ABSTRACT

Availability of adequate and proper drug information helps in rational prescription of essential drugs. This can be obtained from various sources such as National Formularies; other sources such as Current Index of Medical Specialties, Monthly index of medical specialties and the information available with the regulators and drug package inserts (PI). In this study, we assessed the drug information of the drugs used for treatment of both type of lepra reactions. Five drugs used for treating Lepra reactions were analyzed for any variation (Qualitative) in information on various parameters as mentioned in commonly used drug information sources such as CIMS India, MIMS India, Central Drugs and Standards Control Organization (CDSCO) website and National Formulary of India (NFI). We observed some gross qualitative variation regarding drug information given in different commonly used sources. Variation exists in the quality of information available on indications, contraindications, dosage and completeness of the dosing schedule about drugs available in various sources. As management of Lepra reactions is crucial in achievement desirable outcome of treatment of leprosy, it is important that information regarding drugs used for such indications should be 100% uniform in all commonly used and available sources.

ABSTRACT

Objective:To investigate and analyze the rational use of anti-tumor immune enhancement drugs in our hospital. Meth-ods:The annual use data of anti-tumor immune enhancement drugs in our hospital in 2015 were collected, and then statistically ana-lyzed and evaluated combing with the medical history information. Results: There were 12 varieties and 19 specifications of adjuvant anti-tumor immune enhancement drugs in our hospital, which accounted for 15. 87% of the total sum. Totally 25 481 patients used ad-juvant anti-tumor immune enhancement drugs, which occupied 29. 84% of the total medicine afford. The adjuvant anti-tumor immune enhancement drugs showed lots of irrational use in clinics, and the main irrationity was overuse and off-label use. Conclusion: It is necessary to strengthen prescription evaluation for anti-tumor immune enhancement drugs, which needs controlled and rational use.

ABSTRACT

Objective: To investigate the effect of irrational use of traditional Chinese patent medicines (CPM) on medication safety.Methods: The adverse drug reaction (ADR) reports on traditional CPM (including traditional Chinese medicine for injection) in our hospital during January 2013 to October 2016 were collected and analyzed retrospectively.After finding the corresponding prescriptions, the situation of irrational drug use in the reports including indications, usage and dosage, combination of traditional CPM, combination of Chinese and western medicines and course were analyzed, and the rate of irrational drug use was calculated.Results: A total of 217 reports were collected including 217 patients.The primary diseases ranked top three were cardio-cerebral vascular system diseases, digestive system diseases and respiratory system diseases.The cases involved 81 kinds of drugs, and among them, 37 cases were induced by Shenmai injection.The rate of irrational drug use associated with indications, usage and dosage, combination of traditional Chinese patent medicines, combination of Chinese and western medicines and course was 6.45% , 5.99% , 2.76% , 0.92% and 1.84% , respectively, and the total rate was 17.97% (39/217).Conclusion: The irrational drug use maybe one of the important factors related to ADR of CPM.According to syndrome differentiation, CPM should be used rationally, and pharmaceutical care should be strengthened to reduce the incidence of ADR.

ABSTRACT

OBJECTIVE:To provide reference for rational use of essential medicines of TCM preparations.METHODS:The utilization of essential medicines of TCM preparations was analyzed statistically in 21 hospitals of Yancheng area in Jiangsu province during Oct.2015-Jun.2016.RESULTS:The utilization rate of essential medicines of TCM preparations was 66.82% in first-level hospital,and 34.64% in second-level hospitals and 12.50% in third-level hospitals.Top 5 essential medicines of TCM preparations in the list of consumptions sum were all TCM injection,which were mainly used for promoting blood circulation to dissipate blood stasis,invigorating qi,clearing away heat and toxic substances.Irrational use of essential medicines of TCM preparations included unsuitable usage and dosage,unsuitable solvent,drug use without indications.CONCLUSIONS:The utilization of essential medicines of TCM preparations are in good condition in 21 hospitals of Yancheng,but it is necessary to pay more attention to irrational use of them so as to promote safe and rational drug use.

ABSTRACT

Objective To analyze the related factors of ADR/ADE cases caused by Sanqi Panax notoginseng for injection(lyophilization),in order to provide reference for clinical rational use of Sanqi Panax notoginseng for injection.Methods Adopted the method of retrospective study,this paper statistically analyzed ADR/ADE cases caused by Sanqi Panax notoginseng for injection(lyophilization)in the database of Adverse Drug Reaction Monitoring Center in Qingdao from January 2015 to December 2015.Results There were altogether 123 ADR/ADE cases caused by Sanqi Panax notoginseng for injection(lyophilization)in 2015,with 90 cases in general,12 new cases, 21 severe cases;more female patients than male patients;more middle -aged and old patients;irrational use of the drug accounting for 45.53% of the total number of the report;and there was a higher proportion of irrational drug use in grass -roots medical institutions.Conclusion Irrational drug use is an important factor leading to ADR/ADE cases caused by Sanqi Panax notoginseng for injection(lyophilization),so the injection of Sanqi Panax notoginseng (lyophilization)should be regulated in order to reduce the occurrence of ADR/ADE cases.

ABSTRACT

OBJECTIVE:To provide the clinical departments with a reference for promoting rational drug use. METHODS:The inpatient prescriptions given by orthopaedics and burn departments from Jun. to Dec. 2013 were selected randomly. The number of the selected prescriptions by each department accounted for 2% of the total prescriptions given by the corresponding department in that month,including 4 921 cases by orthopaedics department and 1 391 cases by burn department. Statistical analysis of irrational prescriptions was made according to Chinese Pharmacopoeia,New Materia Medica,the package insert, and other relevant references. RESULTS:1 821 prescriptions given by orthopaedics department and 378 by burn department were found to be irrational,accounting for 37.00% and 27.17% respectively. The reasons of irrationality mainly included im-proper compatibility,improper route of administration,contraindication in and use with caution by the elderly and children,re-peated drug use,improper drug combination etc. CONCLUSIONS:The system of prescription review should be strengthened, and clinical staff training system and pharmaceatical knowledge information platform are established to promote rational use of drugs.

ABSTRACT

OBJECTIVE:To provide a reference for reducing adverse events due to off-label use of drugs and promoting ratio-nal use of intravenous drugs in Renji Hospital Affiliated to Shanghai Jiaotong University School of Medicine. METHODS:The pre-scriptions of intravenous drugs in 42 wards of our hospital from Jul. 2013 to Jun. 2014 were reviewed,and then off-label use of drugs were analyzed,where statistics was made mainly to drug overdose,the use of drugs at the concentration beyond the standards (including concentrations too high and too low) and associated conditions. RESULTS:There were 622 irrational prescriptions,in-cluding 60 concerning off-label use of drugs,accounting for 9.6% of irrational prescriptions,in which 40 were involved with the concentration which was too high,accounting for the highest percentage(66.7% of prescriptions related to off-label use of drugs), 17 were related to drug overdose(28.3% thereof),and 3 were in relation to the concentration which was too low(5.0% thereof). CONCLUSIONS:Pharmacy intravenous admixture service may,through the communication between the pharmacist and the doc-tor,refuse to make up the above-mentioned prescription which was found to be irrational(including that involving in off-label use of drugs)or make up such prescription after it is modified according to the requirements as specified in the label to reduce the occur-rence of off-label use of drugs so as to promote rational drug use.

ABSTRACT

Objective:To analyze the unreasonable medical orders at the pharmacy intravenous admixture service ( PIVAS) of our hospital to improve clinical medication. Methods: The medical orders from January to October in 2013 were collected and reviewed, the unreasonable medical orders were analyzed and the results were informed to the clinics. Results:Totally 323 unreasonable medical orders were found, which accounted for 0. 03%. The unreasonable aspects included unreasonable compatibility, input error, irregular medical orders, over dosage or concentration, unsuitability between dosage and age and the disproportion between dosage and diagno-sis. Conclusion:The irrationality in intravenous medical orders still exists in clinics, which shows drug use risks. The unreasonable medical orders can be avoided or reduced through the review of medical orders by pharmacists at PIVAS to improve the safety of clinical drug use.

ABSTRACT

Objective To understand the utilization of antibiotics for acute upper respiratory infection through retrospectively analyzing the prescription in the outpatient department in a whole year,therefore to improve the rational drug using in primary healthy institutions.Methods Prescriptions from the outpatient department,Jan.2010 to Dec.2010 were managed in Excel software and the utilization was analyzed.Results In a total of 6101 prescriptions,2462(40.35% ) were for upper respiratory infection.Of the 2462 prescriptions,1962(79.69% )used antibiotics.The amount of prescriptions and the rate of antibiotics used were significantly higher than those for other infection diseases.Conclusion The investigation shows antibiotics abuse in primary healthy institution has become a severe problem.While the stuff in primary healthy institution are trained for some specific aims,the promotion should be spread to let doctors and patients know the danger of antibiotics abuse.

ABSTRACT

OBJECTIVE:To improve the level of rational use of drugs in hospitals.METHODS:Information on the application of the monitoring system for rational administration of drugs in our hospital was analyzed.RESULTS:Manifestations of irrational drug use in our hospital were mainly manifested as incompatibility of parenteral injection,drug interactions,the repetitive use of drugs,drug dosage exceeding the limit dose,medication warnings for specific group of people.CONCLUSION:Incidences for drug-induced diseases in our hospital have been reduced while the general rational drug-use level in our hospital has been improved through the monitoring system of rational use of drugs.

ABSTRACT

OBJECTIVE:To analyze the manifestations of hospital irrational drug use.METHODS:By retrieving CNKI full - text data base from 1998 to 2008 with"hospital"and"irrational drug use"as keywords,a total of 23 research papers related to irrational drug use were obtained and which were summarized analytically.RESULTS:The irrational drug use in our hospital manifested predominantly as irrational usage and dosage and unreasonable compatibility of drugs followed by irrational selection of drugs,repeated drug use,non-medical error,and abuse of antibiotics or hormones.CONCLUSION:In view of the above manifestations,it is urgent to take full consideration of the hospital pharmaceutical administration system in China and take multi - sided scientific intervention countermeasures so as to effectively improve the level of rational drug use in our hospital.

ABSTRACT

OBJECTIVE:To improve the quality of outpatient prescriptions and promote clinical rational drug use. METHODS:3 600 outpatient prescriptions prescribed in our hospital in 2007 were randomly sampled for statistical analysis of the problems manifested in the prescriptions. RESULTS:Of the 3 600 outpatient prescriptions reviewed,3 314(92.06%) were nonstandard; the errors totaled 10 014 cases/ times,of which,4 102 were foreword errors and 5 719 were errors in normal text. The irrational use totaled 193 cases/ times. CONCLUSION:Clinician and pharmacists should attach importance to the learning of pharmaceutical and medical knowledge so as to improve prescription quality.
