ABSTRACT

Objective:To investigate the effect of the Traditional Chinese Medicine (TCM) intervention on the pregnancy outcomes of patients with a history of spontaneous abortion due to kidney deficiency syndrome. Methods:From February 2015 to July 2020, 41 patients with a history of spontaneous abortion due to kidney deficiency in our hospital were selected as the observation group, and 42 healthy pregnant women with no history of spontaneous abortion were selected as the control group. The observation group was treated with the TCM sequential intervention therapy with treatment principles of nourishing the kidney and promoting blood circulation before pregnancy and solidifying the kidney and tonifying after pregnancy, while the control group received no TCM intervention before and after pregnancy. Color Doppler ultrasonography was used to record the follicle diameter, endometrial thickness and shape during pregnancy, and B-ultrasound was used to monitor the NT value at 12 weeks. The serum D-dimer level was detected by ELISA, and the serum progesterone ,estradiol, human chorionic gonadotropin (HCG) levels were detected by radioimmunoassay. The conditions of term delivery, premature delivery and abortion were observed in both groups, and the successful delivery rate and term delivery rate were compared. Results:On the 30+, 40+, and 50+ days of pregnancy, there was no significant difference in serum progesterone, estradiol, and HCG levels between the two groups ( P>0.05). There was no significant difference in NT value between the two groups at 12 weeks of gestation ( P>0.05). The successful delivery rate of the observation group was 90.2% (37/41), the full-term delivery rate was 85.4% (35/41), the successful delivery rate of the control group was 100.0% (42/42), and the full-term delivery rate was 95.2% (40/42), there was no significant difference in the successful delivery rate and term delivery rate between the two groups ( P>0.05). Conclusion:The sequential intervention of TCM the principle of invigorating kidney and promoting blood circulation before pregnancy and strengthening kidney and calming fetus after pregnancy can improve the pregnancy outcome of spontaneous abortion patients with kidney deficiency syndrome.

ABSTRACT

Objective:To observe the effect of modified Wuzi Yanzongwan periodic staging treatment on the outcome of assisted pregnancy in patients with poor ovarian response (POR) and kidney deficiency syndrome. Method:One hundred and four patients were randomly divided into observation group and control group, with 52 cases in each group. Both groups received gonadotropin releasing hormone (GnRH) antagonist regimen. The patients in control group additionally took Bushen Yutaiwan orally, 5 g/time, 3 times/day. The patients in observation group additionally took modified Wuzi Yanzongwan during pre-ovulation and post-ovulation periods, 1 dose/day. The treatment courses were 3 menstrual cycles (or termination after clinical pregnancy) in both groups. The number of eggs obtained, the number of available embryos, the number of fertilization, the number of high-quality embryos, the number of embryos implanted, the number of cycles cancelled, and the clinical pregnancy were recorded. Human chorionic gonadotropin (HCG) was injected, and then follicle-stimulating hormone (FSH), luteinizing hormone (LH), estrogen (E2), anti-Müllerian hormone (AMH), basal antral follicle count (AFC) and endometrial thickness were measured daily. The number of days and dosage of Gn used, scores of kidney deficiency syndrome were recorded before and after treatment, and the adverse reactions during the study period were recorded. Result:The number of eggs captured, rate of harvested eggs, number of available embryos, rate of available embryos, number of high-quality embryos, rate of high-quality embryos, and fertilization rate in observation group were higher than those in control group (P<0.05 or P<0.01). The cycle cancellation rate was lower than that in the control group; the embryo implantation rate and clinical pregnancy rate were superior than those in control group, but the difference was not statistically significant. The FSH level and FSH/LH ratio in observation group were lower than those in control group during HCG day (P<0.01), while E2, AMH, AFC and endometrial thickness were higher than those in control group (P<0.01). Simultaneously, the number of days and amount of Gn used in observation group was lower than that in control group (P<0.01). Conclusion:The Modified Wuzi Yanzongwan periodic staging treatment combined with GnRH antagonist scheme for patients with POR kidney deficiency syndrome, can regulate the level of endocrine hormones, promote follicular development, improve ovarian reserve, increase the number of eggs obtained, improve egg quality, help improve pregnancy outcomes, and increase the chances of successful pregnancy with assisted reproductive technology. It is worthy of further clinical research.

ABSTRACT

Objective To investigate the changes of saliva biochemical parameters in the dynamic evolution of “normal constitution - kidney deficiency constitution - kidney deficiency syndrome”.Methods Totally 24 male and female SD rats of 3 months old (12 male rats and 12 female rats) were put in packet pair cage. Pregnant rats were randomly divided intoⅠ andⅡ groups, 6 rats in each group.Ⅰ group was without intimidation during pregnancy, with normal feeding, and then after the birth, 20 rats were randomly selected as normal control group and kidney deficiency syndrome model group, 10 rats in each group, half male and half female.Ⅱ group, from gestation day 2, was given intimidation until postnatal. 10 half male and half female rats were randomly selected as model group of kidney deficiency constitution. Rats in each group received normal feeding conditions for 6 weeks. The kidney deficiency syndrome model group was given intramuscular injection of hydrocortisone 25 mg/kg body weight every day, while normal control group and kidney deficiency constitution model group were injected with the same amount of normal saline for 10 consecutive days. The changes of salivary biochemical parameters were observed in each group of rats. Results Compared with the normal control group, salivary pH in the kidney deficiency constitution group and kidney deficiency syndrome group significantly increased (P<0.01). CK, ALT, AST, ALP levels in the kidney deficiency constitution group were reduced, but the contents of K, P, TP, AMY increased. The levels of CK, ALT, AST, ALP and LDH in the kidney deficiency syndrome decreased, while the contents of K, P, TP, and AMY increased (P<0.05,P<0.01). Compared with the kidney deficiency constitution group, the content of LDH in the kidney deficiency syndrome group decreased (P<0.05).Conclusion Rat saliva composition changes did occur in the kidney deficiency constitution group and kidney deficiency syndrome group.

ABSTRACT

Objective:To apply 'HNMR spectroscopic analysis and principal components analysis(PCA) for investigating the fluctuation of metabolites between hydrocortisone-induced kidney deficiency syndrome(Shenxu Zheng) rats and the normal rats. Methods: Using 'HNMR based metabolic profiling and PCA, we investigated the difference of urine metabolites between the normal rats and rats with hydrocortisone-induced kidney deficiency syndrome. We also explained the changes of the related metabolic pathway or metabolic network. Results: The changes following hydrocortisone intervention of rats could be identified by PCA,and the kidney deficiency syndrome rats entered a state of "hyperfunction",involving a series of changes in metabolic pathway and metabolic network. The response integral area of lactic acid(δ 1. 37) increased, indicating the accumulation of metabolites of lactic acid. The amount of dimethylamine(δ 2. 72) also increased, indicating that the disorder of kidney function was related to kidney-deficiency syndrome. The response of creatinine(δ 3. 87) ,asparaginic acid(δ 2.83) ,taurine(δ 3.44,3. 28), and hippurate( 7.84,7. 56,7. 64,3. 97) all decreased significantly, indicating that the hydrocortisone might cause adrenal cortex excretion injury. The response integral area of succinate( 2. 41) and citric acid( 2. 53,2. 68) decreased, which was usually caused by disorder of the mitochondrial function. Conclusion: It is suggested that the metabonomic approach can be used to investigate the pathophysiological changes under certain physiopathologic conditions. This study provides evidence for understanding "Shenxu Zheng"(kidney deficiency syndrome) in traditional Chinese medicine and for diagnosis and therapy of such diseases.
