ABSTRACT

Background: Patient with vernal keratoconjunctivitis faces significant morbidity, which affects quality of life or; moreover, it can lead to vision threatening, corneal complication in severe cases and chronic cases coupled with potential iatrogenic side effects. Aim of this study to assess the demography, clinical features and quality of life of patients with vernal keratoconjunctivitis in a centre in South India.Methods: General objective is to access the health related-quality of life and demographic study of VKC patients in a tertiary eye care hospital in south India. KINDL questionnaire with 30 questions is used for 30 paediatric patients. Percentage analysis for clinical assessment and interpretation method for questionnaire was used as statistical method.Results: In clinical assessment papillary reaction (96.66%), giant papillae (83.34%), Horner Trantas dots (20%), shield ulcer (13.35%), corneal erosions (43.33%), conjunctival congestion (76.68%). In case of vision 36.66% had normal vision, 56.67% had mild vision loss, 6.68% with moderate vision loss.Conclusions: VKC affects the health related quality of life of the patients significantly. The management of VKC should consider this aspect of health related quality of life, in addition to clinical parameters. From the assessment of KINDL questionnaire 30 samples 18 patients had high and 12 patients had average quality of life.

Resumen Introducción: El concepto de calidad de vida se ha tornado de suma importancia y actualmente toda intervención sanitaria es valorada por el impacto que causa en esta misma. La hipoacusia bilateral profunda produce importantes consecuencias, retrasando la adquisición del lenguaje, disminuye significativamente el aprendizaje y produce dificultades en cómo se desarrolla en su entorno. El implante coclear es el tratamiento de elección en esta patología y es de suma importancia conocer el impacto real que tiene en la calidad de vida de aquellos sometidos a esta intervención. Objetivo: Conocer la calidad de vida global de los pacientes implantados en el Hospital Barros Luco Trudeau entre 2015 y 2016, usando el instrumento de medición de calidad de vida KINDL. Material y método: Estudio analítico prospectivo de cohorte, para medición de calidad de vida se utilizó el instrumento KINDL, se aplicó instrumento a menores implantados y padres a través de entrevista directa en periodos preimplante, a 6 meses y al año de implantación, datos y pruebas estadísticas se realizaron con software SPSS 13. Resultados: Se estudiaron 16 pacientes en edad pediátrica implantados en Hospital Barros Luco Trudeau. Se evidenciaron cambios en la calidad de vida global y en las subescalas de bienestar emocional, autoestima, educación, relación con amigos y familia, además de percepción de enfermedad. Conclusiones: El implante coclear en nuestra serie demostró una modificación positiva en la calidad de vida global tanto de los pacientes como de su entorno familiar.


Abstract Introduction: Quality of life has become very important, and nowadays all health interventions are valued for the impact they can have on it. Profound bilateral hearing loss produces major consequences such as delaying the acquisition of language, which results in a significant decrease in learning and creates difficulties in how the individual develops in their environment. The cochlear implant is the treatment of choice in this pathology, and it is imperative to know the real impact it has on the quality of life of those subjected to this intervention. Aim: To know the overall quality of life of the patients implanted in the Barros Luco Trudeau Hospital between 2015 and 2016, using the KINDL quality of life measurement instrument. Material and method: Prospective analytical cohort study, to measure the quality of life, the KINDL instrument was used. The instrument was applied to implanted minors and their parents through a direct interview in pre-implantation periods at six months and after one year of implantation. The data and statistics tests were made with SPSS 13 software. Results: 16 pediatric patients implanted in the Barros Luco Trudeau Hospital were studied. There were changes in the global quality of life and the subscales of emotional well-being, such as the patient's self-esteem, education, relationships with friends and family, as well as the perception of illness. Conclusions: The cochlear implant in our series demonstrated a positive change in the global quality of life for both patients and their family environments.

ABSTRACT

The therapy through the arts included in the high motivation program (HMP), achieves to explore and develop the expressive and communicative potentials, to integrate the affective, cognitive and motor areas of the patient, in an essentially sensory experience, in order to improve quality of life related to health (QLRH). Objectives: To estimate QLRH by using Kindl in children and adolescents attending the HMP in Teletón Santiago between 2008 and 2009, and to analyze the effect of parallel therapeutic treatments. Patients and Methods: We studied a cohort of 84 patients from 4 to 16 years old, who met the selection criteria, we administered the Kind questionnaire in their Kiddy (4-7 years, n = 22), Kid (8-11 years; n = 26) and Kiddo (12-16 years, n = 32) versions to measure QLRH before and after participating in the HMP. Results: The global QLRH increases in the Kid and Kiddy groups except in the school dimension; in Kiddo, there is a significant difference for global QLRH, emotional wellbeing and self-esteem; in Kid we find significant differences for physical and emotional wellbeing, self-esteem and friends (p < 0.05). Conclusions: The QLRH median score measured with Kindl, significantly increases after HMP intervention in the selected study groups. The perception of changes in QLRH reported by the parents and their children from 4 to 7 years old doesn't differ at the end of the intervention between them. Using therapy through the arts helps to enhance the global QLRH in all the Kindl groups and the self-esteem dimension in the Kiddo and Kind groups.


La terapia a través de las artes contenida en el programa de alta motivación (PAM), postula explorar y desarrollar los potenciales expresivos y comunicacionales, integrar las áreas afectivas, cognitivas y motora del paciente, en una experiencia esencialmente sensitiva, a fin de mejorar la calidad de vida relacionada a salud (CVRS). Objetivos: Estimar la CVRS mediante Kindl en niños y adolescentes asistentes al PAM en Teletón Santiago 2008-2009 y, analizar el efecto de tratamientos terapéuticos paralelos. Pacientes y Métodos: Se estudia una cohorte de 84 pacientes de 4 a 16 años, que cumplieron con requisitos de inclusión; se administra cuestionario Kindl en sus versiones Kiddy (4-7 años, n = 22), Kid (8-11 años; n = 26) y Kiddo (12-16 años; n = 32) para medir CVRS antes y después de participar en el PAM. Resultados: La CVRS global aumenta en los grupos Kiddy y Kid excepto en dimensión escuela; en Kiddo, se registra diferencia significativa para CVRS global, bienestar emocional y autoestima; en Kid las diferencias significativas se verifican para bienestar físico, emocional, autoestima y amigos (p < 0,05). Conclusiones: El puntaje mediano de CVRS medido con Kindl, aumenta significativamente después del PAM en los grupos considerados. La percepción de mejoría en CVRS es similar entre padres e hijos de 4 a 7 años al término de la intervención. La terapia a través de las artes, ayuda a potenciar la CVRS global en todos los grupos Kindl y la dimensión autoestima en grupos Kiddo y Kind.
