ABSTRACT

Polycystic Ovarian Disease (PCOD) is an endocrine disorder that causes metabolic changes in women of reproductive age. PCOD has evolved into a lifestyle disorder as a result of sedentary habits, fast food consumption, and a poor lifestyle. The precise cause of PCOD is mysterious, but high levels of insulin, hyper androgen, and LH are the main causes. PCOD symptoms include irregular menstruation, oligomenorrhea, acne, hirsutism, hair loss, obesity, and constipation. PCOD is not directly stated in the Samhita, but clinically it is similar to Aartavavaha strotas dushti, Nastaartava, Granthi, Santarponnth vyadhi, and Yonivyapad. In this present case study, a 22 year old female patient came with symptoms of irregular, delayed menstruation, scanty menses, acne on the face and hair fall. The USG report reveals polycystic patterns of both ovaries with Right ovary volume 11.6cc and Left ovary volume 11cc, both ovaries are bulky in size with increased stromal echogenicity and multiple (10-12) small follicles (2-5mm) arranged in peripheral distribution. She had taken the medication so many times but she had not been completely cured. As a result she came to our hospital for Ayurvedic treatment. Result was made on the basis of Clinical symptoms relief and USG report. So in this case study, we will look at an Ayurvedic approach in the management of Polycystic Ovarian Disease.

ABSTRACT

OBJECTIVE:To evaluate the safety of Tripterysium glycosides tablet systematically,and to provide evidence-based reference.METHODS:Retrieved from Chinese Journal Full-text Database,China Science and Technology Journal Database,Wanfang database,PubMed,Embase,Medline (Ovid) and The Cochrane Library,randomized controlled trials (RCTs) about added or combined with Tripterysium glycosides tablet (trial group) versus basic treatment or combined with other drug (control group) in the treatment of various disease-induced ADR (the incidence of leukopenia,gastrointestinal reaction,irregular menstruation,lover dysfunction) were collected.After data extraction and quality evaluation with Cochrane 5.1.0 quality evaluation tool,RevMan 5.0 statistical software was used to conduct Meta-analysis.RESULTS:A total of 36 RCTs were included,involving 2 814 patients.Results of Meta-analysis showed that the incidence of leucopenia [OR=3.42,95 % CI (1.88,6.22),P< 0.001],irregular menstruation [OR=8.42,95%CI(4.93,14.38),P<0.001] and abnormal liver function [OR=2.99,95%CI(2.01,4.46),P<0.001] in trial group were significantly higher than control group,with statistical significance.Results of subgroup analysis showed that the incidence of irregular menstruation in trial group was significantly higher than control group,with statistical significance [OR=5.39,95% CI (2.50,11.59),P<0.001].When combining with glucocorticoids,the incidence of gastrointestinal adverse reactions in trial group was significantly lower than control group,with statistical significance [OR=0.40,95%CI(0.19,0.84),P=0.02].When the indications were the diseases of immune system,the incidence of leucopenia [OR=3.42,95% CI (1.49,7.82),P=0.004] and abnormal liver function [OR=2.27,95% CI (1.08,4.76),P=0.03] in trial group were significantly higher than control group,with statistical significance.CONCLUSIONS:ADR induced by Tripterysium glycosides tablet mainly include leucopenia,gastrointestinal reaction,irregular menstruation and abnormal liver function.More attentions should be paid on irregular menstruation when combining with other drugs.More attentions should be paid on leucopenia and abnormal liver function when treating the diseases of immune system.

ABSTRACT

Huanshao capsule is widely used in irregular menstruation and has achieved a good effect. Huanshao capsule can promote gonad development in mice, significantly improve the ovarian index in mice, increase estrogen level and reduce FSH level in rats, inhibit the pain response induced by oxytocin and estrogen, inhibit writhing reaction induced by acetic acid pain in mice. Due to the complexity of traditional Chinese medical formula, the pharmacological mechanism of the treatment on the irregular menstruation of the Huanshao capsule is unclear. In this study, the internet-based computation platform (www.tcmip.cn)was used to explore the molecular mechanism of Huanshao capsule on the menstrual. The aim of this study was to find the molecular mechanism of Huanshao capsule in treating menstrual. In the study of the molecular mechanism of Huanshao capsule in the treatment of menstrual by using the internet-based computation platform, Huanshao capsule maybe treat the menstrual by the pathway of endocrine system, GnRH signal transduction pathway, estrogen signal transduction pathway, oxytocin signaling pathway, thyroid hormone signaling pathway, VEGF signaling pathway, FCεRI signaling pathway and purine metabolism and nucleotide metabolism. The early pharmacological study confirmed Huanshao capsule could increase the serum estradiol level and decrease follicle stimulating hormone level and the traditional Chinese medicine pharmacology coincide with the prediction result of internet-based computation platform which roles as the pathway of GnRH signaling pathway and estrogen signal transduction pathway. Other pathway needs further experimental verification.

ABSTRACT

To study and compare the composition regularities of Chinese materia medica (CMM) formulas for four kinds of emmeniopathies, including irregular menstruation, dysmenorrhea, amenorrhea, and uterine bleeding, and contribute to the interpretation and prescription of composition mechanism for the optimization of CMM formulas by using a data-driven approach. A total of 1 761 CMM formulas from the Dictionary of Chinese Medicine Prescription for emmeniopathies were analyzed by the data mining method of Apriori algorithm with the indicators of support, confidence, and lift. The frequencies, composition regularities, and pivotal compositions of CMM were analyzed comparatively. Angelicae Sinensis Radix (ASR), Chuanxiong Rhizoma (CR), CR-ASR, Paeoniae Radix Alba (PRA)-ASR, and PRA-CR-ASR were higher commonly used CMM and CMM combinations for all the four emmeniopathies. Astragali Radix was the highest frequently used CMM for uterine bleeding. Persicae Semen (PS), Rhei Radix et Rhizoma, and PS-ASR were the frequently used CMM and CMM combinations for amenorrhea. Cyperi Rhizoma and Cyperi Rhizoma-ASR were the frequently used CMM and CMM combinations for dysmenorrhea. The latent association rules with significant lift included Hirudo→Tabanus and Olibanum→Myrrha for irregular menstruation, and Sappan Lignum→Carthami Flos and Tabanus→Hirudo for amenorrhea. Based on the CMM formulas from the Dictionary of Chinese Medicine Prescription, the data-driven approach revealed the similarities and differences in CMM compositions for the four emmeniopathies and uncovered the latent composition regularities and the pivotal CMM effectively.

[Objective] To investigate the theory of"Treat Pre-disease"about intermenstrual bleeding from TCM constitution. [Methods] Intermenstrual bleeding is a disease of the gynecology, and it is more likely to relapse. Constitution theory of TCM holds that people's physique has the character of separable, adjustable and physique relating to disease. [Results] The interphase bleeding in patients with often clinical performance for yin deficiency constitution,qi deficiency constitution, damp heat constitution, blood-stasis constitution. [Conclusion] We can use the "Treat Pre-disease" thought to correct the physical deviation of the patients with intermenstrual bleeding, preventing the disease and preventing its recurrence after cure.

ABSTRACT

[Objective] The article analyzed the application of 'Disease Prevention'theory in the hypomenorrhea and provided treatment way for posterity doctor treating the disease. [Method] In this chapter it elaborated the pathogenesis of hypomenorrhea about traditional Chinese medicine and modern medicine. At the same time, it elaborated hypomenorrhea's prevention and treatment from the following aspects:prevention of disease which did not appear, prevention of disease which appeared from worsening,and prevention of disease which was cured from reoccurring to explore the treatment effect and advantage.[Result] The hypomenorrhea was harmful and its incidence rate to a trend of annual increase in recent years. To put'Disease Prevention'theory in the whole diagnosis process had a significant clinical curative effect. [Conclusion] In this paper, starting from the'Disease Prevention'theory treatment of the disease has some practical value, which is worthy of popularization and application.

ABSTRACT

Objective] Research the treatment characteristics of premenstrual abdominal pain and postmenstrual abdominal pain in Fu Qingzhu’s Obstetrics and Gynecology and the clinical medical records.[Method] By reading the relevant documents and medical records to discuss the medication characteristics of premenstrual abdominal pain and postmenstrual abdominal pain in Fu Qingzhu’s Obstetrics and Gynecology along with the medication experience of dysmenorrhea from the traditional Chinese medicine expert Guo Zhenqing. [Results] Fu Qingzhu treated dysmenorrhea by distinguishing between the anterior and posterior with different treatments such as Xuanyu Tongjing decoction and the decoction for regulating liver. Focus on liver qi stagnation and use of white peony root, angelica nourishing blood and liver to relieve pain .Old Guo paid attention to treatment based on syndrome differentiation and modified acupoint prescriptions based on Xuanyu Tongjing Decoction. She treated dysmenorrhea with two prescriptions, two periods of medication and hit the pathogenesis. Achieve satisfactory results in the treatment of this disease. [Conclusion] The clinical dysmenorrhea syndrome is complex, Xuanyu Tongjing Decoction and the decoction for regulating liver should not be confined to the treatment before and after abdominal pain, must have detailed exploration of its thought and use the original prescription flexibly, which can be effective.

ABSTRACT

OBJECTIVE:To investigate the clinical efficacy and safety of Shugan granule in the adjunctive treatment of melas-ma with irregular menstruation. METHODS:140 cases of melasma with irregular menstruation were divided into observation group and control group according to random number table method,with 70 cases in each group. Control group was given vitamin C,vi-tamin E and compound retinoic acid and other routine medicines;observation group was additionally given Shugan granule 10 g, bid. A treatment course lasted for 21 d,and both groups received 2 courses of treatment. The area of melasma,irregular menstrua-tion integral,clinical efficacy of melasma and irregular menstruation were observed in 2 groups,and the occurrence of ADR was re-corded. RESULTS:There was no statistical significance in the area of melasma and irregular menstruation integral between 2 groups before treatment (P>0.05);after treatment,the area of melasma decreased significantly in 2 groups,and the observation group was significantly lower than the control group;irregular menstruation integral decreased significantly,and the observation group was significantly lower than the control group,with statistical significance(P0.05). CONCLUSIONS:Shugan granule shows significant therapeutic efficacy in adjunctive treatment of melasma with irregu-lar menstruation,and can decrease the area of melasma and improve irregular menstruation significantly.

ABSTRACT

Polycystic ovary syndrome (PCOS) is a condition noted in many patients who are seen for menstrual irregularity or infertility in routine practice. The current author previously found that women with PCOS who underwent a unique form of acupuncture, with 1 session taking place each week, had a response rate of about 60%. Subjects of the current study were 3 patients who lacked ovarian follicular development despite 1 acupuncture session per week. In the current study, these patients underwent 2 sessions per week, and the effectiveness of this treatment was examined. When acupuncture was not performed, none of the 3 patients exhibited ovarian follicular development. Two acupuncture sessions per week resulted in ovarian follicular development in 86.7% (13/15 cycles) of all menstrual cycles during which acupuncture was performed. Performing an average of 4-5 acupuncture sessions every 4 days or so was found to result in a mature ovarian follicle on an average of 18.7 days into the menstrual cycle. In this study, acupuncture involved slight insertion of the needles in a pattern. Changing the frequency of sessions from 1 per week to 2 per week resulted in ovarian follicular development within 3 weeks in 3 patients with PCOS, as reported here.

It has been accepted that food customs are closely associated with the quality of life in both men and women’s reproductive life. Food customs are speculated to not only influence the present lifestyle but also to induce gynaecological disorders such as dysmenorrhoea, spermatogenesis and irregular menstruation. though there is no consistent definition of regular or normal menstruation, epidemiologic evaluation of menstrual cycle has been becoming an important issue. In addition, latent development of organic diseases such as endometriosis, which are accompanied by dysmenorrhoea, is a concern under the current nutritional environment. Thus, it is an important issue to evaluate the present situation of eating habits in couples and estimate the influence of these habits on the quality of reproductive functions. A multi-faceted therapeutic approach to improving fertility involves identifying harmful environmental and occupational risk factors, while correcting underlying nutritional imbalances to encourage optimal reproduction and its function.

ABSTRACT

Polycystic ovary syndrome (PCOS) is a condition found in many patients who are seen for menstrual irregularity or irregular vaginal bleeding in routine practice.<br>Subjects were 21 women who were seen on an outpatient basis by this medical center. These women were diagnosed with PCOS and underwent acupuncture. In principle, 6 to 10 sessions of acupuncture were performed, with 1 session taking place each week. After acupuncture concluded, women who did not exhibit ovarian follicular development or ovulation according to ultrasonography and measurements of estradiol and progesterone in blood prior to day 30 of their menstrual cycle were given an oral contraceptive, resulting in withdrawal bleeding. These women then underwent another 4 sessions of acupuncture. Women who did exhibit ovarian follicular development or ovulation prior to day 30 of their menstrual cycle after 10 sessions of acupuncture were deemed to be non-responders. Women who had mature ovarian follicles or ovulated before that time were deemed to be responders.<br>In contrast to women who did not exhibit follicle maturation or ovulation prior to day 30 of their menstrual cycle, 57.14% of women had mature ovarian follicles prior to the 10 sessions of acupuncture. For responders, the average length of time after the onset of menses until ovarian follicle maturation was noted as 20.58 days. An average of 7.92 acupuncture sessions was needed until ovarian follicle maturity or ovulation was noted.<br>The current study found acupuncture to be effective in treating PCOS.

ABSTRACT

OBJECTIVES: This study was aimed to examine the prevalence of neuroleptic-induced irregular menstruation and its related factors as well as the possible relationship of irregular menstruation with psychopathology and extrapyramidal symptoms in schizophrenic patients. METHODS: Thirty five schizophrenic inpatients at Naju National Mental Hospital who satisfied the diagnostic criteria of DSM-IV were selected. They had drug-free periods for more than 1 month before admission and had been taking neuroleptics for more than 3 months after admission. From all patients, reliable information regarding the sociodemographic characteristics, the menstrual histories before and after admission and clinical characteristics, including previous and current medications were investigated. The following were also evaluated: Serum prolactin level(by radioimmunoassay), psychopathology(by positive and negative syndrome scale: PANSS), and extrapyramidal symptoms(by extrapyramidal symptom rating scale: ESRS). RESULTS: Twelve patients(34%) showed regular menstruation and 23 patients(66%) were suffering from irregular menstruation. Of those with irregular menstruation, 13 patients(37%) had oligomenorrhea and 10 patients(29%) had amenorrhea. There were no differences in sociodemographic characteristics and menstrual histories before admission between the regular and irregular menstruation groups. But, the significant differences between the two were found in the following variables: The irregular menstruation group had shorter durations of illness, higher daily doses of current neuroleptics, higher serum prolactin levels, higher scores on the general psychopathology scale of PANSS, and higher scores on the parkinsonian symptom scale, hypokinetic symptom factor and clinical global impression of ESRS. CONCLUSION: The high prevalence of the irregular menstruation in schizophrenic patients reflects a state of hyperprolactinemia due to the interference of dopamine activity by high doses of neuroleptics.
