ABSTRACT
Right colon cancer is a kind of clinically common colorectal cancer.Surgical resection is the main treatment and the only curative method for patients.Improvement of surgical methods in recent years is one of the hotspots in the surgical treatment of colorectal cancer.There are controversies in laparoscopic surgery or traditional open surgery,intestine resection extent,D3 radical surgery or complete mesocolic excision,selective arterial guidance or venous guidance,pyloric lymph nodes (No.6 group lymph nodes) dissection,and most of the issues are currently lacking of prospectively randomized controlled trials.Therefore,this article discusses the above problems.At the same time,in view of the complicated operation of right hemicolectomy,and the greater risk of surgery,the author consulted the relevant literature and combined his clinical experience to introduce the key techniques of the operation.
ABSTRACT
Objective To screen a set of key techniques for new medical rescue aircraft ICU care for peacetime and wartime.Methods The study used the literature review and field research method to propose the initial technical indicators.The consultation questionnaire was developed by expert panel method.Delphi method was applied to conduct two rounds of consultation among 16 experts from different fields.Analytic hierarchy process and TOPSIS comprehensive evaluation method were used to analyze and summarize data.Results In two rounds of consultation,the positive coefficients of experts were 100％,the authority coefficient of experts was 0.958,and Kendall co ordination coefficients were 0.568,0.525,0.474,and the differences were statistically significant (P＜0.05).The new medical rescue aircraft ICU care key techniques for peacetime and wartime was established finally,including 4 firstlevel indicators,37 second-level indicators,and there were 12 basic care techniques,8 specialty care techniques,10 emergency care techniques,and 7 war wound care techniques.Conclusion Key techniques for new medical rescue aircraft ICU care has high scientificity and feasibility,and experts' opinions are concentrated,which can provide additional safeguard for the transit of critically ill patients in the air.
ABSTRACT
Objective: To design a portable real-time monitoring tool for spine surgery and to preliminarily evaluate its application,so as to reduce the iatrogenic injury during the treatment of battle field spinal cord injury. Methods: A portable spine guiding device for spinal fixation was designed and prepared. The spinal specimens were randomly divided into control group (n = 10) and experimental group (n = 10). In the control group the way was cleared for the pedicle according to the doctor's experience; in the experimental group the way was cleared using our self-designed guiding device. The operation time was recorded in the two groups. After manipulation the specimens were opened along the pedicle level; the location and the depth of the needle were compared. Results;The manipulation time of a single vertebral body was (0. 5 + 0. 2) min in the control group and (0. 6 + 0. 1) min in the experimental group (P>0. 05). The edge of the puncture to the pedicle wall in the control group was significantly shorter than that in the experimental group ([1. 1 + 0. 3] vs [1. 8 + 0. 2] mm,P = 0. 037). Conclusion:The designed portable guiding device for spinal fixation is easy to carry, simple to operate; and initial in vitro application is satisfactory and can provide reliable reference for early operation.
ABSTRACT
On spot external fixation and early surgical treatment are the keys to improve the treatment of spinal cord injuries during emergent events. The present treatment of spinal cord injuries in our country still lags behind compared with developed countries. And efforts need to be made to accelerate the development of new technologies and appliances for treatment of spinal cord injuries during emergent events.
