ABSTRACT

Every woman who became pregnant has to undergo the process of delivery. In normal process of delivery the baby is delivered per vagina, an episiotomy is performed by health care provider or midwife. Episiotomy is a surgically planned incision on the perineum and the posterior vaginal wall during the second stage of labour to enlarge the vaginal introits so as to facilitate easy and safe delivery of the fetus. Episiotomy may be advice in the situations such as inelastic perineum, fetal distress, and complicated birth, prolonged second stage of labor, instrumental vaginal delivery and previous perineal surgeries .There are four types of episiotomy: midline, mediolateral, lateral and J shaped. Care of episiotomy involves perineal care, sitz bath, infrared heat, perineal exercises, antiseptic ointments, cold and hot packs The REEDA scale is used for assessing the perineal healing. Complications of episiotomy include perineal discomfort, perineal pain, difficulty with breast feeding and walking, perineal bleeding, infection, wound dehiscence and dyspareunia

ABSTRACT

【Objective】 To explore the clinical efficacy and safety of pelvic floor magnetic and electrical stimulation combined with Kegel exercise training in the treatment of stress urinary incontinence (SUI) after minimally invasive surgery for benign prostatic hyperplasia (BPH). 【Methods】 A total of 52 patients with SUI after minimally invasive surgery for BPH treated during Jan.2016 and Feb.2022 were randomly divided into test group (n=26) and control group (n=26). The test group received pelvic floor magnetic and electrical stimulation and Kegel exercise training, while the control group received Kegel exercise training only. The treatment lasted for 3 months. The scores of International Consultation on Incontinence Modular Questionnaire Short Form (ICIQ-SF), 1 h pad test, International Prostate Symptom Score (IPSS) and Incontinence Quality of Life Questionnaire (I-QoL) were recorded and compared between the two groups before and after treatment. The adverse reactions were observed. 【Results】 The scores of ICIQ-SF, IPSS and I-QoL and 1 h pad test significantly decreased in both groups after treatment (P0.05), but after treatment, the scores of ICIQ-SF,IPSS,I-QoL and 1 h pad test were significantly lower in the test group than in the control group (P<0.05). No severe adverse reactions were observed. 【Conclusion】 Pelvic floor magnetic stimulation combined with Kegel exercise training is safe and effective for SUI after minimally invasive surgery for BPH.

ABSTRACT

Objective:To discuss and analyze the clinical efficacy and application value of transperineal ultrasound dynamic monitoring for the contraction function of pelvic floor muscle to guide training based on its feedback in the treatment of stress incontinence of female.Methods: 169 female patients with stress incontinence were enrolled this study. And all of them were treated by using the Koegel physical training of pelvic floor muscle under the guidance of doctor, at the same time, the relative contraction function indicators of pelvic floor muscle before and after treatment were observed and compared by applied transperineal ultrasound dynamic monitoring.Results: The differences between before and after treatment of physical training on pelvic floor muscle about the length of the levator hiatus, the acreage of pelvic diaphragm hiatus, and the perimeter of pelvic diaphragm hiatus in contraction and silent period were significant (t=-2.858,t=-2.949,t=-4.899,P<0.05), and that of after treatment was higher than that of before treatment. The horizontal distance of pubis combined with margo inferior, vertical distance and thickness of viscera on pubis of after treatment were significant higher than that of before treatment (t=3.674, t=-4.899,t=4.230,P<0.05). The posterior urethra intersection angle and the intersection angle of left and right levator after treatment were significantly higher than that of before treatment (t=-4.087,t=6.584, P<0.05).Conclusion: Transperineal ultrasound dynamic monitoring can correctly evaluate the clinical efficacy of the contraction function of pelvic floor muscle to guide training based on its feedback in the treatment of stress incontinence of female. And this method has important clinical value.

ABSTRACT

Objective To adopt Suitability Assessment of Materials for suitability assessment of domestic Kegel exercise online health education written materials. Methods After Searching Kegel exercise online health education written materials in search engines, four health education professionals assessed the suitability of the collecting materials.One professional evaluated all collecting materials,and all materials were divided into three parts. The other three professionals evaluated one part agian independently.Excel and SPSS 19.0 were used to analyze the results. Results 38 Kegel exercise online health education written materials were collected.2(5.3%)of the materials were assessed as"Superior",23 (60.5%) of the materials were "Adequate", and 13 (34.2%) of the materials were evaluated as "Not suitable". Average percentage score was 44.05% (24% ~74% ), and the overall assessment was"Adequate". Conclusion Most domestic Kegel exercise online health education written materials′quality should be certified and suitability were adequate,which need some improvement.

ABSTRACT

Uterine prolapse is defined as when the uterus slips down into the vagina from its normal position, and in severe cases, outside the vagina. It is one of the most distressful gynaecological problem commonly seen among the menopausal and post-delivery women mainly due to the weakening of pelvic floor muscles. Pelvic floor muscles provide support or act as a “ floor” for the abdominal viscera including the rectum and constrictor or continence mechanism to the urethral, anal and vaginal orifices (in females). Weakening of these muscles not only lead to uterine prolapse, but may also lead to the prolapse of other pelvic organs as a complication that is Cystocoele (prolapse of Bladder) and Rectocoele (prolapse of Rectum). Pelvic floor exercises also called as Kegel exercises mainly works on strengthening the muscles and the ligaments that support the female genital tract which become slack and atonic, and improves their general muscular tone. It also increases the thickness of the vaginal wall and lubrication after menopause. Here, I will discuss briefly about the uterine prolapse, and mainly the importance and process of doing pelvic floor exercises, since it has been mentioned that initial degrees of uterine descent can be managed by it. Need of this paper is to spread the awareness about the conservative treatment, rather than looking for surgery.

ABSTRACT

La incontinencia urinaria es cualquier pérdida involuntaria de orina. El tratamiento actual consiste en ejercicios de fortalecimiento pélvico y cirugía en los casos severos. Dado que el nervio tibial posterior y músculos del piso pélvico presentan inervación común originada en las raíces sacras se plantea que la estimulación eléctrica de dicho nervio es un método terapéutico de la incontinencia urinaria de esfuerzo. Se realizó un estudio cuasi-experimental, longitudinal, analítico de tipo prospectivo en 22 mujeres entre 20 y 60 años durante el periodo de Junio a Diciembre del año 2014, las cuales cumplieron 6 semanas de terapia y se dividieron en dos grupos: el grupo A que realizó ejercicios de Kegel y Grupo B que realizaron electroestimulación del tibial posterior y ejercicios de Kegel. Las pacientes respondieron el cuestionario de incontinencia urinaria ICIQ-SF al inicio de la terapia y a la finalización. Existió una mejoría global del 26% posterior a la terapia con ejercicios de Kegel y 69% en el grupo que realizó la electroestimulación del tibial posterior al finalizar las 6 semanas de tratamiento.


Urinary incontinence consists in any involuntary loss of urine. -The current treatment consists in pelvic strengthening exercises and surgery in severe cases. Since the tibial posterior nerve and the pelvic floor muscles present common innervation originated in the sacral roots a hypothesis arises: Electrical stimulation of the nerve is a therapeutic method of urinary incontinence. A quasi-experimental, longitudinal, prospective analytical study was performed on 22 women between 20 and 60 years during the period from June to December 2014, which met six weeks of therapy and were divided into two groups: group A who performed Kegel exercises and group B who made electrostimulation of the posterior tibial nerve and Kegel exercises. The patients answered the questionnaire urinary incontinence ICIQ-SF at the start of therapy and termination. There was an overall improvement of 26% after therapy with Kegel exercises and 69% in the group that performed electrostimulation of the posterior tibial nerve at the end of 6 weeks of treatment.

ABSTRACT

Objective To study the effect of Sopfrology childbirth method training,Kegel training and Lamaze respiratory training (SLK training) on depression and sexual life of perinatal pregnant woman..Methods The women in 24 weeks of pregnancy who came to the clinic to receive the pregancy test were divided into study group(n=232) and control group(n=126) according whether they voluntered to participate the study.All the women in two group were subjected to regular prenatal care and the women in study group received SLK training additionally.The depression,pelvic floor muscle tension and female sexual function index were evaluated by Self Rating Depression Scale(SDS),the Pelvic Floor Muscle Examination Grading Standards and the Female Sexual Function Inventory (FSFI) respectively.Results 6 months after delivery,the incidence of depression,SDS score,pelvic floor muscle tension score and live index standard rate in study group and the control group respectivelywere(10.3% vs 18.3%),((43.6±5.1) vs(45.3±6.3)),((3.36±0.33) vs(2.46±0 89)),(81.5% vs 65.9%),and the differences were statistically significant(P<0.05).Conclusion SLK training program for perinatal women has the effects of preventing,treating prenatal depression and postpartum depression,improves pregnancy index and the quality of sexual life,worthy of further study.

@#ObjectiveTo investigate the effect of nerve functional reconstruction on urinary incontinence caused by stroke.MethodsMyoelectricity of pelvic muscles was captured and measured by electrode in vagina or anus.According to the type of urinary incontinence,suitable exercise model and corresponding exercise graph were designed,based on the theory of muscle construction and relaxation.23 patients were involved and the therapies last for 4 weeks.Myoelectricity and the degree of urinary incontinence of before and after the treatment.ResultsThe frequency of patients' urinary incontinence and urinations was reduced and the myodynamia of pelvic muscles improved.ConclusionNerve functional reconstruction is helpful in improving the patients' urination function and their life qualities.

ABSTRACT

This study was carried out to assess the perception of decisional balance of Korean subjects about 4 health behaviors and to identify the influencing factor of decisional balance for exercise acquisition, smoking cessation, mammography screening and Kegel's exercise acquisition. All are representative health behaviors nurses can intervene in Korea based on the Transtheoretical model. Convenient samples of 2,484 subjects (191; exercise, 169; smoking cessation, 1903; mammography screening and 221; Kegel's exercise) were selected from cities and counties over 9 provinces throughout Korea, and the data was collected from January 1, 1999 to February 29, 2000. The research instrument were the Decisional Balance Measure for Exercise (Marcus & Owen., 1992), Smoking Cessation (Velicer et al., 1985), Mammography Screening (Rakowski et al.,1992) and Kegel Exercise (Lim, 1999) and Stage of Change Measure for Exercise (Marcus et al, 1992), Smoking Cessation (DiClemente et al., 1991), Mammography Screening (Rakowski et al.,1992) and Kegel's Exercise (Lim, 1999). The data was analyzed by the SAS Program. The results are as follows; 1. According to the stage of change measure, 2,484 subjects were distributed in each stage of change for four health behaviors: 1,233 subjects (49.8%), 745 subjects (30.2%), 113 subjects (4.7%), 156 subjects (6.5%), and 216 (8.7%) belonged to the pre- contemplation stage, contemplation stage, preparation stage, action stage and maintenance stage. They were all series of stages of change in their efforts to do health behavior. 2. Factor analysis identified 3 factors (1 of Pros, 2 of Cons) for the exercise, 4 factors for smoking cessation (2 of Pros, 2 of Cons), 2 factors (1 of Pros, 1 of Cons) for the mammogram screening and 2 factors (1 of Pros, 1 of Cons) for Kegel's exercise of decisional balance. 3. The analysis of variance and multiple comparison analysis showed that for all 4 samples, the Cons of changing the problem behaviors outweighed the Pros for subjects who were in the pre- contemplation stage, The opposite was true for subjects in action and maintenance stage. 4. Through the discriminant analysis, it was found that one factor of Pros for exercise, one factor of Cons for smoking cessation, 1 factor of Cons for mammogram screening and one factor of Cons for Kegel's exercise were the more influencing factors, than others in discriminating the stages of change. Results are consistent with the applications of the Transtheoretical model, which have been used to understand how people change health behaviors. This results provide some evidence that subject's report of his/her health behavior corresponds to beliefs about usefulness of related health behaviors. The results of this study have implications for patients' health education and health intervention strategies. The findings of this study give useful information for nursing educators for 4 health behaviors, especially the factors relating to decision making in the different stages of change.
