ABSTRACT

Oligoasthenozoospermia is the main cause of male infertility, with complex and diverse causes. Currently, there are still some unclear causes of oligoasthenozoospermia in clinical practice, known as idiopathic oligoasthenozoospermia. With the development of high-throughput sequencing technology, it has been found that intestinal microbiota disorder may be an important promoting factor for the onset of oligoasthenozoospermia. Traditional Chinese medicine believes that "deficiency of kidney essence" is the core pathogenesis of oligoasthenozoospermia. In clinical practice, the method of tonifying the kidney and strengthening the essence has a significant therapeutic effect on oligoasthenozoospermia, but its mechanism of action has not been fully elucidated. Based on the basic theories of traditional Chinese medicine and molecular biology research, it has been found that there is a similarity between "kidney essence" and intestinal microbiota. During the onset of oligoasthenozoospermia, the disorder of intestinal microbiota has similarities with the pathogenesis of "deficiency of kidney essence" in traditional Chinese medicine. Moreover, traditional Chinese medicine for tonifying the kidney and strengthening the essence can regulate the disorder of intestinal microbiota, which may be one of the effective mechanisms for the treatment of oligoasthenozoospermia with the Bushen Yijing method. Based on this, this article explored the mechanism of Bushen Yijing method of traditional Chinese medicine in treating oligoasthenozoospermia from the perspective of intestinal microbiota, so as to provide new ideas for the treatment of oligoasthenozoospermia with traditional Chinese medicine.

ABSTRACT

It is believed that the fundamental pathogenesis of the connective tissue diseases-associated interstitial lung disease (CTD-ILD) is kidney essence deficiency, with lung collateral obstruction throughout the disease, and environmental toxin pathogen is the important causative factors for the development of CTD-ILD. This article proposed to restore origin and alleviate bi (痹) for CTD-ILD, for which restoring origin means tonifying the lungs, spleen and kidneys to bank up the roots and consolidate the original qi, with modified Erxian Decoction (二仙汤) plus Liu Junzi Decoction (六君子汤); alleviating bi means expelling wind and dredging collaterals, and eliminating the mass to restore the smoothness of the lung collaterals, with paired medicines of Chuanshanlong (Dioscorea nipponica)-Dilong (Kalanchoe pinnata), Vinegar-processed Sanleng (Sparganium stoloniferum)-Vinegar-processed Ezhu (Curcuma zedoaria), and stem-type medicines, and emphasized on removing the environmental toxin pathogens to facilitate the recovery of healthy qi.

ABSTRACT

Objective:To observe the effect of Wuzi Yanzong Wan made of different processed products on the apoptosis of spermatogenic cells in rats with kidney essence deficiency, and explore its protective effect on spermatogenic cells. Method:SD rats were randomly divided into the blank group, model group, whole raw product group, pharmacopoeia group and salt-processed product group, with 8 rats in each group. The kidney essence deficiency model was replicated by giving tripterygium glycoside tablets (the dose of 20 mg·kg-1). The flow cytometry (FCM) was used to analysis the apoptosis of spermatogenic cells in testis, the immunohistochemistry (IHC) and Western blot were used to detect the expression levels of B-cell lymphoma-2 (Bcl-2) and Bcl-2 associated X protein (Bax) in the testis. High performance liquid chromatography (HPLC) was used to compare the contents of eight components (chlorogenic acid, ellagic acid, hyperoside, isoquercitrin, verbascoside, astragalin, kaempferol and schisandrin) in Wuzi Yanzong Wan made of different processed products, the mobile phase was composed of acetonitrile (A)-0.4% phosphoric acid aqueous solution (B) for gradient elution (0-5 min, 5%-15%A; 5-10 min, 15%-17%A; 10-25 min, 17%A; 25-35 min, 17%-26%A; 35-60 min, 26%-56%A), the detection wavelength was set at 254 nm. Result:Compared with the model group, the total apoptosis rate of spermatogenic cells, protein expression of Bax and Bcl-2 in each administration group were improved. Among them, the pharmacopoeia group and salt-processed product group had significant effects (P<0.01), and the improvement effect of the pharmacopoeia group and salt-processed product group was significantly better than that of the whole raw product group (P<0.05). The contents of chlorogenic acid, hyperoside, isoquercitrin and verbascoside in Wuzi Yanzong Wan were increased after the herbal medicines being processed with salt-water. The content of ellagic acid in the salt-processed product group increased, while it decreased in the pharmacopoeia group. The contents of verbascoside, astragalin, kaempferol and schisandrin in samples from the salt-processed product group were greater than those in samples from the pharmacopoeia group. Conclusion:Wuzi Yanzong Wan may reduce the apoptosis of spermatogenic cells in rat testis by inhibiting the expression of Bax and promoting the expression of Bcl-2, and exert its effect of nourishing kidney and enriching essence. The enhanced anti-spermatogenic effect of Wuzi Yanzong Wan after processing may be related to the changes in chemical composition content after processing.

ABSTRACT

"Kidney essence" is a profound concept in the theory of traditional Chinese medicine. But its biological basis is unknown until now, resulting in the therapeutic effects of traditional Chinese drugs on reinforcing kidney for supplementing essence hard to be evaluated. This study aimed, to explore the potential biological basis and mechanism of traditional Chinese drugs of reinforcing kidney for supplementing essence on diseases related to deficiency of kidney essence through network pharmacology analysis on the intersection of targets of drugs and diseases. The targets for ingredients in Rehmanniae radix praeparata (RRP), Polygoni multiflori radix praeparata (PMRP) and Polygonati rhizome (PR) were gathered from TCMSP and TCMID database. Osteoporosis, Alzheimer's disease, anemia, infertility and oligospermia targets were collected from OMIM and DisGeNET database. Drug-compound-target-disease (DCTD) network was established with Cytoscape 3.6.1 software, then Clue GO and DAVID database was used to acquire the annotation about GO terms and signaling pathways. Natural aging mice, an acknowledged syndrome model of deficiency of kidney essence, and RRP were used to verify the predictive targets by Western blot analysis. All animal experiments were conducted in accordance with the international guidelines and regulations for the care and use of animals. DCTD network showed that the intersection of drugs and diseases included 175 common targets. After topology analysis, 71 key were screened out targets which were associated with GO annotation exhibited that biological processes (including transcription regulation, RNA metabolism regulation, and DNA-dependent transcription regulation), cell composition (including nuclear lumen, organelle lumen, and membrane closure lumen), molecular function (including transcription regulation, transcription factor activity, and enzyme binding), and signaling pathway (including peroxisome proliferator-activated receptor alpha (PPARα), mitogen-activated protein kinase (MAPK), hypoxia-inducible factor 1 (HIF-1), erythropoietin (EPO) and other signaling pathways. In natural aging mice, the expressions of HIF-1α, growth factor receptor-bound protein 2 (GRB2), MAPK3, signal transducer and activator of transcription 5A (STAT5A), transcription factor AP-1 (JUN) and proto-oncogene c-Fos (FOS) in EPO pathway were significantly decreased. RRP significantly reversed the decrease of the above targets. Above all, these results indicated that the therapeutic effects of traditional Chinese drugs of reinforcing kidney for supplementing essence on deficiency of kidney essence may be related to the regulation of nuclear transcriptional activity and EPO signaling pathway.

ABSTRACT

BACKGROUND: The effect of kidney-tonifying prescriptions on proliferation of bone marrow mesenchymal stem cells has been verified, but its mechanism of action is still unclear. In this study, we took Guishen Pil as an example to explore its functional mechanism. OBJECTIVE: To explore the relationship between Guishen Pill nourishing kidney essence and proliferation of bone marrow mesenchymal stem cells by observing the effect of Guishen Pill water extract on the proliferation of bone marrow mesenchymal stem cells. METHODS: Bone marrow mesenchymal stem cells were isolated from the femoral bone marrow of 3-week-old male Sprague-Dawley rats and cultured in vitro. The cells were purified using the whole bone marrow adherence method, and the passage 3 cells of higher purity were selected for the drug intervention. The cells were divided into blank control group and 10, 20, 40, 80 and 100 mg/L Guishen Pill groups. The dose-effect and time-effect relationships were investigated after cultured for 5 days. The cell morphology was observed under inverted microscope. The cell proliferation was evaluated by cell counting kit-8 assay. The expression levels of PI3K and AKT proteins were detected by western blot assay. RESULTS AND CONCLUSION: Guishen Pill significantly increased the cell proliferation in the Guishen Pill groups in a dose-and time-effect manner. The proliferation of bone marrow mesenchymal stem cells in the 80 mg/L Guishen Pill group was strongest at the 3rd day. The expression levels of PI3K and AKT proteins in the Guishen Pill groups were significantly higher than those in the blank control group. In summary, Guishen Pill water extract can effectively boost the proliferation of bone marrow mesenchymal stem cells, which may be related to its involvement in regulating PI3K/AKT signaling pathway.

ABSTRACT

OBJECTIVE@#To assess the effect of electroacupuncture (EA) on expression of cytoskeletal proteins from Sertoli cells (SCs) and spermatogenesis in rats with oligozoospermia of insufficiency of Shen (Kidney) essence syndrome (OIKES).@*METHODS@#Twenty healthy male Sprague-Dawley rats were randomly assigned to four groups using a random number table: control, tripterygium glycosides (TG) treatment, sham and EA groups (n=5 in each group). A rat model of OIKES was established by oral gavage with TG. The EA group was treated with TG and received EA at Shenshu (BL 23) and Zusanli (ST 36) acupoints for 20 min, once daily for 30 days, while the sham group received EA at identical acupoints with skin penetration without stimulation. After 30 days, the final body weight and coefficients for the testis and epididymis were calculated and sperm parameters were measured. Immunohistochemical analyses were performed to detect expression of vimentin and α-tubulin in SCs and proliferating cell nuclear antigen (PCNA) immunoreactivity in germ cells. Apoptosis in germ cells was quantified by the transferase biotin-dUTP nick end labeling assay.@*RESULTS@#Compared with the control group, the final body weight and testis/epididymis coefficients of rats in the TG-treated group were not significantly different, but the sperm count and motility were lower (P<0.05). Expressions of vimentin and α-tubulin were also significantly weaker (P<0.01). The PCNA immunoreactivity of germ cells was decreased (P=0.059), whereas the apoptotic index of germ cells was increased significantly (P<0.01). In contrast, EA at BL 23 and ST 36 acupoints significantly improved the final body weight as well as the sperm count, concentration and motility (P<0.01 or P<0.05). EA increased expression of vimentin and α-tubulin in SCs markedly, and significantly enhanced PCNA immunoreactivity with decreased apoptosis in germ cells (P<0.01 or P<0.05).@*CONCLUSIONS@#EA at BL 23 and ST 36 acupoints has protective effects on spermatogenesis in rats with OIKES. This effect seems to be achieved by attenuating TG-induced disruption of cytoskeletal protein in SCs.

ABSTRACT

The aim of this paper was to observe the changes of EPO in rats with chronic renal failure and low immunity induced by adenine and to investigate the reversal effect of Yougui Yin(YGY)and exogenous EPO.SD rats were randomly divided into normal control group(n=20)and adenine-model group(n=90).The adenine-model group rats were given with adenine 150 mg·kg~(-1)for 14days by gavage administration, and then randomly divided into 8 groups as follows:model group(n=20),YGY groups(10,20,40 g·kg~(-1),10 in each group),rh EPO group(500,1 000,1 500 IU·kg~(-1),10 in each group),and Guilu Erxian Gao 10 g·kg~(-1)group(positive control group,n=10).From the 15th day, every group except normal control group received 150 mg·kg~(-1)adenine by gavage administration once every two days to maintain the model.Meanwhile, the rats in each YGY group and Guilu Erxian Gao group received corresponding drugs by gavage administration once a day for 30 days. The rats in rh EPO groups were subcutaneously injected with rh E-PO once every 3 days for 30 days.On day 46,rats were anesthetized to take blood and then sacrificed. The serum levels of creatinine, urea, glandular hormone, immunoglobulin, complement and interleukin, the proportion of T cells in the spleen, the killing rate of NKcells and the proliferative capacity of spleen cells were measured.Western blot was used to detect the key proteins in JAK2-STAT5 and NF-κB pathways mediated by EPO in kidney and spleen. As compared with the normal control group, the serum levels of CREA and UREA were increased significantly and the serum levels of ACTH,T and T3 were decreased significantly in the model group rats, indicating that the functions of kidney, adrenal gland, gonad and thyroid in rats were decreased. At the same time, the serum levels of Ig A,Ig G,Ig M,C3,C4,IL-2 and IL-6 were significantly decreased, the proportion of CD4~+,CD4~+/CD8~+T cell subsets, the killing rate of NK cell and the proliferation ability of spleen lymphocyte in spleen of the model group rats were significantly declined, indicating that the immune function of model group rats was decreased, and the model of kidney deficiency immunodeficiency was successfully constructed. As compared with the model group, both YGY and rh EPO significantly reduced serum levels of CREA and UREA,significantly increased serum levels of ACTH,T,T3,T4,Ig A,Ig G,Ig M,C3,C4,IL-2,and IL-6,increased the proportion of CD4~+,CD4~+/CD8~+T cell subsets, the killing rate of NK cell and the proliferation ability of spleen lymphocyte in spleen.YGY could significantly increase the content of EPO in serum. Both YGY and rh EPO could regulate the expression of EPOR,p-JAK2/JAK2,STAT5,NF-κB p50,NF-κB p65 and NF-κB IκB of EPO-mediated JAK2-STAT5 and NF-κB pathways in kidney and spleen.EPO is an important factor in the chronic renal failure and low immunity induced by adenine in rats.Exogenous EPO and YGY have significant reversal effects for the model rats. The mechanism of YGY may be related to the up-regulation of EPO in serum and regulating the expression of key proteins in EPO-mediated JAK2-STAT5 and NF-κB pathways in kidney and spleen. The mechanism of exogenous EPO may be related to regulating the expression of the key proteins in EPO-mediated JAK2-STAT5 and NF-κB pathways in kidney and spleen.

ABSTRACT

As the epitome of the modern regenerative medicine, stem cells were proposed in the basic sense no more than 200 years ago. However, the concept of "stem cells" existed long before the modern medical description. The hypothesis that all things, including our sentient body, were generated from a small origin was shared between Western and Chinese people. The ancient Chinese philosophers considered Jing (also known as essence) as the origin of life. In Chinese medicine (CM), Jing is mainly stored in Kidney (Shen) and the so-called Shen-Jing (Kidney essence). Here, we propose that Shen-Jing is the CM term used to express the meaning of "origin and regeneration". This theoretical discovery has at least two applications. First, the actions underlying causing Shen-Jing deficiency, such as excess sexual intercourse, chronic diseases, and aging, might damage the function of stem cells. Second, a large number of Chinese herbs with Shen-Jing-nourishing efficacy had been proven to affect stem cell proliferation and differentiation. Therefore, if Shen-Jing in CM is equivalent with stem cells in regenerative medicine, higher effective modulators for regulating stem-cell behaviors from Kidney-tonifying herbs would be expected.

ABSTRACT

Objective To make a tinnitus matching-based observation of the clinical therapeutic effect of acupuncture on tinnitus of kidney essence deficiency type. Methods Sixty patients with tinnitus of kidney essence deficiency type were randomized to groups A and B, 30 cases each. Group A received acupuncture based on syndrome differentiation- based point selection and group B, acupuncture based on disease differentiation-based point selection. Hearing condition was assessed by pure tone hearing threshold and tinnitus matching (tinnitus frequency and loudness matching test) in the two groups of patients before and after treatment. Results There were statistically significant pre-/post- treatment differences in various thresholds in pure tone audiometry in the two groups (P<0.05). There were statistically significant post-treatment differences in partial thresholds in pure tone audiometry (500 Hz, 750 Hz, 1000 Hz and 2000 Hz) between Group A and group B (P<0.05). Tinnitus matching had no statistically significant pre-/post-treatment difference within the two groups and no statistically significant post-treatment difference between the two groups (P>0.05). There was a statistically significant pre-/post-treatment difference in tinnitus loudness matching in the two groups (P<0.01). There was a statistically significant post-treatment difference in tinnitus loudness matching between Group A and group B (P<0.05).Conclusions Acupuncture is an effective way to treat tinnitus of kidney essence deficiency type. The therapeutic effect of syndrome differentiation-based acupoint selection is better than that of disease differentiation-based acupoint selection.

ABSTRACT

Primary osteoporosis (POP) is one of the most common diseases in the elderly people resulting in high risk of fracture and poor quality of life. In addition to the pathological changes in bone mass, most of the POP patients also suffer from Chinese medicine (CM) syndromes of Shen (Kidney) essence deficiency. Shen essences are highly related to bone. Shen essence deficiency plays an important part in the development of POP and a better diagnosis of POP could be made by combining CM syndromes with Western medicine risk factors. Treatments of POP should aim at both increasing the bone mass and relieving the syndromes of Shen essence deficiency. Clinical study confirmed that treating POP patients with Shen-tonifying herbs could increase the bone mass and relieve the CM syndromes of POP patients. Basic researches clarified the mechanism by which Shen-tonifying herbs increased bone mass in animal models. The mechanisms by which Shentonifying herbs relieve the CM syndromes are still in investigation.

ABSTRACT

This article put forward the ‘heart-kidney centric theory’ on basis of analyzing the physiologic function, pathomechanism, onset rules and measures of prevention and treatment of cardio-cerebrovascular disease and degenerative disease induced by kidney deficiency. According the new theory, a new TCM drug, the Tongmai Qiangshen Wine, was developed. In human body, kidney produces essential qi and produces energy source for human body; heart transports the essential qi and is the physiologic mechanism of transport of energy source. Deficiency of essential qi will result in stasis of blood vessel, on the other hand, stasis of blood vessel can cause defi ciency of essential qi. The opposition, mutual rooting, waxing and waning, mutual convertibility of them reflect the development rules of cardio-cerebrovascular disease and degenerative disease induced by kidney deficiency, and determin the vital process of birth, growth, adult, aging and death. The construction of heart-kidney centric theory and the development and application of Tongmai Qiangshen Wine not only provide new theory and model for prevention and treatment of cardio-cerebrovascular disease and degenerative disease induced by kidney deficiency, but also provide new thoughts for preparation of new TCM drugs from aspects of how to maintain the feature of TCM, how to inherit and create on basis of combing holism with disease, principle, method, medicines.

ABSTRACT

Doctors of past dynasties already understood deficiency of kidney essence, phlegm and blood stasis were important for dementia. As the increasing recognition about Alzheimer disease and the offer of 'toxin damaging brain vessel' hypothesis,we thought ?-Amyloid,the pathology of Alzheimer disease, had the character of damaging brain and marrow,so it belonged to internal toxin, internal toxin damaging brain and marrow was also the pathology of the disease.Deficiency of kidney essence was the precondition, insufficient brain and marrow and internal toxin damaging brain and marrow were the two causes of brain and marrow consumption and spirit dysfunction, which were the pathology of Alzheimer disease in Chinese medicine.
