ABSTRACT

PURPOSE: This study was aimed to evaluate the factors affecting the results of comprehensive pre-internship exam (CPIE) among medicals students of Kermanshah University of Medical Sciences. METHODS: In this descriptive-analytical study, all students (n=240) participating in CPIE over a 3-year period (2012–2014) were selected. Data were gathered by a questionnaire, including the CPIE results and educational and demographic data. Spearman correlation coefficient, Mann-Whitney U-test, and analysis of variance were used to analyze the association of students' success with study variables. Also, regression analysis was applied to determine the role of independent variables in students' success. RESULTS: The frequency of the failed units in apprenticeship course was one of the most important risk factors associated with failure in CPIE. Average scores of pre-internship course were the most important factors of success in CPIE. The CPIE score had the highest direct relationship with grade point average (GPA) of apprenticeship course, total GPA of all three courses, GPAs of physiopathology and basic sciences courses, and score of comprehensive basic sciences examination, respectively. CONCLUSION: CPIE showed the highest inverse correlation with the number of failed units in apprenticeship course. The most important factors influencing this exam were failure in apprenticeship course and GPA of previous educational stages.

ABSTRACT

Background Scorpion stings are an acute health problem in tropical regions. Awareness of this problem is fundamental for establishing preventive interventions, thus prompting the present study to determine the scorpion-sting incidence in tropical areas of Kermanshah province during 2008 and 2009. Methods In a retrospective study, all records related to scorpion sting patients from the health centers of tropical areas of Kermanshah were studied by a census and checklist. Data were analyzed by the software SPSS-16 using descriptive and inferential tests. Results The incidence of scorpion stings was 334.37/100,000 inhabitants in 2008 and 339.07/100000 in 2009. Mean and standard deviation of age were 30.55 ± 16.99. Scorpion stings were more common in rural areas (59.6 %) and occurred more often in summer (52.9 %). Nearly 48 % of bites were to patients' hands and 47.5 % of patients were injured between midnight and 6 a.m. While 92.9 % of patients had mild symptoms, scorpion antivenom was prescribed to 88.8 % of victims, 94.5 % of whom were discharged after outpatient treatment. The relationship between antivenom therapy and clinical symptoms was not significant. Conclusions Due to the relatively high incidence of scorpion stings in tropical areas of Kermanshah, it is recommended that the inhabitants be educated through the mass media about how to prevent the stings and apply preliminary treatment.(AU)

ABSTRACT

Background: Phenylketonuria (PKU) is an inborn error of amino acid metabolism that results from a deficiency of phenylalanine hydroxylase (PAH). According to PAH database, exons 6 and 7 and their flanking introns of PAH gene contain the greatest number of mutant alleles. Therefore, as a preliminary study, nucleotide sequence analysis of exons 6 and 7 of the PAH gene has been performed in 25 PKU patients whose ancestors lived in Kermanshah province of Iran. To date, there has been no mutation data describing the genotypes of the PKU disease in this Kurdish ethnic region background. Materials and Methods: Twenty-five patients (aged between 2 and 23 years) participated in this study. The DNA fragments containing two exons of the PAH gene [6 and 7] and their exon-flanking intronic sequences were amplified and sequenced. Results: The total of detected mutations were R261X (8%), R176X (4%), R243Q (4%), R243X (2%) and R261Q (2%), as they accounted for 20% of all mutant alleles in this study. The identified polymorphisms are: IVS5 -54 G > A (22%), Q232Q (8%) and V245V (4%). All of the detected mutations in this study are related to CpG dinucleotides in the PAH gene sequence. Conclusion: The frequency of R261X, the most common mutation in our study, in Iranian population is <5%. Furthermore, there is no report of detection of R176X and R243Q in Isfahan and Azeri Turkish populations. These findings confirm the common Mediterranean mutations in this local population, although with more or lower frequencies than those reported in other related studies in Iran. Therefore, it may be necessary to study the PAH gene mutations in other provinces of Iran separately.
