ABSTRACT

Huashan Hospital became the first Academic Medical Center Hospital accredited by Joint Commission International (JCI) in 2013. The Department of Clinical Nutrition has constantly improved internship education through the introduction of JCI standards. Based on the flexible combination of teaching and practicing modules, clinical nutrition knowledge is closely integrated with professional dietitian skills, and in-depth practical training helps students to acquire much more experience of the occupation as a dietitian. Teachers think highly of students' initiative and knowledge conversion ability, and in the recent five years, undergraduate projects have been enhanced in both quantity and quality.

ABSTRACT

Objective To explore the influence of perioperative nursing for interventional open operation of patients with symptomatic internal carotid artery occlusion under the guidance of the Joint Commission International(JCI) concept. Methods The clinical data of 90 patients with symptomatic internal carotid artery occlusion treated by elective interventional open operation from January 2016 to February 2018 were retrospectively analyzed, of which 40 patients receiving routine nursing from January 2016 to January 2017 were set as control group, of which 50 patients receiving nursing guided by JCI concept from February 2017 to February 2018 were set as observation group. The Hamilton Depression Scale (HAMD) was used to evaluate the anxiety of patients at preoperative 1 h and postoperative 48 h, and the extubation time and hospitalization time were compared, as well as the incidence of complications and the patient's coordination degree. Results The HAMD scores of preoperative 1 h and postoperative 48 h of the observation group respectively was (26.58 ± 5.79) and (17.41 ± 4.18) points, of which the control group respectively was (29.75 ± 5.21) points and (22.26 ± 4.64) points, and the HAMD scores between the postoperative 48 h and preoperative 1 h in the 2 groups were statistically significant differences (t=9.080, 6.790, all P < 0.01), of which between the 2 groups were statistically significant differences (t=2.697, 5.208, all P < 0.01). The extubation time and hospitalization time of the observation group respectively was (3.81 ± 0.62) and (12.97 ± 3.65) d, of which the control group respectively was (4.39 ± 0.71) and (16.18±3.54) d, with statistically significant differences (t=4.134, 4.201, all P<0.01). The total incidence rate of complications of the observation group was 18.00%(9/50), of which the control group was 40.00%(16/40), with statistically significant differences (χ2=5.361, P<0.05). The good rate of coordination of the observation group was 76.00%(38/50), of which the control group was 55.00%(22/40), with statistically significant differences (χ2=4.410, P < 0.05). Conclusions The perioperative nursing based on the JCI concept guidance can relieve the anxiety of patients with symptomatic internal carotid artery occlusion treated by elective interventional open operation, shorten the postoperative recovery time, reduce the incidence of complications and improve the nursing coordination.

ABSTRACT

This study described the nursing reform at Ningbo No.2 hospital under the guidance of JCI standards.According to the flat network structure, the quality and safety of nursing management was divided into 18 nursing management committees, each assigned to be responsible for a JCI section with specified performance-based distribution reform.Such indexes as nursing sensitivity were also analyzed. Given the workload pushed up by JCI standards,the performance-based reform rationalized the workload of nursing staff in its performance distribution reform, thus conducive to the implementation of the reform, higher nursing quality of care and less turnover rate.

ABSTRACT

Objective To explore the role of Joint Commission International(JCI) standards on the improvement of pharmaceutical care in pharmacy intravenous admixture service (PIVAS) during the JCI accreditation.Methods The changes of PIVAS in Huangshi Central Hospital were analyzed and compared during JCI accreditation in respect of facilities,software management,service processes,safety awareness of the PIVAS staff and pharmacy management,service evaluation,error rate,etc.Results After JCI accreditation,the process of PIVAS were optimized,the software were updated ,and the safety awareness of PIVAS staff was improved;the management of high alert drugs was implemented more strictly;more importance was attached to the details of pharmaceutical care;the satisfaction degree of the clinic units to PIVAS was increased from 87.0% in 2014 to 98.0% in 2015;error rate was decreased from 0.40‰ in 2014 to 0.15‰ in 2015.The incidence rate of defective product was decreased from 0.32‰ in 2014 to 0.07‰ in March 2015.The correct rate of medical waste classification treatment was increased from about 85.5% in 2014 to 100.0% in 2015.Hand hygiene compliance rate and correct rate were increased up to 100.0%.6S site management activity participation rate was increased up to 100.0%.Conclusion The application of JCI standards and the process of JCI accreditation improve the quality of pharmaceutical care in hospital PIVAS.It can improve the medical safety in hospital.

ABSTRACT

Objective Set up a new performance management system with JCI standard that designed for improvement of nursing research.Methods Based on JCI standard,the new system was designed to be performed separately.Re-design a performance appraisal plan that will take account into nursing workload,care quality,nursing risk,nursing research,personal comprehensive quality etc.Results The average performance of the nurses in 42 wards had increased by 24.64%.In this system,research project application,and paper publishing was significantly improved.Projects Standardization was also improved.Enthusiasm of continuing education increased.Conclusions The system is helpful to improve the quality of nursing research.It can also help to assure the occupational stability of the clinical nurses and enthusiasm of nursing research.

The accuracy rate of disposal of significant abnormalities of check-up increased from 90.67%to 96.50%(χ2=69.184, P<0.05). The coincidence rate of high-risk drugs storage increased from 90.17%to 96.50%(χ2=77.357, P<0.05). The hand hygiene compliance rate increased from 75.25%to 88.63%(χ2=145.047, P<0.05). The awareness rate of adverse events report was raised from 84.00%to 96.67% (χ2 = 220.486, P < 0.05). The satisfaction rate of customers at the health management center increased from an average of 93.94 points to 98.66 points (t=-7.903, P<0.05). Above comparisons are all statistically significant. Conclusions The effective implementation of JCI standard patient safety goals helps to achieve scientific, standardized, orderly and detailed nursing management, optimize nursing work processes, standardize operations, reduce nursing risk and improve nursing quality, which makes a contribution to improve patients′satisfaction and gain patients′trust.

ABSTRACT

A presente dissertação tem por objetivo problematizar discursos que constituem as práticas de Acreditação Hospitalar da Joint Commission International mais especificamente, aquelas enunciações voltadas para a formação/educação e empresariamento dos colaboradores. Baseada nas ideias de Michel Foucault, a pesquisa desenvolve-se na operação de conceitos/ferramentas da governamentalidade e, como constituintes desta, do governamento e da subjetivação. Como superfície de análise, foi utilizada a quinta edição do Manual de Padrões de Acreditação Hospitalar da Joint Commission International. As reflexões desenvolvidas permitem ver que os discursos da saúde, empresariais e educacionais ao articularem-se em uma grade de inteligibilidade neoliberal, operam na construção de possibilidades competitivas de formação dos colaboradores, visando a transformação do éthos hospitalar, a partir da transformação do colaborador. Portanto, há um novo éthos hospitalar formativo e homogeneizador, construído na Contemporaneidade, que impõem à Joint Commission International a escuta permanente dos clientes e, para estes, investimentos em sua formação profissional e em padronização de serviços e atendimentos.


This work aims to debate the discourses that constitute the practices of Hospital Accreditation of the Joint Commission International; more specifically, those enunciations directed to the training/education and entrepreneurship of employees. Based on Michel Foucault's ideas, the research develops operating concepts/tools of governmentality and, as constituents thereof, of the government and subjectivation. As surface to be analysed was used the fifth edition of Hospital Accreditation Standards Manual of the Joint Commission International. The developed analyses allow us to see that the discourses of health, business and education are articulated in a neoliberal intelligibility grid and operate in building competitive opportunities for employee training, aiming at the transformation of the hospital ethos from the transformation of the employee on a businessman himself. So there is a new hospital ethos, training and homogenizing, built in contemporary times, which impose to the Joint Commission International permanent listening to customers, and for them, investments in their training and standardization of services and attendances.


Esta tesis pretende problematizar los discursos que constituyen las prácticas del Hospital de acreditación de la Joint Commission International, más concretamente, esas declaraciones dirigidas a la formación / educación y el espíritu empresarial de los empleados. Sobre la base de las ideas de Michel Foucault, la investigación se desarrolla los conceptos de operaciones / herramientas gubernamentalidad y como componentes de los mismos, del governamento y subjetividad. Como análisis de superficie, se utilizó la quinta edición del Manual de Normas de acreditación de hospitales de la Joint Commission International. Las reflexiones desarrolladas nos permiten ver que los discursos de la salud, los negocios y la educación para articular en una cuadrícula inteligibilidad neoliberal, trabajamos en la construcción de oportunidades competitivas para la capacitación del personal, orientadas a la transformación de la ética del hospital, de la transformación del empleado. Así que hay un nuevo hospital éthos y la formación homogeneizador, construida en el contemporáneo, que practican la escucha permanente de la Comisión Conjunta Internacional a los clientes y, por estas inversiones en su formación y estandarización de servicios y comodidades...

ABSTRACT

Recently many hospitals in Korea have become interested in JCI (Joint Commission International) accreditation. As the medical market opens to medical tourism and to attract foreign patients, JCI accreditation has become the prime object. It is compatible with government policy for upgrading the medical industry and necessary to strengthen compatibility with foreign hospitals. JCI accreditation means that the medical services provided by a hospital are equivalent in quality and patient safety to medical services internationally. It also means that the hospital is reliable in treating patients according to international policies and regulations. The most important meaning of JCI accreditation is that the staff in the hospital have promised to provide safe patient care according to the hospital policies and bylaws made and approved by the staff members. During the process for JCI accreditation, the hospital staff's concept regarding patient safety has been changed and many of them are now voluntarily involved in quality improvement and patient safety activities in the hospital.

ABSTRACT

Although the practice of sedation has markedly increased and expanded beyond the operating room with the development of new diagnostic and minimally invasive surgical procedures, there is no guideline for hospitals to set up a sedation policy in Korea. The Joint Commission International (JCI), which is a division of Joint Commission of United States, has urged Korean hospitals to develop guidelines for sedation. Because JCI defines the sedation standards as a minimum, there is much for Korean medical association and physicians in charge of sedation to do in order to improve the safety of sedated patients and set up an economical and efficient sedation system. There are three main requirements to meet JCI standards. Aqualified individual responsible for sedation should complete a formal training program supervised by an anesthesiologist or experienced practitioner, and adequate facilities and monitoring systems must be available to safely care for patients throughout sedation. Secondly, a documentation process around sedation, including informed consent, pre-operative assessment, the parameters observed during sedation, and discharge criteria, should be organized according to the sedation policy, which in turn should be determined by an institutional sedation committee. Lastly, JCI requires continuous quality improvement activity, which can improve the efficiency of the sedation process and the safety of sedation care. Multi-departmental joint discussion may be necessary to advance the practice of sedation and avoid conflicts among departments. This review aims to highlight the starting points of adiscussionon local standards for sedation.

OBJECTIVE:To strengthen management of hospital drug storehouse.METHODS:According to Joint Commission International (JCI) accreditation,management of drug storehouse was guided through perfecting working system,workflow,qualification confirmation of employees,reasonable management of drug stock and information management,etc.Related investigation and discussion were carried out.RESULTS & CONCLUSIONS:Management of hospital drug storehouse combining with JCI accreditation and quality assurance system of drug storehouse management can improve workflow and management of drug storehouse,guaranteeing efficiency and quality of drug supply.
