. Of the patients, 13 were operated under local anaesthesia and the other 9 under intravenous anaesthesia. There were 4 operations performed by a resident doctor, 15 by an attending doctor and 3 by a senior doctor. No intraoperative or postoperative complication was found. The mean follow-up was 11.51 months and no patient had a recurrence of LNG-IUS expulsion during the follow-up period. The mean level of hemoglobin at 1 month after operation was significantly higher than that before (P<0.001). VAS scores and PBAC scores at 3 and 6 months postoperatively were all improved significantly than those preoperatively (P<0.001). ConclusionsEffectively preventing the recurrence of LNG-IUS expulsion, modified LNG-IUS fixation is a safe and efficient method for adenomyosis patients with previous LNG-IUS expulsion. Modified LNG-IUS fixation deserves the clinical application due to its easy operation and wide range of use on women.

ABSTRACT

ObjectiveTo evaluate the effect of women's body mass index (BMI) on pregnancy outcomes of ovulation induction intrauterine insemination (OI-IUI) in patients with unexplained primary infertility. MethodsThe study included 764 OI-IUI cycles from January 2016 to December 2022 in reproductive center of Sun Yat-sen Memorial Hospital. According to BMI,patients were divided into three groups:low BMI (BMI<18.5 kg/m2), normal BMI (18.5 kg/m2 ≤BMI<23.0 kg/m2), and high BMI (BMI≥23.0 kg/m2). Comparison of clinical data and pregnancy outcomes was performed between the groups. Logistic regression was used to analyze the association between BMI and live birth rate. ResultsFrom the low BMI group to the high BMI group, the HCG positive rate (7.08%,9.74%, 13.19%), clinical pregnancy rate(5.51%, 7.91%, 13.19%), and live birth rate (4.72%, 6.90%, 12.50%) increased. Among them, the live birth rate of the high BMI group was significantly higher than that of the low BMI group and the normal BMI group, with a statistically significant difference (P=0.034). While the early miscarriage rate (14.28%, 10.26%, 5.26%) decreased from the low BMI group to the high BMI group. The binary logistic regression analysis revealed that BMI was an independent factor in live birth, and high BMI resulted in a better live birth rate than low BMI (OR=3.15,95%CI=1.191-8.329,P=0.021). ConclusionLow BMI is associated with poor OI-IUI outcomes in patients with unexplained primary infertility. These patients are encouraged to gain weight in a healthy manner.

ABSTRACT

Aim To observe the effects of oral puerarin (Pue) on the maternal gestational diabetes mellitus (GDM) rats and its fetal growth and development, so as to provide references for the application of Pue in the treatment of GDM. Methods The GDM rat model was established by injecting streptozotocin (STZ) into the tail vein of pregnant female rats, and the rats were treated with Pue orally for 12 days; the body weight and abortion of pregnant rats were recorded. The fasting blood glucose of pregnant rats was detected before and at the end of the treatment, and the glucose tolerance was tested on the 5th and 10th days after the administration of the drug. The cesarean sections were carried out on the 20th day of pregnancy. The blood glucose content of the fetal rats was detected, and the state of development was observed. The body weight and length were measured, as well as the placenta and the important organs weight, and the indexes of the organs were calculated. Results Compared with the model group, Pue could significantly reduce the fasting blood glucose of GDM pregnant rats and fetal rats, improve the glucose tolerance of pregnant rats, effectively alleviate the excessive weight gain of pregnant rats and overweight of fetal rats caused by GDM, and reduce the abortion rate; it could also reverse the decrease in the indexes of the organs of brain, heart, and liver, and the increase in the indexes of organs of kidney in fetal rats caused by GDM. Conclusions Pue can relieve the maternal and the fetal hyperglycemia in GDM, reduce the rate of miscarriage, reduce the incidence of macrosomia, and promote the development of vital fetal organs.

ABSTRACT

ABSTRACT OBJECTIVE To describe the prevalence of contraindicated use of combined hormonal contraceptives, progesterone-only contraceptives, and intrauterine devices in mothers participating in the 2015 Pelotas Birth Cohort according to the WHO medical eligibility criteria. METHODS The biological mothers of children belonging to the 2015 Pelotas birth cohort who attended the 48-month follow-up were studied. The 48-month follow-up data were collected from January 1, 2019, to December 31, 2019. Contraindicated use of modern contraceptives was considered to occur when these women presented at least one of the contraindications for the use of modern contraceptives and were using these methods. The prevalence of contraindicated use was calculated according to each independent variable and their respective 95% confidence intervals (95%CI). RESULTS The analyzed sample consisted of 3,053 women who used any modern contraceptive method. The prevalence of contraindicated use of modern contraceptives totaled 25.9% (95%CI: 24.4-27.5). Combined hormonal contraceptives showed the highest prevalence of contraindicated use (52.1%; 95%CI: 49.3-54.8). The prevalence of contraindicated use of modern contraceptives methods was greater in women with family income between one and three minimum wages, a 25-30 kg/m2 body mass index, indication by a gynecologist for the used method, and purchasing the contraceptive method at a pharmacy. The higher the women's education, the lower the prevalence of inappropriate use of modern contraceptives. CONCLUSION In total, one in four women used modern contraceptives despite showing at least one contraindication. Policies regarding women's reproductive health should be strengthened.

ABSTRACT

RESUMO Objetivo identificar quais os desfechos das inserções de dispositivos intrauterinos por enfermeiros em instituições de saúde. Métodos revisão integrativa realizada em oito bases de dados, com auxílio de um gerenciador de referências bibliográficas, utilizando o modelo de JBI para a análise do nível de evidência. Os dados foram organizados, categorizados e discutidos por meio de síntese descritiva. Resultados 10 estudos compuseram a amostra final, sendo dois apenas com enfermeiros e os demais com enfermeiros e médicos. Os principais desfechos avaliados foram perfuração e expulsão, sem diferenças significativas entre os profissionais que realizaram o procedimento. As taxas de sucesso, continuidade e satisfação foram semelhantes entre médicos e enfermeiros. Conclusão os desfechos das inserções de dispositivos intrauterinos por enfermeiros em instituições de saúde são similares aos realizados por médicos, com ampliação do acesso, sem aumentar as complicações relacionadas a esse método contraceptivo, contribuindo para a integralidade da assistência no campo do planejamento reprodutivo. Contribuições para a prática: os achados podem contribuir para o enfrentamento de barreiras pelos enfermeiros, servindo como subsídio para diretrizes e políticas de saúde que incentivem a inserção do dispositivo intrauterino por estes profissionais, principalmente, em contextos onde esta prática ainda não ocorre.


ABSTRACT Objective to identify the outcomes of intrauterine device insertions by nurses in healthcare institutions. Methods integrative review carried out in eight databases, with the help of a bibliographic reference manager, using the JBI model for analyzing the evidence level. The data was organized, categorized and discussed using descriptive summaries. Results 10 studies made up the final sample, two with nurses only and the others with nurses and physicians. The main outcomes assessed were perforation and expulsion, with no significant differences between the professionals who performed the procedure. Success rates, continuity and satisfaction were similar between physicians and nurses. Conclusion the outcomes of intrauterine device insertions by nurses in health institutions are similar to those carried out by physicians, with increased access, without increasing the complications related to this contraceptive method, contributing to comprehensive care in the field of reproductive planning. Contributions to practice: the findings could help nurses to tackle barriers and serve as a basis for guidelines and health policies that encourage the insertion of the intrauterine device by these professionals, especially in contexts where this practice does not yet take place.

ABSTRACT

ABSTRACT Objectives: to synthesize and analyze evidence on intrauterine device insertion by nurses in Primary Health Care. Methods: an integrative review, carried out in the BDENF, CINAHL, LILACS, SciELO, Scopus, PubMed and Web of Science databases in June 2022, delimiting the period from 1960 to 2022. Results: 141 articles were identified in the initial search, and 10 studies made up the final sample. Four (40%) were developed in the United States and one (10%) in Brazil, with publications from 1979 to 2021. The findings were grouped into three categories: Nurse training to insert an intrauterine device; Nurses' competency to insert an intrauterine device; and Women's access to intrauterine devices. Conclusions: nurse theoretical and practical training is a prominent element, consolidated in the favorable outcomes of insertions performed by nurses and satisfaction among women, a practice that has expanded access to the contraceptive method in Primary Health Care.

Métodos: revisión integrativa, realizada en las bases de datos BDENF, CINAHL, LILACS, SciELO, Scopus, PubMed y Web of Science en junio de 2022, delimitando el período de 1960 a 2022. Resultados: se identificaron 141 artículos en la búsqueda inicial y 10 publicaciones conformaron la muestra final. Cuatro (40%) fueron desarrollados en Estados Unidos y uno (10%) en Brasil, con publicaciones de 1979 a 2021. Los hallazgos se agruparon en tres categorías: Capacitación de enfermeras para insertar un dispositivo intrauterino; Competencia de las enfermeras para insertar un dispositivo intrauterino; y Acceso de las mujeres a los dispositivos intrauterinos. Conclusiones: la formación teórica y práctica de los enfermeros es un elemento destacado, consolidado en los resultados favorables de las inserciones realizadas por los enfermeros y la satisfacción de las mujeres, práctica que ha ampliado el acceso al método anticonceptivo en la Atención Primaria de Salud.

ABSTRACT

Objetivo: verificar as barreiras enfrentadas pelos enfermeiros na ampliação da inserção de dispositivo intrauterino (DIU) em pacientes no contexto das Unidades Básicas de Saúde. Metodologia: estudo observacional realizado com 66 enfermeiros da Atenção Primária. O instrumento de coleta foi elaborado por revisão de literatura e validado por especialistas da área da saúde da mulher. A coleta de dados ocorreu de forma remota entre outubro de 2021 e janeiro de 2022. Todas as considerações éticas para pesquisas com seres humanos foram respeitadas. Resultados: observou-se que a maioria não realizou treinamento de inserção de DIU (86,4%), não possuíam experiência de inserção (100%) e nem segurança na técnica (71,2%). Ademais, o DIU não é inserido (83,3%) nas unidades de saúde e, para a inserção, há necessidade de realização de exames (84,8%), como ultrassonografia transvaginal (76,2%), exames de citologia oncótica (92,1%) e teste de gravidez (76,2%). Conclusão: as barreiras observadas neste estudo dificultam o acesso ao DIU na Atenção Primária e afetam os direitos à liberdade, à saúde e ao planejamento reprodutivo. Verificou-se a necessidade da criação de um protocolo assistencial sobre o DIU de cobre e elaboração de uma estratégia de qualificação teórico-prática como forma de ampliação do método e garantia dos direitos sexuais e reprodutivos das mulheres previstos na Constituição Federal.


Objective: to assess the impediments confronted by nurses in the expansion of intrauterine device (IUD) insertion within the context of Basic Health Units.Methodology: an observational study was conducted involving 66 Primary Care nurses. The data collection instrument was formulated through an extensive literature review and subsequently validated by experts specializing in women's health. Data collection transpired remotely between October 2021 and January 2022, adhering to all ethical considerations pertinent to research involving human subjects. Results:findings revealed that a majority of participants had not undergone specific training for IUD insertion (86.4%), lacked practical experience in the procedure (100%), and expressed a lack of confidence in executing the technique (71.2%). Furthermore, the IUD insertion was infrequently performed within health units (83.3%), necessitating ancillary tests, such as transvaginal ultrasound (76.2%), oncotic cytology tests (92.1%), and pregnancy tests (76.2%). Conclusion:the identified barriers in this study impede access to IUDs in Primary Care, thereby encroaching upon fundamental rights related to autonomy, health, and reproductive planning. The study underscores the necessity for the formulation of a comprehensive care protocol concerning the copper IUD and the development of a theoretical-practical training strategy. This strategy aims to broaden the utilization of the method, thereby safeguarding women's sexual and reproductive rights as enshrined in the Federal Constitution.

El instrumento de recolección fue desarrollado a través de una revisión de la literatura y validado por expertos en el campo de la salud de la mujer. La recolección de datos se realizó de forma remota entre octubre de 2021 y enero de 2022. Se respetaron todas las consideraciones éticas para la investigación con seres humanos. Resultados:se observó que la mayoría no realizó entrenamiento para la inserción del DIU (86,4%), no tenía experiencia en la inserción (100%) y no tenía confianza en la técnica (71,2%). Además, el DIU no se inserta (83,3%) en las unidades de salud y, para su inserción, es necesario realizar pruebas (84,8%), como ecografía transvaginal (76,2%), citología oncótica (92,1%) y test de embarazo. (76,2%). Conclusión:las barreras observadas en este estudio dificultan el acceso a los DIU en Atención Primaria y afectan los derechos a la libertad, la salud y la planificación reproductiva. Era necesario crear un protocolo de atención al DIU de cobre y desarrollar una estrategia de capacitación teórico-práctica como forma de ampliar el método y garantizar los derechos sexuales y reproductivos de las mujeres previstos en la Constitución Federal

ABSTRACT

Objetivo: Analizar el efecto sobre el peso corporal de la anticoncepción hormonal continua mediante implante sub-dérmico liberador de etonogestrel (ENG) en mujeres en edad reproductiva atendidas en la consulta de planificación familiar de un hospital venezolano. Métodos: Investigación comparativa, con diseño cuasi experimental, de casos y controles, a etiqueta abierta, y prospectivo; con una muestra intencionada de 60 mujeres separadas para recibir bien sea un implante subdérmico (Implanon NXT®; casos) o un dispositivo intrauterino (DIU) de cobre (T de cobre; controles). Se evaluaron el peso corporal e índice de masa corporal antes y posterior de doce meses del uso del contraceptivo; así como las características demográficas de las usuarias, efectos adversos y efectividad anticonceptiva de cada método. Resultados: Luego de un año con el implante de ENG no se encontraron variaciones significativas respecto a las mediciones iniciales del peso corporal (61,21±8,30 vs. 61,23±9,50, p>0,5) e IMC (25,23±3.89 vs. 25,26 ±4,30; p>0,05); contrariamente, a lo observado entre las usuarias del DIU donde tanto el peso corporal como el IMC tuvieron un aumento significativo (P<0,05). Asimismo, la mayoría de las usuarias se mantuvieron en el mismo rango de peso donde se encontraban al iniciar el método (p<0,001); mientras que la ganancia ponderal fue mayor entre las usuarias del DIU (1,530±2,04 vs. 3,700±3,02; p<0,05). Conclusiones: El implante de ENG no produce aumento del peso corporal luego de 12 meses de uso, con mínimos efectos adversos y alta efectividad contraceptiva.


Aim: To analyse the effect on body weight of continuous hormonal contraception by releasing subdermal implant etonogestrel (ENG) in women of reproductive age treated in the family planning consultation of a Venezuelan hospital. Methods: Comparative and applied research, with quasiexperimental, case-control, open label and prospective design, with an intentional sample of 60 women separated to receive the contraceptive implant (Implanon NXT®; cases) or a cooper intrauterine device (IUD) (Cooper T; controls) was carried out. Body weight and body mass index were evaluated before and after 12 months of contraceptive use; as well as demographic characteristics of users, side effects, and contraceptive effectiveness of each method. Results: After one year with the ENG implant, no significant variations were found with respect to initial measurements of body weight (61.21±8.30 vs. 61.23±9.50, p>0.5) and BMI (25.23±3.89 vs 25.26±4.30; p>0.05); on the contrary, to what was observed among IUD users where both body weight and BMI had a significant increase (P<0.05). Likewise, most users remained in the same weight range as when starting the method (p<0.001); while weight gain was greater among IUD users (1,530±2.04 vs. 3,700±3.02; p<0.05). Conclusions: The ENG implant does not produce an increase in body weight after 12 months of use, with minimal adverse effects and high contraceptive efficacy.

ABSTRACT

Objetivo: Analisar o uso dos contraceptivos hormonais em mulheres com asma e a escolha desses métodos contraceptivos para essa população, com avaliação de eventuais repercussões sobre novos episódios de asma e sibilos. Métodos: Foram selecionados estudos longitudinais, ensaios clínicos, revisões sistemáticas e metanálises. As plataformas consultadas foram PubMed, Embase, Cochrane e SciELO, com a utilização dos descritores: "contracepção", "contracepção hormonal", "sistema intrauterino liberador de levonorgestrel" e "asma". Resultados: Dois grandes estudos demonstraram que o uso de contraceptivos hormonais esteve associado à redução do risco de novos episódios de asma. Uma revisão sistemática concluiu que os resultados para o uso de contraceptivos hormonais para mulheres com asma foram mistos, com aumento ou redução dos seguintes riscos: novo episódio de asma e aumento da frequência das crises e dos sibilos. O uso da contracepção hormonal em pacientes obesas portadoras de asma é controverso. Conclusão: Os resultados para o uso de contraceptivos hormonais em mulheres com asma são inconsistentes, com relatos de aumento ou de redução do risco de novos episódios. O uso do método contraceptivo deve ser discutido individualmente, levando-se em consideração outros fatores de risco associados e o desejo da mulher. A paciente deverá ser orientada se houver piora dos sintomas clínicos de asma na vigência do uso de qualquer método contraceptivo hormonal.


Objective: To analyze the use of hormonal contraceptives in women with asthma and the choice of this contraceptive method for this population, evaluating possible repercussions on new episodes of asthma and wheezing. Methods: Longitudinal studies, clinical trials, systematic reviews and meta-analyses were selected. Platforms consulted: PubMed, Embase, Cochrane, SciELO, using the descriptors: "contraception", "hormonal contraception", "levonorgestrel-releasing intrauterine system" and "asthma". Results: Two large studies demonstrated that the use of hormonal contraceptives was associated with a reduced risk of new episodes of asthma. A systematic review concluded that the results for the use of hormonal contraceptives for women with asthma were mixed, with increased or decrease in the following risks: new asthma episodes, increased frequency and wheezing. The use of hormonal contraception in obese patients with asthma is controversial. Conclusion: The results for the use of hormonal contraceptives in women with asthma are inconsistent, with reports of increased or reduced risk of new episodes. The use of the contraceptive method should be discussed individually, taking into account other associated risk factors and the woman's desire. The patient will be advised if there is a worsening of the clinical symptoms of asthma while using any hormonal contraceptive method.

Objective To report a case of early pregnancy loss with septate uterus, successfully managed by hysteroscopy guided suction & evacuation of product of conception along with removal of uterine septum in same sitting. Case report. Tertiary care hospital. One woman with early pregnancy loss with septate Design Setting Patient uterus. Hysteroscopy guided suction & evacuation of product of conception along with removal of uterine septum Intervention in same sitting. Hysteroscopy guided suction & evacuation of product of conception followed by septum removal with Result hysteroscopic scissor was performed, with minimal blood loss, in same sitting. Products of conception were sent for pathologic examination. There were no intra-operative or postoperative complications. Even though the role of septum as a Conclusion contributing factor to miscarriage in not certain, early pregnancy loss may be seen in patients with septate uterus. Blind removal of POCs with dilatation & curettage, may be complicated with retained products of conception (RPOC) or intrauterine adhesion formation, which can lead to adverse fertility outcomes in the future. Also removal of septum, which may or may not be the cause, is often performed before further pregnancy. This requires another admission & exposure to anaesthesia, along with added cost. Hysteroscopy guided resection of POC with septum removal in same sitting has been associated with complete tissue removal under vision, less damage to surrounding endometrium, cost effective combination of two surgical procedures & earlier time to conception compared with 2 sitting procedure (dilation and curettage than septum removal). Thus, hysteroscopy guided suction & evacuation of POC & septum removal in same sitting can be considered as an alternative surgical technique for management of early pregnancy loss in patients diagnosed with septate uterus. This case report demonstrates the successful application of hysteroscopic procedure in a case of early pregnancy loss with septate uterus.

Background & objectives: Postpartum intrauterine contraceptive device (PPIUCD) is well accepted and recommended for contraception. However, anxiety at the time of delivery may restrict the acceptance of a PPIUCD for its immediate insertion. So far there is limited evidence to conclude anything concrete on the association between the expulsion rates and the timing of insertion following a vaginal delivery. Thus, this study was undertaken to compare the expulsion rates in immediate and early insertions and their safety and complications. Methods: This prospective comparative study was carried out over 17 months on women delivering vaginally in a tertiary care teaching hospital in South India. A copper device (CuT380A) was inserted using Kelly’s placental forceps either within 10 min of placental delivery (immediate group, n=160) or between 10 min upto 48 h postpartum (early group, n=160). Ultrasound was done before discharge from the hospital. The expulsion rates and any other complications at six-week and three-month follow up were studied. Chi-square test was used to compare the difference in expulsion rates. Results: The expulsion rate was five per cent in the immediate compared to 3.7 per cent in the early group (no significant difference). In ten cases, the device was found to be in the lower uterus upon ultrasound before discharge. These were repositioned. There was no case with perforation, irregular bleeding or infection up to the three-month follow up. Higher age, higher parity, lack of satisfaction and motivation to continue were predictors of expulsion. Interpretation & conclusions: In the present study PPIUCD was found to be safe with overall expulsion in 4.3 per cent. It was marginally, though not significantly, higher in the immediate group.

Objetivo: El uso de anticonceptivos hormonales ha crecido progresivamente en muchos países del mundo. Determinar los patrones de utilización de anticonceptivos hormonales, frecuencia de comorbilidades y medicaciones concomitantes en mujeres mayores de 15 años afiliadas al Sistema de Salud de Colombia. Materiales y métodos: Estudio de corte, que incluyó datos de mujeres mayores de 15 años con prescripciones y dispensaciones de algún anticonceptivo hormonal durante al menos tres meses continuos (abril a junio-2016). Se creó una base de datos con información sociodemográfica, farmacológica (tipo de anticonceptivos, dosis, comedicaciones y comorbilidades. Se realizaron análisis descriptivos y multivariados buscando identificar factores asociados con comedicaciones de riesgo. Resultados: Se identificaron 34309 mujeres que recibieron anticonceptivos hormonales, con edad media de 27,2±7,0 años (rango:13-60,8 años). Los anticonceptivos más utilizados fueron inyectables de aplicación mensual (63,0 %), inyectables de aplicación trimestral (19,1 %), de administración oral (12,1 %), los implantes subdérmicos (7,4 %) y finalmente los dispositivos intrauterinos hormonales con 0,4 %. El 5,7 % de las pacientes (n=1957), estaban recibiendo alguna comedicación, especialmente con antihipertensivos (2,9 %) y antimigrañosos (1,9 %). Ser mayor de 45 años (OR:2,3; IC95 %:1,7-3,0), utilizar dispositivo intrauterino hormonal (OR:2,4; IC95 %:1,4-4,1) y anticonceptivo inyectable trimestral (OR: 1,7; IC95 %:1,3-2,3) se asociaron con mayor probabilidad de recibir comedicaciones. Conclusiones. Las mujeres colombianas que acceden a anticonceptivos hormonales a través del Sistema de Salud están empleando principalmente presentaciones inyectables, con muy baja frecuencia de las orales, y en general tienen pocas comorbilidades que requieran tratamiento farmacológico, pese a que algunas tienen condiciones cardiovasculares que pueden implicar un potencial riesgo de eventos trombóticos.


Objective: The use of hormonal contraceptives has grown progressively in many countries of the world. The aim was to determine the patterns of use of hormonal contraceptives, frequency of comorbidities and concomitant medications in women over 15 years of age affiliated with the Colombian Health System. Methods: Cross-sectional study, which included data on women over 15 years of age with prescriptions and dispensations of a hormonal contraceptive for at least three continuous months (April-June-2016). A database with sociodemographic, pharmacological information (type of contraceptives, doses, comedications and comorbidities) was created. Descriptive and multivariate analyzes were conducted seeking to identify factors associated with risk comedications. Results: 34309 women who received hormonal contraceptives were identified, with a mean age of 27.2 ± 7.0 years (range: 13-60.8 years). The most commonly used contraceptives were injectable of monthly application (63.0 %), injectable of quarterly application (19.1 %), oral administration (12.1 %), subdermal implants (7.4 %) and finally the hormonal intrauterine devices with 0.4 %. 5.7 % of the patients (n = 1957) were receiving some medication, especially with antihypertensives (2.9 %) and anti-migraines (1.9 %). Be over 45 years old (OR:2.3; 95 %CI: 1.7-3.0), use hormonal intrauterine device (OR: 2.4; 95 % CI:1.4-4.1) and quarterly injectable contraceptive (OR:1.7; 95 %CI:1.3-2.3) were associated with a higher probability of receiving comedications. Conclusions: Colombian women who access hormonal contraceptives through the Health System are mainly using injectable presentations, with very low frequency of oral ones, and in general they have few comorbidities that require pharmacological treatment, although some have cardiovascular conditions that may involve potential risk of thrombotic events.

ABSTRACT

La prevención de las enfermedades crónicas no trasmisibles de elevada prevalencia a nivel mundial, como la hipertensión arterial, la diabetes mellitus, las enfermedades cardiovasculares y el cáncer, representa una prioridad en salud. La nueva perspectiva brindada por la epigenética sobre el origen intrauterino de las enfermedades que afectarán al ser humano durante la etapa posnatal, obliga a replantearse una nueva visión preventiva, que debe iniciarse desde el período prenatal de la vida. Con el objetivo de estructurar los referentes teóricos que sustentan la relevancia de un nuevo enfoque preventivo de las enfermedades crónicas no trasmisibles, basado en intervenciones modificatorias de los perfiles epigenéticos desfavorables durante la etapa prenatal del desarrollo humano, se realizó una actualizada búsqueda sobre el tema, consultándose 28 referencias bibliográficas. Condiciones maternas durante la gestación, como la malnutrición, el estrés, los hábitos tóxicos y la obesidad, constituyen factores causantes de modificaciones epigenéticas desfavorables, que serán trasmitidas a futuras generaciones e incrementarán en estas el riesgo de enfermedades durante la etapa posnatal de la vida. Se concluye que las intervenciones realizadas durante el período prenatal del desarrollo humano pueden contribuir a la prevención de enfermedades crónicas no trasmisibles, a lo que la epigenética aporta un nuevo enfoque en la prevención de este importante problema de salud.


The prevention of chronic non-communicable diseases with high prevalence around the world, such arterial hypertension, diabetes mellitus, cardiovascular diseases and cancer, represents a health priority. The new perspective given by epigenetics on the intrauterine origin of the diseases that will affect the human being during the post-natal stage, forces us to reconsider a new preventive vision which must begin from the prenatal period of life. With the aim of structuring the theoretical references that support the relevance of a new preventive approach to chronic non-communicable diseases, based on modifying interventions of unfavorable epigenetic profiles during the prenatal stage of the human development, an updated search on the subject was conducted, consulting 28 bibliographic references. Maternal conditions during pregnancy, such as malnourishing, stress, toxic habits and obesity, constitute factors that cause unfavorable epigenetic modifications, which will be transmitted to future generations and will increase in them the risk of disease during the post-natal stage of life. It is concluded that the interventions carried out during the prenatal period of human development can contribute to the prevention of chronic non-communicable diseases, to which epigenetics provides a new approach in the prevention of this important health problem.

Resumo: A gestação é um fenômeno fisiológico e mesmo com o manejo de pré-natal adequado, as perdas gestacionais podem ocorrer. A Organização Mundial da Saúde estima uma ocorrência de 4,9 milhões de mortes perinatais no mundo todos os anos. No Brasil, a incidência em 2020, mostrou um total de 28.993 casos, sendo 1.062 no estado do Paraná e, destes, 117 na cidade de Curitiba. Não obstante, para além dos dados, tem-se o impacto emocional para todos os envolvidos, inquestionáveis para a mulher e família que sofre com a perda gestacional, mas também para os profissionais de saúde, em especial os enfermeiros, que se deparam com estas situações ao longo da vida profissional. Desta forma, este estudo teve como questão de pesquisa: qual a percepção do enfermeiro sobre o cuidado oferecido à mulher que sofreu perda gestacional? Ainda, como objetivo geral, compreender a percepção do enfermeiro que presta cuidados às mulheres diante das perdas gestacionais; e objetivos específicos: identificar elementos do cuidado do enfermeiro frente à mulher que sofreu perda gestacional e descrever a experiência dos enfermeiros sobre seu cuidado diante da mulher que diante perda gestacional. Trata-se de estudo qualitativo exploratório, realizado em um hospital universitário da Região Sul do país. A coleta de dados deu-se através de entrevistas semiestruturadas, as quais foram audiogravadas e transcritas na íntegra, durante os meses de abril a junho de 2022. Contou com a participação de 11 enfermeiros que atuam diretamente com mulheres em situação de perda gestacional. Para análise dos dados, foram seguidos os passos da Análise de Conteúdo do tipo temática proposta por Bardin e apoiada pelo uso do software de análise qualitativa o webQDA. Resultados: foram levantados 13 temas, os quais, por afinidade e exclusão, resultaram na elaboração de 3 categorias: Fragilidades do processo de cuidar frente às perdas gestacionais; Potencialidades do processo de cuidar frente às perdas gestacionais; e Cuidados de enfermagem frente às perdas gestacionais. Foi possível evidenciar que os enfermeiros conseguiam reconhecer as lacunas assistenciais e, mesmo com suas dificuldades, prestavam um cuidado respeitoso e empático, além de proporcionar momento de criação de memórias para as famílias enlutadas, através de fotos ou guarda de pertences que foram do bebê, que passou tão brevemente pela vida. O estudo permitiu compreender que os enfermeiros entrevistados reconheciam a importância da sua presença como um profissional que oferece um cuidado direcionado a cada mulher/família em situação de perda gestacional nas suas necessidades, enfatizaram a comunicação verbal e não verbal, sendo uma de suas preocupações que esta compreendesse, para além da assistência oferecida, este olhar singular do profissional. Os impactos do desenvolvimento deste trabalho podem resultar na elaboração de documentos, materiais educativos e protocolos assistenciais voltados aos profissionais de saúde que prestam cuidados diante das perdas gestacionais dentro das instituições hospitalares.


Abstract: Pregnancy is a physiological phenomenon. However, even with proper prenatal care management, pregnancy losses may occur. The World Health Organization estimates 4.9 million perinatal deaths worldwide every year. In Brazil, showed an incidence of 28,993 cases in 2020, with 1,062 cases in Parana State and, among these, 117 in its capital city, Curitiba. Therefore, that is a global health problem, but there still have been scarce public policies addressing this theme. Apart from the data, there is also the emotional impact on all the involved individuals, unquestionably to the woman and family who suffer the pregnancy loss, but also to the healthcare professionals, ultimately nurses, who are confronted with those situations along their professional lives. Thus, the research question in this study was: what is the nurses' perception on the health care delivered to the woman facing a pregnancy loss? In addition, the general objective was to understand the nurse's perception who renders health care to the woman who suffered a pregnancy loss. It is an exploratory qualitative study held at a university hospital in Southern Brazil. Data collection was conducted by means of semi-structured interviews, which were audio recorded and fully transcribed between April and June 2022. The participants were 11 nurses who care for women in a situation of pregnancy loss. For the data analysis, Thematic Content Analysis proposed by Bardin was applied, supported by the use of the webQDAE software for qualitative analysis. Results: 13 themes by affinity and exclusion stood out by means of the analysis, which resulted in the elaboration of 3 categories: Fragilities in the caring process in the face of pregnancy losses; Potentialities of the caring process in the face of pregnancy losses; and Nursing Care in the face of pregnancy losses. It was possible to evidence that nurses could recognize caring gaps, and even facing difficulties, they could deliver respectful and empathetic care, in addition to providing a moment of creating memories to bereaved families through photos or belongings of the baby who briefly got through their lives. The study enabled to understand that the interviewed nurses acknowledged the importance of their presence as professionals who delivered care to meet the needs of each woman/family going through a pregnancy loss. They also pointed out verbal and non-verbal communication as one of their concerns so that the women could understand their unique professional look, beyond care delivery. The study outcomes may result in the elaboration of documents, educational materials and care protocols to those health professionals who are confronted with pregnancy losses while rendering care within hospital institutions.

ABSTRACT

Idealmente, a contracepção deve ser iniciada o mais precocemente possível após o parto ou após o abortamento, permitindo que as mulheres sejam protegidas contra uma gravidez não programada subsequente. O objetivo desta revisão é discutir a contracepção no pós-parto e pós-aborto, por meio da análise de ensaios clínicos e metanálises, além das principais diretrizes internacionais, com ênfase nas indicações e contraindicações, tempo de início do uso dos métodos contraceptivos e possíveis complicações. Nesta revisão não sistemática, são discutidos os principais métodos contraceptivos: dispositivos intrauterinos, métodos somente de progestagênios, métodos hormonais combinados, métodos de barreira, método de amenorreia lactacional e esterilização. O aconselhamento contraceptivo pós-parto deve começar durante o pré-natal e, em pacientes após abortamento, durante a internação hospitalar. Todas as mulheres devem ter acesso a informações claras sobre cada método contraceptivo, e o processo de tomada de decisão deve ser compartilhado com o médico assistente. Idealmente, métodos de contracepção reversíveis de longa duração devem ser priorizados em relação aos outros. Em conjunto, todas as evidências demonstram que o melhor método para cada paciente é aquele que combine altas taxas de segurança com o desejo da paciente de iniciá-lo e mantê-lo pelo tempo que desejar.


Contraception should ideally be started as early as possible after childbirth or abortion to allow women to be protected against a subsequent unplanned pregnancy. The aim of this review is to discuss postpartum and postabortion contraception, through the analysis of clinical trials and meta-analyses, in addition to the main international guidelines, with emphasis on indications and contraindications, time to start contraceptive method and possible complications. In this review, the main contraceptive methods are discussed: intrauterine devices, progestin-only methods, combined hormonal methods, barrier methods, lactational amenorrhea method and sterilization. Postpartum contraceptive counseling should start during prenatal care and during hospital stay in post-abortion patients. All women should have access to clear information about each contraceptive method and the decision-making process must be shared. Ideally, long acting reversible contraception methods should be prioritized over others. Taken together, all the evidence shows that the best method for each patient is the one that combines high safety rates with the patient's desire to start and maintain it for as long as she wants.

ABSTRACT

Background: Post-placental and intra-cesarean insertion of intrauterine contraceptive devices (PPIUCD) is a lucrative family planning method which provides safe, effective reversible, inexpensive, non-hormonal, and long-acting contraception. Aims and Objectives: The objective of the study is to evaluate the postpartum insertion of intrauterine contraceptive device in terms of awareness, acceptance, safety, efficacy, continuation, and removal rate and its complications in both vaginal as well as intra-cesarean section (CS) insertion. Materials and Methods: Prospective observational study conducted in Obstetrics and Gynaecology Department GMERS sola for 6 months. Two thousand and ninety-four pregnant women from labor room who delivered vaginally or by CS during study period were asked regarding their knowledge and acceptability for PPIUCD. Results: Total 2094 women evaluated for awareness and acceptance for PPIUCD. About 68.66% were familiar with the concept with 29.99% giving verbal consent for its insertion. About 60.01% women refused for various reasons. Conclusion: This study shows that PPIUCD insertion is a safe and convenient method which requires more population awareness, counseling, and usage.

OBJECTIVES@#To study the left heart structure and functional characteristics of term neonates with intrauterine growth restriction (IUGR).@*METHODS@#This study included 86 term neonates with IUGR admitted to the Neonatal Ward of Beijing Friendship Hospital, Capital Medical University from January 2019 to January 2022 as the IUGR group, as well as randomly selected 86 term neonates without IUGR born during the same period as the non-IUGR group. The clinical data and echocardiographic data were compared between the two groups.@*RESULTS@#The analysis of left heart structure and function showed that compared with the non-IUGR group, the IUGR group had significantly lower left ventricular mass, left ventricular end-diastolic diameter, left ventricular end-systolic diameter, left atrial diameter, end-diastolic interventricular septal thickness, left ventricular posterior wall thickness, left ventricular end-diastolic volume, left ventricular end-systolic volume, and stroke volume (P<0.05) and significantly higher ratio of end-diastolic interventricular septal thickness to left ventricular posterior wall thickness, proportion of neonates with a mitral peak E/A ratio of ≥1, and cardiac index (P<0.05). The Spearman correlation analysis suggested that stroke volume was positively correlated with birth weight and body surface area (rs=0.241 and 0.241 respectively; P<0.05) and that the ratio of end-diastolic interventricular septal thickness to left ventricular posterior wall thickness was negatively correlated with birth weight and body surface area (rs=-0.229 and -0.225 respectively; P<0.05).@*CONCLUSIONS@#The left ventricular systolic function of neonates with IUGR is not significantly different from that of neonates without IUGR. However, the ventricular septum is thicker in neonates with IUGR. This change is negatively correlated with birth weight and body surface area. The left ventricular diastolic function may be impaired in neonates with IUGR.

@#Antibody cross-reactivity among flaviviruses is a major limitation in understanding the prevalence without vector control measures. In this study, we investigated the presence of Zika virus (ZIKV)-specific antibodies and the significance of their cross-reactivity with other flaviviruses, which could affect the serological specificity in both symptomatic and asymptomatic pregnant women. Among the results obtained from 217 serum samples tested for ZIKV-specific IgM and IgG, no specific predictions regarding seropositivity or exposure due to extensive cross-reactivity with dengue virus (DENV) serology could be made. Clear-cut positivity was observed in 1.8% (n = 4) and 1.0% (n = 2) for ZIKV IgM and IgG, respectively. The same samples assessed for DENV showed 1.3% (n = 3) seropositivity each for IgM and IgG levels. None of the samples were positive for ZIKV and DENV IgM or IgG. However, one sample (0.4%) tested positive for ZIKV and DENV IgM. No significant correlation was observed between DENV IgM and IgG when comparing the overlapped serotiters. On the other hand, the ZIKV IgG-positive sample showed higher serotiters for DENV IgG, indicating cross-reactivity with ZIKV but without statistical significance. Therefore, screening for the incidence of ZIKV becomes particularly challenging in a population where the presence or pre-exposure to DENV is observed. Our observations further suggest that unless flavivirus prevalence is properly addressed, determining the prevalence of ZIKV antibodies, which may be confounded with other uninvestigated flaviviruses, will be complicated.
